The spatially and temporally resolved x-ray emission in the W7-AS stellarator is analyzed by three diagnostic systems. The first consists of two cameras using arrays of surface barrier diodes. A variety of absorber-foils can be used to change the spectral response of the system. This flexibility allows one to analyze impurities and magnetohydrodynamic (MHD) activity. The measurements are also used for the reconstruction of equilibrium parameters (e.g., Shafranov shift) and of the topology of MHD modes. The second system serves to monitor the electron temperature profile by using the two-foil method. Pairs of large ion-implanted Si diodes in combination with different foil combinations are used. In addition, the measurements provide routine informations about Zeff, plasma position and profile width. Finally, a compact camera system (ten cameras with planar pin-diode arrays and curved Be-foils, 320 viewing chords) inside the vacuum vessel allows one to obtain proper two-dimensional tomograms of the equilibrium flux surfaces and of mode structures without prior information. In addition to the camera systems spectral information is obtained from x-ray pulse height measurements with two Si(Li) detectors. These data are correlated with the x-ray profile measurements by an impurity radiation code. The contribution of x-ray measurements to the investigation of various physics issues is illustrated by selected examples.
At W7-AS, confinement properties are analysed and compared mainly with neoclassical predictions for quite different conditions. Low-density electron cyclotron resonance heating (ECRH) discharges allow access to the very-long-mean-free-path regime for electrons ( up to 6 keV) whereas pure neutral beam injections (NBI) and combined NBI/ECRH discharges at high density ( at ) lead to high performance ( up to 50 ms). Depending on the achieved temperatures, the experimental transport analysis in the plasma core is consistent with the neoclassical predictions. The experimentally observed `electron root' feature with strong is driven by the convective flux of ripple-trapped suprathermal electrons generated by the ECRH absorption. `Optimum' confinement is obtained in discharges with narrow density, but broad temperature profiles with steep gradients in the region of low densities and strong close to the plasma edge. The large radial electric fields, both positive and negative, strongly reduce neoclassical transport. The achieved temperatures, however, are limited by the strong temperature dependence of the neoclassical transport.
The CAS3D stability code package has been augmented by a code version (CAS3D3) by means of which the stable part of the ideal magnetohydrodynamic (MHD) spectrum may be analyzed for fully three-dimensional (3-d) configurations. Calculations far tokamak equilibria recover all the typical properties of the stable MHD spectrum, e.g. the existence of spectral gaps and gap modes. In contrast to the other parts of the code package the new code version includes the fluid compression term and a physical normalization. For this new code version one interesting held of application is the computational reconstruction of MHD activities (global and toroidal Alfven eigenmodes) which have been measured by the Mirnov-coil and soft-X-ray diagnostics in the magnetic confinement experiment W7-AS in neutral beam heated discharges. Computational analysis of two representative cases, one with very low shear case typical of the presence of GAE's and one with moderate shear case typical of the presence of TAE's, indicates that the numerical results from CAS3D3 and the experimentally measured frequency and spatial structure are in good agreement.
Reliability of Energetic Meridian Measurement with Prognos A(R) OBJECTIVE: Is energetic measurement of the Traditional Chinese medicine (TCM) meridians with Prognos A a reliable method? PATIENTS: 30 healthy volunteers from the staff of the Stauferklinik, Schwä bisch Gmü nd, Germany. DESIGN: Measuremeuts are taken 4 times from each person at all terminal locations of the meridians (24 points) on fingers and toes with Prognos A. Breaks between each measurement run lastet 2-5 min. The values of skin resistance are compared. RESULTS: Reliabilities of the single meridian measurements range between 0.44 and 0.82 with an overall reliability of 0.72. Lowest reliability is seen in yang -hollow organs, probably a due to their intense lability. Mediating of 4 single runs shows a reliability of 0.9. CONCLUSIONS: Our measurement of reliability confirms Prognos A as a reliable device of determinating skin resistance, especially with repeated measurements. Variations of the readings are probably due to the special quality of the meridian system; increased variations of special meridians seem to be explainable by their characteristic features defined in TCM.
