В статье проанализирована эффективность барьерных методов терапии при аллергическом рините. Рассмотрены особенности действия микродисперсного порошка целлюлозы Назаваль® – назального спрея для местного применения в составе комплексной лекарственной терапии аллергического ринита. На основании данных ряда зарубежных и российских исследований показано, что Назаваль® уменьшает клинические симптомы сезонного и круглогодичного АР, эффективен и безопасен, а также способствует улучшению качества жизни пациентов. The article examines the effectiveness of barrier methods in the treatment of allergic rhinitis. The effects of Nasaval® – a nasal spray for topical application which contains microdispersed cellulose powder as part of a complex treatment for allergic rhinitis are being discussed. According to the data from numerous foreign and Russian studies Nasaval® reduces the clinical symptoms of seasonal and perennial allergic rhinitis, it is effective and safe and improves the quality of life of patients.
Сезонный аллергический ринит (САР) – одно из самых распространенных аллергических заболеваний человека, особенно в экономически развитых странах, включая Россию. В Москве и Московском регионе среди пыльцевых аллергенов доминирующим является пыльца деревьев (51,7%) [1]. Цель исследования – изучить эффективность предсезонного курса препарата Рузам в достижении контроля САР у пациентов с и без бронхиальной астмы в рутинной практике врача в сравнении с группой пациентов, получающих симптоматическую терапию САР. Материал и методы. В проспективное открытое контролируемое исследование было включено 40 взрослых пациентов с САР. Участники исследования были распределены в две группы: 30 пациентов (основная группа), получавших предсезонно-сезонный курс препарата Рузам, и 10 пациентов (группа сравнения), которым предсезонная терапия не проводилась. В сезон цветения деревьев пациенты обеих групп получали стандартную симптоматическую фармакотерапию поллиноза. У большей части пациентов, включенных в исследование, подтверждена поливалентная аллергия: в группе исследования – у 89,3%, в группе контроля – у 77,8%. Дизайн исследования предусматривал три визита. Первый визит проходил за 1–1,5 месяца до начала сезона цветения деревьев, когда пациенты группы исследования начали профилактический курс инъекций препарата Рузам в дозе 0,2 мл подкожно с интервалами 5–7 дней, всего 10 инъекций. Второй визит соответствовал периоду высокого уровня пыления причинно-значимого аллергена, согласно данным мониторинга пыльцы, третий визит – окончанию пыления причинно-значимого аллергена, согласно мониторингу пыления. Все пациенты оценивали назальные и неназальные симптомы поллиноза по опросникам TNSS/TNNSS (Total Nasal/Non Nasal Symptom Score), визуальной аналоговой шкале (ВАШ), шкале оценки назальных симптомов с учетом потребности в медикаментах (дневник пациента с поллинозом), качество жизни по опроснику RQLQ (Rinoconjunctivitis Quality of Life Questionnaire), удовлетворенность терапией по шкале глобальной удовлетворенности. Результаты. Оценка тяжести назальных симптомов аллергического ринита по шкале TNSS в группе препарата Рузам в пиковый период пыления деревьев составила 3,59 балла, после окончания палинации – 1,54 балла, выраженность назальных симптомов снизилась на 57,2% (р < 0,001). Оценка неназальных симптомов по шкале TNNSS в группе препарата Рузам в пик пыления деревьев составила 2,41 балла, на момент окончания палинации – 0,5 балла, выраженность неназальных симптомов снизилась на 79,3% (р < 0,001). В контрольной группе достоверного уменьшения назальных и неназальных симптомов к концу пыления не зафиксировано (показатели TNSS составили 3,44 и 3,17 балла, показатели TNNSS – 3,06 и 2,89 балла соответственно). Средний балл ВАШ в обеих группах свидетельствовал о контролируемом течении САР на фоне проводимой терапии, однако лекарственная нагрузка для контроля САР была достоверно меньше в группе пациентов, получивших курс препарата Рузам. Скорректированный балл назальных симптомов, который учитывает интенсивность симптомов АР и максимальную потребность в лекарственной терапии (максимальный фармакологический класс терапии) САР за последние 7 дней до назначенного визита, в группе препарата Рузам на втором визите составил 4,64 балла, в группе контроля – 6,29 балла (р = 0,008), на третьем визите в группе исследования – 0,97 балла, в группе контроля – 4,78 балла (р < 0,001). В динамике