Visceral Pain is a common debilitating symptom of inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS). The endocannabinoid system (ECS) is a prime target for alleviation of visceral pain, given its important role in both gastrointestinal physiology and pain. We will conduct a systematic review of randomised controlled trials (RCTs) of cannabis, cannabinoids, cannabis-based medicines (CBMs), and other ECS modulators for patients with IBD and IBS, comparing any preparation of cannabis, any cannabinoid, CBM, or other pharmacological modulator of the ECS (any dose, by any route of administration), with any control (placebo, or pharmacological / psychological / dietary intervention). We will search CENTRAL, MEDLINE, PubMed, EMBASE, and Web of Science databases, as well as the WHO International Clinical Trials Registry Platform and ClinicalTrials.gov Trials Registries, together with reference checking and citation searching, following PRISMA guidelines. Our objectives are to evaluate the benefits and harms of pharmacological modulation of the ECS for visceral abdominal pain in patients with IBD or IBS, compared to placebo or other interventions. The primary outcomes will be the proportion of people with (a) at least a 30% reduction and (b) at least a 50% reduction in pain. Secondary outcomes will include any change in pain intensity, physical and emotional functioning, fatigue and sleep measures, quality of life, gastrointestinal disease or symptom severity, and adverse effects. We will assess risk of bias in the RCTs using the Cochrane Risk of Bias 2 tool. Where there are sufficient data that are directly comparable, we will conduct meta-analyses of the results for each outcome. We will use the GRADEpro GDT tool to assess certainty of evidence for each outcome. This review will synthesise the available evidence regarding all types of ECS modulation for the treatment of visceral abdominal pain and its related comorbidities in IBS and IBD patients.
Introduction Diabetes-related foot ulcers (DFUs) are a major complication of diabetes and are associated with high morbidity and mortality rates. Psychological factors have been found to play a role in wound healing (Frasier et al., 2024; Robinson et al., 2017; Walburn et al., 2009) but it remains uncertain if psychological interventions can help individuals with an active DFU, or a history of DFUs, to achieve complete or improved wound healing or prevent recurrence. Objective A recent Cochrane review by McGloin et al. (2021) examined existing psychological interventions for individuals with DFU and emphasized the need for more interventions and stronger study designs to evaluate their efficacy. Building on this, the proposed scoping review aims to: 1) Investigate the emotional consequences and burdens of living with DFU, and 2) Examine how psychosocial factors influence the progression and management of ulcerations. Methods This review will be conducted in accordance with the Joanna Briggs Institute methodology for scoping reviews and the Preferred Reporting Items for Systematic reviews and Meta-Analysis extension for Scoping Reviews (PRISMA-ScR) will guide the reporting of results. Discussion While factors such as stress, depression and social support have been identified as variables that may negatively affect DFU healing, there is a need for a greater understanding of how variables such as these may influence progression and recurrence. This review will comprise of a broad and systematically mapped synthesis of the identified data. Following this review, qualitative studies will be conducted, and their findings, along with those from the scoping review, will inform the development of a psychological intervention that will aim to improve psychosocial functioning and mitigate the progression and recurrence of DFUs.
Chronic conditions are extremely common, with approximately 1 million people in Ireland currently affected by the four most common chronic conditions alone. This is expected to significantly increase in the near future due to Ireland’s aging population. Identifying the priorities of patients, carers, and healthcare professionals for primary care research in chronic condition management could ensure future work is relevant and that resulting service changes and policy decisions align with the needs of those most affected. An initial survey to collect potential research questions about the management of all chronic diseases was shared with patients, carers, and healthcare professionals from March to May 2023. Submissions were sorted and checked against existing evidence resulting in a list of 30 unanswered questions. An interim priority setting survey was shared in late 2023, and a final workshop to rank the top ten research priorities took place in January 2024. The first survey resulted in 350 individual statements (n = 185 participants). Seventy-three respondents had a chronic disease and 72 were primary healthcare professionals. Rankings were informed by an initial priority setting survey (n = 108), followed by an in-person workshop (n = 16) to decide the final order. The resulting top ten research priorities offer a starting point for funding bodies and researchers to ensure that future primary care research in chronic condition management is relevant, meaningful, and impactful. The top ten priorities reflect current Irish and global challenges in healthcare, with top items including communication, non-pharmacological treatments, and multidisciplinary care. Low back pain (LBP) is a major unmet clinical need. The endocannabinoid system (ECS) has emerged as a promising therapeutic target for pain, including LBP. This review examines the evidence for the ECS as a therapeutic target for LBP. While preclinical studies demonstrate the potential of the ECS as a viable therapeutic target, clinical trials have presented conflicting findings. This review underscores the need for innovative LBP treatments and biomarkers and proposes the ECS as a promising avenue for their exploration. A deeper mechanistic understanding of the ECS in LBP could inform the development of new pain management strategies.

