Allergy laboratory tests provide an important contribution to the diagnosis and monitoring of respiratory allergies. The increase in the prevalence of these diseases in the French population during the last decades and the increasingly frequent use of biological diagnosis requires the optimization of the use of this tool. Its rational use according to updated scientific data and an approach favoring the cost/benefit ratio can guarantee diagnostic efficiency. Aware of this issue, the French Society of Allergology (SFA) has set up a multidisciplinary task force to update allergy biology recommendations, the previous recommendations drawn up in 2005 by the High Authority for Health (HAS) having become partly obsolete due to the evolution of techniques and concepts. The methodology used is that of the recommendations for clinical practice (RPC) developed by the HAS, even if the text has not been labeled by that institution. The working group drafted 11 graded recommendations for the management of respiratory allergy. Although not exhaustive, these 11 questions respond to the most common concerns of prescribers whether or not they are allergists for the management of patients with allergies or with a suspected respiratory allergy, while taking into account the French regulatory context. Les examens biologiques d’allergie contribuent de façon importante au diagnostic et au suivi des maladies allergiques. L’augmentation de la prévalence de ces maladies dans la population française durant les dernières décennies et le recours de plus en plus fréquent au diagnostic biologique nécessite l’optimisation de l’utilisation de cet outil. Son utilisation rationnelle selon les données actualisées de la science et une démarche privilégiant le rapport coût/bénéfice sont à même de garantir l’efficience diagnostique. Consciente de cet enjeu, la Société française d’allergologie (SFA) a mis en place un groupe de travail multidisciplinaire pour l’actualisation des recommandations de biologie de l’allergie, les précédentes recommandations élaborées en 2005 par la Haute Autorité de santé (HAS) étant devenues en partie obsolètes en raison de l’évolution des techniques et concepts. La méthodologie utilisée est celle des recommandations pour la pratique clinique (RPC) élaborée par la HAS, même si le texte n’a pas été labellisé par celle institution. Le groupe de travail a rédigé 73 recommandations gradées et un préambule comportant 6 questions d’ordre général. Bien que non exhaustives, ces 79 questions répondent aux préoccupations les plus courantes des prescripteurs qu’ils soient allergologues ou non pour la prise en charge des patients allergiques ou ayant une suspicion d’allergie, tout en tenant compte du contexte réglementaire français.

The evolution of knowledge, techniques and the routine availability of a large number of molecular allergens, as well as regulatory developments which have eliminated certain allergenic extracts for prick tests, have had repercussions on the biological diagnosis of allergy. The French Allergology Society has set up a working group whose objective was to update the allergy biology recommendations initially developed by HAS(2) in 2005. The methodology used for the drafting is that of the "Good recommendation practice", according to the HAS method "Recommendation for clinical practice". Objective: The aim of our study was to analyze the dynamics of HIV-DNA levels in CD4+ Tcell subsets in individuals starting successful dolutegravir-based regimens. Design: