Die richtige Einschätzung und Einteilung von unfallchirurgischen Verletzungen ermöglichen eine passgenaue Therapie und Nachbehandlung. Mit einfachen Regeln lassen sich fast alle Frakturen korrekt nachbehandeln und in einem Arztbrief leicht an die Weiterbehandler vermitteln.
The nursing shortage, legal requirements, and funding deficits have led to increasingly poor patient care in German hospitals.Volunteer patient companions offer support by conversing with and caring for patients. This can work to relieve hospital staff while diminishing patients' fears/worries.Volunteer companions visited selected lonely and thus, delirium-prone patients in hospital wards and spent approximately one hour with patients conversing, playing, reading aloud, or mostly listening. A surgical escort service was also offered. Patients met the escort one day in advance. The escort then accompanied the patient to the operating room and later from recovery back to the ward. Transition companions were assigned to provide support after discharge. They contacted patients prior to discharge and assisted patients in organizing their home environment as well as continued social and medical care.From 1 July 2017 until 1 April 2023, 12000 patient companions were provided in 14 wards of 4 hospitals. Using a German school grading system (1 to 6, with 1 as the optimum), the volunteers' evaluation of their success averaged 1.3, and the patients' assessments averaged 1.2. Since November 2020, 120 surgery escorts were carried out by volunteers in trauma surgery. These were so successful that we now also make a comparable offer with a family member for all older patients. Since 2022, 30 transition companions have successfully supported patients at home in a pilot project.Volunteering in and around the hospital offers important support to older and single patients while relieving the hospital medical staff. It is a "win-win-win-win situation" for patients, volunteers, staff, and management.
ZusammenfassungDer Pflegemangel, gesetzliche Vorgaben und die Finanzierungsdefizite führen zu einer zunehmend schlechteren Patientenversorgung in deutschen Krankenhäusern.Ehrenamtliche Patientenbegleiter können hier eine sinnvolle Unterstützung bei der Unterhaltung und Betreuung der Patienten sein. Durch die Betreuung der Patienten wird auf den Stationen das medizinische Fachpersonal entlastet und den Patienten Ängste und Sorgen genommen.Die Begleiter besuchten ausgesuchte, einsame und somit delirgefährdete Patienten auf den Stationen und verbrachten ca. 1 h Zeit mit Gesprächen, spielen, vorlesen oder meistens nur zuhören. Zusätzlich wurde eine OP-Begleitung angeboten, bei der die Patienten am Vortag den Begleiter kennenlernten, der sie dann bis in den OP und später vom Aufwachraum wieder auf die Station begleitete. Zur Unterstützung nach der Entlassung wurden Übergangsbegleiter eingesetzt. Sie nahmen vor der Entlassung Kontakt zu den Patienten auf und unterstützten die Patienten bei der Organisation des häuslichen Umfeldes und der weiteren sozialen und medizinischen Betreuung.Vom 01.07.2017 bis 01.04.2023 wurden 12000 Patientenbegleitungen in 4 Krankenhäusern in 14 Stationen durchgeführt. Die Bewertung der Ehrenamtlichen nach Schulnoten zum Erfolg ihrer Arbeit lag im Mittel bei 1,3, die Bewertung der Patienten betrug im Mittel 1,2. Seit November 2020 wurden in der Unfallchirurgie 120 OP-Begleitungen durch Ehrenamtliche vorgenommen, die so erfolgreich waren, dass wir dies bei allen alten Patienten auch grundsätzlich bei einem Familienangehörigen anbieten. Seit 2022 konnten in einem Pilotprojekt 30 Übergangsbegleiter die Patienten erfolgreich zu Hause unterstützen.Ehrenamtliches Engagement im und um das Krankenhaus ist eine wichtige Unterstützung bei alten und alleinstehenden Patienten und entlastet das medizinische Personal im Krankenhaus. Es ist eine „Win-win-win-win-Situation“ für die Patienten, Begleiter, Personal und Geschäftsführung.
