La mastocytose cutanée (MC) pédiatrique est une maladie rare et bénigne, affectant principalement les enfants dès la naissance jusqu’à l’âge de 2 ans. Quatre formes cliniques sont distinguées : l’urticaire pigmentaire, la MC papulo-nodulaire (ou mastocytome), la MC maculeuse télangiectasique et la MC diffuse. Notre étude rétrospective a porté sur 7 cas de mastocytose cutanée colligées aux services d’anatomie pathologique et de dermatologie de l’Hôpital Habib Thameur de Tunis sur une période de 4 ans allant de 2010 à 2014). L’âge moyen de nos patients était de 7.71 mois (avec des extrêmes allant de 3 mois à 1 an). Le sex-ratio était de 6 (6 garçons et 1 fille). Pour 4 patients, le diagnostic de MC a été suspecté cliniquement. Le signe de Darrier était positif dans un seul cas. L’étude histopathologique des biopsies cutanées a confirmé le diagnostic de MC dans tous les cas. L’étude immunohistochimique, faite dans 2 cas, a monté une positivité diffuse et intense aux CD-117 (C-kit). La confrontation anatomo-clinique a permis de retenir le diagnostic d’urticaire pigmentaire chez 3 patients. Chez 3 autres enfants, il s’agissait d’un mastocytome. Enfin le dernier patient a présenté à la fois des lésions d’urticaire pigmentaire dans sa forme bulleuse associée à des lésions de mastocy- tose papulo-nodulaire. Aucun patient n’a présenté de manifestation extra-cutanée de la maladie. La MC pédiatrique est une maladie de bon pronostic qui régresse spontanément dans près de la moitié des cas. Elle est rarement diffuse mettant en jeu le pronostic vital.
L’érythrokératodermie progressive symétrique est une génodermatose rare à transmission variable le plus souvent autosomique dominante. Elle est proche de l’érythrokératodermie variable, autre génodermatose héréditaire, mais s’en distingue par le caractère bilatéral et symétrique des lésions. Ces dernières apparaissent le plus souvent dès les premières années de vie mais la maladie peut aussi débuter à l’âge adulte. Elles siègent principalement aux dos des mains et des pieds, plus rarement à la face ou au tronc. Sur le plan histologique, la maladie est caractérisée par une importante hyperkératose orthokératosique avec parfois, focalement une parakératose. L’épiderme est hyperplasique, pa- pillomateux et acanthosique sans lésions de la granuleuse. Tout au plus, on peut noter des vacuoles claires périnucléaires dans les granulocytes. Le derme papillaire comporte un infiltrat inflammatoire péri-vasculaire d’abondance variable. Un homme âgé de 22 ans consultait pour des placards lichénoïdes, pigmentés, symétriques, localisés au niveau des dos des mains et des pieds et évoluant depuis deux ans. L’examen histologique de la biopsie de la lésion de la main gauche montrait un épiderme légèrement papillomateux, acanthosique surmonté par une importante hyperkératose orthokératosique. La granuleuse était conservée avec par endroits des vacuoles péri-nucléaires. Le diagnostic d’érythrokératodermie progressive symétrique a été retenu. Le malade a été convoqué pour une enquête génétique.
Schwannomas are benign tumors mostly arising from the head and neck. Retroperitoneal schwannomas are rare as only 0.7–2.6% of schwannomas occur in the retroperitoneal space, and approximately 1–6% of retroperitoneal tumors are schwannomas. Most retroperitoneal schwannomas are benign; approximately 1.2% of cases are malignant. They are diagnosed in patients with an average age of 48 years with mild female predominance. Given the large loose areolar space available in the retroperitoneum, tumors in this location often present late with vague complaints related to compression of surrounding structures and often develop degenerative changes inside them. Size at diagnosis is usually large with a median of 9cm. Imaging is not specific and diagnosis remains histological. In lights of the two observations, we will discuss the clinical, radiological and pathological features of this tumor. We report two cases of retroperitoneal schwannoma in a 17 year-old man and a 39 year-old woman. In the first case, the patient had a two years lasting mass of the left flank. The second patient consulted for a lower back pain. The radiological examination showed a retroperitoneal mass. Macroscopically, these masses were nodular, well defined, measuring respectively 8.5 and 12 cm with a firm, yellowishcut-section. Histological examination showed, in both cases, fusocellular, encapsulated proliferation made of a compact spindle cells component with focal nuclear palisadingwhich was altering with a loosely textured, histiocyte-rich component. There were numerous thick-walled vessels with fibrinoid and hyaline changes as well as a marked lymphoid infiltrate. Foci of ischemic necrosis were noted in the first case but without atypia or increased mitotic activity. Both tumors were stained positively and strongly with S100 protein. Diagnosis of benign retroperitoneal schwannomas was made. Retroperitoneal schwannoma was rare and preoperative diagnosis was difficult. Most of retroperitoneal schwannoma were benign and the surgical treatment was the first choice.
