Background/Objectives: The prognostic value of baseline clinical parameters in predicting the survival prolonging effect of Radium-223-dichloride (223RaCl2) for metastatic castration resistant prostate cancer (mCRPC) patients has been the object of intensive research and remains an open issue. This national multicenter study aimed to corroborate the evidence of ten years of clinical experience with 223RaCl2 by collecting data from eight Italian Nuclear Medicine Units. Methods: Data from 581 consecutive mCRPC patients treated with 223RaCl2 were retrospectively analyzed. Several baseline variables relevant to the overall survival (OS) analysis were considered, including age, previous radical prostatectomy/radiotherapy, number of previous treatment lines, prior chemotherapy, Gleason score, presence of lymphoadenopaties, number of bone metastases, concomitant use of bisphosphonates/Denosumab, Eastern Cooperative Oncology Group Performance Status (ECOG-PS), as well as baseline values of hemoglobin (Hb), platelets, Total Alkaline Phosphatase (tALP), Lactate Dehydrogenase (LDH), and Prostate-Specific Antigen (PSA). Data were summarized using descriptive statistics, univariate analysis and multivariate analysis with the Cox model. Results: The median OS time was 14 months (95%CI 12-17 months). At univariate analysis age, the number of previous treatment lines, number of bone metastases, ECOG-PS, presence of lymphadenopathies at the time of enrollment, as well as baseline tALP, PSA, and Hb, were independently associated with OS. After multivariate analysis, the number of previous treatment lines (HR = 1.1670, CI = 1.0095-1.3491, p = 0.0368), the prior chemotherapy (HR = 0.6461, CI = 0.4372-0.9549, p = 0.0284), the presence of lymphadenopathies (HR = 1.5083, CI = 1.1210-2.0296, p = 0.0066), the number of bone metastases (HR = 0.6990, CI = 0.5416-0.9020, p = 0.0059), ECOG-PS (HR = 1.3551, CI = 1.1238-1.6339, p = 0.0015), and baseline values of tALP (HR = 1.0008, CI = 1.0003-1.0013, p = 0.0016) and PSA (HR = 1.0004, CI = 1.0002-1.0006, p = 0.0005) remained statistically significant. Conclusions: In the era of precision medicine and in the landscape of novel therapies for mCRPC, the prognostic stratification of patients undergoing 223RaCl2 has a fundamental role for clinical decision-making, ranging from treatment choice to optimal sequencing and potential associations. This large Italian multicenter study corroborated the prognostic value of several variables, emerging from ten years of clinical experience with 223RaCl2.
Purpose whole body scan (WBS) performed following diagnostic or therapeutic administration of I-131 is useful in patients with differentiated thyroid carcinoma. However, it can be falsely positive in various circumstances. We aimed to report a series of pitfalls in a clinical perspective. Methods A search in the database PubMed utilizing the following terms: “false radioiodine uptake” and “false positive iodine 131 scan” has been made in January 2023. Among the 346 studies screened, 230 were included in this review, with a total of 370 cases collected. Physiological uptakes were excluded. For each patient, sex, age, dose of I-131 administered, region and specific organ of uptake and cause of false uptake were evaluated. Results 370 cases of false radioiodine uptake were reported, 19.1% in the head-neck region, 34.2% in the chest, 14.8% in the abdomen, 20.8% in the pelvis, and 11.1% in the soft tissues and skeletal system. The origin of false radioiodine uptake was referred to non-tumoral diseases in 205/370 cases (55.1%), benign tumors in 108/370 cases (29.5%), malignant tumors in 25/370 cases (6.7%), and other causes in 32/370 cases (8.7%). Conclusions WBS is useful in the follow-up of patients with differentiated thyroid carcinoma, however it can be falsely positive in various circumstances. For this reason, it is critically important to correlate the scintigraphic result with patient’s medical history, serum thyroglobulin levels, additional imaging studies and cytologic and/or histologic result.
Targeted radionuclide therapy, as described by Goldsmith using Ehrlich's words, is a magic bullet capable of delivering energy to a specific target to destroy it (1). Many new radiotracers have been rising, useful both for diagnosis and for therapy. Patients undergoing radiopharmaceutical therapy (RPT) become a radioactive source, requiring safety protocols for caregivers and the public. In Italy, patients may be hospitalized until the radioactive dose decays to safe levels such as in the long-practiced therapy with 131I (2). Similar precautions are often adopted for emerging radionuclides, resulting in the application of the same habits as for 131I. Instead, each case should be individually assessed to keep radiation exposure as low as reasonably achievable, balancing cost-benefit considerations. That became a necessity for [177Lu]LuPSMA-RPT, for which discharge regulations differ: some countries allow outpatient administration, whereas others require hospitalization (3). This heterogeneity impacts the worldwide diffusion of this promising cancer treatment. This editorial assesses the feasibility of [177Lu]Lu-PSMA administration as an outpatient procedure for prostate cancer, analyzing safety aspects, advantages, and disadvantages.

We are recently faced with a progressive evolution of the therapeutic paradigm for radioiodine refractory differentiated thyroid cancer (RAI-R DTC), since the advent of tissue agnostic inhibitors. Thus, tumor genotype assessment is always more relevant and is playing a crucial role into clinical practice. We report the case of an elderly patient with advanced papillary thyroid carcinoma (PTC) harboring RET-CCDC6 fusion with four co-occurring mutations involving PI3KCA, TP53, and hTERT mutations, treated with pralsetinib under a compassionate use program. Despite the high histological grade and the coexistence of aggressive RET co-mutations, an impressive metabolic and structural tumor response has been obtained, together with a patient's prolonged clinical benefit. A timely comprehensive molecular testing of those cases wild-type for the common thyroid carcinoma BRAF V600E-like and RAS-like driver mutations may uncover actionable gene rearrangements that can be targeted by highly selective inhibitors with great potential benefit for the patients.