Background Fragility fractures of the sacrum (FFS) have been detected more and more frequently in recent times, and the incidence will continue to increase due to increasing life expectancy. The aim of this study was to compare the clinical outcome of conservative, interventional and surgical treatment of FFS. Methods Retrospectively, 292 patients (276 women, 16 men) with confirmed FFS were followed up over a period of 2 years. The age of the women was Ø 81.2 (58 - 99) and that of the men Ø 78.1 (76 - 85) years. The pain was quantified using a VAS. Fractures were classified in accordance with the Rommens and Hofmann and with the Denis classification using conventional X-rays, CT and MRI. A QCT of the lumbar spine was performed to quantify bone mineral density. Concomitant diseases of every patient were recorded. An interdisciplinary case conference determined the individual treatment concept considering the age, type of fracture, pain level and comorbidities with classification into conservative, interventional (any type of sacroplasty) or surgical treatment. Over the course pain and independence were measured, complications and patient satisfaction were documented. A vitamin D determination was done, and existing comorbidities were included. Results Patients with a pain level of ≤5 benefited from the conservative therapy measures, with pain levels > 5 significantly delaying the development of mobility. After sacroplasty, the pain reduced significantly, which caused a rapid improvement in mobility without any significant difference being found between vertebro- (VSP), balloon (BSP), radiofrequency (RFS) and cement sacroplasty (CSP). In terms of pain reduction and mobilization, the surgical treated patients benefited from osteosynthesis, although more complex fracture types with lumbopelvic stabilization took longer. Overall, there were no deaths during the hospital stay. Mortality after 12 months was 21.7% for the conservative, 8.4% for the interventional and 13.6% for the surgical therapy group; the differences are significant. For patients in the conservative therapy group who were difficult to mobilize due to pain, the mortality increased to 24.3%. Over 24 months, patients achieved the best independence after sacroplasty. At 12 and 24 months, subjective satisfaction with the therapies was best after sacroplasty, followed by osteosynthesis and conservative measures. All patients had a pronounced vitamin D deficiency and manifest osteoporosis. Cardiovascular pathologies were the main concomitant diseases. Conclusions Patients with FFS with a low level of pain benefit from conservative therapy measures, whereby complications and mortality increase significantly in the case of persistent immobilizing pain. Patients with an unacceptable level of pain resulting from non-dislocated fractures benefit significantly from sacroplasty. Patients with unstable and displaced fractures (Rommens and Hofmann type III and IV) should be operated on promptly. Different techniques are available for sacroplasty and osteosynthesis, which lead to an improvement of independence and a reduction in mortality.
Einleitung Insuffizienzfrakturen des Sakrums werden in letzter Zeit immer häufiger detektiert, wobei aufgrund der steigenden Lebenserwartung die Inzidenz weiter zunehmen wird. Vermutet wird eine Inzidenz von circa 5%, bei PatientInnen>80 Jahre noch deutlich höher. Als wichtigste Risikofaktoren gilt das weibliche Geschlecht, das Alter>70 Jahre und eine vorhandene Osteoporose. Ziel der retrospektiven Untersuchung bei Patientinnen mit sakralen Insuffizienzfrakturen ist die Bestimmung des Ausmaßes eines möglichen Vitamin-D-Mangels und einer vorhandenen Osteoporose.
Einleitung Bei malignen Erkrankungen wie dem Multiplen Myelom sind ossäre Destruktionen mit pathologischen Frakturen am Achsenskelett keine Seltenheit. Sie verschlechtern die Lebensqualität und Überlebensprognose des Pat. Klinisch stehen stärkste Schmerzen im Vordergrund. In unserer Studie sollten die Durchführbarkeit, Sicherheit und klinischen Ergebnisse der Ballonkyphoplastie bei Pat. mit myelombedingten ossären Destruktionen und/oder pathologischen Frakturen überprüft werden.
Der Oberschenkelhalsbruch ist eine typische und häufige Verletzung des alten Menschen. Aufgrund der durch die Osteoporose verminderten Knochendichte und der zunehmenden Varisierung des Schenkelhalses ist es ein Prädilektionsort für Frakturen auch nach Minimaltraumen [1]. Mit zunehmendem Alter steigt die Inzidenz drastisch an. In Deutschland liegt die Häufigkeit über alle Einwohner gesehen bei 90/100000 Einwohnern [2] [3]. Bei den über 65-Jährigen beträgt die Inzidenz schon 966/100000 Einwohner, dabei schwanken die Zahlen in Abhängigkeit vom Wohnort. In Baden-Württemberg ist die Inzidenz mit 507 deutlich geringer als z. B. in Hamburg mit 1404/100000 Einwohner. Bis 2050 wird mit einer Verfünffachung der Inzidenz gerechnet [4]. Die Frakturen führen heute wie vor 20 Jahren zu einer signifikanten Verminderung der Lebenserwartung und haben für die betroffenen Patienten eine oft dramatische Verschlechterung der sozialen und gesundheitlichen Situation zur Folge [5] [6]. Viele Patienten verlieren ihre Selbstständigkeit und bedürfen zusätzlicher Hilfe oder werden sogar zum Pflegefall [7].
