Background. - The implementation of efficient health and vigilance strategy is one of the essential aspects of the health policy of public and private health establishments, in order to reduce the risk of incidents due to medical devices. Aim. - The objective of this study is to demonstrate the importance of user notification and to recognize the role of nursing staff in the materiovigilance process. Methods. - This is a retrospective study of materiovigilance cases notified for three years (2016, 2017, and 2018) at the National Institute of Oncology. The evaluation of the incidents was carried out with the aim of taking the necessary measures to prevent and minimize risk. Results. - Seven thousand three hundred and eight cases of materiovigilance during the 3 years were collected. A spontaneous collection of reports was predominant (70%). The distribution of the number of incidents by the profile of notifiers shows that reports come from surgeons and much more from nurses. The surgical block (35.7%) and medical oncology services (14.3%) were the services that reported more; the majority of the observed adverse events were attributed to infusion sets (n = 7105). Four serious cases of materiovigilance. reported during these 3 years at the level of the vigilance unit considered the most relevant were detailed with examples of immediate actions taken and risk minimization actions. Conclusion. - This study highlights the characteristics of incidents reported. The rote of the nursing staff and the strong presence of nurses in the vigilance system has been well demonstrated, thus reducing the risk of side effects due to medical devices. La présente étude vise à déterminer les principaux effets indésirables survenant après l’injection du vaccin BCG contre la tuberculose chez l’enfant au Maroc. Il s’agit d’une étude rétrospective descriptive des effets indésirables du vaccin BCG chez l’enfant de moins de 4 ans, notifiés au CAPM, durant la période 2010–2012. Durant cette période, 194 déclarations ont été enregistrées, soit en moyenne 27 déclarations/an. Les effets indésirables les plus observés sont généralement : bécégites (53 %), abcès (26 %), inflammation (8 %), tuméfaction (6 %), suppuration (3 %), … Le sex-ratio (M/F) est de 1,3. Les nourrissons sont les plus touchés avec une fréquence de 91 %. Les réactions sont modérées dans 79 % des cas, sévères dans 11 % des cas et graves dans 4 % des cas. L’évolution était favorable pour 92 % des cas, pour le reste, l’évolution est non précisée. Le vaccin BCG provoque des effets indésirables surtout de nature locale, la déclaration au CAPM par les professionnels de santé est nécessaire pour détecter les signaux émergents et pour surveiller l’innocuité des vaccins.
Le présent travail consiste à dresser le profil épidémiologique du nécrolyse épidermique déclaré au Centre antipoison et de pharmacovigilance du Maroc (CAPM). Il s’agit d’une étude rétrospective descriptive des cas spontanés du (NE), notifiés au CAPM entre 2004 et 2013. Un total de 37 cas du (NE) a été déclaré au CAPM, dont 84 % des cas sont des cas graves. L’âge moyen des patients était de 47 ± 2,6 ans. Le sex-ratio (F/M) est de 4 (χ2 = 52 ; p < 0,001). Les médicaments les plus incriminés sont généralement : l’allopurinol (19 %), l’acide acétylsalicylique (16 %), carbamazipine et l’amoxicilline sodium/clavulanate potassium dans 8 % pour chacun d’eux, le phénobarbital (5 %), … La relation de cause à effet selon la méthode de l’imputabilité de l’OMS est probable dans 51 % des couples, possible dans 38 % des couples, certaine dans 8 % des couples et inconnue dans 3 %. L’évolution est favorable dans 51 % des cas, 8 % ont guéri avec des séquelles, fatale dans 22 % des cas, alors que cette évolution est restée indéterminée dans 19 % des cas. La nécrolyse épidermique est l’urgence dermatologique la plus grave, qui impose une déclaration obligatoire à la pharmacovigilance. Néanmoins une sensibilisation de la population sur les risques liés à l’automédication et des médecins aux risques de la prescription médicamenteuse irréfléchie reste le meilleur moyen de prévention.
