ObjectiveThis retrospective study in individuals with chronic respiratory diseases (CRDs) compared changes in quality of life, anxiety and depressive symptoms, and exercise capacity after a home pulmonary rehabilitation (PR) programme, based on the care manager's professional background.MethodsPR team was composed of nurses (Gr1, n = 989 patients), physiotherapists (Gr2, n = 466), one dietician and one sociomedical beautician (Gr3, n = 794) and one respiratory physician (Gr4, n = 201) who supervised one home session per week during 8 weeks. Quality of life, anxiety and depressive symptoms, dyspnea and exercise capacity were assessed at the beginning and end of PR, and at 12 months after the end of PR.Results2450 participants were included (females: 41.6%, age: 64.1 ± 13.0 years, FEV1: 50.7 ± 25.2% of predicted, people with COPD: 51%, with ILD: 23%, with asthma: 10%). Gr 1, Gr 3 and Gr 4 improved all the assessments at short- and long-term (p < 0.01). Improvement in the Visual Simplified Respiratory Questionnaire in Gr2 was not maintained at long-term. According to the linear mixed models, changes over time were not statistically different between groups.ConclusionA personalised PR programme delivered by one care manager, led to similar short-term benefits regardless of the care manager's initial professional background.
Background Severe allergic asthma is a chronic inflammatory airway disease. T2 low asthma is associated with bronchial remodelling, neutrophil recruitment in the lung, and Th17 cytokine production. Among Th17 cytokines, IL-22 is involved in neutrophil recruitment, tissue remodelling and is induced in response to infectious agents, some involved in the exacerbation of asthma. The aim of this study was to determine the role of IL-22 in an asthma model by using IL-22 deficient mice. Materials and The role of IL-22 was investigated in a model of dog allergen-induced chronic asthma, using Wild-Type and IL22 deficient mice. Asthma parameters were evaluated including airway resistances, bronchoalveolar lavage (BAL) cell counts, lung cytokine expression, and bronchial remodelling. Induced Bronchus associated lymphoid tissue (iBALT) was assessed by immunostaining against T cells, B cells and follicular dendritic cells (FdC). Dog-sensitized/challenged IL22 deficient mice exhibited decreased total cell and neutrophil BAL recruitment, abolished airway hyperresponsiveness compared with wild-type mice. The number of iBALT was also decreased together with loss of FdC. However, there was no modification in lung cytokine expression. In contrast, a strong decrease in the parameters associated with bronchial remodelling was observed. These results suggest that therapies targeting IL-22 may be of interest in neutrophilic T2 low asthma with strong bronchial remodelling. Background Work productivity is impaired in severe asthma; however, its evolution under biologics is poorly known, particularly regarding presenteeism, fact of being present at work while ill. Objective To investigate the evolution and factors associated with presenteeism in severe asthma after treatment with biologics. Methods We conducted a national, multicentric, uncontrolled cohort study. Patients with severe asthma eligible for a biologic and having a professional activity were included. Patients were assessed at baseline and after six months of treatment. Outcomes (percentages of presenteeism, absenteeism and work productivity loss) were measured using the Work Productivity and Activity Impairment (WPAI):Asthma questionnaire. Results A total of 167 patients were included and 122 were analyzed (59.8% women, mean age at 45.7 years). At inclusion, median presenteeism was at 30%. Under biologic, we observed a significant decrease in mean presenteeism (-14.9%, p<0.001) and work productivity loss (-15.4%, p<0.001) but no difference in absenteeism (-1.8%, p=0.41). In the multivariate logistic model, high presenteeism (WPAI:Asthma-Q5 ≥4) at inclusion was associated with uncontrolled asthma (ACQ-6 ≥1.5) (OR=18.9 [2.7; 403]) and hyperventilation symptoms (Nijmegen >17) (OR=4.6 [1.3; 19.8]). In the multivariate linear regression model, we found an association between presenteeism evolution, ACQ-6 score at inclusion (Beta=15.9 [8.6; 23.2], per 1-point increase) and ACQ-6 score evolution (Beta=17.1 [9.9; 24.2], per 1-point increase). Conclusion Although limited by the lack of a control group, our results suggest that biologics can reduce presenteeism and work impairment in severe asthma, and that asthma control is the main factor associated with presenteeism.