от пика цветения к концу сезона пыления скорректированный балл назальных симптомов в группе исследования уменьшился на 79% (р < 0,001), в контрольной группе снижение составило 24% (р = 0,085). Качество жизни пациентов, согласно анализу опросника RQLQ, на момент окончания пыления деревьев в группе исследования соответствовало 6,32 балла, в группе контроля – 11,11 балла (р = 0,003). Согласно оценке по шкале глобальной удовлетворенности терапией, 64,3% пациентов, получивших курс препарата Рузам, отметили более высокую удовлетворенность терапией в сравнении с предыдущим сезоном пыления, в группе контроля изменений не произошло. Уровень общего IgE в сыворотке крови в основной группе практически не изменился (отмечено недостоверное снижение на 7,1%; p = 0,700), в группе контроля зарегистрировано его достоверное повышение на 123,7% (р = 0,007). Заключение. В группе препарата Рузам интенсивность назальных и неназальных симптомов САР в пик пыления деревьев и на момент окончания палинации была достоверно ниже, чем в группе сравнения, был меньше объем медикаментозной терапии, использованной в сезон цветения, и ниже класс противоаллергических препаратов. Между группами достигнута достоверная разница по скорректированному баллу назальных симптомов с учетом потребности в медикаментах и по среднему суммарному баллу потребности в медикаментах. В группе контроля к концу пыления сохранялась высокая лекарственная нагрузка (достоверного снижения не получено). Клиническая эффективность препарата Рузам проявляется уже после 6 инъекций, однако максимальный клинический эффект достигается после полного курса терапии из 10 инъекций, что позволяет рекомендовать начало курса терапии за 2–2,5 месяца до сезона пыления. Seasonal allergic rhinitis (SAR), or hay fever, is one of the most common allergic diseases, especially in economically developed countries including Russia. In the Moscow region tree pollen prevails among pollen allergens (51.7%) [1]. Aim of the study. To analyze the effectiveness of a pre-season course of Ruzam in routine clinical practice in obtaining control over SAR symptoms in patients with or without asthma compared to patients receiving only symptomatic treatment for SAR. Material and methods. This was a prospective, non-interventional, controlled study of 40 adult patients with SAR. Patients were divided into two groups: 30 patients (study group) received a pre-season and season course of Ruzam and 10 patients (comparison group) who did not receive it. The majority of participants had confirmed polyvalent allergy: 89.3% in study group, 77.8% in control group. The study design implied 3 visits. At the first visit (1–1.5 month before the flowering season onset), patients in the study group started a pre-season course of Ruzam – 0.2 ml subcutaneously each 5–7 days, 10 injections totally. The second visit was scheduled during the period of high concentrations of the causative allergen, according to the pollen monitoring data; the third visit was set after the end of the causative allergen dispersion period, according to the pollen monitoring data. Symptoms in all patients with hay fever were assessed using the Total Nasal Symptom Score and Total Non-nasal Symptom Score (TNSS/TNNSS), the Visual Analogue Scale (VAS), the Combined Symptom and Medication Score (CSMS), the Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ), satisfaction with therapy using the Global Satisfaction Scale of the treatment. Results. The severity of nasal symptoms according to the TNSS in Ruzam group was 3.59 points at the peak of pollen dispersion and 1.54 points at the end of pollen dispersion period, thus the severity of symptoms decreased by 57.2% (p < 0.001). The severity of non-nasal symptoms according to the TNNSS in Ruzam group was 2.41 points at the peak of pollen dispersion and 0.5 points at the end of the flowering season, thus the severity of symptoms decreased by 79.3% (p < 0.001). In the control group no significant decrease in nasal or non-nasal symptoms was recorded by the end of pollination period (3.44 and 3.17 points according to TNSS scale; 3.06 and 2.89 points according to TNNSS scale). The average VAS score indicates control over SAR symptoms during pollination period in both groups due to the pharmacotherapy, however, the need for medication in Ruzam group was reliably lower. The adjusted TNSS score which evaluates the intensity of SAR symptoms