Abstract This work planned to evaluate the biomechanical response of the ligamentum collaterale mediale (LCM) of the knee joint to valgus loadingwhen the posteromedial capsule (PMC) was cut and when it was intact. The contributory role of the posteromedial capsule to medial stability was tested and the results were presented.\nOn human knee joint, it was shown that the PMC has a 15 % contributory role in the stability of LCM. Consequently applied forces had produced laxity of the joint. When it was continued to apply a force after LCM had ruptured, the posterior crossing ligament also had ruptured. Most ruptures of LCM was from the point of insertion to femur.\nPosteromedial complex had been shown to be an important structure in restoration of knee instability and in reconstruction of ligament ruptures. ÖzetBu çalışma; disde Ligamentum Collaterale Mediale nin (LCM), Posteromedial Kapsül (PMK) sağlamken ve PMK kesildikten sonra valgus yüklemelerine karşı verdiği biomekanik cevaplara açıklık getirmek için. PMK nın medial stabiliteye katkısı incelenmiş ve sonuçları sunulmuştur.\nPMK nın insan dizinde; LCM nin stabilitesine %15 kadar katkısı olduğu tesbit edilmiştir. Müteaddit zorlamalarda diste laksite oluştuğu gözlenmiştir. LCM koptuktan sonra zorlamaya devam edildiğinde arka çapraz bağın da koptuğu görülmüştür. LCM deki kopmalar genellikle femura yapışma yerinden olmuştur.\nDizin instabilite ve bağ yaralanmalarının rekonstrüksiyonununda, bu posteromedial kompleksin stabiliteyi sağlamada çok önemli bir yapı olduğu sonucuna varılmıştır.
Abstract This study was done to light the biomechanical responses of the medial collateral (MCL) and the lateral collateral ligament (LCL) of the knee joint were investigated and the results obtained were presented here.\nMCL was strained primarily at the valgus. On the other hand, it was strained both in extension and flexion. In extension and flexion, when internal and external rotations was applied, a strain in MCL occurs.\nLCL was also extremely strained in varus loading. During flexion and extension movements, it was observed a strain in LCL. In the flexion andextension, when it was forced to external rotation a strain was occurred in LCL, but not in internal rotation. When anterior Drawer Test (ADT) and posterior Drawer Test (PDT) were applied MCL was strained, but LCN not. \nIt was estabilished that MCL of the knee joint plays primarily role in the valgus stability; as a result of this concept it resists to rotation Ioadings and contributes to antero-posterior stability of the knee joint.\nIt was concluded that LCL primarily resists to varus stability and to a Iesser extent to rotational loading. ÖzetBu çalışma, diz ekleminde Medial Collateral Ligament (MCL) ve Lateral Collateral Ligament (LCL) in muhtelif yönlerde yüklenmelere karşı verdiği biomekanik cevapları ortaya çıkarmak için yapılmıştır. MCL ve LCL’nin dizdeki stabiliteye iştirakleri incelenmiş ve sonuçları sunulmuştur. MCL Valgusda primer olarak gerilmiştir. Ayrıca ekstansiyonda daha fazla olmak üzere, flexionda da gerilmiştir. Flexion ve Extensionda, iç ve dış rotasyon uygulanırsa, MCL’de gerilme ortaya çıkmıştır. LCL de varus yüklemesinde aşırı derecede gerilmiştir. LCL’de Flexion ve extension hareketleri esnasında gerilme olmamıştır. Flexion ve extensionda dış rotasyona zorlandığında LCL’de gerilme olmuş, iç rotasyonlarda ise gerilme olmamıştır. Anterior Drawer Test (ADT) ve posterior Drawer Test (PDT) uygulandığında MCL gerilmiş, LCL ise gerilmemiştir. Diz ekleminde MCL’nin primer olarak valgus stabilitesinde rol oynadığı, bununla birlikte rotasyonel yüklemelere karşı koyduğu ve dizin antero-posterior stabilitesine iştirak ettiği tesbit edilmiştir. LCL ise öncelikle varus stabilitesine, az miktarda da rotasyonel zorlamaya karşı koyduğu sonucuna varılmıştır.
