Non-adherence to the RT protocol was associated with an inferior OS and loco-regional control. These results underline the importance of RTQA.
Concurrent chemotherapy is the standard of care for locally advanced NSCLC treated with definitive radiotherapy. The optimal chemotherapy regimen for use with concurrent therapy is not known. In this secondary analysis we evaluated the role of different chemotherapy regimens in patients treated in the phase III PET Plan trial. The prospective randomized controlled phase-III-type PET-Plan trial was conducted in 24 centers in Germany, Austria and Switzerland. Patients with inoperable locally advanced NSCLC suitable for radio-chemotherapy were randomized at a 1:1 ratio concerning the target volume (TV) definition and received quality-assured isotoxically dose-escalated IMRT or 3D-RT (60–74 Gy, 2 Gy per fraction) was applied to the respective TVs. Concurrent platinum-based chemotherapy was mostly conducted according to Vokes et al.(Vokes, II et al. 2002), using cisplatin 80mg/m2 (day 1 and 22) and vinorelbin 15mg/m2 (day 1+8 and 22+29) (protocol 1, P1) or cisplatin 20mg/m2 (day 1-5 and 29-33) and vinorelbin 12.5 mg/m2 (day 1, 8, 15 and 29, 36, 43) (protocol 1, P2) or carboplatin AUC1 (day 1-5 and 29-33) and vinorelbin 12.5 mg/m2 (day 1, 8, 15 and 29, 36, 43) (protocol 1, P3) according to Semrau et al. (Semrau, Klautke et al. 2008), at the discretion of the treating physician. Between 05/2009 and 11/2016, 172 patients in the per-protocol (PP) analysis (A: n = 84, B: n = 88). 30 patients were treated according to P1, 92 according to P2 and 28 according to P3 (1 patient did not receive any treatment and 21 patients received different treatments). All three protocols were well tolerated. Patients in P1 (17%) und P2 (15%) developed more grade 3 leucopenia than P3 (4%) but less grade 3 renal toxicities 0% (P1,2) vs 4% (P3). Only patients in P1 developed grade 4 hematologic toxicities (7% leucopenia, 3% anemia and 3% thrombocytopenia). Patents treated with P1 or P2 had a better OS as opposed to P3 (p = 0.008 and p = 0.009 respectively) but there was no difference between P1 and P2 (p = 0.6). There was no difference observed concerning PFS between the different protocols. Patients treated with cisplatin (n = 122) had a better OS as opposed to carboplatin (n = 28) (p = 0,003), which remained also adjusting for age, ECOG, ejection fraction using cardiac ultrasound and creatinine at baseline. Vinorelbin in combination with cisplatin either as a single dose of 80mg/m2 per cycle or daily doses of 20mg/m2 (5 days per cycle) for concurrent chemoradiation show similar efficacy in this patient cohort. Patients treated with carboplatin had a significant worse survival. This may be due to the selection of the drug due to restriction in normal tissue function, which may also cause an impaired prognosis.

In an attempt to find the best condition for PSMA radioligand therapy in metastatic castration-resistant prostate cancer (mCRPC) patients, we evaluated the effect of oral enzalutamide in each individual.
Ziel/Aim Die Bestimmung der radiochemischen Reinheit von 225Ac-markierten Radiotracern, wie z.B. [225Ac] Ac-PSMA-617, erfolgt in der Regel mittels Dünnschichtchromatographie (DC). Aufgrund der nicht vorhanden Gamma-Emissionen des Ac-225 erfolgt die Messung über die Gammalinien der Tochternuklide Fr-225 bzw. Bi-213. Mit konventioneller Messtechnik (Radio DC Detektor) ist die Bestimmung der Reinheit sowohl zeitaufwendig (Wartezeit bis zur Einstellung des radioaktiven Gleichgewichts) als auch inakkurat (geringe Gamma Emissionswahrscheinlichkeiten). Ziel dieser Arbeit war der Vergleich unterschiedlicher Trenn- und Messmethoden zur Bestimmung der radiochemischen Reinheit, um eine zeitnahe und verlässliche Qualitätskontrolle zu etablieren.
Ziel/Aim Einige Patienten mit anfänglich intensiver PSMA-Expression entwickeln unter laufender Behandlung Läsionen mit geringer oder fehlender PSMA-Expression. Die FDG-PET/CT eignet sich zum Nachweis solcher therapeutisch hochrelevanter Mismatch-Herde, nämlich glukosemetabole vitale Tumorläsionen mit weitgehend fehlender PSMA-Expression (GLUT(+)/PSMA(-) mismatch). Untersucht wurden mögliche laborchemische Prädiktoren für das Auftreten eines solchen, eine PSMA-Therapie kontraindizierenden, mismatch-Befundes.
