Context: Anxiety is a common comorbidity in patients with chronic diseases. This study aims to assess the prevalence of anxiety among Moroccan patients with chronic diseases and identify the key factors associated with increased anxiety levels. Methods: A cross-sectional study was conducted at Hassan II University Hospital of Fez, among patients diagnosed with various chronic diseases. A structured questionnaire was used to collect sociodemographic, clinical, and economic data. Anxiety levels were measured using HADS questionnaire in its version Arabic validated. The socio-economic status (SES) was assessed using a composite index based on key indicators, including income, educational level, employment status, and health insurance coverage. Bivarite and multivariate (logistic regression) statistical analyses were performed to determine associations between anxiety and potential risk factors. Results: 1122 subjects were included in the study. The findings revealed a high prevalence of anxiety among patients with chronic diseases (48.9%) CI95% = (46%-52%). It varies according to the type of disease. Older age, female gender, low socioeconomic status, and duration of disease over or equal 6 years were significantly associated with higher anxiety levels. Conclusion: The study highlights the urgent need for integrated mental health care within chronic disease management.
Primary mucosal tuberculosis is an exceptionally rare extrapulmonary manifestation, representing a diagnostic challenge due to its marked anatomo-clinical polymorphism and paucity of systemic signs. We report a 62-year-old immunocompetent woman with multifocal mucosal facial plaques showing a fluctuating course. Dermoscopy demonstrated a lupoid pattern. Radiologic evaluation revealed sinonasal extension with osseous lysis. Histopathology confirmed confluent Koester follicles with central caseous necrosis. Systemic evaluation showed no pulmonary or visceral involvement, with only a positive tuberculin skin test supporting prior sensitization. Prompt recognition enabled timely antituberculous therapy, preventing further structural and functional sequelae.
Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by hypogammaglobulinemia, impaired vaccine response, and increased susceptibility to infections. Advances in molecular genetics have enabled the identification of monogenic forms in an increasing proportion of patients. In this study, a targeted genetic panel based on the 2022 International Union of Immunological Societies classification was performed in five patients presenting with a phenotype compatible with CVID. A genetic abnormality was identified in two (40%) patients, including a variant in the NFKB1 gene in one patient and variants in the CD3G and IL7R genes in another. No detectable genetic abnormalities were found in three (60%) patients. These results are consistent with the marked genetic heterogeneity of CVID and highlight the value of molecular sequencing in improving diagnostic accuracy and guiding therapeutic strategies.
Abstract Background Rhinoscleroma is a chronic granulomatous infection caused by Klebsiella rhinoscleromatis , predominantly affecting the upper respiratory tract. Tracheal involvement is uncommon and may lead to progressive airway obstruction, delayed diagnosis, and severe respiratory complications. Methods We conducted a retrospective descriptive study of patients diagnosed with tracheal rhinoscleroma at the Department of Pulmonology of Hassan II University Hospital, Fez, between 2011 and January 2026. Clinical presentation, radiological findings, bronchoscopic characteristics, histopathological features, therapeutic management, and outcomes were analyzed. Results Five patients were included, comprising three women and two men, with a median age of 49 years (range: 10–50 years). Progressive dyspnea was present in all patients; while wheezing and inspiratory stridor were observed in selected cases. Subglottic involvement was the predominant localization. Bronchoscopic findings included circumferential inflammatory stenosis, pseudomembranous lesions, and complex subglottic narrowing. Computed tomography demonstrated tracheal wall thickening and luminal narrowing in most patients. Histopathological examination demonstrated chronic granulomatous inflammatory infiltrates compatible with rhinoscleroma. Three patients required interventional bronchoscopic management, including repeated bronchoscopic dilatations in one pediatric patient and emergency tracheostomy. All patients received prolonged ciprofloxacin therapy. Clinical outcome was favorable in most cases, although one patient developed ENT recurrence during follow-up. Conclusion Tracheal rhinoscleroma remains a rare but potentially severe cause of upper airway obstruction. Diagnosis is frequently delayed because of nonspecific clinical manifestations and its resemblance to other chronic airway diseases. Histopathological confirmation remains essential. Early recognition associated with prolonged antibiotic therapy and multidisciplinary airway management may improve prognosis and reduce irreversible airway stenosis.