The structure of the major protein constituent of photosynthetic membranes in higher plants, the chlorophyll a/b-light harvesting complex (LHC), was studied by x-ray diffraction and electron microscopy. The LHC was purified from Triton X-100 solubilized thylakoid membranes of the pea, and contained 6 mol of chlorophylls a and b per mole of a polypeptide of 27,000 molecular weight. X-ray diffraction showed that in the presence of 10 mM MgCl2, purified LHC forms planar aggregates that stack with a period of 51 A. Within each layer, LHC molecules pack with a center-to-center distance of 85 A but without long-range order. However, when LHC is incorporated into single-walled vesicles of plant lecithin, the addition of NaCl above 10 mM, or MgCl2 above 2 mM, led to the formation of plaques of hexagonal lattices, with a lattice constant of 125 A. The large domain size and high degree of order in the plane of the membrane are evident from the sharp lattice lines observed to 7 A resolution on the equator of the x-ray pattern. Freeze-fracture electron micrographs demonstrated an aligned stacking of the lattices in adjacent membranes, resulting in crystallinity in the third dimension over short distances. Micrographs of negatively stained membranes revealed a hexagonal lattice of the same lattice constant, formed by surface-exposed parts of the LHC molecules which are probably responsible for the ordered stacking of lattices. In both the LHC aggregates and in the reconstituted membrane lattices the diffracted x-ray intensities at 10-A spacing on the equator indicate that the LHC molecule contains paralled alpha-helices or beta-sheets that are oriented perpendicular to the planar arrays.
Experimental and numerical studies have been conducted with the aim of modeling pressure rises which occur in the helium, during quenches of the 11.7-T superconducting magnet named Iseult. Iseult is based on a double-pancake winding internally cooled by superfluid helium channels opening into a pressurized He II bath at 1.8 K. A scale mock-up has been built of 10 copper equivalent pancake slices and 7 helium channels per pancake. The heat produced by a quench of the Iseult magnet is simulated by electrical heaters put inside each copper plate. Cryogenic pressure and temperature sensors have been fitted in the helium channels and in the bath. Bath pressure measurements are given for various heating powers, various numbers of heated plates and various bath volumes. Comparisons with a simple numerical model permit to identify the main physical mechanisms which drive the pressure rise during a quench.
Une étude sur le contrôle des infections à Staphylococcus aureus résistant à la méticilline (SARM) a été réalisée sur un échantillon d'établissements hospitaliers représentatif de l'ensemble des hôpitaux français. Cette étude a montré la lenteur avec laquelle les établissements de soins ont mis en place des programmes de surveillance et de contrôle de ces infections puisqu'en 1990 seulement 6 % des hôpitaux surveillaient les infections à SARM alors que l'incidence de ces infections à SARM était déjà élevée. Toutefois, des programmes de surveillance ont été mis en place progressivement dans la plupart des hôpitaux avec une collaboration pluridisciplinaire entre microbiologistes, hygiénistes et personnel soignant. Ces programmes se sont d'ailleurs révélés efficaces puisque la fréquence des infections à SARM a diminué dans les établissements surveillant ces infections.An assessment of the management of infections caused by methicillin-resistant Staphylococcus aureus (MRSA) was performed on a representative sample of French hospitals randomizely selected. This study shows that French hospitals implemented lately MRSA surveillance program since in 1990 only 6 % these hospitals had such surveillance program while the incidence of MRSA infections was already high (0.4 % admissions). However, since 1990 surveillance programs have been implemented in most hospitals with a collaboration between microbiologists, infection control personnel and clinical units' personnel. The implementation of these programs was associated with a decrease in the incidence of MRSA infections.