The impact of 18-FDG-PET/CT based target volume delineation on outcome in radio-chemotherapy for locally advanced (LA) non-small cell lung cancer (NSCLC) was evaluated in an international prospective randomized multicenter trial (PET-Plan). A phase II non-inferiority study was performed with LA NSCLC scheduled for combined radio-chemotherapy in 23 trial sites. In 205 of 311 screened patients, target volumes were randomized between a conventional (adjuvant mediastinum to 50 Gy plus dose escalation volume of tumor, atelectasis, FDG-positive and CT-positive nodal stations; arm A) and an experimental (escalation volume FDG-positive tumor and nodal stations only; arm B) approach. In every case, respecting normal tissue constraints (pre-defined in analogy to RTOG 0617), isotoxic dose escalation was performed to achieve the individually highest achievable doses between 60 and 74 Gy/2 Gy. Stringent prospective and retrospective imaging and RT-QA was done during recruitment and follow-up. Simultaneous chemotherapy was given, mainly using platinum and vinorelbine. Primary endpoint was loco-regional progression, main secondary endpoints were infield and outfield progression-free survival, overall survival, and toxicity. After a minimum/mean follow up of 6/22 months, we here report the first results. In the well balanced patient group of 205 patients, 172 were treated as per protocol, being mainly in UICC-stage IIIa (36%) and IIIb (56%) with GTVs of in mean 105 ml. Dose escalation reached in mean 65.3 Gy (A) and 67.3 Gy (B; p=0.007). Loco-regional progression (LRP) was more frequent in arm A (conventional target volume) with a cumulative incidence of 0.29 / 0.39 vs. 0.14 / 0.20 (arm B) after 12 / 24 months (p=0.078). In stratified Cox-models, independent prognostic factors for LRP were study center (p=0.046) and tumor volume (GTV) (p=0.022) while study arm did not reach statistical significance. IMRT (50%) technique did not lead to significantly different results as compared to 3-D-CRT (50%). The 2-year overall survival was 57% (A) vs. 54% (B; n.s.). Neither for local control nor for survival, a positive or negative influence of radiation therapy dose was observed. There was no significant difference for in-field (A: 23% vs. B: 11%; n.s.) and out-field- (A: 11% vs. B: 8%; n.s.) regional recurrences between the study arms. Acute and late toxicity (CTC/ RTOG-EORTC) was mild with no clinically significant difference between the treatment arms. However, SUEs appeared more frequent in arm B. FDG-PET based radiation therapy planning is recommended for isotoxically dose escalated radio-chemotherapy in locally advanced NSCLC. It enabled higher dose escalation and a clear trend to improved local control as compared to conventional planning. The favorable survival results well compare to other recent trials. In contrast to RTOG 0617, no adverse effect of higher treatment doses were observed.
Ziel: Fluoro-2-deoxy-D-Glukose (FDG)-Positronenemissionstomografie (PET) und PET / CT werden zunehmend zur Strahlentherapie (RT)-Planung bei Patienten mit nicht kleinzelligen Bronchialkarzinomen (NSCLC) eingesetzt. Der Planungsprozess basiert oft auf separat erstellten FDG-PET / CT- und Planungs-CT-Aufnahmen. Wir haben intraindividuelle Unterschiede zwischen diagnostischen PET-Aufnahmen (D-PET) und PET-Aufnahmen in Bestrahlungsplanungsposition (RT-PET), die zuvor mit in unterschiedlichen Atemphasen angefertigten Planungs-CT-Aufnahmen koregistriert wurden, miteinander verglichen. Methoden: 16 Patienten mit NSCLC erhielten jeweils eine D-PET-Untersuchung, eine RT-PET-Untersuchung und 3 Planungs-CT-Untersuchungen in unterschiedlichen Atemphasen (Exspiration = EXP, Inspiration = INS und Atemmittellage = MID) am selben Tag. Alle Aufnahmen wurden mit Hilfe eines rigiden Algorithmus koregistriert, wobei insgesamt 6 fusionierte Datensätze entstanden: D-INS, D-EXP, D-MID, RT-INS, RT-EXP und RT-MID. Die Genauigkeit der Fusion wurde durch 3 Betrachter anhand von 8 anatomischen Landmarken (Lungenspitzen, Aortenbogen, Herz, Wirbelsäule, Sternum, Carina tracheae, Diaphragma und Tumor) beurteilt, dabei wurde ein Fusions-Score von 1 (keine Fusion) bis 5 (exakte Fusion) angewandt. Ergebnisse: Alle RT-PET-Fusionsdatensätze wiesen im Vergleich zu den D-PET-Datensätzen eine bessere Fusionsqualität auf (p < 0,001). In Bezug auf das Atmungsprotokoll zeigte RT-MID die beste Fusion (3,7 ± 1,0), gefolgt von RT-EXP (3,5 ± 0,9) und RT-INS (3,0 ± 0,8) (p < 0,001). Im Vergleich der Fusionsqualität der einzelnen anatomischen Landmarken fanden sich die größten Abweichungen an Zwerchfell, Herz und Lungenspitzen. Die Übereinstimmung unter den Betrachtern war mäßig (κ = 0,48; p < 0,001). Schlussfolgerung: Durch Durchführung der PET-Untersuchung in RT-Position kann eine signifikant bessere Fusion von PET und Planungs-CT erreicht werden. Das beste intraindividuelle Ergebnis wird erreicht, wenn die Planungs-CT in Atemmittellage durchgeführt wird. Unsere Daten rechtfertigen die Anfertigung einer separaten Planungs-PET-Untersuchung in RT-Position, falls nur eine diagnostische PET-Untersuchung verfügbar ist.
Einleitung: Mit der FDG-PET basierten Strahlentherapieplanung beim NSCLC können möglicherweise höhere Gesamtdosen und dadurch eine bessere lokale Tumorkontrolle erreicht werden. Daher konzipiert die AG Nuklearmedizin und Strahlentherapie der DEGRO und DGN derzeit eine randomisierte Multicenterstudie zur FDG-PET geplanten dosiseskalierten Radiochemotherapie bei inoperablen NSCLC. Seit Januar 2006 wird in Homburg die Pilotphase als offene, nicht randomisierte Phase I-Studie durchgeführt. Ziel ist die Etablierung und Standardisierung des Vorgehens, sowie der Evaluation der Rate von out-field-Rezidiven.