Zusammenfassung Hintergrund Ziel der Untersuchung bei PatientInnen mit Fragilitätsfrakturen des Os sacrum (FFS) war die Erfassung von vorhandenen Risikofaktoren sowie der klinischen Ergebnisse nach Zementsakroplastie (ZSP). PatientInnen und Methoden Retrospektiv wurden 68 PatientInnen (64 Frauen, 4 Männer) mit stattgehabten FFS nachuntersucht. Anhand von CT- und MRT-Schnittbildern erfolgte eine Fraktureinteilung nach Denis et al. sowie Rommens und Hofmann. Bei allen PatientInnen wurde eine Knochenmineralgehaltsbestimmung mittels QCT durchgeführt. Unter Berücksichtigung von Anamnese und Röntgenaufnahmen wurden Begleiterkrankungen sowie zentrale und periphere Frakturen miterfasst. Vitamin-D-Werte wurden zusätzlich bestimmt. Nach einem frustranen konservativen Therapieversuch erfolgte eine ZSP. Anhand der Schmerzentwicklung, der körperlichen Selbstständigkeit, der PatientInnen-Zufriedenheit, der Komplikationsrate und der Mortalität wurden die Ergebnisse dokumentiert. Ergebnisse Das Alter der Frauen betrugt Ø 83,2 (72–99), dass der Männer Ø 77,8 (76–85) Jahre. Zu 42,4 % fand sich eine Denis-Typ-1-, zu 4,2 % eine Denis-Typ-2-, zu 0 % eine Denis-Typ-3-, zu 43,3 % eine Denis-Typ-1–2- und zu 10,1 % eine Denis-Typ-1–2–3-Frakturzone. Es fand sich ein FFP-Typ-II a-bis -II c-Frakturgeschehen zu 88,2 %, ein FFP-Typ III c zu 7,4 % sowie ein FFP-Typ IV b zu 4,4 %. Bei 68,8 % fanden sich bilaterale FFS. Der Knochenmineralgehalt (KMG) betrug im Ø 35,4 (2–74) mg/ml, der Vitamin-D-Wert im Ø 8,8 (0–28) nmol/l. Weitere osteoporoseassoziierte Frakturen fanden sich in circa 50 %. Nach der ZSP zeigten die PatientInnen eine schnelle und signifikante ( p < 0,001) Schmerzreduktion sowie nachhaltige klinische Verbesserung. Schlussfolgerung Als Frakturrisikofaktoren von FFS fanden sich das weibliche Geschlecht, das hohe Alter, eine vorhandene Osteoporose und ein schwerer Vitamin-D-Mangel. PatientInnen mit nichtdislozierten FFS, welche schmerzbedingt nicht zu mobilisieren waren, profitierten von einer ZSP nachhaltig.
Background: The objective of the present study on patients with fragility fractures of the sacrum (FFS) was to assess existing risk factors and clinical outcomes after cement sacroplasty (CSP). Patients andmethods: 68 patients (64 women, 4 men) with previous FFS were followed up retrospectively. CT and MRI images were used to classify fractures according to Denis et al. and Rommens and Hofmann. Bone mineral content was determined by QCT in all patients. Concomitant diseases as well as central and peripheral fractures were recorded, considering the patient's medical history and X-ray images. Vitamin D levels were also determined. If conservative therapy was unsuccessful, CSP was performed. The results were documented on the basis of pain development, physical independence, patient satisfaction, complication rate and mortality. Results: The age of the women in the study was empty set 83.2 (72-99) and that of the men empty set 77.8 (76-85) years. 42.4% had a Denis type 1, 4.2% a Denis type 2, 0% a Denis type 3, 43.3% a Denis type 1-2 and 10.1% a Denis type 1-2-3 fracture zone. FFP type II a to II c fractures were found in 88.2%, FFP type III c in 7.4% and FFP type IV b in 4.4%. Bilateral FFS were found in 68.8%. The average bone mineral content (BMC) was 35.4 (2-74) mg/ml, and the average vitamin D value was 8.8 (0-28) nmol/l. Other osteoporosis-associated fractures were found in around 50% of the patients. After CSP, patients showed a rapid and significant (p<0.001) reduction in pain and sustained clinical improvement. Conclusion: FFS fracture risk factors were found to be female gender, advanced age, existing osteoporosis and severe vitamin-D deficiency. Patients with non-displaced FFS who could not be mobilised due to pain experienced sustained benefit from CSP.
