A comprehensive clinical and laboratory examination of 50 pregnant women with premature rupture of membranes at 22-34 weeks of gestation who were treated at the Department of Pathology of pregnancy of Perinatal Center in Saratov region. The level of proinflammatory cytokines (IL-6, IL-8, TNFα) was increased in the blood and the amniotic fluid, the level of proinflammatory cytokines correlated with systemic activation of lipid peroxidation. Monitoring of the blood levels of pro-inflammatory cytokines (IL-6, IL-8, TNFα) and intermediate products of lipid peroxidation (diene conjugates and malondialdehyde) can be used to predict the threat of prenatal rupture of membranes, followed by the onset of preterm labor.
Studied dynamics of 34 pregnancies and development of newborns at a placenta inflammation. Necroses, heart attacks, infiltrates and anchor in a placenta are characteristic for prematurely born, in the low-spring, resuscitation group of newborns with the waterless period of 8-10 days. Childbirth to the term of 32 weeks – a consequence of the broken invasion of a placenta with traces of a long inflammation because of a number of factors. Active tactics is offered.
The analysis of lipid peroxidation in 64 pregnant women with premature rupture of membranes on gestation period 22 34 weeks, were treated in the Perinatal center of Saratov region in 2012-2014. The objectives of the study included a comparative assessment of the content in the blood of pregnant women surveyed lipid peroxidation products diene conjugates and malondialdehyde, quantitative determination of peroxides as well as assessment of the overall antioxidant status, superoxide dismutase and ceruloplasmin content. Identified pathogenetic relationship between the activation of lipid peroxidation, which is manifested by an excessive accumulation of blood and amniotic fluid diene conjugates and malondialdehyde on a background of relative insufficiency of the antioxidant system, and the development of degenerative processes in the amniotic and chorionic membranes of the fetus in terms of prolongation of pregnancy in patients with premature rupture of membranes. The possibility of using indicators of lipid peroxidation in the blood as objective criteria for assessing the degree of damage to the amniotic and chorionic membranes of the fetus in patients with premature rupture of membranes is exist.
Проведен анализ цитокинового профиля крови у 50 беременных с преждевременным излитием околоплодных вод в сроки, соответствующие 22–34 недели гестации, находившихся на стационарном обследовании и лечении в отделении патологии беременности Перинатального центра Саратовской области в 2012 году. Определение содержания провоспалительных цитокинов (IL-1β, IL-6, IL-8, TNFα) в крови беременных проведено с использованием метода твердофазного иммуноферментного анализа с помощью тест-систем (производство ЗАО «Вектор–Бест», г. Новосибирск, Россия). При клинико-лабораторном обследовании выявлено резкое увеличение содержания в крови обследуемого контингента беременных провоспалительных цитокинов (IL-1β, IL-6, IL-8, TNFα), что является прогностически неблагоприятным признаком возможного дородового излития околоплодных вод с последующим наступлением преждевременных родов.
A comparative evaluation of anti-radical protection mechanisms was done in the "mother-placenta-fetus" system of the pregnant women with prenatal rupture of amniotic membranes at the gestation period of 22–34 weeks, who were examined and treated in the department of pregnancy pathology in the Perinatal Center of Saratov region. A cross-sectional increase in levels of total antioxidant status (TAS), ceruloplasmin content, and superoxide dismutase activity in the amniotic fluid and in the blood of pregnant women with specified complication of gestation were established. The findings suggest, on the one hand, the development of adaptation reactions against excessive production of free radicals, and on the other hand, the fact that monitoring the indicators of antioxidant system of blood and amniotic fluid may have prognostic value in case of a threat of preterm labor.
