Aristolochic acids (AAs) are powerful nephrotoxins that cause severe tubulointerstitial fibrosis. The biopsy-proven peritubular capillary rarefaction may worsen the progression of renal lesions via tissue hypoxia. As we previously observed the overproduction of reactive oxygen species (ROS) by cultured endothelial cells exposed to AA, we here investigated in vitro AA-induced metabolic changes by 1H-NMR spectroscopy on intracellular medium and cell extracts. We also tested the effects of nebivolol (NEB), a β-blocker agent exhibiting antioxidant properties. After 24 h of AA exposure, significantly reduced cell viability and intracellular ROS overproduction were observed in EAhy926 cells; both effects were counteracted by NEB pretreatment. After 48 h of exposure to AA, the most prominent metabolite changes were significant decreases in arginine, glutamate, glutamine and glutathione levels, along with a significant increase in the aspartate, glycerophosphocholine and UDP-N-acetylglucosamine contents. NEB pretreatment slightly inhibited the changes in glutathione and glycerophosphocholine. In the supernatants from exposed cells, a decrease in lactate and glutamate levels, together with an increase in glucose concentration, was found. The AA-induced reduction in glutamate was significantly inhibited by NEB. These findings confirm the involvement of oxidative stress in AA toxicity for endothelial cells and the potential benefit of NEB in preventing endothelial injury.
Aristolochic acid nephropathy (AAN) is a progressive tubulointerstitial nephritis caused by the intake of aristolochic acids (AA) contained in Chinese herbal remedies or contaminated food. AAN is characterized by tubular atrophy and interstitial fibrosis, characterizing advanced kidney disease. It is established that sustained or recurrent acute kidney injury (AKI) episodes contribute to the progression of CKD. Therefore, the study of underlying mechanisms of AA-induced nephrotoxicity could be useful in understanding the complex AKI-to-CKD transition. We developed a translational approach of AKI-to-CKD transition by reproducing human AAN in rodent models. Indeed, in such models, an early phase of acute tubular necrosis was rapidly followed by a massive interstitial recruitment of activated monocytes/macrophages followed by cytotoxic T lymphocytes, resulting in a transient AKI episode. A later chronic phase was then observed with progressive tubular atrophy related to dedifferentiation and necrosis of tubular epithelial cells. The accumulation of vimentin and αSMA-positive cells expressing TGFβ in interstitial areas suggested an increase in resident fibroblasts and their activation into myofibroblasts resulting in collagen deposition and CKD. In addition, we identified 4 major actors in the AKI-to-CKD transition: (1) the tubular epithelial cells, (2) the endothelial cells of the interstitial capillary network, (3) the inflammatory infiltrate, and (4) the myofibroblasts. This review provides the most comprehensive and informative data we were able to collect and examines the pending questions.

L’hypoxie tissulaire induite pourrait contribuer à la progression rapide des lésions tubulo-interstitielles.DescriptionVu la production d’espèces réactives de l’oxygène rapportée dans une lignée de cellules endothéliales humaines (EAhy926) exposées aux AA [Toxicol In Vitro 2020 ;65 :104811], le but de ce travail est d’investiguer in vitro les changements métabolomiques AA-induits et les effets cytoprotecteurs éventuels du nébivolol (NEB), un β-bloquant présentant des propriétés antioxydantes [J Hum Hypertens 2017 ; 31 :376].MéthodesLes cellules EAhy926 sont incubées en présence ou non de AA (50μM ou 100μM) et les effets d’une préincubation au NEB (10nM) sont évalués par le test de viabilité à la résazurine. La production d’H2O2 est mesurée par cytomètre en flux. Pour chaque condition expérimentale, les profils métabolomiques des extraits intracellulaires et des surnageants sont analysés par la spectroscopie 1H-RMN.RésultatsAprès 24h, les AA produisent des effets cytotoxiques marqués dose-dépendants et une production d’H2O2 nette, effets atténués par le NEB. Après 48h d’exposition aux AA, les principaux changements métabolomiques sont une diminution significative de l’arginine, du glutamate, de la glutamine et du glutathion ainsi qu’une augmentation significative de l’aspartate, de la glycérolphosphocholine et de l’UDP-N-acétylglucosamine. Le NEB inhibe significativement les changements en glutathion et glycérophosphocholine. Dans les surnageants des cellules intoxiquées, on observe une augmentation en glucose ainsi qu’une réduction en lactate et en glutamate, cette dernière étant significativement inhibée par le NEB.ConclusionCette étude confirme la toxicité des AA vis-à-vis de la cellule endothéliale et la cytoprotection partielle du NEB. L’analyse métabolomique corrobore l’implication du stress oxydatif dans les lésions endothéliales. Le NEB semble être une molécule intéressante à explorer dans la prévention de l’hypoxie rénale d’origine toxique.