Twenty-seven individuals with acute (AI, n=8) or chronic (CI, n=5) infection and patients in virological success (VS, n=10) or failure (VF, n=4) on ART who initiated a dolutegravir-based regimen were enrolled (NCT02557997). Methods: CD4+ T-cells from baseline and week 48 of successful treatment were sorted into effector memory (TEM), transitional memory (TTM), central memory (TCM) and naïve (TN) cell groups for total HIV-DNA measurements by qPCR. Bayesian methods were used to estimate the posterior probability of a HIV-DNA decrease >0.25 log copies/10 cells at week 48. Results: All patients achieved HIV-RNA suppression at 48 weeks. At baseline and week 48, the highest contributions to the HIV-DNA-infected pool from CD4+ T-cells were observed in TTM cells in the AI group (62.4% and 60.2%, respectively), but in TCM cells for the CI, VS and VF groups (54.6% and 59.4%, 58.2% and 62.9%, 62.4% and 67.2%), respectively. HIV-DNA burden declined in all subsets after 48 weeks of treatment in the AI (probability (Pr)>91%), CI (Pr>52%) and VF (Pr>52%) groups, but only in TEM cells in the VS group (Pr=95%). Conclusions: Our study showed that dolutegravir-based treatment reduced the HIV-DNA cellular burden in individuals from the AI, CI and VF groups, though the reduction levels differed between the patient subgroups. Early treated patients had the highest probability of HIVDNA reduction. Interestingly, in the aviremic VS group, HIV-DNA reduction was limited to TEM cells. L’histamine et la tryptase beta sont les médiateurs mastocytaires utilisables pour confirmer la nature anaphylactique d’un choc. Le dosage de la tryptase n’est pas spécifique de l’isoforme beta et implique de comparer la valeur mesurée au pic à la valeur à l’état basal. Une augmentation de la tryptase n’est pas toujours détectable dans les réactions de grade 1. Le dosage de l’histamine de demi-vie courte nécessite un prélèvement précoce (idéalement < 30 min après les 1er signes). La mise à jour de la nomenclature des actes de biologie médicale considère cet acte comme obsolète. Nous avons voulu évaluer la pertinence du dosage de l’histamine dans l’exploration d’un choc en complément de la tryptase. Une étude rétrospective menée par 7 laboratoires français entre 2011 et 2015 a permis l’analyse de 273 réactions (dont 38 provenaient de l’étude francilienne NASA). Pour 269, l’agent responsable était un médicament. Les réactions ont été classées en grade 1 (15 %), 2 (25 %), 3 (27 %), 4 (5 %) ou indéterminé (28 %). Seuls les patients avec tests cutanés positifs (162, 60 %) ont été classés allergiques. Les courbes ROC calculées avec les valeurs au pic pour la tryptase (ThermoFisher) et pour l’histamine (Beckman Coulter RIA/ELISA) donnent des AUC à 0,88 et 0,86 respectivement. L’index de Youden permet de calculer un seuil optimal de positivité de 8 μg/L pour la tryptase et de 18 nmol/L pour l’histamine. L’analyse de la cinétique (pic > taux basal × 1,2 +2 μg/L), y compris quand le pic de tryptase est < 8 μg/L permet d’obtenir une sensibilité et specificité de 84 %. Cependant, l’utilisation de l’histamine au seuil de 18 nmol/L permet de diminuer le taux de faux-négatifs de 24 (analyse uniquement de la tryptase) à 19 %. L’analyse des variations de cinétique de la tryptase et l’optimisation du seuil de positivité à 8 μg/L permettent d’obtenir une bonne sensibilité pour ce seul médiateur. Mais, en absence d’augmentation par rapport au taux basal, le dosage de l’histamine peut être informatif pour l’allergologue.
