Cette étude de cas explore les vécus et les ressentis de deux receveurs d’une greffe de cornée. Lors d’entretiens, deux hommes de 60 et 78 ans ont été invités à préciser leurs anamnèses, à être évalués (NEI-VFQ25, SF-26), puis à s’exprimer plus librement lors d’un entretien semi-structuré. Aucune anxiété et aucune dépression n’ont été diagnostiquées. S’il a été pour eux « miraculeux » de recouvrer la vue, la cornée reste difficilement matérialisable, représentable. Ils l’ont comparée tous deux à une substance liquide. Ces hommes ont communément évoqué un tribut esthétique, une préparation psychique spécifique à la chirurgie. Embarrassés de penser et de parler d’un donneur, ils ont alors considéré ce qu’ils pourraient donner en retour. Ces éléments appuient la nécessité d’élaborer les vécus et les ressentis des patients receveurs de greffes cornéennes. Ceci, afin de les préparer à la chirurgie et de mieux délimiter leurs questionnements autour du don et du contre-don.
Keratoconus has a significant impact on patients’ quality of life (QoL), from diagnosis to the advanced stages of the disease. The aim of this research was to identify domains of QoL affected by this disease and its treatment. Phone interviews were conducted using a semi-structured interview guide, with patients with keratoconus stratified according to their current treatment. A board of keratoconus experts helped identify the guide’s main themes. Thirty-five patients (rigid contact lenses, n = 9; cross-linking, n = 9; corneal ring implants, n = 8; and corneal transplantation, n = 9) were interviewed by qualitative researchers. Phone interviews revealed several QoL domains affected by the disease and its treatments: “psychological”, “social life”, “professional life”, “financial costs” and “student life”. All domains were impacted, independently of the treatment history. Few differences were found between treatment regimens and keratoconus stages. Qualitative analysis enabled the development of a conceptual framework based on Wilson and Cleary’s model for patient outcomes common to all patients. This conceptual model describes the relationship between patients’ characteristics, their symptoms, their environment, their functional visual impairment and the impact on their QoL. These qualitative findings supported the generation of a questionnaire to evaluate the impact of keratoconus and its treatment on patients’ QoL. Cognitive debriefings confirmed its content validity. The questionnaire is applicable for all stages of keratoconus and treatments and may help tracking change over time in regular clinical settings. Psychometric validation is yet to be performed before its use in research and clinical practices.
Purpose To investigate the repeat corneal transplantation trend in France from 2004 to 2019. Methods Review of the prospectively compiled French Biomedicine Agency electronic database containing all corneal transplantation records from 2004 to 2019. The surgical technique, demographic characteristics, diagnosis, and previous graft data were retrieved and analyzed using the Cochran-Armitage trend test. Results A total of 66,584 corneal transplantations were performed, 51,260 of which were first grafts and 15,324 (23%) were regrafts. For regrafts, 77% were penetrating keratoplasties (PK) and 19.6% were lamellar keratoplasties (LK). Age, hypertonia, glaucoma, trauma, lens surgery, immune disorders, diameter > 8.5 mm, and neovessels in > 2 quadrants were associated with a higher rate of repeat keratoplasty. Keratoconus, secondary endothelial dystrophy, and Fuchs’ dystrophy were the principal indications for regrafting. When a previous graft failed, it occurred earlier for patients with LK (4.6 years, median = 2, SD = 7.54) than PK (8.48 years, median = 5, SD = 9.51). Failure within a year was the reason why 28.3% of the LK regrafts and 12.5% of PK regrafts were performed, while for failure within two years these values were 49.9% and 27.8%, respectively. Graft survival decreased with the number of repeat keratoplasty, being more pronounced after a second LK regraft and after a first PK regraft. Conclusion The number of LK regrafts increased continuously, and 1/3 were performed for failure within the year. This rate increased until 2015, after which it stabilized until 2019, probably due to the better mastery of the technique.

The purpose of this study is to report the 24-month outcomes of a pro re nata (PRN) compared with a treat and extend (T&E) regimen in patients previously treated for neovascular age-related macular degeneration (nAMD). This was a 2-year prospective, single-center study. Previously treated patients for nAMD were randomized into two regimen groups: T&E and PRN groups. Main outcome measured was change in best corrected visual acuity (BCVA) from baseline to month 24. Secondary outcomes encompassed anatomical features such as central retinal thickness (CRT), number of intravitreal injections (IVI), and visits required. A total of 124 eyes received the T&E (n = 61) or PRN (n = 63) regimen. At month 24, the mean BCVA change was −4.4 early treatment diabetic retinopathy study (ETDRS) letters (T&E) and −3.4 ETDRS letters (PRN), with a difference of +1.1 ETDRS letters (95% CI [−2.25]; p = 0.006). The mean change in CRT was −10.6 µm (T&E) and −7.9 µm (PRN), with a difference of +2.6 µm (95% CI [+19.2]; p = 0.004). The T&E group had received a mean of +4.6 more injections (95% CI [−7.06; −2.12]; p < 0.001) at month 24. There was statistically proven non-inferiority between the PRN and T&E regimens in terms of visual and anatomical outcomes at 24 months, with significantly more IVI administered in the T&E regimen.