Das umstrittene Krankenversicherungsgesetz fuÈ hrt dazu, dass die KomplementaÈrmedizin eine Chance erhaÈ lt. Im Abstimmungskampf gemachte Versprechungen werden damit umgesetzt; dies betrifft auch die Phytotherapie. Erstmals wieder seit Jahren wurden in letzter Zeit pflanzliche Heilmittel in die SpezialitaÈ tenliste (SL) aufgenommen. Ein neuer Teil wurde aufgenommen ± Teil III, KomplementaÈrmedizin. Eine Analyse der SMGP hat ergeben, dass einige wenige pflanzliche Heilmittel in der SL unter den SpezialitaÈ ten der Schulmedizin «uÈ berlebt» haben: Neben einigen Besonderheiten handelt es sich dabei zum groÈ ssten Teil um Laxantien und einige KamillenpraÈparate. Noch ist die Liste der SpezialitaÈ ten der KomplementaÈrmedizin nicht sehr gross und daher entsprechend uÈ bersichtlich. Die SMGP verfolgt die Neuaufnahmen mit Interesse und fuÈ hrt seit einigen Monaten eine separate Liste der PraÈparate, die bei einer Verschreibung von der Grundversicherung bezahlt werden muÈ ssen. Die aktuelle Liste (Stand Oktober 1998) wird in diesen Gesellschaftsmitteilungen publiziert, im Sinne einer Dienstleistung an die Miglieder (Tab. 1). ErgaÈnzungen werden in Zukunft ebenfalls publiziert; die aktuellste Liste ist jeweils auf der SMGPHomepage per Internet abrufbar. Die im neuen Teil III der SL publizierten PraÈparate sind vorlaÈufig provisorisch aufgenommen bis 1999. Der Ausschuss fuÈ r KomplementaÈrmedizin der EidgenoÈ ssischen Arzneimittelkommission (EAK) bemuÈ ht sich zur Zeit um die Ausarbeitung der Kriterien zur definitiven Beurteilung der komplementaÈrmedizinischen PraÈparate. Dieser Ausschuss besteht aus 4 Personen mit komplementaÈrmedizinischen Fachkenntnissen (2 EAK-Mitglieder, 2 Expertinnen), 1 Vertreter der Krankenkassen, 1 Vertreter der pharmazeutischen Industrie, 1 Vertreterin der Patientenorganisationen und den Mitgliedern der Verwaltung (Bundesamt fuÈ r Sozialversicherung, IKS, Kantone, PreisuÈ berwacher). Der Ausschuss hat den klaren Auftrag, komplementaÈrmedizinische SpezialitaÈ ten, welche die Anforderungen erfuÈ llen (wirksam, zweckmaÈssig und wirtschaftlich), in die SL aufzunehmen. Da jedoch in den Auschusssitzungen die unterschiedlichen Betrachtungsweisen der BefuÈ rworter und Gegner der KomplementaÈrmedizin immer wieder aufeinanderprallen, gestaltet sich der Entscheidungsprozess z. T. recht schwierig. Mit jedem neuen Aufnahmeantrag fuÈ r eine SpezialitaÈ t muÈ ssen die Kriterien entsprechend der jeweiligen Therapieform wieder neu definiert werden. Dabei stellen die pflanzlichen Heilmittel im allgemeinen eine der einfacheren und meist fuÈ r allle verstaÈndlichen PraÈparategruppen dar. Es bleibt zu hoffen, dass moÈ glichst viele Firmen ihre SpezialitaÈ ten fuÈ r die Aufnahme in die SL anmelden. Anerkennung fanden zuletzt nicht nur pflanzliche Heilmittel, sondern auch die Phytotherapie als solche. Das EidgenoÈ ssische Departement des Innern hat, gestuÈ tzt auf die VorschlaÈge der EidgenoÈ ssischen Leistungskommission, den Pflichtleistungskatalog der obligatorischen Krankenversicherung angepasst. Leistungen der Phytotherapie werden ab dem 1. Juli 1999 kassenpflichtig. Diese Leistungen duÈ rfen von Ørztinnen und Ørzten erbracht werden, deren Weiterbildung in dieser Disziplin durch die Verbindung der Schweizer Ørzte anerkannt ist. Die SMGP hat ihr Ausbildungskurrikulum schon vor laÈngerer Zeit bei der FMH eingereicht. Ein Fertigkeitsprogramm «Phytotherapie» ist in Bearbeitung und kann zu Beginn des naÈchsten Jahres verabschiedet und publiziert werden. Bis dahin wird auch definiert sein, wie solche phytotherapeutischen Leistungen aussehen werden. SMGP-Mitglieder, die das bisherige SMGPZertifikat bereits erworben haben, werden unter Nachweis einer seither erfolgten Weiterbildung den Fertigkeitsausweis problemlos erhalten. Sowohl die Aufnahme von SpezialiaÈ ten in die SL als auch die Aufnahme der Phytotherapie in die Leistungspflicht sind vorlaÈufig befristet. Eine Evaluation steht bevor, deren primaÈres Ziel es natuÈ rlich ist, zu zeigen, dass phytotherapeutische Massnahmen kostenguÈ nstig sind. Diesen Bereich gilt es im Auge zu behalten. Dass bei der Medikation mit Phytopharmaka ein Sparpotential vorhanden ist, hat die in den Gesellschaftsmitteilungen vor einem Jahr publizierte SMGP-Preisstudie gezeigt. Wie sehr befuÈ rchtet wird, dass mit der Anerkennung komplementaÈrmedizinischer Verfahren ± zu diesen wid die Phytotherapie in der Schweiz gezaÈhlt, wohingegen in Deutschland die Gesellschaft fuÈ r Phytotherapie sich als Bestandteil der Schulmedizin erklaÈrt ± die Kosten im Gesundheitswesen steigen, zeigt die im Vergleich zum Gesamtprojekt mit hohem Aufwand gefoÈ rderte NFP-34-Studie «KomplementaÈrmedizin in der Krankenversicherung» (NFP 34: «Nationales Forschungsprogramm KomplementaÈrmedizin», das vom Schweizerischen Nationalfonds betreut wird). Die Studie kommt zum Schluss, dass die KomplementaÈrmedizin kein Ausweg aus dem Teufelskreis der Kostenspirale ist. Die SMGP definiert die AusuÈ bung der Phytotherapie allerdings anders, als dies in der Studie getan wurde. Hier wurden saÈmtliche Angebote sogenannter KomplementaÈrmedizin mit einbezogen, da die Helsana, mit deren Daten die Studie durchgefuÈ hrt wurde, in der Zusatzversicherung auch NaturaÈrzte und Personen mit Hilfsberufen der Alternativmedizin als Leistungserbringer anerkennt. Die SMGP hingegen erachtet es als sinnvoll, ausschliesslich die von ØrztInnen ± und in zuÈ kuÈ nftigen Modellen allenfalls von ApothekerInnen ± erbrachten Leistungen in die Grundversicherung aufzunehmen. Dass jede zusaÈtzliche Erweiterung zu Kostensteigerungen fuÈ hrt, ist naheliegend.