Findings will be used to provide a better understanding of the bi-directional relationship between DFU and psychological variables and will provide direction for the development or adaptation of a tailored psychological intervention that will aim to optimise wellbeing and improve outcomes for individuals with DFU.
This international multicenter randomized controlled trial aimed to compare the effectiveness of virtual reality (VR) distraction with an identical non-VR game in reducing needle-related pain and anxiety in children undergoing venous blood draw. The study involved 304 children aged 5-9 years undergoing a blood draw procedure, randomly allocated to one of three groups: VR distraction, non-VR distraction, and control group (usual care). The distraction task was based on the Multiple Object Tracking (MOT) paradigm, and the game was identical in design and gameplay for both VR and non-VR distraction groups. The primary outcome was self-reported pain intensity using the Faces Pain Scale-Revised (FPS-R). Secondary outcomes included child distress, attention/distraction to the blood draw, and parent and medical staff satisfaction with procedure. Analyses were conducted using analysis of variance and multivariable linear regression models. The results showed that VR distraction and non-VR distraction performed similarly, showing large effect sizes compared with standard care. There was no significant difference between the two types of distraction. The study's findings suggest that VR and non-VR distraction are similarly effective in reducing needle-related pain and anxiety in children undergoing venous blood draw. This is the first well-powered study comparing modern VR distraction with an identical task displayed on a smartphone or monitor screen. The study's results have important implications for using VR in clinical settings and suggest that investing in expensive VR equipment for acute pain management may not be necessary. BACKGROUND:Evidence suggests that women's abdominal pain is more likely to be minimised or dismissed by healthcare professionals than men's. This can have a detrimental impact on health-related outcomes as well as quality of life. The aim of this study was to explore women's experiences of seeking healthcare for abdominal pain in Ireland. METHOD:A qualitative design and opportunity sampling approach were employed in this study. Fourteen women living in Ireland with experience of seeking healthcare for abdominal pain took part in one-to-one semi-structured interviews via video-conferencing software. Data were analysed using reflexive thematic analysis. RESULTS:Four themes were constructed from the data: [1] "Just Get on with It" - Normalisation and Invalidation; [2] "Bad Enough"? Costs of (Not) Seeking Help; [3] "Fight Your Case," Fight for Care; and [4] "Out of the Loop" - Systemic Barriers to Care. Perceived invalidation of pain by healthcare professionals was common, as was internalised normalisation of pain. This created challenges when negotiating pain management solutions. Despite functional interference, participants felt their pain needed to reach an extreme level of severity before seeking help. Costs of private healthcare were implicated in delayed help-seeking. Participants felt the onus was on them to fight for care. Social support and information-seeking facilitated participants in this fight while systemic issues were identified as barriers to adequate care. Despite their frustrations, participants expressed empathy for healthcare professionals operating in a flawed system. CONCLUSIONS:Participants described mostly negative experiences of seeking healthcare for abdominal pain, characterised by dismissal of symptoms and internalisation of normative views of women's pain as less worthy of care. These experiences reinforced participants' views that self-advocacy is essential to access care for their pain. There are systemic issues at play within the Irish healthcare system that limit women's ability to access abdominal pain management support. Education and training for healthcare professionals on the Gender Pain Gap and its implications for patient care, as well as clear referral pathways for women presenting with abdominal pain, may help to ensure more equitable healthcare delivery for individuals with abdominal pain in Ireland.
ABSTRACT:Phantom limb pain (PLP) represents a significant challenge after amputation. This study investigated the use of phantom motor execution (PME) and phantom motor imagery (PMI) facilitated by extended reality (XR) for the treatment of PLP. Both treatments used XR, but PME involved overt execution of phantom movements, relying on the decoding of motor intent using machine learning to enable real-time control in XR. In contrast, PMI involved mental rehearsal of phantom movements guided by XR. The study hypothesized that PME would be superior to PMI. A multicenter, double-blind, randomized controlled trial was conducted in 9 outpatient clinics across 7 countries. Eighty-one participants with PLP were randomly assigned to PME or PMI training. The primary outcome was the change in PLP, measured by the Pain Rating Index, from baseline to treatment cessation. Secondary outcomes included various aspects related to PLP, such as the rate of clinically meaningful reduction in pain (CMRP; >50% pain decrease). No evidence was found for superiority of overt execution (PME) over imagery (PMI) using XR. PLP decreased by 64.5% and 68.2% in PME and PMI groups, respectively. Thirty-seven PME participants (71%) and 19 PMI participants (68%) experienced CMRP. Positive changes were recorded in all other outcomes, without group differences. Pain reduction for PME was larger than previously reported. Despite our initial hypothesis not being confirmed, PME and PMI, aided by XR, are likely to offer meaningful PLP relief to most patients. These findings merit consideration of these therapies as viable treatment options and alternatives to pharmacological treatments.