Although cisplatin is used as a first-line therapy in many cancers, its nephrotoxicity remains a real problem. Acute kidney injuries induced by cisplatin can cause proximal tubular necrosis, possibly leading to interstitial fibrosis, chronic dysfunction, and finally to a cessation of chemotherapy. There are only a few nephroprotective actions that can help reduce cisplatin nephrotoxicity. This study aims to identify new prophylactic properties with respect to medicinal mushrooms. Among five Ganoderma species, the methanolic extracts of Ganoderma tuberculosum Murill., Ganoderma parvigibbosum Welti & Courtec. (10 µg/mL), and their association (5 + 5 µg/mL) were selected to study respective in vitro effects on human proximal tubular cells (HK-2) intoxicated by cisplatin. Measurements were performed after a pretreatment of 1 h with the extracts before adding cisplatin (20 µM). A viability assay, antioxidant activity, intracytoplasmic β-catenin, calcium, caspase-3, p53, cytochrome C, IL-6, NFκB, membranous KIM-1, and ROS overproduction were studied. Tests showed that both methanolic extracts and their association prevented a loss of viability, apoptosis, and its signaling pathway. G. parvigibbosum and the association prevented an increase in intracytoplasmic β-catenin. G. parvigibbosum prevented ROS overproduction and exhibited scavenger activity. None of the extracts could interfere with pro-inflammatory markers or calcium homeostasis. Our in vitro data demonstrate that these mushroom extracts have interesting nephroprotective properties. Finally, the chemical content was investigated through a phytochemical screening, and the determination of the total phenolic and triterpenoid content. Further studies about the chemical composition need to be conducted.
Aristolochic acid nephropathy (AAN) is characterized by interstitial fibrosis, proximal tubular atrophy, and hypoxia. A correlation between a reduced peritubular capillary density and the severity of fibrosis has been demonstrated. As calcium, redox and energetic homeostasis are crucial in maintaining endothelial cell function and survival, we aimed to investigate AA-induced disturbances involved in endothelial cell injury. Our results showed a cytotoxic effect of AA on EAhy926 endothelial cells. Exposure of aortic rings to AA impaired vascular relaxation to Acetylcholine (ACh). Increased levels of intracellular reactive oxygen species (ROS) were observed in cells exposed to AA. Pre-treatment with antioxidant N-acetyl cysteine inhibited AA-induced cell death. Superoxide dismutase resulted in restoring ACh-induced relaxation. An increase in intracellular calcium level ([Ca2+]i) was observed on endothelial cells. Calcium chelators BAPTA-AM or APB, a specific inhibitor of IP3R, improved cell viability. Moreover, AA exposure led to reduced AMP-activated protein kinase (AMPK) expression. AICAR, an activator of AMPK, improved the viability of AA-intoxicated cells and inhibited the rise of cytosolic [Ca2+]i levels. This study provides evidence that AA exposure increases ROS generation, disrupts calcium homeostasis and decreases AMPK activity. It also suggests that significant damage observed in endothelial cells may enhance microcirculation defects, worsening hypoxia and tubulointerstitial lesions.