Reliability of Mandel's Energy Emission Analysis (EEA) in Sick PersonsOBJECTIVE: Is the Energy Emission Analysis (EEA) according to Mandel a reliable diagnostic instrument in severely diseased patients? PATIENTS: 30 patients of the Stauferklinik Schwäbisch Gmünd, Germany, suffering from a great variety of diseases. STUDY DESIGN: At regular intervals of 10-15 min 4 Kirlian photos of fingers and toes were taken from each patient. An experienced rater blindly assessed all photos according to the EEA sectors with respect to remarkable phenomena. Additionally he defined the most affected circles of functions of Traditional Chinese Medicine (TCM). RESULTS: The circles of functions of kidney/bladder and spleen/pancreas/stomach appeared to be the most affected in nearly all photos, the reliability is estimated by a rather low intraclass-kappa of 0.14 (95% CI: 0.00-0.29, p = 0.0279. Reliabilities of single EEA sectors also are low and similar to those of healthy persons. CONCLUSIONS: The reasons for the moderate reliability may be attributed to fluctuations of Chi, increasing regulation rigidity of diseased persons or simply to technical limitations of the method.Possibly new realtime Kirlian devices can provide additional information.
Fragestellung: Handelt es sich bei der ETD nach Mandel um eine reliable diagnostische Methode? Patienten: 12 freiwillige gesunde Probanden aus dem Personal der Stauferklinik, Schwäbisch Gmünd. Studiendesign: In Abständen von ca. 10–15 min werden hintereinander jeweils 10 Kirlian-Fotos von alien Fingern und Zehen angefertigt. Die einzelnen Sektoren der ETD nach Mandel werden von einem erfahrenen Rater verblindet befundet. Ergebnisse: Die Befundung der einzelnen Sektoren der ETD zeigt eine überzufällige Reliabilität auf, die mit einem Intraklass-Kappa von 0,30 allerdings deutlich kleiner ist als bei konventionellen diagnostischen Verfahren. An den Füssen sind höhere Reliabilitäten als an den Händen zu erkennen. Die Berücksichtigung komplexerer Zusammenhänge zwischen den Sektoren bringt keine erhöhten Reliabilitätskoeffizienten. Schlussfolgerungen: Die Reliabilität ist möglicherweise deshalb relativ gering, da bei den gesunden Probanden die energetischen Belastungen klein sind und im physiologischen Rauschen untergehen. Zusätzlich handelt es sich um Momentaufnahmen eines fluktuierenden Phänomens, das zudem durch wechselnde Aufnahmepositionen verändert wird. Untersuchungen an Kranken könnten weitere Informationen geben.
In order to study impurity transport and radiation behavior nitrogen has been injected into the scrape-off plasma of limiter-dominated discharges by gas puffing or into natural magnetic islands at the plasma edge of separatrix-dominated discharges by a reciprocating erosion probe. Strong radiative plasma edge cooling with a reduction of the power flux to the limiter could be obtained. The accompanied degradation of the energy confinement and the observed decrease of the central electron temperature can partly be explained by the reduction of the effective heating power. At high plasma densities and sufficiently strong impurity injection phenomena well known from tokamaks, like plasma shrinking, plasma detachment from the limiters and MARFE's were transiently observed.
In the presence of fast particle populations marginally stable global modes in the shear Alfven branch can be destabilized by wave particle resonances. This is particularly of concern in future large devices, where losses of resonant particles ({alpha}-particles in a reactor) may then limit the available heating power and also may cause damage of the first wall. In tokamaks TAE modes inside toroidicity induced gaps of the shear Alfven continua have been found. In stellarators with very weak shear like W7-AS low-n TAE-gaps do not occur but gaps below the shear Alfven continua with mode numbers m and n, if the resonant values {tau} = n/m do not exist in the plasma volume (k{sub {parallel}} = (m{sm_bullet}{tau} - n )/R {ne} 0 ). Under these conditions GAE modes with frequencies {omega}{sub GAE} < (k{sub {parallel}}{sm_bullet}V{sub A}){sub min} are the favoured modes. The investigation of GAE modes could also be of relevance in the case of advanced tokamak equilibria with flat or inverted q-profiles in the central region.