Idiopathic Pulmonary Fibrosis (IPF) is a severe, chronic, progressive lung disease classified within interstitial lung disorders. It predominantly affects individuals aged 50 to 70 years, with a prognosis of 3-5 years post-diagnosis. The pathophysiology of IPF is complex, involving an interplay of genetic predisposition, environmental exposures, and age-related factors. A significant genetic component is evident, with key contributions from rare variants in telomere maintenance genes (e.g., TERT and TERC) and surfactant protein genes (e.g., SFTPA and SFTPC), as well as a strong association with a common promoter variant in the MUC5B gene. The diagnosis is established through high-resolution computed tomography (HRCT) and, when necessary, histopathological analysis. The search for reliable biomarkers is a key area of research, with molecules such as KL-6, SP-A, SP-D, and MMP-7 showing potential for aiding in diagnosis, prognosis, and monitoring disease activity. While antifibrotic therapies (Pirfenidone and Nintedanib) have revolutionized management by slowing the decline in lung function, the therapeutic landscape continues to evolve. Ongoing research efforts are focused on integrating clinical, radiological, genetic, and biomarker data to facilitate early diagnosis and develop personalized treatment strategies to improve patient outcomes.
Pycnodysostosis is a rare genetic disorder associated with craniofacial anomalies that promote the occurrence of obstructive sleep apnea (OSA). We report the 15-year longitudinal follow-up of a patient with severe OSA treated with continuous positive airway pressure (CPAP). From the age of 2 years, the patient presented severe OSA confirmed by polysomnography and CPAP was initiated. Throughout follow-up, adherence was excellent (≥90%). Despite multidisciplinary follow-up and the addition of a dental prosthesis aimed at targeted expansion of the oral cavity, persistent maxillo-mandibular retrognathia was still present at age 17 years along with OSA persistence. This case highlights the importance of early and periodic follow-up and evaluation of alternative therapeutic approaches to improve upper airway patency in children with craniofacial syndromes such as pycnodysostosis.
Introduction Le cancer bronchique primitif représente plus de 20 % des cancers chez l’homme, son incidence est en nette augmentation chez le sujet jeune, touche toutes les origines ethniques et de mauvais pronostic et de diagnostic souvent tardif. À côté du rôle déterminant du tabagisme, d’autres facteurs sont incriminés : environnementaux, génétiques et hormonaux. Notre étude a comme objectif d’identifier les particularités radiocliniques et histologiques du cancer du poumon chez les patients jeunes moins de 50 ans dans la région Fès-Meknès. Méthodes Il s’agit d’une étude rétrospective descriptive et analytique menée au sein du service de pneumologie CHU Hassan II Fès, incluant les patients âgés de 50 ans et moins porteurs d’un cancer bronchopulmonaire confirmé histologiquement entre 1er juillet 2019 et le 30 juin 2024. Résultats Nous avons colligé durant cette période 66 patients (46 hommes et 20 femmes), sex-ratio=2,3 H/F. L’âge moyen de nos patients était de 43 ans, la moitié de nos patients étaient des fumeurs dans 57 % (36 hommes soit 78 % et 2 femmes 10 %). Les circonstances de découverte sont principalement la toux associée à une dyspnée chez 39 patients soit 59,09 %, la toux isolée trouvée chez 7 patients soit 11 %, l’hémoptysie chez 17 patients (26,56 %), asymptomatique sur le plan respiratoire dans 30 % des cas, des signes extrarespiratoires neurologique chez 16 patients (24,24 %). 30 % des patients avaient un score OMS a 1, et 23 % avaient un score OMS a 2, tandis que presque la moitié avaient un score OMS à 0. La tomodensitométrie a montré un processus tissulaire dans tous les cas (droit dans 55 % et gauche dans 33 % et bilatérale dans 10 %), associé à des adénopathies médiastinales dans 46 % et une pleurésie dans 23 %. La fibroscopie bronchique réalisée chez 55 patients (83 %), un bourgeon a été trouvé chez 22 patients (33,33 %), une sténose infiltrative chez 9 patientes (13,63 %), un aspect normal chez 10 patients (15 %). Le diagnostic histologique a été obtenu par des biopsies bronchiques chez 38 patients (57,57 %), des biopsies guidées par le scanner chez 7 patients (10,6 %), guidées par échographie chez 4 patients (6,06 %), biopsie des ADP chez 4 patients (6,06 %), biopsie pleurale chez 8 patients (12,12 %), et des biopsies sur une pièce opératoire chez 5 patients soit (7,57 %). L’adénocarcinome représente le type histologique le plus fréquent avec 42 