La pollution de l’air a augmenté régulièrement depuis plusieurs décennies. Les effets sur la santé humaine ont été largement étudiés concernant l’augmentation de la mortalité, l’incidence des accidents vasculaires cérébraux, des pathologies respiratoires et allergiques. Les effets de la pollution et des conditions atmosphériques au niveau de la surface oculaire, en contact direct avec l’environnement extérieur, ont été en comparaison nettement moins décrits.Nous avons réalisé une revue de la littérature des articles publiés sur le sujet de 1966 à octobre 2020. Parmi les 661 articles recensés, 33 ont été retenus. Les atteintes de la surface oculaire en lien avec la pollution incluaient les conjonctivites non spécifiques, le syndrome sec, la blépharite, et la conjonctivite allergique. Les principaux polluants étudiés étaient les PM2,5 (Particulate Matter), PM10, l’ozone (O3), le dioxyde d’azote (NO2), le monoxyde de carbone (CO) et le dioxyde de soufre (SO2). Certains paramètres météorologiques influant la qualité de l’air tels que la température ou l’humidité relative ont été pris en compte.Parmi les marqueurs de pollution de l’air susceptibles d’être responsables de pathologies ophtalmologiques, le NO2 et le SO2 apparaissent être le plus fréquemment et fortement corrélés. Des températures élevées et des faibles taux d’humidité apparaissent aussi comme des facteurs aggravants pour la surface oculaire. Les résultats doivent cependant être interprétés prudemment devant l’hétérogénéité des études, tant au niveau de la méthodologie que de la variabilité des résultats. L’inclusion des patients et l’analyse des données environnementales ainsi que la corrélation entre ces deux paramètres posent en effet de nombreuses questions méthodologiques.Il apparaît cependant essentiel de diminuer les taux de pollution mais aussi de développer de nouvelles études fondées sur des méthodes d’analyse de l’environnement et d’imputabilité clinique fiables.Air pollution has steadily increased for several decades, with widely studied effects on human health, including increased mortality, incidence of stroke, respiratory and allergic disease. However, the effects of pollution on the ocular surface, in direct contact with the outside world, have been less precisely studied.We conducted a literature review of articles on the subject published from 1966 to October 2020. Among the 661 articles identified, 33 were retained. Ocular surface disease associated with pollution included non-specific conjunctivitis, dry eye disease, blepharitis, and allergic conjunctivitis. The studied pollutants were particulate matter less than 2.5 μm and 10 μm (PM2.5, PM10), ozone (O3), nitrogen dioxide (NO2), carbon monoxide (CO) and sulfur dioxide (SO2). Certain air quality parameters such as temperature and relative humidity were also studied.Among the markers of air pollution possibly associated with ophthalmic disease, NO2 and SO2 appear to be the most frequent and highly correlated. High temperatures and low humidity levels also appear to be aggravating factors for the ocular surface. However, due to the heterogeneity of the studies, the results must be interpreted with caution. Indeed, the methodology and the results of the various studies are sometimes contradictory. The inclusion of patients, the analysis of environmental data, and the correlation between these two elements indeed raise numerous methodological questions.Air pollution control would appear essential, as well as the development of new studies based on reliable methods of studying the environmental and its clinical effects.
