Experimental findings in neonatal mice demonstrated massive apoptosis induced by phenobarbital (PB), which raised considerable concern about the use of PB in the treatment of seizures in neonates. We studied 28 children aged 8-14 years who had received PB as neonates. The children underwent the Kaufman ABC test, various questionnaires, and functional MRI. " Best friends " served as a control group. Some children had an underlying disease independent of PB medication. With the exception of five children, all attended a regular school, three even the higher branch of the German school system and four the middle branch. Ten children are athletes, five children have demanding hobbies, and four children suffer from ADHD (but attend a regular school). The fMRI findings revealed small differences between children treated with PB and healthy controls as to the activation of the right frontal gyrus. If massive apoptosis had been induced by PB, some kind of impairment should be expected. Our finding that even prematures < 1000 g (four children) can perform perfectly well in regular school contradicts the view that PB is as dangerous in humans as it is in rodents.
Introducing: An apoptotic effect of PB to the immature brain of neonatal rodents has been demonstrated by Bittigau et al. These oberservations raised considerable concern about the use of PB in the treatment of seizures in neonates. We have investigated the clinical sequelae of these findings in children at the age of 8–14 years.
Phenobarbital induziert bei Tieren neuronale Apoptose, was zu großer Verunsicherung hinsichtlich der Behandlung neonataler Krampfanfälle mit dieser Substanz führte. Wir untersuchten 28 Kinder im Alter zwischen 8 und 14 Jahren, die in der Neugeborenenphase Phenobarbital erhalten hatten, mit Hilfe von Kaufman-ABC, verschiedenen Fragebögen und Funktions-MRT-Untersuchung. Kontrollgruppe waren bester Freund/Freundin. Einige der Kinder hatten zusätzliche, von der Phenobarbitalmedikation unabhängige Erkrankungen. Mit Ausnahme von 5 Kindern besuchen alle eine Regelschule (Gymnasium: 3, Realschule: 4). 10 Kinder betreiben Vereinssport, 5 haben anspruchsvolle Hobbys, 4 Kinder leiden unter ADHD (Regelschule). Die fMRT-Untersuchung ergab eine kleine Minderaktivierung im rechten frontalen Gyrus bei der Phenobarbital- im Vergleich zur Kontrollgruppe. Wäre durch die Phenobarbitalapplikation eine massive neuronale Apoptose induziert worden, sollte diese Schädigung im Schulalter sichtbar sein. Unsere Nachuntersuchungen zeigen, dass selbst Frühgeborene <1000 g (4 Kinder) nach neonataler Phenobarbitalapplikation erfolgreich eine Regelschule besuchen, was der Annahme widerspricht, dass Phenobarbital für das menschliche Gehirn ebenso schädigend ist wie bei Ratten.