Experimental aristolochic acid nephropathy is characterized by transient acute proximal tubule necrosis and inflammatory cell infiltrates followed by interstitial fibrosis and tubular atrophy. The respective role of T-cell subpopulations has never been studied in the acute phase of the mouse model, and was heretofore exclusively investigated by the use of several depletion protocols. As compared to mice injected with aristolochic acids alone, more severe acute kidney injury was observed after CD4 + or CD8 + T-cells depletion. TNF-alpha and MCP-1 mRNA renal expressions were also increased. In contrast, regulatory T-cells depletion did not modify the severity of the aristolochic acids induced acute kidney injury, suggesting an independent mechanism. Aristolochic acids nephropathy was also associated with an increased proportion of myeloid CD11b high F4/80 mid and a decreased proportion of their counterpart CD11b low F4/80 high population. After CD4 + T-cell depletion the increase in the CD11b high F4/80 mid population was even higher whereas the decrease in the CD11b low F4/80 high population was more marked after CD8+ T cells depletion. Our results suggest that CD4 + and CD8 + T-cells provide protection against AA-induced acute tubular necrosis. Interestingly, T-cell depletion was associated with an imbalance of the CD11b high F4/80 mid and CD11b low F4/80 high populations.
La néphropathie aux acides aristolochiques (NAA) se caractérise non seulement par une fibrose interstitielle et une atrophie tubulaire proximale mais également par une raréfaction des capillaires péri-tubulaires [1]. Les AA acheminés par la circulation sont internalisés au niveau de l’épithélium tubulaire par les transporteurs d’anions organiques (OAT) [2]. Une corrélation entre la sévérité des lésions rénales et la diminution de la biodisponibilité du NO a été démontrée in vivo [3]. Alors que la toxicité des AA sur la cellule tubulaire est bien établie, la toxicité au niveau endothélial est moins documentée. Vu le contact direct des AA avec l’endothélium des capillaires péri-tubulaires avant d’atteindre leur cible préférentielle, le but de notre travail a été d’évaluer les effets des AA sur deux types de cellules endothéliales en culture. Dans cette étude in vitro, des cellules endothéliales primaires de veine ombilicale humaine (HUVEC) et immortalisées (Eahy926) ont été incubées avec ou sans AA (50 μM ou 100 μM) afin d’évaluer par immunofluorescence (IF) l’expression du facteur de von Willebrand (vWF), de la vimentine et le nombre de noyaux (DAPI). Le comptage cellulaire a été effectué à l’aide du logiciel imageJ. La viabilité des cellules Eahy926, comparée à celle des cellules tubulaires proximales de rein humain (Human Kidney-2, HK-2), a été évaluée par le test à la résazurine. L’observation de marqueurs endothéliaux tels que le CD31 a été réalisée par la technique de Polymerase Chain Reaction (PCR). La cytométrie de flux a permis d’étudier le stress oxydatif (production de H2O2). Les cellules endothéliales ont présenté une sensibilité moindre à la toxicité des AA que les cellules cellules HK-2. Les AA diminuent de manière dose-dépendante la viabilité et le nombre des cellules endothéliales de types HUVEC et Eahy926. On observe en IF une légère augmentation de l’expression du vWF par les cellules exposées intoxiquées aux AA ainsi qu’une surexpression de vimentine. La PCR montre une diminution de l’expression du CD31. De plus, on mesure une augmentation de la production de H2O2. Nos résultats confirment une cytotoxicité dose-dépendante des AA sur les deux types de cellules endothéliales. Même si les cellules endothéliales semblent moins sensibles que les cellules HK-2 à la toxicité des AA, les altérations significatives observées sur les cellules endothéliales pourraient être suffisantes pour engendrer de l’hypoxie et entretenir/aggraver les lésions tubulo-interstitielles.

