Próteses articulares são fabricadas com ligas de Ti-6Al-4V ou Co-Cr-Mo, porém, a liberação de íons nestas ligas pode causar efeitos tóxicos. O revestimento de superfícies articulares com um “filme protetor“ visa aumentar a longevidade das próteses e evitar a liberação de íons causadores de hipersensibilidade, além de diminuir a dor, ocasionalmente provocada pela alteração da temperatura do metal. O filme de Diamond-Like Carbon (DLC) vem sendo pesquisado, devido sua biocompatibilidade, alta resistência ao desgaste e à corrosão, baixo coeficiente de atrito, inércia química e efeito bactericida. O objetivo deste estudo foi avaliar a neoformação óssea em amostras de liga Ti-6Al-4V recobertas com filme de DLC associados ou não a nanopartículas de prata (nAg). Após caracterização estrutural as amostras foram fixadas bilateralmente no ângulo da mandíbula de 30 coelhos. Quinze e 90 dias após a cirurgia, as amostras foram removidas junto ao osso, e novamente caracterizadas. Os filmes de DLC e DLC-Ag mostraram alta resistência elétrica e não sofreram alterações significativas no espectro Raman após exposição in vivo. No teste de Micro scratch test observou- se que os filmes aumentaram sua resistência mecânica após o teste in vivo. Em todos os animais, foi ocorreu neoformação óssea na interface osso-amostra, caracterizando a osseointegração da mesma e não observou-se diferença estatística entre os grupos no mesmo período (p>0,05). Concluiu-se que os filmes de DLC e DLC-Ag reagem in vivo de forma semelhante à liga Ti-6Al-4V, que é considerada o padrão ouro para uso biomédico.Palavras-chave: Osseointegração. Carbono. Calcific Uraemic Arteriolopathy (CUA) or calciphylaxis, is a thrombotic disorder of skin and subcutaneous tissue which typically presents with painful purpuric nodules that may progress to necrotic ulcers, and is a severe, life-threatening condition. CUA is an uncommon clinical entity that affects mostly haemodialysis (HD) patients. Although the process of vascular calcification was initially thought to be the result of a passive deposition of calcium -phosphate crystals, current knowledge suggests a distinct mechanism, including cellular activity with differentiation of vascular smooth muscle cells (VSMCs) into chondrocyte as well as osteoblast -like cellular phenotypes and deficiencies in calcification inhibitors. Although multiple studies suggest a potential relationship between warfarin and CUA, larger prospective studies are needed in order to better evaluate this association, and randomised controlled trials are needed to assess the benefit of distinct interventions in this setting. In this article the topic of CUA is reviewed based on a clinical case of a 65 -year -old man undergoing haemodialysis, who underwent an aortic valve replacement one year earlier, receiving a mechanical heart valve, and who has been under warfarin therapy since then.

Methodology validation is an important tool to support the activities of metrology in the quantification analysis. Extraneous materials may be present in the pulps due to the conditions or the practices of production, storage and distribution. Since there is no specific methodology for fruit pulp light filth quantification a research has been carried out to test two methods for isolation of light filth in mango, strawberry, tomato and guava pulps. For guava, mango and tomato pulps the AOAC Official Method 964.23 was used, and for strawberry pulp AOAC Official Method 950.89, with adaptations. The micro-analytic standard for insect fragment and rodent hair was prepared in the laboratory. The study has been conducted on 63 samples of 100 g of pulp analyzed in duplicate after contamination with 5, 15, 30 insect fragments (IF) per 100 g and 5, 10, 15 rodent hair (RH) per 100 g, with blank samples as a control. The validation parameters used were precision and accuracy. Recovery of insect fragment was considered stisfactory in mango, tomato, guava pulps, and the average recovery ranged from 87 to 96%. For strawberry pulp, the recovery rate ranged from 68 to 80%. For rodent hairs, the best recovery rate was observed in mango pulp ranging from 73 to 81%, followed by guava pulp ranging from 65 to 76%, tomato pulp ranging from 50 to 67%, and strawberry pulp ranging from 33 to 35%. It was concluded that the methodologies used to detect light filth in guava, mango, tomato and strawberry pulps can be adopted in the monitoring routine of fruit pulp extraneous material contamination even though they were found not very efficient for rodent hairs. Traditional open repair of aortic arch aneurysms is a complex and demanding surgery that requires cardiopulmonary bypass, hypothermia and circulatory arrest, being associated with considerable morbidity and mortality. Even with significant advances in pre-operative assessment and optimization, advanced anaesthetic techniques and intensive care, conventional aortic arch repairs remain amongst the highest risk of across all surgical specialties. Therefore, a hybrid approach with sequential transposition of the supra-aortic branches and endovascular stent-graft placement is a less invasive alternative treatment that opens the indication for repair to more critical patients. We report our institution's first experience in the hybrid surgery of aortic arch aneurysms.
Unlike carotid bifurcation atherosclerotic stenosis, supra-aortic trunks (SAT) occlusive disease is rare and its revascularization uncommon, accouting for less than 10% of the operations performed on the extracranial brain-irrigating arteries. There are three different treatment approaches: transthoracic, extra-anatomic cervical and endovascular. Endovascular repair is gaining popularity as first-line therapy for proximal lesions with favorable anatomy because of its low morbidity and rare mortality. Extra-anatomic bypass is a safe and durable reconstruction and should be considered in patients with single vessel disease, with cardiopulmonary high-risk or with limited life expectancy. If cardiac surgery is needed, central transthoracic reconstruction is preferable, and the two procedures should be combined. The long-term patency of bypasses with aortic origin, specially when multiple vessels are involved, is superior to other repair techniques. We present three clinical cases that illustrate each of these therapeutic strategies: central brachiocephalic revascularization and synchronous cardiac surgery in a patient with complex SAT atherosclerosis disease; subclavian-carotid transposition for disabling upper limb claudication; and subclavian artery stenting for subclavian-steal syndrome. Surgical approach selection should be based on the individual patient's anatomy and operative risk.