cas (63,63 %) (24 H soit 52,17 % et 18 F soit 90 %) suivi de carcinome épidermoïde et processus carcinomateux peu différencié dont 7 cas (10,60 %), 6 cas (9 %), de carcinome à petites cellules, 3 carcinomes neuroendocrine et un cas de carcinome adénoïde kystique. Tous les cas ont été discutés en RCP thoracique, 54,5 % étaient classés stade IV dont seulement 6 patients étaient traités chirurgicalement. Des métastases ont été notées chez 41 patients (62 %). Une chimiothérapie était indiquée chez 46 cas, on avait noté une rémission complète chez 2 patientes, une rémission partielle chez 7 patients (soit 11 % des cas), une progression était notée chez 12 patients (soit 18 % des cas), stabilité radiologique était noté chez 2 patients par ailleurs, on avait noté 25 décès (soit 37,87 % des cas) et 18 cas perdus de vue. Discussion Le cancer bronchogénique primitif dans la moitié des cas était diagnostiqué chez des patients fumeurs, et la deuxième moitié avaient d’autres facteurs de risque qu’il faut chercher systématiquement à savoir le tabagisme passif, les expositions professionnelles et plus fréquemment l’exposition à l’amiante ainsi que la pollution atmosphérique. Ce dernier facteur de risque ne peut pas être considéré comme tel pour la région de Fès-Meknès vu sa caractéristique montagneuse et sa qualité d’air acceptable. Dans le registre des Cancers du Grand Casablanca 2018–2021, une prédominance du sexe masculin a été notée constituant ainsi 26 % environ de tous les cancers de l’homme vs 3 % de tous les cancers de la femme, alors que dans notre étude les patients de sexe féminin sont significativement atteints à hauteur de (30 %), ce qui nous mène à se poser des questions sur l’influence du facteur hormonal sur la fréquence du cancer bronchogénique chez les femmes. Conclusion Le cancer bronchique primitif reste assez fréquent chez le sujet jeune, le tabac est le facteur le plus incriminé chez l’homme et reste nettement inférieure chez la femme où d’autres facteurs étiologiques interviennent, avec comme caractéristiques une grande prédominance d’adénocarcinomes et malheureusement, ces patients présentent plus fréquemment un stade avancé au moment du diagnostic et ont un très mauvais pronostic. À noter que la lutte contre le tabac représente un moyen de prévention très important, puisque la moitié de nos patients étaient tabagique.
Introduction Le tabagisme constitue un facteur de risque commun entre la fibrose pulmonaire idiopathique (FPI) et le cancer broncho-pulmonaire (KBP). Ces deux entités sont particulièrement graves et leur association affecte le pronostic des deux pathologies. L’objectif de ce travail est d’étudier les caractéristiques cliniques, anatomopathologiques et évolutives des patients ayant un cancer pulmonaire associé à une FPI. Matériels et méthodes Étude rétrospective menée dans le service de pneumologie au CHU Hassan II Fès, sur une période allant de janvier 2016 à février 2024 des cas suivis pour un cancer broncho-pulmonaire primitif confirmé histologiquement associé à une FPI. Résultats Nous avons colligé 10 patients, tous des hommes, avec un âge moyen 69 ans. Parmi eux, 8 sont des fumeurs. Les symptômes les plus fréquents sont la dyspnée (8 cas), la toux (7 cas), la douleur thoracique (6 cas) et l’hémoptysie (5 cas). À l’examen physique, des râles crépitants sont présents chez 8 patients, un hippocratisme digital chez 5 patients avec une altération de l’état général de tous les patients. Les scanners thoraciques révèlent la présence de fibrose chez tous les patients, accompagnée d’un processus de localisation à droite chez 6 patients et à gauche chez 4. En outre, l’emphysème est observé chez 5 cas, et des adénopathies médiastinales sont présentes chez 6 cas. Suite à la confirmation histologique obtenue par fibroscopie avec des biopsies bronchiques réalisée chez huit patients, ainsi qu’une biopsie pleurale et une BSG chez les deux autres, le type histologique prédominant est le carcinome épidermoïde chez 6 patients et l’adénocarcinome chez 3 cas, un cas d’un carcinome non à petite cellules de type NOS a également été identifié. La plupart des patients était au stade de métastase (7cas), huit patients ont reçu une chimiothérapie, alors que deux autres ont subi une radio-chimiothérapie concomitante. Neuf patients sont décédés, tandis que le dernier est toujours sous traitement. Conclusion Les patients ayant une association entre une FPI et un KBP avaient un état général plus altéré, un pronostic sombre et une survie réduite. La lutte contre la consommation tabagique reste la solution pour prévenir ces deux pathologies graves et potentiellement mortelles.