Insufficiency fractures of the sacrum are being detected increasingly frequently, whereby their incidence will no doubt increase further as a result of the rise in life expectancy. Some of the patients are disabled by the severe fracture pain and cannot be treated adequately by means of conservative treatment. A marked pain reduction and clinical improvement can be achieved by inserting cement into the respective fracture zone. The objective of this review article is to present the different cement augmentation procedures, balloon, radiofrequency, vertebro- and cement sacroplasty, with regard to their correct indication, technical feasibility, possible complications and clinical outcome. A literature search was conducted in PubMed and Google Scholar using the keywords: pelvic insufficiency fracture, fragility fractures of the pelvic ring, sacral insufficiency fracture, sacrum, osteoporosis, interventional pain therapy, sacroplasty, cement augmentation and pelvic osteosyntheses. Experience gained from our own interdisciplinary, multicentric working group, which has been in existence for many years, was also taken into consideration. New classifications in the categorisation of pelvic insufficiency fractures must be taken into account when determining the appropriate therapeutic procedure, while making allowance for the individual clinical situation. Conservative therapy is of primary importance initially, although not all patients achieve freedom from pain here and cannot be mobilised adequately, subsequently leading to comorbidities and increased mortality. In the case of non-dislocated fractures, sacroplasty can be performed to achieve rapid and sustained pain reduction. With regard to cement augmentation, the osteoplastic procedures such as balloon, radiofrequency and cement sacroplasty can be differentiated from the non-osteoplastic procedure of vertebrosacroplasty. In the case of vertebrosacroplasty, a higher rate of cement leakage is to be expected. The complex sacral anatomy, the osteopenic bone structure, the alignment of the fracture zones, the differences in technical approach, the properties of the cements and the experience of the surgeons have an influence on the occurrence of possible complications. If conservative measures do not lead to a clinical improvement, patients with painful, non-dislocated sacral insufficiency fractures achieve an effective and sustained benefit from cement augmentation. Through the use of individually adaptable sacroplasty procedures, relevant complications are generally very rare. Patients with unstable fractures benefit from osteosynthesis performed at an early stage.
Zusammenfassung Insuffizienzfrakturen des Os sacrum werden in letzter Zeit immer häufiger detektiert, wobei aufgrund der steigenden Lebenserwartung die Inzidenz weiter zunehmen wird. Ein Teil der Patienten ist, bedingt durch die starken Frakturschmerzen, invalidisiert und lässt sich durch eine konservative Behandlung nicht suffizient therapieren. Mittels Zementeinbringung in die entsprechende Frakturzone lässt sich eine deutliche Schmerzreduktion und klinische Verbesserung herbeiführen. Ziel dieser Übersichtsarbeit ist eine Darstellung der unterschiedlichen Zementaugmentationsverfahren Ballon-, Radiofrequenz-, Vertebro- und Zementsakroplastie hinsichtlich der richtigen Indikation, der technischen Durchführbarkeit, den möglichen Komplikationen und des klinischen Outcomes. Die Grenzen der Sakroplastie und die Indikation zur Durchführung unterschiedlicher Osteosyntheseverfahren werden aufgezeigt. Eine Literaturrecherche in PubMed und GoogleScholar wurde zu den Stichworten durchgeführt: Beckeninsuffizienzfraktur, Fragilitätsfrakturen des Beckenrings, sacrale Insuffizienzfraktur, Os sacrum, Osteoporose, interventionelle Schmerztherapie, Sakroplastie, Zementaugmentation und Beckenosteosynthesen. Erfahrungen aus der seit Jahren bestehenden eigenen interdisziplinären und multizentrischen Arbeitsgruppe wurden mitberücksichtigt. Neue Klassifikationen in der Einteilung von Beckeninsuffizienzfrakturen sind bei der Festlegung des therapeutischen Vorgehens unter Beachtung der individuellen Klinik zu berücksichtigen. Eine konservative Therapie steht zunächst im Vordergrund, jedoch werden hier nicht alle Patienten schmerzfrei und lassen sich nicht adäquat mobilisieren, konsekutiv folgen Komorbiditäten und eine erhöhte Mortalität. Bei nichtdislozierten Frakturen lässt sich mittels Sakroplastie eine schnelle und nachhaltige Schmerzreduktion herbeiführen. Hinsichtlich der Zementaugmentation lassen sich osteoplastische Verfahren wie die Ballon-, Radiofrequenz- und Zementsakroplastie von dem nichtosteoplastischen Verfahren der Vertebrosakroplastie unterscheiden. Bei der Vertebrosakroplastie muss man mit einer höheren Zementleckagerate rechnen. Die komplexe sacrale Anatomie, die osteopene Knochenstruktur, die Ausrichtung der Frakturzonen, das unterschiedlich technische Vorgehen, die Eigenschaften der Zemente und die Erfahrung des Operateurs haben einen Einfluss auf das Auftreten möglicher Komplikationen. Wenn konservative Maßnahmen zu keiner klinischen Verbesserung führen, profitieren Patienten mit schmerzhaften, nichtdislozierten sacralen Insuffizienzfrakturen von einer Zementaugmentation effektiv und nachhaltig. Durch den Einsatz von individuell anzupassenden Sakroplastieverfahren sind relevante Komplikationen insgesamt sehr selten. Patienten mit instabilen Frakturen profitieren von einer rechtzeitig durchgeführten Osteosynthese.