L’observation de marqueurs endothéliaux tels que le CD31 a été réalisée par la technique de polymerase chain reaction (PCR). La cytométrie de flux a permis d’étudier le stress oxydatif (production de H2O2) ainsi que l’évolution du calcium cytosolique marqué au Fluo-3. Les AA diminuent de manière dose-dépendante la viabilité des cellules endothéliales Eahy926. Un pré-traitement avec le nébivolol permet d’améliorer la viabilité des cellules Eahy926 intoxiquées aux AA pendant 24 h. La diminution de l’expression du CD31 induite par les AA est atténuée par le nébivolol. L’augmentation de la production intracellulaire d’H2O2 par les cellules intoxiquées aux AA est contrecarrée par le nébivolol. L’augmentation du calcium cytosolique induite par les AA à leur plus haute dose n’est pas inhibée par le β-bloquant. Nos résultats confirment une cytotoxicité dose-dépendante des AA sur la cellule endothéliale en culture. Un pré-traitement au nébivolol permet de réduire l’intensité du stress oxydatif généré par les AA, suggérant une approche thérapeutique prometteuse. D’un point de vue physiopathologique, nos résultats in vitro suggèrent que l’atteinte de l’endothélium vasculaire de l’interstitium rénal (capillaires péritubulaires) joue un rôle non négligeable dans la progression de la néphropathie aux AA. Ces données sont en cours d’investigation in vivo.
L’atteinte tubulo-interstitielle chronique de la néphropathie induite par les acides aristolochiques (AA) est caractérisée par une atrophie tubulaire et une fibrose interstitielle, mais aussi par une raréfaction significative des capillaires péri-tubulaires [1] , [2] . Le but de notre étude était d’évaluer les mécanismes impliqués dans la toxicité endothéliale et la dysfonction vasculaire induites par les AA I et II (mélange 50/50) sur les cellules endothéliales de la veine ombilicale humaine (HUVEC), les cellules endothéliales immortalisées Eahy926 et sur les anneaux aortiques de rat. La viabilité des cellules HUVEC a été mesurée par le test MTT. Le nombre de noyaux a été compté par le DAPI. L’expression de la molécule d’adhésion cellulaire PECAM (CD31) a été analysée par immunofluorescence. Le test de l’activité superoxyde dismutase (SOD) intracellulaire a été réalisé sur les cellules Eahy926 par une méthode enzymatique (Kit Cayman Chemical Company). Nous avons étudié les relaxations endothélium-dépendantes à l’acétylcholine (ACh) sur les anneaux d’aorte de rat incubés pendant 1 h avec 200 μM des AA, en présence ou en absence de SOD et de N-acétylcystéine (NAC). L’exposition aux AA entraîne une diminution significative du nombre total de cellules CD31 + (nombre total de cellules ( %) vs. contrôle (100 %) : 60,27 ± 13,50 avec AA 50 μM et 53,70 ± 8,82 avec AA 100 μM, respectivement). Le test MTT montre une toxicité dose-dépendante des AA (viabilité cellulaire ( %) vs. contrôle (100 %) : 70,26 ± 4,80 avec AA 50 μM et 12,50 ± 3,26 avec AA 100 μM, respectivement). L’activité de la SOD intracellulaire est augmentée en présence des AA et rétablie par la NAC. Les relaxations induites par l’ACh sont significativement diminuées dans les anneaux d’aorte de rat pré-incubés avec 200 μM d’AA : l’aire sous la courbe était de 170,9 ± 8,9 dans les contrôles vs 236,0 ± 8,7 en présence des AA ( p < 0,0001). La NAC (2,5 mM) n’a pas protégé les anneaux aortiques de l’altération des relaxations induite par les AA. Nos résultats montrent 1) une toxicité dose-dépendante des AA sur les HUVEC ; 2) une augmentation de l’activité de la SOD intracellulaire qui peut être antagonisée en présence de NAC ; 3) une diminution par les AA de la relaxation des anneaux d’aorte induite par l’ACh, diminution atténuée par la SOD mais pas par la NAC, suggérant que l’anion superoxide est responsable de la dysfonction endothéliale induite par les AA.