Paul Tessier was a leading French oculoplastic surgeon who took part in several surgical missions in Iran to manage victims of the Iraq-Iran conflict in the late 1980's and early 1990's.We collected the records of 322 patients who underwent surgical procedures for the management of wartime injuries by Paul Tessier's team in Iran from 1990 to 1993. We also report one of the most representative cases of orbital reconstruction performed by Tessier.Mean age at the time of trauma was 20.65 ± 7.04 years (range: 2-–62). Craniofacial CT-scans were available for 54 patients. The bones of the upper third of the face and the orbital contents were affected in 124/322 patients (38.50%). Soft-tissue lesions of the upper third included 13 frontal lacerations (4.04%), 60 orbital injuries (18.63%) and 95 uni- or bilateral enucleations (29.50%). Thirty-nine uni- or bilateral lid injuries (12.11%) and 8 tear duct injuries (2.48%) were reported. A specific case of orbital reconstruction using antero-internal and posterior iliac bone grafts was reported as a representative example of Tessier's techniques.This study highlights the challenges of orbital reconstruction in wartime injuries and provides insights on the work of one of the most renowned surgeons in this field.Paul Tessier fut l’un des pionniers de la chirurgie orbitopalpébrale en France, il a participé à plusieurs missions chirurgicales en Iran pour prendre en charge des victimes du conflit Iran-Iraq à la fin des années 1980 et au début des années 1990.Nous avons colligé les dossiers de 322 patients opérés par l’équipe de Paul Tessier entre 1990 et 1993. Nous développerons également l’un des cas les plus représentatifs des techniques de reconstruction orbitaire utilisées par Paul Tessier.L’âge moyen au traumatisme était 20,65 ± 7,04 ans (2–62). Un scanner craniofacial était disponible chez 54 patients. Le tiers supérieur de la face et l’orbite étaient touchés chez 124 patients sur 322 (38,50 %). Les atteintes des tissus mous du tiers supérieur de la face incluaient 13 dilacérations frontales (4,04 %), 60 lésions orbitaires (18,63 %) et 95 énucléations uni- ou bilatérales (29,50 %). Trente-neuf lésions palpébrales uni- ou bilatérales (12,11 %) et 8 plaies de la voie lacrymale (2,48 %) ont été reportées. Nous avons détaillé un cas spécifique de reconstruction orbitopalpébrale par greffons d’os iliaque du fait de sa représentativité vis-à-vis des techniques de lambeaux et greffes osseuses utilisées par Tessier.Cette étude met en avant le défi que constitue la reconstruction orbitopalpébrale en temps de guerre. Elle permet de comprendre les techniques de réhabilitation faciale mises en œuvre par l’un des précurseurs de cette discipline.
Le dupilumab est la première biothérapie disponible pour la dermatite atopique (DA). Il repose sur l’inhibition de l’IL-4 et de l’IL-13. Cette thérapie peut entraîner des conjonctivites, dont l’incidence est évaluée à 5–28 % selon les études cliniques. Les mécanismes impliqués dans la conjonctivite associée au dupilumab (CAD) ne sont pas élucidés. Notre objectif était d’étudier la faisabilité et de comparer les profils cytokiniques des larmes collectées de façon non invasive avant puis sous traitement par dupilumab, chez les patients avec et sans conjonctivite induite. Nous avons mené une étude prospective monocentrique de patients recevant du dupilumab pour une DA. Des échantillons lacrymaux étaient collectés par capillarité à l’initiation du traitement, puis à 4 mois et 8 mois après l’instauration. Les patients ayant développé une conjonctivite étaient suivis ensuite tous les deux mois. Une recherche de demodex sur les cils était aussi effectuée à l’instauration. Les dosages d’IL-2, 4, 6, 10, 17A, d’IFNg et de TNFa étaient réalisés en cytométrie de flux. Une numération et formule des leucocytes lacrymaux était aussi effectuée. Vingt et un patients traités par dupilumab étaient inclus et 10 contrôles sains étaient aussi prélevés pour comparaison. Cinq patients (24 %) ont développé une conjonctivite induite, après une moyenne de 2,3 mois. À l’instauration du dupilumab, le temps de rupture du film lacrymal au diagnostic était plus court (6 secondes versus 4,5 s, NS) et le taux de lymphocytes lacrymaux étaient plus élevé chez les patients qui développaient ensuite une conjonctivite. Les taux de cytokines lacrymales IL2, IL17A, IL10, IFNg semblaient plus élevés chez les patients qui ont ensuite développé une CAD, mais non significatifs (p > 0,1), par rapport aux autres patients et témoins. L’IFNg, le TNF et l’IL-10 diminuaient sous traitement avec dupilumab (p < 0,05). La présence de demodex était détectée chez un seul patient avec DA sans CAD. Le taux des éosinophiles lacrymaux avait tendance à augmenter au cours du suivi dans le groupe DA avec une conjonctivite induite. Notre étude démontre la possibilité d’analyser de façon non invasive les cytokines dans les larmes de patients. Nos résultats préliminaires n’ont pas permis d’identifier de marqueur lacrymal significatif de CAD, probablement à cause d’un manque de puissance. Un taux élevé de lymphocytes et de polynucléaires éosinophiles au niveau lacrymal pourrait être associé au développement ultérieur d’une CAD.