Abstract This study was done to investigate the biomechanical responses of (ACL) and (PCL) of knee against compulsions of valgus, varus, extension flexion, internal and external rotations. Participations of ACL and PCL on stabIity of the knee were investigated and the results obtained were presented there.\nAn extension occured on ACL when tibia was pulled in the direction of front side, whereas on PCL when tibia was pushed in the direction of back side. If was observed that ACL is stretched in internal and external rotation of the tibia during flexion and extension, PCL is stretched only during extantion, and both ACL and PCL are strecthed during hyperextantion, which higher on PCL. Also stretchings were observed in varus on ACL, in varus-val- gus on PCL. Rupture on ACL occured on the part of tibia, where as on PCL occured on the part of femur. It was concluded that ACL on the knee prevents sliding to the front of tibia resists to compulsions of rotational and varus in the case of extension and that PCL participates the stabilities of varus and valgus, stretches highly in hyperextension, and plays a major role in the rotational stability in extension. Özet Bu çalışma, dizde ACL ve PCL’nin valgus, varus, extansion, flexion, iç ve dış rotasyon zorlamalarına karşı verdiği biomekanik cevapları aydınlatmak için yapılmıştır. ACL ve PCL’in dizdeki stabiliteye iştirakleri incelenmiş ve sonuçları sunulmuştur.\nACL’de tibianın öne doğru çekilmesinde, PCL’de ise tibianın arkaya doğru itilmesi esnasmda gerilme ortaya çıkmıştır. Plexion ve extansionda tibianın iç dış rotasyonlarında ACL’nin, extansionda iç ve dış rotasyonda ise PCL’nin de gerildiği, hiperextansionda PCL’de daha fazla olmak üzere, her iki bağda da gerilme olduğu müşahade edilmiştir. Ayrıca varusda ACL’de, varus-valgusda PCL’de gerilme gözlenmiştir. ACL’de kopma tibia tarafından, PCL’de ise femur tarafından olmuştur. Dizde ACL’nin tibianın öne kaymasını engellediği, Rexionextansion durumlarında rotasyonel ve dizin varus zorlamasına karşı koyduğu, PCL’nin ise varus vc valgus stabilitesine iştirak ettiği, hiperextansionda çok gerildiği ve extansionda rotasyonel stabilitede mühim rol oynadığı sonucuna varılmıştır.
Abstract In this study, after attaching strain-gauge to the femoral side of the musculus popliteus, various direction had been loaded and results were evaluated biomechanicaly. In varus loading, the M. popliteus had been strained, moreover in external rotation loading much more strain had been observed. In valgus, internal rotation and flexion loading, relaxation had been observed on muscle. Also, when the tibia was pulled backward in hyperflexion, strain had been occured on muscle. Özet Bu ça1ışmada musculus popliteusun femoral yapışma yerine strain gauge yapıştırılarak çeşitli yönlere doğru yüklenmiş ve ortaya çıkan biomekanik cevaplar araştırılmıştır. M. popliteus dizin varusa yüklenmesinde gerilmiş, bununla birlikte dış rotasyon yüklenmesinde de gerilme daha çok olmuştur. Valgus, iç rotasyon ve fleksiyonda adalede büzülme görülmüştür. Hiperfleksiyonda tibia geri doğru çekilince adalede gerilme görülmüştür. Musculus popliteus dizin varus ve dış rotasyon stabilitesinde aktif rol oynadığı, ligamentum cruciatum posteriora aktif destek verdiği ve lateral meniscusu posteriordan koruduğu sonucuna varılmıştır.
We present a new device for femoral and tibial fixation in anterior cruciate ligament reconstruction using soft tissue. We evaluated the method biomechanically in pullout tests using human cadaver knee joints. Fixation strengths ranged from 300 ± 60 to 600 ± 50 N (mean 400 ± 50 N). This corresponds to loads in the graft during aggressive rehabilitation although cadaveric specimens were used. With respect to primary fixation strength this implant is a reasonable alternative for anterior cruciate ligament graft fixation.
This study was planned as an alternative method to tibial osteotomies or knee prostheses. The semitendinosus tendon was attached on the tibial condyl like rail. As a consequence. the tibial condyl was escaped from the pressure, on the other hand the femoral condyl was moved on the tendon.