Myoglobinuric acute kidney injury (AKI) is a severe condition requiring early therapeutic strategies. Early recognition and treatment are crucial to reduce morbidity and mortality rate. Here, we report a kidney recipient with severe rhabdomyolysis and AKI secondary to parvovirus B19 infection. Initiation of hemodialysis with the super high-flux filter Theralite® (Gambro, cut-off 45 kDa, 2.1 m2) resulted in the clearance of myoglobin from 61 to 71% after 3 hours. Elimination rates of IL-6 and β2-microglobulin were ~ 30 - 64% and 55 - 71% after 3 hours, respectively. Renal graft function rapidly recovered. The place of this effective but expensive procedure still needs to be defined and validated in high-risk patients. .
ETHNOPHARMACOLOGICAL RELEVANCE:The world prevalence of kidney stones is increasing and plants are frequently used to treat urolithiasis. Pistacia lentiscus L, a plant which freely grows around the Mediterranean basin areas, is widely used for various pathologies. P. lentiscus has an important impact as it has economical value on top of its pharmacological interest. Decoctions of its aerial parts and/or resin are used to treat kidney stones. AIM OF THE STUDY:To in vitro assess the potential nephroprotective effect of Pistacia lentiscus ethanolic fruit extract (PLEF) on proximal tubular cells in response to the adhesion of calcium oxalate monohydrate (COM) crystals. MATERIALS AND METHODS:Human Kidney [HK]-2 cells were incubated with and without COM in the presence or absence of PLEF. Cell viability was measured by the resazurin assay. The expression of E-cadherin was analyzed by PCR. The extracellular production of H2O2 was measured by Amplex® Red H2O2 Assay. The numbers of detached or non-adherent COM crystals in the presence of PLEF were microscopically captured and counted using ImageJ software. The interaction of PLEF with COM and the effect of PLEF on crystal size were analyzed by flow cytometry. The spectrophotometric measurement of turbidity was performed for assessing the COM concentration. RESULTS:PLEF incubated with COM was able to increase the cell viability. The decrease of E-cadherin expression after incubation with COM was counteracted by PLEF. Overproduction of H2O2 induced by COM was also inhibited by PLEF. Observations using flow cytometry showed that interactions between PLEF and the COM crystals occurred. PLEF was also effective in reducing the particles size and in lowering COM concentration. CONCLUSION:Our data show that COM tubulotoxicity can be significantly reversed by PLEF -at least in part- via an inhibition of COM crystals adhesion onto the apical membrane. This early beneficial effect of PLEF needs to be further investigated as a useful strategy in nephrolithiasis prevention.
Background The platelet-derived growth factor receptor β (PDGFRβ)+ perivascular cell activation becomes increasingly recognized as a main source of scar-associated kidney myofibroblasts and recently emerged as a new cellular therapeutic target. Aims In this regard, we first confirmed the presence of PDGFRβ+ perivascular cells in a human case of end-stage aristolochic acid nephropathy (AAN) and thereafter we focused on the early fibrosis events of transforming growth factor β (TGFβ) inhibition in a rat model of AAN. Materials and Methods Neutralizing anti-TGFβ antibody (1D11) and its control isotype (13C4) were administered (5 mg/kg, i.p.) at Days -1, 0, 2 and 4; AA (15 mg/kg, sc) was injected daily. Results At Day 5, 1D11 significantly suppressed p-Smad2/3 signaling pathway improving renal function impairment, reduced the score of acute tubular necrosis, peritubular capillaritis, interstitial inflammation and neoangiogenesis. 1D11 markedly decreased interstitial edema, disruption of tubular basement membrane loss of brush border, cytoplasmic edema and organelle ultrastructure alterations (mitochondrial disruption and endoplasmic reticulum edema) in proximal tubular epithelial cells. Moreover, 1D11 significantly inhibited p-PERK activation and attenuated dysregulation of unfolded protein response (UPR) pathways, endoplasmic reticulum and mitochondrial proteostasis in vivo and in vitro. Conclusions The early inhibition of p-Smad2/3 signaling pathway improved acute renal function impairment, partially prevented epithelial-endothelial axis activation by maintaining PTEC proteostasis and reduced early PDGFRβ+ pericytes-derived myofibroblasts accumulation.