BACKGROUND:Despite the increased risk for people with an intellectual disability developing dementia, post-diagnostic psychosocial supports such as cognitive stimulation therapy (CST) are not routinely offered and there is limited research examining this intervention with people with intellectual disabilities. The aim of this study was to explore the feasibility of CST for older adults with intellectual disability to support active ageing. METHODS:Five client participants attended a 14-session CST group and four staff attended a focus group. Reflexive thematic analysis was used to investigate the client and staff narratives. RESULTS:Three key themes were generated: (1) Brain Health, (2) Connecting with others, and (3) Barriers and Enablers. CONCLUSION:Findings indicated the suitability of CST as a way of supporting active ageing for older adults with intellectual disability. This study adds to the growing knowledge about service provision for older adults and their changing needs as they age and identifies clinical implications such as staff training to support intervention adherence.
PURPOSE:To explore women's lived experiences of intimacy, sexuality and sexual functioning in the context of cauda equina syndrome (CES). METHODS:Ten women completed an in-depth semi-structured interview exploring the psychosocial impact of CES on their sexuality and data were analyzed using Interpretative Phenomenological Analysis (IPA). RESULTS:One superordinate theme was generated: "Sexuality forever altered, it's changed completely," reflecting the enduring impact of CES on women's sexuality. Five subordinate themes emerged reflecting a range of changed roles and relationships, primarily associated with loss of sexual identity and a sense that health professionals did not prioritize sexual functioning (1): Loss of worth as a sexual being: "Feel like a nothing" (2) Relationships have changed: "It's not a meeting of equals anymore" (3) Lack of professional support: "We don't like to talk about anything below the waist" (4) Challenges for motherhood: "What kind of future will they have with a mother like me…" and (5) Finding ways of coping: "Closed the door on it." CONCLUSION:Findings highlight the detrimental effect of CES, directly and indirectly, on women's sexuality. There is a need for a multidisciplinary approach to address the physical, emotional, cognitive, and behavioural effects experienced in relation to sexuality as part of a rehabilitative process.
AimsPsychological care is recognised as an integral part of quality diabetes care. We set out to describe the roles and competencies of the clinical psychologist as a member of the multidisciplinary adult diabetes care team, focused on secondary care.MethodsThe authors are clinically experienced psychologists involved in adult diabetes care, from Australia, Europe and North America, and active members of the international psychosocial aspects of diabetes study group. Consensus was reached as a group on the roles and competencies of the clinical psychologist working in adult diabetes secondary care, building both on expert opinion and a selective review and discussion of the literature on psychological care in diabetes, clinical guidelines and competency frameworks.ResultsThe clinical psychologist fulfils multiple roles: (1) as a clinician (psychological assessment and therapy), (2) as advisor to the healthcare team (training, consulting), (3) as a communicator and promotor of person-centred care initiatives and (4) as a researcher. Four competencies that are key to successfully fulfilling the above-mentioned roles in a diabetes setting are as follows: (a) specialised knowledge, (b) teamwork and advice, (c) assessment, (d) psychotherapy (referred to as STAP framework).ConclusionsThe roles and competencies of clinical psychologists working in diabetes extend beyond the requirements of most university and post-graduate curricula. There is a need for a comprehensive, accredited specialist post-graduate training for clinical psychologists working in diabetes care, building on the proposed STAP framework. This calls for a collaborative effort involving diabetes organisations, clinical psychology societies and diabetes psychology interest groups.
Despite the growing international move away from the term 'mental age', the term continues to be used in some settings with adults with an intellectual disability. Arguably, the construct of 'mental age' conflicts with current rights-based approaches yet its continued use suggests that it may have certain utilities. This study aimed to explore the use of the construct of mental age in the context of supporting adults with an intellectual disability in Ireland, and its perceived value and limitations in clinical practice. Forty-three psychologists in Ireland responded to a 10-item online survey using a mix of closed and open-ended questions. Descriptive statistics were used and analysis was informed by reflexive thematic analysis. Twenty of the 43 respondents reported that mental age continues to be used in clinical practice. Analysis was informed by reflexive thematic analysis and identified five main themes: (1) negative connotations, (2) inconsistent with best practice, (3) supporting the person with an intellectual disability, (4) communicating about the person with intellectual disability and (5) moving on from mental age. The findings suggest that this construct is considered problematic and limited in its meaning and while there is a desire for change, there are challenges in finding an alternative method of communicating information about ability in a brief and easily understood way.