Le tatouage oculaire est une technique ancienne déjà utilisée sur la cornée il y a 2000 ans afin d'améliorer l'aspect de cicatrices cornéennes disgracieuses. Depuis 2007, des tatoueurs réalisent des tatouages scléraux ou « eyeball tattoo » malgré les risques encourus et la désapprobation des autorités sanitaires et de la sphère médicale. Nous rapportons le cas d'un patient de 30 ans, tatoué sur les deux yeux à l'encre noire qui s'est présenté aux urgences ophtalmologiques avec des nodules épiscléraux apparus aux niveaux des points d'injection de l'encre. L'évolution clinique a été stable sous traitement anti inflammatoire topique. Des cas similaires sont retrouvés dans la littérature dont un où le patient a bénéficié d'une exérèse de lésion d'aspect similaire. L'analyse tissulaire retrouvait une prolifération lympho-histiocytaire sans critère de malignité. À ce jour, 20 cas de « eyeball tattoo » ont été rapportés dans la littérature internationale avec souvent des complications très sévères. On identifie d'une part les complications en lien avec des traumatismes perforants et d'autre part les complications aigues et chroniques inhérentes aux produits utilisés. Le tatouage scléral gagne en popularité à travers le monde or on sait aujourd'hui que les risques sont nombreux et que les patients s'exposent à des conséquences traumatiques, inflammatoires, immunitaires, infectieuses et probablement néoplasiques à plus long terme. Les ophtalmologues doivent être avertis de cette pratique et de ses conséquences afin de prendre en charge au mieux les cas rencontrés.Tattooing of the glove is an old practice used 2000 years ago on the cornea to improve the appearance of unsightly corneal scars. Since 2007, tattoo artists have been performing scleral tattoos, also called "eyeball tattoos," despite the risks involved and the disapproval of health authorities and the medical community. We report the case of a 30-year-old man with bilateral black ink scleral tattoos who came to our ophthalmologic emergency service with episcleral nodules, which had appeared at the ink injection sites. The clinical course stabilized with topical anti-inflammatory treatment. Similar cases have been reported in the literature, in one of which the patient underwent surgical excision of a similar lesion. Histological analysis showed a lympho-histiocytic reaction without any signs of malignancy. To date, 20 cases of scleral tattoos have been reported in the international literature, often with severe complications. We see, on the one hand, complications related to perforating trauma, and on the other hand, acute and chronic complications inherent to the products used. Scleral tattooing is gaining popularity around the world despite significant risk exposure, including traumatic, inflammatory, immune, infectious and probably long-term neoplastic consequences. Ophthalmologists must be aware of this practice and its consequences in order to make the most appropriate therapeutic decisions.
PURPOSE This study aimed to assess how the contamination rate of organ-cultured corneas has evolved and to analyze the evolution of microorganisms involved. METHODS Data from the Besançon eye bank were reviewed over 14 years (2005-2018). The changes in the contamination rate and the contaminant species found during the organ culture storage were analyzed. Microbiological tests were performed twice on the storage media-at day 5 and before the deswelling phase. RESULTS Among the 17,979 donor corneas collected, 1240 corneas were microbiological-test positive. The average annual contamination rate was 6.8% (range: 5.2%-8.9%). Seventy-five percent of contaminations were bacterial. The most frequently found bacterium was Staphylococcus spp. (31.3%), followed by non-Enterobacteriaceae Gram-negative Bacilli (GNB) (27.3%), with most Sphingomonas spp. and Pseudomonas spp. Fungal contamination (21.9%) was dominated by Candida (82.7%). Seventy-seven types of microorganisms were identified. The Staphylococcus rate tended to decrease, whereas non-Enterobacteriaceae GNB rate has increased in the past few years to reach 46% of bacteria. Most of the contaminations were detected in the early phase of organ culture at day 5 (89.2%). The second microbiological test found 44.8% of fungal contaminations (predominantly Candida spp.). CONCLUSIONS The annual contamination rate was stable and remains low, but the types of contaminating microorganisms varied from year to year. Staphylococcus spp. and non-Enterobacteriaceae GNB accounted for a significant proportion of the contaminations. We found a significant proportion of contamination, especially fungal, at the late phase of storage. Reassessing the antibiotics and antifungals in the storage medium may be useful to limit corneal disposal.