Le dépistage néonatal et la prise en charge chirurgicale précoce des malformations cardiaques chez les enfants atteints de trisomie 21 (T21) a permis d’améliorer considérablement leur espérance de vie et de réduire leur morbi-mortalité. Les anomalies immunologiques et hématologiques sont bien caractérisées chez ces enfants : susceptibilité aux infections, auto-immunité (diabète, dysthyroïdie, maladie cœliaque) et hémopathies malignes. L’état des lieux à l’âge adulte est moins bien décrit. Dans le cadre d’un PHRC interrégional (2012-A00486-37) sur l’état de santé des adultes atteints de trisomie 21, 26 patients T21 adultes (> 18 ans) ont bénéficié d’une consultation complète avec analyse systématique des données du carnet de santé, examen clinique et bilan biologique, après recueil du consentement éclairé. Les données biologiques ont été comparées à celles de 23 témoins. En complément, les données rétrospectives de 42 patients T21 adultes hospitalisés dans notre centre entre 2005 et 2015 ont été colligées. Parmi les 68 patients inclus au total, on comptait 39 hommes et 29 femmes (SR 1,34), avec un âge médian de 44 ans [18–74]. Vingt-cinq patients (36 %) vivaient en institution. Trente-trois patients (49 %) souffraient d’hypothyroïdie, 1 (1,5 %) de maladie cœliaque. Treize patients (19 %) présentaient un antécédent de malformation cardiaque opérée (CAV ou CIV), 13 étaient atteints d’épilepsie, 9 (13 %) de démence, 2 (5 %) d’une hémopathie (LAM et myélodysplasie). Trente-trois patients (49 %) présentaient des infections respiratoires répétées (> 2 épisodes) ayant nécessité une antibiothérapie, avec 2 pics d’incidence aux âges extrêmes : l’un dans l’enfance à 5 ans et l’autre à 50 ans. Parmi les 26 patients inclus via le PHRC, l’âge médian était de 27 ans [19–52]. Onze patients (42 %) présentaient des épisodes infectieux répétés dans l’enfance (entre 1 mois et 12 ans), dont 7 avec des infections respiratoires basses, avec une prédominance entre 5 et 8 ans, 2 avec des infections ORL, 2 ayant les 2 types d’infections. Ces épisodes infectieux récurrents ne persistaient à l’âge adulte que pour 1 patient. Six patients étaient vaccinés contre l’haemophilus, 1 seul contre le pneumocoque. Comparés aux témoins, les patients présentaient une lymphopénie relative (lymphocytes totaux 1 508 vs 2 170/mm3, p = 0,002), prédominant sur le compartiment lymphocytaire B (73 vs 197/mm3, p < 0,001) et portant particulièrement sur les LB mémoires switchés (10 vs 26/mm3, p < 0,01), mais sans baisse des plasmocytes (3,72 vs 3,44/mm3, p > 0,05). Les lymphocytes immatures atypiques CD21lowCD38low (décrits chez les enfants T21 et au cours de certains patients atteints de déficit immunitaire commun variable avec auto-immunité) n’étaient pas augmentés (1,3 vs 2,8/mm3). Les taux d’IgG étaient de 14,5 g/L (10,5–21,2 g/L), avec une augmentation des IgG1 et IgG3 (10,5 et 1,5 g/L respectivement) telle que décrite chez l’enfant T21, sans anomalies pour les autres sous-classes d’immunoglobulines. Des anomalies immunobiologiques étaient retrouvées chez 12 patients (46 %) : des anticorps antinucléaires (> 1/160e) chez 10, une hypergammaglobulinémie (> 15 g/L) chez 4, sans arguments cliniques pour une maladie auto-immune. Concernant les 42 patients inclus rétrospectivement via le PMSI, l’âge médian était supérieur à celui du groupe du PHRC (51 vs 27 ans, p < 0,05) avec un taux d’institutionnalisation de 60 %. Les principaux motifs d’hospitalisation étaient une infection respiratoire (n = 25), une infection urinaire (n = 3), un syndrome occlusif (n = 3), une crise comitiale (n = 2). Parmi les patients inclus, 16 (38 %) présentaient des pneumopathies d’inhalations répétées, avec un âge médian de 51 ans [26–55] lors du premier épisode, dont 13 étaient atteints d’affections neurologiques (épilepsie, n = 10 ; démence, n = 9) apparaissant à 50 ans [1–63]. Sept présentaient des pneumopathies aiguës communautaires à 52 ans [19–73] pour le premier épisode, dont 3 souffraient de malformations cardiaques et 2 de dilatations des bronches. Les taux de lymphocytes et de gammaglobulines n’étaient pas différents dans ces 2 groupes. Au total, on comptait 8 décès dont 6 d’origine infectieuse respiratoire, avec un âge médian au décès de 55 ans [18–73]. Malgré la persistance d’anomalies immunobiologiques à l’âge adulte, la susceptibilité aux infections décrite chez l’enfant T21 n’est pas retrouvée dans notre cohorte. Il existe une recrudescence des infections respiratoires après 50 ans qui semble être davantage la conséquence des comorbidités (principalement neurologiques) que le résultat d’un déficit immunitaire et qui représente la principale cause d’hospitalisation et de décès.