Objective: Alexithymia, a deficit in identifying and describing feelings, is prevalent in traumatic brain injury (TBI). Sometimes referred to as "emotional unawareness," we sought to investigate whether alexithymia after TBI was related to, or distinct from, impaired self-awareness (ISA) and whether the two predicted differentiable emotional and aggression profiles. Further, the mediating role of frontal system behaviors (disinhibition, dysexecutive function, apathy) was explored. Method: Participants with TBI (N = 40) from diverse backgrounds completed self-report measures of alexithymia, emotional distress, aggression, and frontal system behaviors. For the assessment of ISA, significant other ratings were obtained to identify discrepancies from self-ratings. Data were analyzed quantitatively using independent samples t tests, correlations, partial correlations, and simple mediation. Results: There was a negative correlation between alexithymia and ISA. Alexithymia, but not ISA, was associated with higher expressions of emotional distress and aggression even after controlling for the effects of ISA via partial correlations. Exploratory analyses found that frontal system behaviors mediated the relationships between alexithymia and aggression and alexithymia and emotional distress. Conclusions: Alexithymia is more accurately conceptualized as an emotional processing deficit than an awareness deficit. Indeed, self-awareness may be a prerequisite for the ability to identify alexithymic tendencies. Negative psychological effects of alexithymia are compounded by poorer executive function and disinhibition and call for the development of TBI-specific alexithymia screening tools and interventions. Alexithymia interventions are best delivered in conjunction with rehabilitation of emotion regulation and executive function.
BACKGROUND Technology is ubiquitous in all aspects of daily life and research interest in the topic of digital mental health interventions (DMHI) for people with intellectual disabilities is growing. In addition, the COVID-19 pandemic has accelerated technology use in delivering mental health supports. METHOD This review used a qualitative systematic literature review methodology with the aim of collating the views and experiences of adults with intellectual disabilities accessing DMHI. The electronic databases MEDLINE, PsycINFO, CINAHL and SCOPUS were searched in March 2022. RESULTS Five studies were selected in this review, and three major narrative themes were generated, each with sub-themes: (1) User experience with DMHI (personal experience, digital experience); (2) Relationships fostered through technology (interpersonal support, relating to virtual characters, computer as skilled helper); (3) Psychological benefits (new skills development, personal autonomy development). CONCLUSION This review highlights that, although there is a diversity of views and experiences of people with intellectual disabilities, the evidence suggests that DMHI has potential to support this population given the right opportunities. Findings also point to the benefit of technology as a complement to therapy, but not necessarily as a replacement to in-person therapeutic encounters.
BackgroundIndividual responses to behavioural weight loss interventions can vary significantly, and a better understanding of the factors associated with successful treatment might help to target interventions for those who will benefit the most. We sought to identify demographic and clinical characteristics that predicted intervention “success” (defined as ≥5% weight loss) and other health gains in patients with severe obesity attending a ten-week structured lifestyle modification programme.MethodsWe conducted a prospective cohort study of all 1122 patients (751 (66.9%) female, mean age 47.3 ± 11.9 years, mean body mass index (BMI) 46.7 ± 7.8 kgm-2) referred from our hospital-based obesity clinic, who started the structured lifestyle programme between 2012-2019. We compared routine clinical measures such as weight, fitness, blood pressure, lipids and HbA1c at baseline and follow-up. We also used validated questionnaires to quantify anxiety, depression and health-related quality of life.ResultsOf 1122 patients who started, 877 (78.2%) completed the programme and attended for follow up. Of these, 12.8% lost ≥5% body weight. The amount of weight lost was a strong and consistent predictor of improvements in metabolic, cardiovascular, and mental health, even after adjusting for age, sex, programme attendance and baseline fitness. Older age, male sex, being physically active and having lower anxiety and depression scores at baseline predicted greater weight loss. Younger age, depression and longer wait time to start the intervention were associated with drop-out.ConclusionsIn adults with severe obesity completing a structured lifestyle modification programme, older age and good mental health were associated with programme completion and attaining ≥5% weight loss. The magnitude of weight lost was a strong predictor of improvements in cardiovascular, metabolic and mental health associated with programme completion.
Background: Anterior capsular tears are often associated with traumatic glenohumeral instability episodes; however, there are not many previous descriptions in the literature without occurrence of frank dislocation. This video will discuss the presentation of a throwing athlete with an anteroinferior capsular tear in his dominant shoulder who underwent successful arthroscopic repair. Indications: Based on the patient's clinical symptoms, magnetic resonance imaging arthrogram findings, and failure to return to play despite attempts at conservative management, he was determined to be a candidate for arthroscopic repair. Technique Description: Arthroscopic repair consisted of reapproximation of the avulsed capsular tissue to its attachment point medially using suture anchors in the glenoid and a side-to-side repair of the mid-substance extension of the capsular split. Results: Arthroscopic repair of the anteroinferior capsular tear allowed the patient to return to his previous level of sport. Discussion/Conclusion: In the present case and a previous case series, surgical repair of anterior capsular tears, whether arthroscopic or open, can yield good results in throwing athletes aiming to return to prior levels of performance.
