Introduction: The present study aims to investigate whether values of venous blood gas sampling can be used instead of arterial blood gas values in the evaluation of blood gas for the emergency service patients with acute exacerbation of chronic obstructive pulmonary disease (COPD).Methods: Patients diagnosed COPD and identified to have acute exacerbation of COPD based on acute exacerbation of COPD criteria participated in the study. Data from arterial and venous samples were compared using Spearman and Pearson correlation and Bland-Altman analysis.Results: Ninety patients were included in this study. The results indicated statistically significant correlations between venous blood gas pH, pO(2), pCO(2) and HCO3 values and arterial pH, pO(2), pCO(2) and HCO3 values of the patients. The correlation for pH, pCO(2) and HCO3 values were found to be significantly important (p < 0.0001).Discussion and conclusion: The findings of the study suggest that some formulations can be used to estimate pH and pCO(2) values when evaluating the lung functions of the emergency service patients with acute exacerbation of COPD.

Ilac tedavisinin sadece semptomlari azalttigi ve vestibuler testlerin ise tedavinin temeli oldugu bilinmektedir. Bu derlemede periferik ve santral vertigo ayirici tanisi yapilarak, basit manevra onerileri ile acilden taburcu edilebilecek hastalarla daha ileri tetkik ve tedavi gerektirecek hastalara genel yaklasimin aktarilmasi amaclanmistir.
Meyan kökü glycyrrhizic acid içeren Glycyrrhiza glabra"nın köküdür. Aşırı meyan kökü alımı sodyum ve su retansiyonu, hipokalemi, hipertansiyon, metabolik alkaloz, düşük renin aktivitesi ve hipoaldosteronizm ile karekterize hipermineralokortikoidizm benzeri sendroma yol açabilir. Bu yazıda aşırı meyan kökü şerbeti içimi sonrası acil servise solunum sıkıntısı, kol ve bacaklarında güçsüzlük nedeniyle getirilen hipopotasemik paralizili 34 yaşındaki erkek hasta sunuldu. Acil Servise paralizi ve solunum sıkıntısıyla gelen hastalarda aşırı meyan kökü alımı olup olmadığının sorgulanması gerekliliğine dikkat çekilmesi amaçlandı. Kan potasyum düzeyi 1.4 mmol/L olan hastanın potasyum replasmanı sonrası solunum sıkıntısı ve kas kuvvet kaybı tamamen düzeldi.
Licorice is the root of Glycyrrhiza glabra, which has a herbal ingredient, glycyrrhizic acid. Excessive intake of licorice may cause a hypermineralocorticoidism-like syndrome characterized by sodium and water retention, hypokalemia, hypertension, metabolic alkalosis, low-renin activity, and hypoaldosteronism. In this paper, an 34 years old man who admitted to the emergency department with respiratory failure and marked muscle weakness of all extremities that progressed to paralysis after excessive intake of licorice syrup was presented. It was aimed to draw attention to the necessity of questioning whether there is excessive intake of licorice or not in patients who admitted to emergency department with paralysis and dyspnea. Plasma potassium concentration of the patient was 1.4 mmol/L. The patient's respiratory distress and loss of muscle strength recovered completely after potassium replacement.
Aims: The purpose of the study was to establish whether the patients presenting with abdominal pain should undergo to contrast enhanced CT (Computed tomography). For this purpose, we have investigated the possible relationship between CRP (C-reactive protein),WBC (White blood cell), neutrophil levels and the presence of pathological findings determined on abdominal pelvic CT images.Materials and methods: Patients who were admitted to Emergency Department between 01.01.2012 - 31.12.2012 with abdominal pain and underwent intravenous contrast-enhanced abdominal-pelvic CT scan were enrolled in this retrospective study.Results: Totally 130 patients were included in the study. A statistically significant correlation was found between the presence of pathological findings determined on abdominal-pelvic CT images and CRP, WBC and neutrophil levels. Pathological findings were found to be positive on CT images of patients with CRP > 0.8 mg / dl (78.33% sensitivity and 98% specificity), WBC > 12.91 x1031uL (sensitivity 48.3%, specificity 86%), neutrophils > 8.78 x1031uL (sensitivity 56.67%, specificity 86%).Conclusion: Abdominal-pelvic CT is a very useful diagnostic tool for determining the differential diagnosis of abdominal pain. CRP, WBC and neutrophil levels could be used for the determination of the need to contrast enhanced CT in patients with abdominal pain.
Introduction: Tricyclic antideprressants have been commonly used for he treatment of depression, neuropathic pain, migren for a long time. The most toxic effects of tricyclic antideprressants are on cardiovascular system, otonomic nervous system and central nervous sytem.Case Report: Forty five years old female patient admitted to the emergency department with the complaints of altered mental status and agitation. Her glasgow coma scale was 11 (E2, M5, V4) and there was supraventricular tachycardia in her electrocardiography and corrected QT was 0.54 sec. Since there was nothing in the diagnosis of coma, poisoning thought and sodium bicarbonate treatment was begun and after treatment the rhythm was sinus tachycardia. After the history was detailed, it was learned that she was using amytriptillin 10 mg/ day because of major depression and administred plentous amount of this drug in order to suicide when her conscious was opened. On the third day of hospitalisation her vital signs were stable and she was discharged.Conclusion: Poisoning should be kept in mind in the differantial diagnosis of coma. Treatment of coma, electrocardiographic changes, hypotension, and seizures secondary to tricyclic antideprressants poisoning should be well known by emergency physicians and should not lost time for treatment.