Background Pycnodysostosis is a rare skeletal disorder often associated with craniofacial anomalies that may increase the propensity for obstructive sleep apnea (OSA). This retrospective study aimed to explore the prevalence and treatment of OSA in pediatric patients with Pycnodysostosis. Objectives To describe the clinical characteristics, polysomnographic findings, and treatment in 10 children with Pycnodysostosis. Methods Retrospective study conducted at the Sleep Medicine Center of CHU Hassan II, Fès, Morocco of 10 children with Pycnodysostosis diagnosed between 2010 and 2024 and based on clinical and/or genetic criteria who underwent overnight polysomnography. Results All patients suffered from moderate to severe OSA (apnea-hypopnea index: 5.9-43.8 events/h). Treatment included adenoidectomy, tonsillectomy, and CPAP, the latter leading to near normalization of respiratory patterns and improvement in sleep quality. Conclusion OSA in Pycnodysostosis imposes a significant impact on quality of life which responds favorably to stepwise interventions aimed at eliminating or mitigating OSA severity. This study highlights the importance of early diagnosis and appropriate treatment to prevent long-term complications, especially involving maxillofacial growth.
Background: This study aimed to develop a nomogram based on the most relevant clinical, CT, and radiomic features comprising 11 key signatures (2 clinical, 2 CT-based, and 7 radiomic) for the non-invasive prediction of the EGFR mutation status and to support the timely initiation of tyrosine kinase inhibitor (TKI) therapy in patients with non-small cell lung cancer (NSCLC) adenocarcinoma. Methods: Retrospective real-world data were collected from 521 patients with histologically confirmed NSCLC adenocarcinoma who underwent CT imaging and either surgical resection or pathological biopsy for EGFR mutation testing. Five Random Forest classification models were developed and trained on various datasets constructed by combining clinical, CT, and radiomic features extracted from CT image regions of interest (ROIs), with and without feature preselection. Results: The model trained exclusively on the most relevant clinical, CT, and radiomic features demonstrated superior predictive performance compared to the other models, with strong discrimination between EGFR-mutant and wild-type cases (AUC = 0.88; macro-average = 0.90; micro-average = 0.89; precision = 0.90; recall = 0.94; F1-score = 0.91; and accuracy = 0.87). Conclusions: A nomogram constructed using a Random Forest model trained solely on the most informative clinical, CT, and radiomic features outperformed alternative approaches in the non-invasive prediction of the EGFR mutation status, offering a promising decision-support tool for precision treatment planning in NSCLC.
Introduction Le SOH est défini par la présence d’une hypercapnie persistant la journée à l’état d’éveil, chez un sujet obèse, en l’absence d’autres causes pneumologiques, neurologiques ou mécaniques expliquant l’hypercapnie ; c’est une pathologie de plus en plus fréquente au vu de l’augmentation de la prévalence de l’obésité. Méthodes C’est une étude rétrospective descriptive incluant 112 patients porteurs de SOH, suivis au service de pneumologie et centre de sommeil de CHU Hassan II de Fès, colligés entre janvier 2017 et décembre 2023. Résultats Nous avons inclut 112 patients dont 10 % de sexe masculin et 90 % de sexe féminin, la moyenne d’âge est de 69,2 ans, avec IMC moyen de 40,3 ; la dyspnée d’effort, la somnolence diurne, le ronflement et la fatigue ont été retrouvés chez tous nos patients. Concernant les comorbidités ; HTA a été retrouvée chez 40 % des patients ; le diabète chez 25 %, une cardiopathie chez 26,7 %, une pathologie thyroïdienne chez 4,4 % et le tabagisme actif chez 5,3 %. Le mode de révélation a été une exacerbation chez 64,2 % des patients, les causes d’exacerbation étaient une surinfection bronchique chez 28,5 % des cas, une embolie pulmonaire chez 4,4 %, une association des deux chez 1,7 % et une décompensation cardiaque gauche chez 9,8 %. Tous les patients ont rapporté une dyspnée chronique depuis 3 ans en moyenne. La gazométrie artérielle a objectivé un pH moyen de 7,4, une PaO2 moyenne de 60,5mmHg avec une PaCO2 moyenne de 50,2, et une polyglobulie chez 16 % des cas sur la NFS. Une polysomnographie a été réalisée chez 39,3 % des cas, elle a objectivé un SAOS sévère dans 88,6 %, une moyenne d’IAH de 52,4/h, et une hypoventilation nocturne chez tous nos patients. Une spirométrie a été réalisée chez 41 des cas objectivant un profile ventilatoire restrictif chez 97,5 %. La VNI chronique a été indiquée chez tous les patients, instaurée chez 42,8 % des cas. La bonne observance thérapeutique a été notée chez 79,1 % des cas et une bonne tolérance chez 89,6 %. Apres une moyenne de 3,5 ans de suivi, on a noté que l’évolution clinique et gazométrique était favorable chez 93,7 % des patients appareillés ; tandis que chez les patients non appareillés, l’évolution vers l’aggravation a été objectivée chez 23,8 % des cas et 76,2 % étaient perdus de vue et chez qui on ne pourrait pas prédire l’évolution. Conclusion Nous concluons que le SOH est le plus souvent associé à un SAOS sévère, désaturant, qui est mis en évidence grâce à un enregistrement par polysomnographie.
