The staging of Hodgkin lymphoma lays the groundwork for optimal treatment decision making. For a long time, bone marrow assessment has been an integral part of staging. The study of bone marrow involvement in tumor progression includes radiological method and morphological analysis of its core biopsy samples. During the last five decades of using bone marrow core biopsy, the attitude of oncologists and hematologists to this invasive and painful procedure remained ambivalent between denying and affirming the need to carry it out in all or most Hodgkin lymphoma cases. The present review provides the historical background of bone marrow core biopsy and considers its appropriateness for patients with classical Hodgkin lymphoma.
Second malignant tumors occurring in classical Hodgkin’s lymphoma (cHL) patients after treatment include mainly solid neoplasms and far more rarely acute myeloid leuke-mias (AML). At the same time, a relative risk of developing secondary AML substantially exceeds the risks of second (solid) tumors, and the efficacy of secondary AML treatment is considerably lower compared to the outcomes of primary AML treatment. All that implies the importance and relevance of this issue. The present literature review discusses the epidemiology of developing secondary AMLs in patents after cHL treatment. In addition to that, it focuses on modern drugs and technologies for effective treatment of secondary AMLs.
Вторые злокачественные опухоли, развивающиеся у больных классической лимфомой Ходжкина (кЛХ) после лечения, представлены преимущественно солидными новообразованиями и в значительно меньшей степени острыми миелоидными лейкозами (ОМЛ). Вместе с тем относительный риск развития вторичного ОМЛ существенно превышает риск развития вторых (солидных) опухолей, а эффективность лечения больных вторичным ОМЛ значительно уступает результатам лечения первичного ОМЛ, что делает проблему значимой и актуальной. Настоящий обзор литературы посвящен эпидемиологии развития вторичных ОМЛ у больных, получавших лечение по поводу кЛХ. Кроме того, уделяется внимание современным лекарственным препаратам и технологиям, эффективным в отношении вторичных ОМЛ.
Эпидемия COVID-19, разразившаяся в Ухане (Китай), быстро переросла в пандемию. Высокая смертность от этого заболевания ставит его в ряд наиболее опасных вирусных инфекционных болезней настоящего времени. В то время как наибольшему риску летального исхода подвержены лица пожилого возраста, некоторые сопутствующие заболевания, среди которых вполне могут быть и злокачественные опухоли, также существенно ухудшают течение COVID-19. В силу присущего иммунодефицита, усугубляемого иммуносупрессивной противоопухолевой терапией, ведущее место по влиянию на течение COVID-19 занимают онкогематологические заболевания. В обзоре представлены немногочисленные опубликованные сведения о влиянии коронавирусной инфекции на прогноз у пациентов с опухолями кроветворной и лимфоидной тканей. Кроме того, обсуждаются возможные меры по снижению у этих больных риска летального исхода.
After initially appearing in Wuhan (China), the COVID-19 epidemic rapidly escalated to pandemic level. Due to its high mortality COVID-19 belongs to the group of the most dangerous viral infectious diseases of today. While elderly people are at greatest risk of death, some comorbidities, including also malignant tumors, considerably worsen the course of COVID-19. In view of inherent immunodeficiency exacerbated by immunosuppressive chemotherapy, oncohematological diseases most greatly affect the course of COVID-19. The review presents few published data on coronavirus disease affecting the prognosis of hematopoietic and lymphoid tumors. In addition, the control of mortality risk in these patients is discussed.