Allergen‐specific serum immunoglobulin E detection and quantification have become an important step in allergy diagnosis and follow‐up. In line with the current trend of laboratory test accreditation to international standards, we set out to design and assess an accreditation procedure for allergen‐specific serum IgE.

RATIONALE: Few studies have performed nasal and bronchial allergen challenges in the same patients. The aim of our study was to assess the effect of the allergen delivery route (nasal and bronchial) on plasma cytokine levels in grass pollen sensitized patients. METHODS: Twenty-one subjects with seasonal grass pollen rhinitis and/or intermittent asthma underwent outside the grass pollen season one nasal provocation test (NPT) and one bronchial provocation test (BPT) with a grass pollen extract in a randomized order. BPT, performed by using a dosimeter jet nebulizer (Spira Electro 2) producing 10 μm particles, was considered positive when FEV 1 dropped by 20%. NPT, performed by means of a nasal pump spray in both nostrils, was considered positive when nasal flow measured by anterior-rhinometry dropped by 40% or symptom score was ≥ 5. Plasma cytokine levels (IL-4, IL-5, IL-10, IL-2, TNF-α and IFN-γ) were measured before and 3, 6 and 24h after allergen challenge by flow cytometry using a bead array immunoassay. RESULTS: A significant increase in plasma levels of IL-5 was found 24 hours following NPT (0.78 to 2.50 pg/ml; p=0.008) and 6 and 24 hours following BPT (0.95 to 40.69 pg/ml; p=0.029). The increase of IL-5 after BPT was significantly higher than after NPT (p=0.034). IL-4, IL-10, IL-2, TNF-α and IFN-γ plasma levels were undetectable in most of the samples. CONCLUSIONS: Our results suggest that introducing allergens through the bronchi induces significantly higher concentrations of IL-5 than through the nose.
INTRODUCTION:Deficiencies in components of the classical pathway of complement activation are strong risk factors for lupus erythematosus (LE).Yet, it has not been addressed whether the conventional measurements of the serum hemolytic CH50 activity and antigenic concentrations of C3 and C4 are sufficient to asses a deficiency in C4A, C4B or C2 components, the most common deficiencies associated with LE. PATIENTS AND METHODS:In a retrospective series, we performed complement analyses in 35 patients with LE who were systematically screened for a complement deficiency. The majority of patients had cutaneous LE with mild systemic involvement and no complement consumption. Of 25 patients (72%) with complement deficiency we found 13 with a partial C4A deficiency, 2 with a complete C4A deficiency, 6 with a partial C4B deficiency, 2 with a complete C4B deficiency and 2 with a combined partial C2 and C4A deficiency. RESULTS:The total complement activity (CH50) was decreased in only one out of two patients with complete C4B deficiency. CH50 level was found to be low-normal (35-38 U/ml(-1)) in one patient with partial C4B deficiency, one patient with complete C4B deficiency and both patients with combined partial C4A and C2 deficiency. Total C4 levels were normal in 9 out of 13 the patients with a partial C4A deficiency and in 2 out of 6 patients with a complete C4B deficiency. The antigenic concentration of C3 was low in only 1 patients with a complete C4B deficiency and within the normal range in all the others patients. Overall, 50% of the patients had normal or elevated C3, C4, and CH50 levels. DISCUSSION:This study emphasizes that the usual measurements of CH50, C3 and C4 levels are not adequate to detect a C4 and/or C2 deficiency in patients with LE. In epidemiologic or investigative studies addressing the prevalence of complement deficiency, more elaborated diagnostic tests, such as C4 protein allotyping, C2 level measurement and genetic screening for type I C2 deficiency should also be performed.