OBJECTIVE:Lung cancer is the second most common malignancy worldwide and remains a leading cause of cancer-related mortality. Identifying predictive biomarkers is essential to improve personalized treatment. This prospective six-year study investigated the correlation between thyroid transcription factor-1 (TTF-1) expression and epidermal growth factor receptor (EGFR) mutations in Moroccan patients with lung adenocarcinoma. MATERIALS AND METHODS:Between 2017 and 2022, 234 patients with histologically confirmed lung adenocarcinoma were prospectively enrolled. TTF-1 expression was assessed by immunohistochemistry, and EGFR mutations were evaluated using molecular testing. Statistical analyses examined associations with clinicopathological parameters and overall survival. RESULTS:TTF-1 expression was detected in 83.8 % of cases and was more frequent among females (85.5 %) and never-smokers (88.2 %). EGFR mutations were identified in 21.36 % of patients, with exon 19 deletions (15.38 %) and exon 21 mutations (4.27 %) being the most common. A strong association was observed between TTF-1 expression and EGFR mutations (p < 0.001), particularly exon 19 deletions (p = 0.009). No significant correlation was found with exon 21 mutations or rare/dual variants. Combined exon 19 and 21 mutations were also significantly associated with TTF-1 positivity (p = 0.001). CONCLUSION:This study demonstrates a strong correlation between TTF-1 expression and common EGFR mutations, especially exon 19 deletions, in Moroccan patients with advanced lung adenocarcinoma. The association was most evident in never-smokers, supporting the role of TTF-1 and EGFR as complementary biomarkers in guiding diagnosis and treatment. Larger multicenter studies are warranted to further explore their prognostic value.
Congenital absence or deficiency of tracheal rings is exceptionally rare and may cause diffuse tracheobronchomalacia (TBM) with life-threatening airway collapse. A 6-month-old girl had persistent stridor from birth, cyanotic spells during feeding, and nocturnal desaturation. Cervicothoracic CT suggested severe airway collapse, and rigid bronchoscopy showed a flattened trachea and main bronchi without visible cartilaginous rings, consistent with ring agenesis and diffuse TBM. Curative surgery was not feasible. After in-hospital titration during a nap, face-mask CPAP with heated humidification was prescribed during sleep and postprandial windows. A minimal effective pressure of 6 cmH2O stabilized oxygenation and reduced cyanotic episodes, with good tolerance and device-recorded adherence. Despite transient clinical benefit, the infant died at one and a half years of age (18 months) due to disease progression. In extensive congenital TBM from ring agenesis not amenable to reconstruction, targeted face-mask CPAP can provide palliative/bridging support by counteracting dynamic airway collapse at times of highest vulnerability (sleep and after feeding). This case highlights practical aspects of titration, interface choice, and monitoring in infants.