La néphropathie expérimentale aux acides aristolochiques (AA) est caractérisée par une atteinte tubulo-interstitielle aiguë (nécrose tubulaire et infiltrat inflammatoire) suivie de lésions chroniques (atrophie tubulaire et fibrose). Dans ce modèle, l’infiltrat inflammatoire est considéré comme le lien physiopathologique entre ces 2 phases. Nous avons étudié le rôle des populations lymphocytaires au moyen d’anticorps (Ac) déplétants au cours de la phase aiguë du modèle murin. Des souris C57Bl/6 ont été injectées avec des AA ou la solution contrôle et sacrifiées après 5 jours. De plus, elles ont reçu un isotype contrôle (ctrl) ou des Ac déplétants les lymphocytes T-CD4 + (αCD4), CD8 + (αCD8) ou les lymphocytes T régulateurs (T-reg) (αCD25). Par rapport au groupe AA + ctrl, une augmentation significative de la créatinine plasmatique (pCr), de l’urée, du score de nécrose tubulaire ainsi que de l’expression rénale de l’ARNm du MCP-1 et du MIP-1α a été observée dans le groupe AA + αCD4. De plus, la déplétion a provoqué l’augmentation de l’IL-6, du TNFα, du CXCL10 et de l’IL-1b. L’injection d’AA induisant une augmentation de la proportion de T-reg rénaux par rapport aux souris contrôles, nous avons comparé le groupe AA + αCD25 avec le groupe AA + ctrl. Étonnamment, aucune augmentation de pCr, d’urée ou du score de nécrose n’a été observée. D’autre part, la comparaison du groupe AA + ctrl au groupe AA + αCD8 a montré une augmentation significative de la pCr, de l’urée ainsi que de l’ARNm, du TNFα de même qu’une augmentation du score de nécrose, du MCP-1 et du CXCL10 dans le groupe déplété. L’analyse de l’infiltrat inflammatoire rénal a démontré que l’injection d’AA provoquait l’augmentation d’une population rénale CD11bhiLy6G-F4/80lo par rapport aux souris contrôles. Cette augmentation était significativement plus importante dans les groupes AA + αCD8 et αCD4. L’effet délétère de la déplétion des lymphocytes T-CD4+ n’est pas lié à la déplétion des T-reg. L’augmentation des lésions observée lors de la déplétion en lymphocytes T-CD4+ ou CD8+ est associée à l’apparition d’une population myéloïde spécifique. Ces résultats suggèrent que les lymphocytes T-CD4+ et CD8+ sont protecteurs dans ce modèle murin de néphropathie aux AA. Le rôle des populations myéloïdes reste à préciser.
Aristolochic acids (AA) are nephrotoxic and profibrotic agents, leading to chronic kidney disease. As some controversial studies have reported a nephroprotective effect of exogenous recombinant human bone morphogenetic protein (rhBMP)-7 in several models of renal fibrosis, we investigated the putative effect of rhBMP-7 to prevent progressive tubulointerstitial damage after AA intoxication in vitro and in vivo. In vitro, the toxicity of AA on renal tubular cells was demonstrated by an increase in vimentin as well as a decrease in -catenin expressions, reflecting a dedifferentiation process. Increased fibronectin and interleukin-6 levels were measured in the supernatants. Enhanced -SMA mRNA levels associated to decreased E-cadherin mRNA levels were also measured. Incubation with rhBMP-7 only prevented the increase in vimentin and the decrease in -catenin expressions. In vivo, in a rat model of AA nephropathy, severe tubulointerstitial lesions induced by AA after 10 and 35days (collagen IV deposition and tubular atrophy), were not prevented by the rhBMP-7 treatment. Similarly, rhBMP-7 did not ameliorate the significant increase in urinary concentrations of transforming growth factor-. In summary, our in vitro data demonstrated a poor beneficial effect of rhBMP-7 to reverse cell toxicity while, in vivo, there was no beneficial effect of rhBMP-7. Therefore, further investigations are needed to confirm the exact role of BMP-7 in progressive chronic kidney disease.