The goal of this study was to assess the efficacy of transcanalicular diode laser assisted dacryocystorhinostomy (TDL-DCR) for the treatment of primary acquired nasolacrimal duct obstruction.This was a single center retrospective study. We included all patients who underwent TDL-DCR between July 2015 and October 2017 for chronic epiphora or chronic dacryocystitis. All procedures were performed under local anesthesia with sedation. The anatomical and functional success rates were studied as a function of each operative indication.Thirty-seven patients were included (18 had chronic epiphora and 19 had chronic dacryocystitis). All but one patient had bicanalicular silicone stent intubation. Irrigation of the lacrimal system demonstrated postoperative patency in 78% of patients operated for chronic epiphora and 76% of those with chronic dacryocystitis. The functional success of the surgery (as defined by the Munk score) was higher if the operative indication was chronic dacryocystitis (62%) (whereas it was only 41% for the indication of chronic epiphora).TDL-DCR has a lower anatomical and functional success rate than those reported in the literature for external or endonasal dacryocystorhinostomy. However, it retains a place in the treatment of primary acquired nasolacrimal duct obstruction (in particular when it is complicated by chronic dacryocystitis), especially if there is an absolute or relative contraindication to the two other dacryocystorhinostomy techniques.Cette étude a pour objectif d’évaluer l’efficacité de la dacryocystorhinostomie transcanaliculaire au laser diode pour le traitement de l’obstruction primaire du conduit lacrymo-nasal.Étude rétrospective monocentrique. Nous avons inclus les patients opérés de dacryocystorhinostomie transcanaliculaire au laser diode entre juillet 2015 et octobre 2017 pour un épiphora chronique ou une dacryocystite chronique. Toutes les interventions ont été réalisées sous anesthésie locale et sédation. Les taux de succès anatomique et fonctionnel ont été étudiés en fonction de chaque indication opératoire.Trente-sept patients ont été inclus (18 avaient un épiphora chronique et 19 une dacryocystite chronique). Tous les patients ont eu une intubation bicanaliculaire en silicone, sauf un. L’irrigation du système lacrymal était perméable en postopératoire chez 78 % des patients opérés pour un épiphora chronique et 76 % de ceux qui avaient une dacryocystite chronique. Le succès fonctionnel de la chirurgie (défini par le score de Munk) a été plus élevé si l’indication opératoire était une dacryocystite chronique (62 %) (alors qu’il n’était que de 41 % pour une indication d’épiphora chronique).La dacryocystorhinostomie transcanaliculaire au laser diode a un taux de succès anatomique et fonctionnel inférieurs à ceux rapportés dans la littérature pour la dacryocystorhinostomie externe ou endonasale. Elle garde cependant une place pour le traitement de l’obstruction primaire du conduit lacrymo-nasal (en particulier si elle est compliquée de dacryocystite chronique), notamment s’il existe une contre-indication absolue ou relative aux deux autres techniques de dacryocystorhinostomie.
Histidine kinases (HKs), together with their partner proteins, the response regulators (RRs), form the ubiquitous two-component systems that are global players in control and adjustment of microbial lifestyle. Although their basic function (i.e. the transfer of a phosphate group from the HK to its RR partner) is simple to articulate, deciphering the molecular details of this process has proven anything but simple, especially when quantitative aspects come into play. Bouillet et al. report a series of elegant and sophisticated experiments to quantitatively understand HK functions, clearing up several open questions and providing a new strategy for future work in the field.
Le syndrome de Radius-Maumenee est une cause rare et grave de glaucome secondaire à angle ouvert. Il a pour particularité d’être lié à un mécanisme d’hyperpression veineuse responsable d’une hypertonie oculaire. Ce syndrome associe un glaucome à angle ouvert à une dilatation des vaisseaux épiscléraux sans autres anomalies orbitaires ou systémiques. Il s’agit d’un diagnostic d’élimination après avoir vérifié l’absence de fistule carotidocaverneuse, d’orbitopathie dysthyroïdienne, ou encore d’un processus invasif intraorbitaire pouvant expliquer l’hyperpression veineuse observée. La prise en charge consiste en un traitement par collyres hypotonisants et, en cas d’échec, d’une chirurgie filtrante. Celle-ci se complique fréquemment d’effusion uvéale et de décollements choroïdiens postopératoires. Nous présentons ici le cas d’une patiente de 59 ans atteinte d’un Syndrome de Radius Maumenee.Radius-Maumenee syndrome is a rare cause of severe secondary open-angle glaucoma. The ocular hypertension is due to episcleral venous hypertension. This syndrome consists of open angle glaucoma and episcleral venous dilatation without orbital or systemic abnormalities. It is diagnosis of exclusion after ruling out carotid-cavernous fistula, thyroid orbitopathy and an invasive orbital process that might explain the elevated episcleral venous pressure. Treatment begins with glaucoma eyedrops and, in case of failure, filtering surgery. Uveal effusion syndrome and postoperative choroidal detachment are frequent complications of filtering surgery in patients with this syndrome. We present the case report of a 59-year-old woman with Radius-Maumenee syndrome.