Introduction La fibrose pulmonaire idiopathique (FPI) est une maladie respiratoire appartenant à la famille des pneumopathies interstitielles diffuse (PID). Cliniquement, elle se manifeste par une toux et une dyspnée d’installation progressive et radiologiquement, c’est la TDM thoracique qui permet retenir le diagnostic. C’est une pathologie chronique et progressive évoluant vers l’insuffisance respiratoire terminale sur un intervalle de temps variable pouvant aller de plusieurs mois à plusieurs années. Méthodes Notre étude est une étude rétrospective des patients pour fibrose pulmonaire idiopathique, suivis au sein du service de pneumologie du CHU Hassan II de Fès de 2016 à février 2024. Résultats Soixante-trois cas ont été colligés. La moyenne d’âge était de 67,7 ans avec un sex-ratio de 1F/9H, 59 % des patients étaient tabagiques et 44 % avaient une exposition environnementale. Les maîtres symptômes étaient la toux chez 90 % des patients et la dyspnée chez 95 % des cas. À l’examen clinique, des râles crépitants ont été retrouvés chez 95 % avec un hippocratisme digital chez 38 % des patients. À la TDM thoracique, 62 % des patients avaient un pattern de PIC certaine et 33 % une PIC indéterminée. Sur le plan fonctionnel, les explorations fonctionnelles respiratoires ont révélé un trouble ventilatoire restrictif dans 83,6 % des cas (51 cas), et lors du test de marche de 6minutes, une désaturation a été observée chez 53 des participants. Tous les dossiers ont été discutés en discussion multidisciplinaire avant de retenir ce diagnostic de FPI. Sur le plan thérapeutique, seul 30 % des patients ont pris un traitement anti-fibrosant, le reste des patients n’avaient pas les moyens ni la couverture sociale pour se le permettre. L’évolution a été marquée par la progression de 27 % des malades vers une insuffisance respiratoire chronique et malheureusement le décès de 24 % des patients. Conclusion La FPI est une pathologie certes rare mais grave engageant le pronostic fonctionnel et vital d’où l’intérêt d’un diagnostic précoce et d’une prise en charge adéquate permettant d’améliorer la qualité de vie et de prolonger la survie.
Introduction Le syndrome obésité hypoventilation (SOH) est défini par l’existence d’une hypoventilation alvéolaire chronique (PaO2<70mmHg, PaCO2≥45mmHg), chez des patients obèses, ayant un index de masse corporelle>30kg/m2, et ne présentant pas d’affection respiratoire associée susceptible d’expliquer les perturbations gazométriques. La ventilation non invasive (VNI) est la technique d’assistance ventilatoire appliquée en premier intention à des patients en insuffisance respiratoire hypercapnique. Nous rapportons l’expérience de service de pneumologie de CHU Hassan II Fès. Méthodes Il s’agit d’une étude descriptive rétrospective incluant les patients atteints de syndrome d’obésité hypoventilation hospitalisés en en unité de soins intensifs mis sous VNI aiguë et sous VNI au long cours au service de pneumologie CHU Hassan II de Fès au Maroc durant la période allant de janvier 2017 au 31 décembre 2023. Résultats Notre étude a porté sur 73 patients colligés. L’âge moyen de nos patients a été 70,84 ans [31–91 ans], avec une nette prédominance féminine. Le sex-ratio femme/homme est de 9,43. Le motif d’admission a été une aggravation aiguë d’une dyspnée chronique chez tous les malades, des expectorations purulentes chez 30 cas (41,09 %), une orthopnée dans 10 cas (13,69 %), hémoptysie 7(9 %), aggravation des signes cliniques d’hypercapnie dans 54 cas (74 %). IMC moyen à 41 la gazométrie artérielle a montré une SaO2 à AA moyenne à 70 % (47 à 89 %), une acidose respiratoire chez 29 malades (39,7 %), une hypoxémie chez tous le malades (PaO2 moyenne à 56mmHg) avec hypercapnie (PaCO2 moyenne à 68mmHg). La cause d’exacerbation a été : une surinfection bronchique dans 29 cas (39 %), une embolie pulmonaire dans 6 cas (8 %), une embolie pulmonaire associée à une surinfection bronchique dans 2 cas (3 %), une décompensation cardiaque gauche dans 11 cas (15 %). Tous nos malades ont été mis sous ventilation non invasive aiguë avec un réglage adapté à chaque patient, on a noté une amélioration clinique et de la PaCO2 avec une moyenne de 47mmHg après VNI aiguë nous déplorons deux décédés. Tous nos malades étaient sortis avec indication de VNI chronique avec réglage transitoire et adressaient en unité de sommeil. Une polysomnographie a été réalisé chez 31 patients (42 %). Le syndrome d’apnée obstructive a été retrouvé chez 30 patients (97 %) La VNI chronique était mise en place avant enregistrement de sommeil chez 24 patients (32 %) avec un réglage transitoire et après enregistrement chez 14 malades (20 %). Au total 38 patients (52 %) étaient mis sous VNI chronique IPAP moy à 20,57 et PEP à 9,67. L’évolution clinique et gazométrique sous VNI avec un recueil d’un an en moyenne (1 mois–7 ans) était favorable chez 29 patients parmi les 38 (76 %), des effets secondaires à type de rhinite chronique, de sécheresse buccale, escarres étaient notées respectivement dans 15, 20 et 9 % des cas. Conclusion La VNI est devenue l’un des piliers de la PEC de l’insuffisance respiratoire hypercapnique. Ainsi que la proximité de l’unité de soins intensifs et du centre de sommeil joue un rôle important dans la Prise en charge des exacerbations de SOH en permettant la réduction de la mortalité et de diminuer les hospitalisations en réanimation et permettant aussi un suivi adapté des malades.