Ziel dieser Übersichtsarbeit ist eine Darstellung der unterschiedlichen Zementaugmentationsverfahren Ballon-, Radiofrequenz-, Vertebro- und Zementsakroplastie hinsichtlich richtiger Indikation, der technischen Durchführbarkeit, den möglichen Komplikationen und dem klinischen Outcome.

Beispielhaft werden die Klinik der sakralen Insuffizienzfraktur, das therapeutische Vorgehen unter Berücksichtigung des klinischen Outcomes und der DRG-Erlöse besprochen. Drei weibliche Patienten (Ø Alter 78,3 Jahre) mit sakralen Insuffizienzfrakturen wurden aufgrund von zunehmenden immobilisierenden Schmerzen stationär aufgenommen. Unter Berücksichtigung der Klinik und der Empfehlung aus einer interdisziplinären Fallkonferenz wurde eine Patientin konservativ mit kurzfristiger Bettruhe, begleitender Schmerzmedikation und schmerzadaptierten Bewegungsmaßnahmen behandelt. Bei einer Patientin erfolgte eine CT-gesteuerte Ballonsakroplastie. Bei einer Patientin wurde eine transiliosakrale Verschraubung durchgeführt. Die Schmerzen wurden im Verlauf mittels VAS und die Selbstständigkeit anhand des Barthel-Index dokumentiert. Die Frakturen wurden nach Denis et al. und der Klassifikation der „fragility fractures of the pelvis“ (FFP) nach Rommens und Hofmann eingeteilt. Anschließend erfolgte für jeden Fall eine Darstellung der DRG-Erlöse der Abrechnungsperiode 2020. Patientin Nr. 1: konservative Therapie, einseitige Denis-1-Frakturzone, entsprechend einer FFP Typ IIa, Ausgangsschmerz 7 Score-Punkte, bei Entlassung 4 Score-Punkte; der Barthel-Index stieg von 55 auf 75 Punkte. Nach 6 stationären Tagen Verlegung in die Reha. Der DRG-Erlös lag bei € 3817,95. Patientin Nr. 2: Ballonsakroplastie, beidseitige Denis 1–2 Frakturzone, entsprechend einer FFP Typ IIa, Ausgangsschmerz 9 Score-Punkte, bei Entlassung 2 Score-Punkte, der Barthel-Index stieg von 35 auf 95 Punkte. Nach 4 stationären Tagen Entlassung in die ambulante Weiterbehandlung. Der DRG-Erlös lag bei € 7409,44. Patientin Nr. 3: Osteosynthese, beidseitige Denis-1-Frakturzone, entsprechend einer FFP Typ IIa, Ausgangsschmerz 7 Score-Punkte, bei Entlassung 2 Score-Punkte, der Barthel-Index stieg von 40 auf 90 Punkte. Nach 5 stationären Tagen Verlegung in die Reha. Der DRG-Erlös lag bei € 6714,30. Die sakrale Insuffizienzfraktur ist ein starker Indikator für das Vorliegen einer manifesten Osteoporose. Als Frakturrisikofaktoren finden sich das weibliche Geschlecht, das hohe Alter, eine vorhanden Osteoporose und ein Vitamin-D-Mangel. Zunächst steht die konservative Therapie im Vordergrund der Behandlungskaskade. Bei Patienten mit persistierenden, immobilisierenden Schmerzen und keiner Möglichkeit der Mobilisierung sollte jedoch rechtzeitig eine Sakroplastie oder Osteosynthese durchgeführt werden. Bei komplikationsfreien Verläufen und abgestimmten Therapieprozessen sind alle 3 Behandlungsansätze wirtschaftlich zu erbringen.