and the maximum need for medication (maximum pharmacological class) in the last 7 days before the patient’s visit. The adjusted TNSS score at the second visit in Ruzam group was 4.46, in control group 6.29 (p = 0.008); at the third visit in Ruzam group 0.97 and 4.78 in contol group (p < 0.0001). Overall, the adjusted TNSS score decreased from the peak to the end of flowering season by 79% (p < 0.001) in Ruzam group and only by 24% in control group (p = 0.085). Serum IgE levels did not decrease significantly, only by 7.1% (p = 0.700); in control group we observed a significant increase in serum IgE levels by 123.7% (p = 0.007). Conclusions. In Ruzam group a significant reduction in SAR symptoms as well as in the drug load during the flowering season was observed compared to the control group. Moreover, patients in the study group used fewer medications and lower classes of antiallergic drugs during the pollination period. There was a significant difference between the groups according to the Adjusted TNSS taking into account the need for medications and in the Average Total Score for the need for medications. In the control group a high drug load remained (no significant reduction was obtained) by the end of the dusting season. The clinical effectiveness of Ruzam appears after 6 injections already, however, the maximum clinical effect is achieved after a full course of therapy including 10 injections, which allows for recommendations of starting Ruzam course 2–2.5 months before onset of pollination.
Allergic rhinitis (AR) is the most common form of rhinitis and the most common allergic disease in children and adults. Numerous clinical studies and daily practice indicate a pronounced negative effect of symptoms of AR on patients’ quality of life, their usual daily activity, cognitive functions, mood and sleep. The type and severity of individual symptoms can vary from patient to patient, and therapy should be directed to the overall control of the symptoms of AR. It is important to obtain reliable and comparable information regarding the severity of AR symptoms before, during and after the treatment, which can be assessed using a simple and accessible tool - a visual analogue scale (VAS). VAS validated for patients with AR allows to define a controlled, partially controlled and uncontrolled AR. The article proposes an algorithm for the treatment of AR, depending on the severity of the symptoms, assessed using VAS.
The article is devoted to the actual problem of clinical allergy – diagnosis and treatment of allergic rhinitis (AR). The article highlights the issues of prevalence of AR, discusses classification, etiology, pathogenesis of the disease. Detailed discussion of approaches to its diagnosis and treatment, depending on the severity of AP and the presence of comorbidities. The mechanism of action of the drug desloratadine – non-sedating, long-acting antihistamine that has high affinity for H1-receptors. On the basis of a number of studies have shown that desloratadine is highly effective in relieving the symptoms of allergic rhinitis, is well tolerated and improves the quality of life of patients.
The aim of this study was to investigate efficacy and safety of inhalational treatment with Ruzam in 63 adult patients with persistent mild to moderate atopic bronchial asthma (BA). In this placebo controlled trial, clinical, functional and laboratory parameters were assessed at the end of 2-week treatment with Ruzam and in 6 months after the treatment. All patients also received inhaled corticosteroids. In patients treated with Ruzam, asthma symptoms, bronchial hyperresponsiveness and nitric oxide in the exhaled air decreased and morning PEF increased when compared to patients treated with inhaled corticosteroids plus placebo. During 6 months after the treatment, there were no asthma exacerbations in the Ruzam group and asthma was well-controlled in these patients. The patients treated with Ruzam significantly decreased a frequency of acute respiratory infections. In the control group ( n = 32), exacerbation frequency and asthma control level remained unchanged compared to baseline. Achieving clinical control of asthma in Ruzam group was accompanied by a significant reduction in concentrations of inflammatory markers (IFN-γ, ECP) in serum and in the induced sputum that could be used as a measure of systemic anti-inflammatory effect of Ruzam.