Phishing is a kind of embezzlement that uses social engineering in order to obtain personal information from its victims, aiming to cause losses. In the technical literature only the hit rate of the classifiers is mentioned to justify the effectiveness of the phishing detecting techniques. Aspects such as the accuracy of the classifier results (false positive rate), computational effort and the number of features used for phishing detection are rarely taken into account. In this work we propose a technique that yields the minimum set of relevant features providing reliability, good performance and flexibility to the phishing detection engine. The experimental results reported in this work show that the proposed technique could be used to optimize the detection engine of the anti-phishing scheme.
Les cancers thyroïdiens sont rares et représentent 1 % des tumeurs malignes. Les femmes sont plus fréquemment atteintes que les hommes (sex-ratio 3/1). Le cancer thyroïdien peut survenir à tout âge bien qu'il soit fréquent après 30 ans et significativement plus agressif chez les sujets âgés. La majorité des patients présente un nodule thyroïdien. Les nodules thyroïdiens sont fréquents, mais seulement 5 % d'entre eux sont malins. Durant les vingt dernières années, l'essor des moyens diagnostiques (échographie et cytoponction) et les modifications des pratiques de prise en charge des pathologies thyroïdiennes ont entraîné une augmentation des cancers thyroïdiens en particulier des microcancers papillaires. Le cancer thyroïdien est plus fréquent chez les patients ayant eu un antécédent d'irradiation thyroïdienne pendant l'enfance. Le diagnostic repose sur l'examen anatomopathologique de la pièce opératoire qui reste difficile dans certains cas. Les cancers thyroïdiens les plus fréquents (papillaire et vésiculaire) sont guéris dans 80 à 90 % des cas, si le traitement est approprié. Le cancer anaplasique est rare et de pronostic très défavorable. Le traitement initial du cancer thyroïdien consiste en une thyroïdectomie totale ou quasi totale. Le curage ganglionnaire n'est pas systématique. Le traitement par l'iode radioactif est conseillé chez les patients à haut risque et nécessite un taux élevé de thyroid stimulating hormone (TSH) pour être efficace. Par la suite, le patient doit être mis sous hormonothérapie thyroïdienne à vie. Les facteurs pronostiques sont bien définis et le pronostic est favorable en cas de cancer thyroïdien différencié. Les métastases à distance sont observées dans 10 % des cas et les poumons et les os sont les localisations les plus fréquentes. Les récidives locorégionales surviennent dans environ 7 % des cas. Une surveillance prolongée est conseillée. Elle est fondée sur l'examen clinique, le dosage de thyroglobuline plasmatique sous stimulation par la TSH (arrêt de la L-thyroxine ou utilisation de la TSH recombinante humaine), l'échographie cervicale et la scintigraphie corps entier sur dose thérapeutique d'iode 131.Thyroid carcinomas are rare and represent 1% of malignant tumours. Women are more likely to have thyroid cancer, at a ratio of three to one. Thyroid cancer may occur in any age group, although it is most common after the age of 30, and its aggressiveness increases significantly in older patients. Most patients present with a thyroid nodule. Thyroid nodules are frequent but only 5% of them are malignant. Over the last two decades, the widespread of diagnostic tools (ultrasonography, cytology) and changes in the surgical and pathological management of thyroid diseases have led to the increase of thyroid cancer, especially papillary microcarcinomas. Thyroid cancer is more frequent in people who have a history of thyroid exposure to radiation during childhood. The diagnosis is based on the histopathological examination that remains difficult in some cases. The most frequent thyroid cancers (papillary and follicular) are cured in 80-90 % of the cases if treated appropriately. Anaplastic cancer is rare and has a very poor prognosis. The primary therapy for thyroid cancer is total or near total thyroidectomy. Lymph node dissection is not systematically undertaken. Radioiodine treatment is advised for high risk patients and needs high levels of TSH to be effective. Then, patients must remain on thyroid hormone therapy for the rest of their lives. Prognosis factors are well defined and prognosis is good in case of differentiated thyroid cancer. Distant metastases are observed in 10% of the patients, and lung and bones are the most common sites. Locoregional recurrences are observed in 7% of the cases. Prolonged follow-up is recommended, based on physical examination, serum thyroglobulin assessment with TSH stimulation (L-thyroxin off or use of human recombinant TSH), ultrasonography and post therapeutic whole body scan.