Täglich erleben wir in deutschen Krankenhäusern und Praxen die Probleme unseres Gesundheitswesens: überfüllte Notaufnahmen und Praxen mit gestresstem Personal bei Vollversorgermentalität der Patienten. Fachkräftemangel bei Pflege und Ärzten in Krankenhäusern führt zu Bettenschließungen, fehlende haus- und fachärztliche Versorgung bedingen eine Unterversorgung im ambulanten Bereich mit langen Wartezeiten und Mehrbelastung der alternativ genutzten Notaufnahmen der Krankenhäuser. Dies alles führt zu weiter schwindenden Ressourcen, unzufriedenen Patienten und Behandlern.
Fragestellung Die Inzidenz multimorbider Patienten mit Frakturen im hohen Alter wird weiter zunehmen. Eine kooperative Behandlung mit Unfallchirurgen, Geriatern und Therapeuten ist daher ein tragfähiges Zukunftskonzept. Seit 10 Jahren werden wir vom TÜV als Alterstraumatologiezentrum zertifiziert.
Demographic development in Germany has led to an aging of the population. Particularly for these patients, osteoporosis-induced vertebral fractures represent a significant decrease in quality of life and level of activity. According to current guidelines, the initial treatment of stable osteoporotic vertebral fractures is conservative management with analgesic, anti-osteoporotic, physical therapy, and orthotic measures as first line options. Personal experience, however, suggests that patients benefit from timely surgical treatment through rapid improvement of pain symptoms and thus, more rapid mobilization. The poor bone quality of elderly patients presents the treating spine surgeon a challenge in achieving stable spinal fusion with or without support, for example, through augmentation. Minimally invasive procedures have increasingly established themselves for such purposes in recent years. With over 1000 fracture treatments in the last 3.5 years, we have developed a differentiated treatment concept depending on patient age and fracture morphology, which we would like to introduce. Unstable fractures with posterior edge involvement are stabilized from posterior with a percutaneous fixator. Patients over 60 years were treated percutaneously with a polyaxial screw system. Increased stability was achieved by PMMA cement augmentation of the fenestrated screws. In elderly patients with Magerl A3 fractures without neurologic deficit, the index vertebra is supplementally treated with kyphoplasty (hybrid treatment). In acute, stable osteoporotic vertebral fractures with severe pain despite analgesics, we perform kyphoplasty, which is possible even in high thoracic fractures to T3 with smaller balloons and thinner trocars. Vertebroplasty is another option in the lumbar and lower thoracic spine. Because of invasiveness, extended posterior-anterior correction procedures are generally avoided in this population, which has frequent multiple comorbidities.
Background Today, nearly two thirds of inpatients in trauma surgery hospitals are over 70 years old and at significant risk for comorbidities. These patients frequently suffer delirium. Delirium occurs in 15-30% of these trauma patients and increases the risk of mortality in up to 15-25% of cases. Conversation, attention, and activity significantly reduce this risk. Objectives Thus, the Centre for Geriatric Traumatology and the District Seniors Council have initiated a new project of a visiting service, to prevent delirium and anxiety conditions in elderly inpatients. Volunteers from the welfare program offer daily care for selected patients. Methods They care for and interact with the patients for approximately one hour per day. They accompany the patients in the hospital and during mobilization, explain, read aloud, converse, or simply listen, and thus provide calm, support, and company. The attendants are volunteers who have been previously trained by the clinic, who regularly attend team meetings and draw up reports from each admission. All patients and relatives are informed prior to these visits and are notified with an accompanying letter. There are structured visiting plans for each week. Results From July 17 to May 19, 4031 patients in our clinic and in three other hospitals have been accompanied in the program. None of the patients suffered delirium. The patients and attendants found the care very good and personally satisfying. Evaluations of the volunteers for the success of their work, using the German school-grading system, was an average 1.3. Noticeable was the markedly reduced frequency of patient calls to nursing, who were thus relieved. Travel expenses and insurance costs for the volunteers were reimbursed. Conclusion In times of skilled labour shortages, patient support provided by volunteer patient attendants enables innovative care for patients at risk for delirium. The assistance of the attendants alone reduced the workload on the nursing staff.Y