Purpose: to establish the role of activation of lipid peroxidation and antioxidant deficiency of blood and amniotic fluid in the destabilization of membranes with early prenatal discharge of amniotic fluid. Material and methods. The analysis of lipid peroxidation products content in blood and amniotic fluid — diene conjugates (DC), malondialdehyde (MDA) and the quantitative determination of total peroxides (OxyStat), the estimation of the total antioxidant status (TAS), the activity of SOD and the content of ceruloplasmin in 20 pregnant with premature rupture of membranes in the gestation period of 22-34 weeks, were hospitalized at the Department of Pathology of Pregnancy of Saratov Perinatal Center in 2012. DC and MDA determination was carried out by conventional spectrophotometric methods of research, quantitative determination of peroxides (OxyStat) was carried out by enzyme immunoassay analyzer «Alfa Prime» («Meredith Diagnostics», 2008 using chemicals made by the company «Bender Medsystems»). The study of antioxidant status of serum and amniotic fluid was performed by enzyme immunoassay analyzer «Alfa Prime» («Meredith Diagnostics», England, 2008). Reagents («Bender Medsystems», Austria) were used to determine the total antioxidant status (TAS) and the SOD activity. To assess the content of ceruloplasmin reagent «Sentinel» (Italy) was used. Results. Indicators of lipid peroxidation in blood and amniotic fluid and anti-radical protection of biological membranes of cells and body fluids in the mother-placenta-fetus increased. Conclusion. Systemic activation of lipid peroxidation has been proved to be a pathogenetic factor in the failure of membranes in prenatal discharge of amniotic fluid. The increase in the blood levels of lipid peroxidation products on the background of the activation of antioxidant system is considered as a prognostic sign of abortion.
Проведен анализ состояния процессов липопероксидации и оксидативного статуса у 20 беременных с преждевременным излитием околоплодных вод (в сроки гестации 22–34 недели), находившихся на стационарном обследовании и лечении в отделении патологии беременности Перинатального центра Саратовской области в 2012 г. В задачи данного исследования входила сравнительная оценка содержания в крови и околоплодных водах обследованных беременных продуктов липопероксидации – диеновых коньюгатов (ДК) и малонового диальдегида (МДА), а также количественное определение перекисей (OxyStat). Выявлена закономерность одновременного возрастания суммарного показателя OxyStat крови и околоплодных вод, а также увеличение содержания в крови и околоплодных водах промежуточных продуктов липопероксидации: диеновых коньюгатов и малонового диальдегида. Последнее свидетельствует о важной роли локальной и системной активации свободнорадикальной дестабилизации биомембран в патогенезе дородового излития околоплодных вод.
The analysis of the blood cytokine profile of 50 pregnant women with preterm rupture of amniotic membranes at 22–34 weeks´ gestation who were in hospital examination and treatment in the Department of Pathology of pregnancy of the Perinatal Center of Saratov region in 2012. Determination of the cytokine level in the blood (IL-4, IL-10, IL-2) of pregnant women conducted using the enzyme linked immunosorbent assay using the test system (production of JSC Vector-Best, Novosibirsk, Russia) have been made. The clinical and laboratory examination revealed elevated level of IL-2, while reducing the content of IL-10, IL-4 in the blood of local population of pregnant women, which indicates the need for monitoring the content of these cytokines in the blood to predict complications during pregnancy.

Materials and methods: The analysis of anti-inflammatory and pro-inflammatory cytokines in the blood of 50 pregnant women with preterm rupture of membranes in gestation of 22-34 weeks, who were treated at the Department of Pathology in pregnancy of Saratov Perinatal Center in 2012, was performed. The method of enzyme linked immunosorbent assay using test systems «Vector— Best» (Novosibirsk, Russia). Results:.Clinical and laboratory examination revealed an increase levels of IL-1(3, IL-2, IL-6, IL-8, TNF-a, while reducing the content of IL-4, IL-10 in blood. Conclusion: Manifest signs of possibility of prenatal rupture of membranes and premature labour in 22-34 weeks gestation are combined with increasing of IL-1(3, IL-6, IL-8, TNF-a, IL-2 and with decreasing of IL-10, IL-4 level in the blood, which indicates the need for monitoring the content of these cytokines in the blood for the diagnosis and prognosis of pregnancy.
