Purpose. To evaluate feasibility and potential of a three-dimensional diffusion-weighted Echo-Planar imaging in a Whole-body MRI protocol to improve the detection of lesions, particularly lymph nodes, as compared to PET/CT.Material and methods. 22 patients with various primary malignancies underwent tumor staging with whole-body PET/CT and whole-body MRI. our protocol began with a single-shot diffusion EPI sequence. A S-station coronal Fast STIR was acquired and finally, a S-stations 3D T1 weighted Fast Spoiled Gradient Echo with a spectral fat suppression (LAVA) following gadolinium administration was added.Results. 120 (PET/CT) and 165 (WBMRI) lymph nodes were identified in 15 patients (hypermetabolic in PET and in hyperintense signal in STIR and diffusion with MRI). Complete concordance between whole-body MRI and PET/CT occurred in 10 of these 15 patients (66 %). Concordant N staclification occurred in 14 (95 %) of the 15 patients. In 1 patient, retroperitoneal and inguinal lymph nodes considered as non-hypermetabolic in PET/CT, were positive with MRI (STIR and DWI) and histologically proven lymphomatous. 36 and 37 distant metastasis were detected respectively by MRI and PET/CT: in 1 patient (pulmonary adenocarcinoma), PET/CT revealed lesions one on clivus and one paravertebral C2, non clearly visible with MRI.Conclusion. MRI was as sensitive as PET/CT in the detection of lymph nodes. Axial acquisition is very helpful because coronal MRI sequences appear less sensitive in the analysis of coronal-oriented anatomical structures (clivus, scapula...). This emerging technique also opens the way to better characterization and therapeutic follow up.
Evaluer les résultats d’un protocole IRM corps entier avec 3D diffusion Echoplanar, STIR et LAVA après injection de gadolinium. Les résultats ont été comparés à ceux du TEP-CT, actuel gold standard pour les bilans d’extension. Quinze patients avec des tumeurs primitives variées ont bénéficié d’un bilan d’extension par TEP-CT et IRM corps entier. L’intervalle entre les deux procédures n’a pas excédé deux semaines. L’IRM débutait par une séquence diffusion single-shot EPI : 5 paliers de 30 coupes axiales chevauchées (épaisseur 7 mm), couvrant le corps, reconstruites en MIP. Suivait une séquence Fast STIR coronale en 5 paliers et une séquence 3D LAVA après administration IV de gadolinium (0,lmmol/kg). L’étude a été réalisée en double aveugle et les conclusions diagnostiques comparées pour évaluer le score TNM. Concordance des résultats de l’IRM et du TEP-CT dans 10 cas sur 15 (66 %). Deux métastases vertébrales et une infiltration médullaire diffuse ont échappé au TEP-CT. Cent vingt huit et 122 adénopathies recensées respectivement en IRM et TEP-CT. 36 et 37 métastases viscérales étaient détectées. L’utilisation d’une séquence diffusion avec haut gradient b dans un protocle d’IRM corps entier potentialise la détection des adénopathies. Cette procédure reste moins sensible pour l’analyse des structures anatomiques coronales (clivus, scapula…) et la base du crâne.
The medical and imaging data of a 59 year old male is reported. The patient presented with cough, fever, night sweats and weight loss, for which a final diagnosis of multivisceral tuberculosis (with peritoneal involvement and mycotic aneurysm) was made, by means of CT, MRI and laparoscopic findings.This disease is uncommon in developed countries with subjects lacking the usual risk factors. Because delayed treatment may be lethal, especially with such serious vascular complication, the authors review the value and limitations of CT and MRI along with the suggestive features.
Les auteurs rapportent le cas d’un homme de 59 ans, présentant un tableau clinique respiratoire initial associant toux irritative, fièvre, complété secondairement d’un syndrome abdominal douloureux dans un contexte d’atteinte marquée de l’état général et pour lequel le diagnostic final de tuberculose extra-pulmonaire (avec localisations cérébrales, péritonéale et aortique) a été porté sur les résultats du scanner, de l’IRM et de la laparoscopie. Si l’incidence de la tuberculose augmente dans les pays industrialisés, la forme extra-pulmonaire reste rare chez les sujets dénués de tout facteur de risque, son diagnostic ne devant pas être retardé, au risque de compromettre le pronostic, parfois même vital devant certaines localisations. Les auteurs présentent les résultats du scanner et de l’IRM, leur intérêt dans le bilan lésionnel et leurs limites quant au diagnostic étiologique.
Nous pouvons aujourd’hui, avec l’expansion de machines performantes, réaliser des IRM de l’ensemble du corps en un seul temps (Total body MR). Même si les indications ne sont à l’heure actuelle pas bien définies, les premiers résultats pratiques pourraient en élargir l’éventail. Nous présenterons, après une explication détaillée de la technique, la réalisation pratique de l’examen par le manipulateur, moment capital influençant la qualité finale de l’interprétation de l’examen. A travers différents exemples nous verrons quelle peut être la place de cette technique non-irradiante et non-invasive face aux modalités d’exploration de l’ensemble du corps telles que la scintigraphie corps entier au Tc99 m ou au PET. Malgré quelques difficultés de réalisation pratique cette technique, de part son inocuité, peut s’avérer être un moyen performant de localisation de certaines lésions. En conclusion nous tenterons de prédire quelle pourra être l’évolution de cette technique en analysant son incidence future sur différentes pathologies.

In the invasion stage of the disease maturation and sexual differentiation of schistosomules is restricted to portal vessels. Liver ultrasonography may show concomitant abnormalities.EXEGESIS:Eosinophilia was diagnosed in a 36-year-old man residing in Central Africa. Clinical examination, chest X-ray and biology were normal. Liver ultrasonography showed numerous hypoechoic lesions, up to 3 cm in diameter. Ultrasonography-directed biopsy demonstrated infiltration of both portal vessels and liver sinusoids, with sometimes microabcesses. No parasite was found in either blood, stool, or rectal biopsy. Immunoelectrophoresis and hemagglutination showed positive bilharziosis serology, whereas serology for other helminthiases was negative. Four months after treatment eosinophilia was still high, liver ultrasonography was normal, and hemagglutination was negative.CONCLUSION:The transient shistosomules maturation-related liver abnormalities detected by ultrasonography should be differentiated from the laying phase of the disease.

We report the case of a rare glial tumor of the conus medullaris classified as pilocytic astrocytoma in a young adult. This tumor is most commonly found in the brain. The imaging findings will be correlated with the findings at histology.;The literature will also be reviewed.
We report one case of massive inguino-scrotal bladder herniation which was responsible for an acute obstructive renal insufficiency. The different types of bladder hernias and their anatomic factors are described. The clinical - radiological findings and surgical management are discussed.