Introduction:Prader-Willi syndrome (PWS) is a genetic disorder characterized by hypothalamic dysfunction, resulting in a combination of neurodevelopmental, metabolic, and behavioral abnormalities. Sleep disorders are frequent and often severe and constitute a major component of the clinical phenotype. However, sleep disorders affecting PWS patients remain under-recognized and undertreated. Case report:We report two pediatric cases of PWS illustrating divergently evolving pattern of sleep-disordered breathing.The first patient, L.K., aged 10, presented with severe sleep apnea-hypopnea syndrome including a prominent central component (AHI: 31.8/h; central apneas: 15.2/h). Auto-SV ventilation followed by fixed PAP therapy led to a significant reduction in indices (final AHI: 5/h), improvement in deep sleep (N3: 50.2 %), and a decrease in micro-arousals, with observed behavioral benefits reported by caregivers.The second patient, R.K., followed since the age of 10, progressed from moderate OSA (AHI: 5.6/h) to a phenotype dominated by central events (AHI: 27.8/h including 25.4/h central). A personalized fixed-pressure PAP titration resulted in long-term control of respiratory events, maintained over six years, with adherence exceeding 5 h/night and a residual AHI of 1.3/h. Discussion:These cases highlight the complex pathophysiology of sleep disorders in PWS, involving peripheral hypotonia, central hypothalamic dysregulation, and ventilatory control instability. They also underscore the central role of polysomnography in screening and tailoring ventilatory support treatment. Conclusion:Tailored nocturnal ventilation improves not only breathing but also sleep quality and behavioral functioning, as suggested by previous clinical observations linking ventilatory correction to behavioral improvements in PWS.
Background Lung cancer is the leading cause of cancer-related mortality worldwide, with a notable association between interstitial lung diseases, particularly usual interstitial pneumonia (UIP), and lung cancer. This study investigates the epidemiology, clinical characteristics, histological findings, and treatment implications of this relationship. Methodology A retrospective study was conducted at the Department of Pneumology of Hassan II University Hospital in Fez, Morocco, from January 2016 to September 2024. Patient records of those with established UIP/idiopathic pulmonary fibrosis who developed lung cancer during follow-up, as well as lung cancer patients discussed in thoracic oncology multidisciplinary meetings with UIP pattern identified on CT, were reviewed. Data on demographics, clinical presentation, radiological and histological findings, treatment, and outcomes were collected and analyzed using Epi Info version 7.2.6.0. Results In total, 10 male patients with UIP and lung cancer were included, with a mean age of 66.2 years. Cough 8 (80%) and dyspnea 7 (70%) were the most common symptoms. Radiologically, 7 (70%) had definite UIP, and tumors were predominantly located in the lower lobes. Histological analysis revealed an equal distribution of adenocarcinoma and squamous carcinoma at four (40%) each, with six (60%) patients diagnosed at stage IV. Treatment varied, with one patient undergoing surgery, while others received chemotherapy or radiotherapy. Unfortunately, four patients died, highlighting the aggressive nature of the disease. Conclusions The association between UIP and lung cancer presents significant clinical challenges, impacting treatment strategies and survival outcomes. Continued research is vital to developing effective management protocols. A multidisciplinary approach is essential to optimize care for patients facing the dual burden of UIP and lung cancer, ultimately aiming to improve their prognosis and quality of life.