Les effets indésirables médicamenteux (EIM) restent un problème de santé publique, en termes de nombre d’hospitalisations et de décès. Notre étude consiste à évaluer la qualité de rédaction des EIM déclarés par les professionnels de santé de l’Hôpital militaire d’instruction Mohammed V de Rabat lors de la journée nationale de notification en pharmacovigilance, ainsi que leur pertinence globale. Une étude prospective observationnelle a été instaurée le 23 mai 2019 lors de la journée nationale de notification en pharmacovigilance, Dans le cadre de la collaboration et d’échanges d’informations avec le centre antipoison et de pharmacovigilance du Maroc, concernant les déclarations d’EIM notifiés par les professionnels de santé de l’Hôpital militaire d’instruction Mohammed V. L’ampleur dans laquelle les informations cliniques pertinentes ont été rapportées a été évaluée, à l’aide d’un outil de documentation clinique (ClinDoc) Au cours de la période d’étude, 103 fiches de notification d’EI rédigées par les professionnels de santé de l’Hôpital militaire d’instruction Mohammed V correspondant à 103 patients ont été analysées. On note une prédominance masculine (77,67 %) et un sex-ratio égal à 3,47, l’âge moyen est de 49,27 ans avec des extrêmes allant de 7 à 72 ans. Il y avait 125 médicaments incriminés pour 103 déclarations. Les principales classes médicamenteuses en cause selon la classification ATC sont les anti-infectieux, les médicaments des voies digestives et du métabolisme, les médicaments du système nerveux central, et cardiovasculaire. Dans notre étude, les EIM les plus fréquents sont les affections gastro-intestinales, les affections respiratoires et les affections du métabolisme et de la nutrition. Un tiers des EIM dans notre étude étaient évitables. Dans notre étude, 82,52 % des fiches de notifications étaient classées comme « bien documentée », 16,50 % comme « légèrement documentée » et 0,97 % comme « mal documentée ». L’organisation des journées nationales de notification en pharmacovigilance est indispensable permettant d’encourager les professionnels de santé à déclarer davantage les effets indésirables médicamenteux. As part of collaboration and exchange of information with the Poison and Pharmacovigilance Centre of Morocco, concerning ADR declarations notified by health professionals at the Mohammed V Military Training Hospital. The extent to which the relevant clinical information was reported was assessed, using a clinical documentation tool (ClinDoc). During the study period, 103 IE notification forms drawn up by health professionals at the Mohammed V Military Training Hospital corresponding to 103 patients were analyzed. There is a male predominance (77.67%) and a sex ratio equal to 3.47, the average age is 49.27 years with extremes ranging from 7 to 72 years. There were 125 drugs incriminated for 103 reports. The main drug classes involved according to the ATC classification are anti-infectives, digestive tract and metabolism drugs, central nervous system, and cardiovascular drugs. In our study, the most common ADRs are gastrointestinal conditions, respiratory conditions, and metabolic and nutritional conditions. A third of the ADRs in our study were preventable. In our study, 82.52% of the notices were classified as “well documented”, 16.50% as “slightly documented” and 0.97% as “Poorly documented”. The organization of national pharmacovigilance notification days is essential to encourage healthcare professionals to report more adverse drug reactions.
Background Our national pharmacovigilance system is based on the spontaneous reporting of adverse drug reactions (ADRs) which requires successful participation of health professionals and pharmacists, in particular their specialisation in medicines and their proximity and availability for the patient. Purpose To study the knowledge, and the perception of the pharmacists of the largest city of our country in terms of pharmacovigilance. Material and methods This was a descriptive study conducted in the form of a survey of pharmacists practising in 153 pharmacies in the economic capital of the country, chosen at random, through an anonymous self-administered questionnaire of 19 questions organised around three items, over a period of 4 months from September to December 2017. Results One-hundred and thirty pharmacists (85%) responded, of whom 40% had experience of less than 10 years. Regarding their pharmacovigilance knowledge, n=108 (83.1%) confirmed that they were aware of the existence of a national pharmacovigilance organisation in our country. Among pharmacists surveyed, 1.7% could not give a definition of pharmacovigilance, while 67.8% defined it as the activity of identifying, assessing and preventing ADRs resulting from the use of drugs. As for their opinion on the ADRs to be reported, the exceptional or unexpected ADRs were the most chosen by respondents with 25.9%. Sixty-four per cent of pharmacists confirmed that they had already been asked about ADRs in patients. But only 10.7% of these reports were sent to competent authorities. Among the proposed answers concerning the under-reporting, the ignorance of the reporting circuit remains the most chosen cause, with a rate of 44.2%. Finally, a more simplified statement was the way to improve the number of statements most cited, with a rate of 32.7%. The other means proposed, with a rate of 1.2%, were continuing education and awareness-raising through the media. Conclusion This study showed a moderate level of knowledge and a low perception of pharmacovigilance. There is therefore a real interest in sensitising the teams of pharmacists so that they can play their role in the spontaneous reporting of adverse effects.