Objectives To evaluate the short-term and long-term benefits of adding a weekly educational session to a traditional 8-week home-based pulmonary rehabilitation (PR) programme in people with chronic obstructive pulmonary disease (COPD). Primary hypothesis was that 8 home-based supervised sessions will be equivalent to 16 home-based supervised sessions at both short- and long-term after PR.Design Retrospective cohort study conducted on prospectively collected real-life data, from January 2010 to December 2021.Setting FormAction Santé, Pérenchies France.Participants Eligible individuals were aged >18 years with a diagnosis of COPD and referred to the home-based PR programme by their respiratory physician. Participants were retrospectively divided into two groups (Gr 1, 8 PR sessions, n=759, and Gr 2, 8 PR sessions+8 educational sessions, n=262).Intervention All participants received an 8-week personalised home PR programme. A subgroup of participants received one additional supervised home session per week, including education and motivational support for daily physical activities and walking.Outcomes Health-related quality of life, dyspnoea, anxiety and depressive symptoms, fatigue and exercise tolerance were assessed at baseline (M0), at the end of PR (M2), and 14 months (M14) after M0.Results Baseline characteristics and assessments were similar between groups with an exception for long-term oxygen therapy (Gr1: 69.8% vs Gr2 53.0%, p<0.001) and noninvasive ventilation (Gr1: 38.6% vs Gr2: 29.8%, p=0.015). At M2 and M14, all the assessments were improved in both groups (p<0.01). At M2, the improvement in health status and exercise tolerance was higher in Gr 2 compared with Gr 1 (p<0.05). From M0 to M14, 90 (11.9%) participants and 29 (11.1%) participants died in Gr 1 and Gr 2, respectively (p=0.794).Conclusion People with COPD benefited, at short and long terms, from both 8 or 16 supervised home-based PR sessions. Once-weekly home-based supervised sessions during 8 weeks, combined with unsupervised physical training sessions and self-management plan for the other health behaviours, might be the best compromise between patients, health professionals and policy makers.
Airway remodelling (AR) is a disabling phenomenon in patients with severe asthma, yet suitable models are lacking. We previously developed a dog allergen-induced murine asthma model characterized by T2low Th17-driven neutrophilic airway inflammation and AR. To assess its relevance to human AR associated with T2low severe asthma, a condition characterised by poor response to inhaled steroids, we tested the steroid sensitivity of the key features of this model. Asthma was induced in C57BL/6 J mice by intranasal sensitization, followed by a three-week challenge with dog allergen. Treatment: Daily intraperitoneal 1 mg kg−1 dexamethasone was administrated during the last week of challenge. Methods: We measured airway resistances in response to methacholine, cellular inflammation in bronchoalveolar lavage, lung cytokines, and quantified AR features, in response to dexamethasone. Dexamethasone-treated mice showed persistent airway hyperresponsiveness, neutrophilic inflammation, and Il17a overexpression, whereas Il22 expression was abrogated. Pathological AR features, including mucus hyperproduction, subepithelial fibrosis and smooth muscle hypertrophy were not eliminated by dexamethasone. Our dog allergen-induced murine model of asthma mirrors the steroid-insensitive traits of human severe T2low asthma with AR, making it a relevant tool for identifying novel therapeutic targets in this orphan asthma subset.
Some interstitial lung diseases involve pulmonary fibrosis, which is a process that is characterized by the excessive and abnormal accumulation of extracellular matrix in the pulmonary interalveolar space. Although the current anti-fibrotic therapy aims at slowing down the progression of pulmonary fibrosis, it does not reverse it, and many of the drugs that were identified in basic-research studies failed in clinical phases, mainly because of the lack of a model that can recapitulate the pathophysiological mechanisms of human pulmonary fibrosis. We developed a novel experimental model of pulmonary fibrosis induced by a cocktail of molecules on an air/liquid interface culture of mouse embryonic lung explants. Histological analyses revealed a pattern of usual interstitial pneumonia, the worst-prognosis form of pulmonary fibrosis. We performed a transcriptomics analysis at the single-cell level after the induction of fibrosis and before any histological signs of fibrosis could be observed. The results revealed increased expression of several gene families that are involved in early inflammation, fibrosis and iron homeostasis, as well as potential new genetic targets.
Unexplained dyspnea refers to unpleasant breathing sensations without any disorder being diagnosed. We aimed to study the burden of unexplained dyspnea, by assessing health-related quality of life (SF-36 questionnaire), characteristics of dyspnea (mMRC, multidimensional dyspnea profile (MDP) and Borg score at exercise), hyperventilation symptoms (Nijmegen questionnaire), anxiety and depression (HAD scale), exercise capacity (peak V̇O2) and ventilatory response to exercise. Of the 62 patients referred to our Dyspnea center, we included 29 patients with confirmed unexplained dyspnea after a complete systematic evaluation (a cause of dyspnea was found in 29 patients and 4 cases were excluded for missing values). They were aged 57 years [47;66], including 20 (69%) women. All SF-36 scores were low: physical functioning 50 [33;75], limitation due to physical health 50 [0;63], limitation due to emotional problems 67 [0;100], fatigue 30 [15;43], emotional well-being 52 [44;66], social functioning 63 [38;75], bodily pain 45 [23;58], general health 50 [30;55]. Most patients (23 (79%)) had activity-limiting dyspnea (mMRC≥2), 15 (55%) had a low exercise capacity (peakV’O2<85%) and 20 (74%) had either exertional hyperventilation (threshold V̇’E/V̇CO2>34) or low Vt expansion (peak Vt/FVC<40%). Hyperventilation symptoms (Nijmegen>23) were present in 17 (59%) patients, and associated with lower scores in some domains of the SF-36 questionnaire and higher HAD and MDP anxiety subscores. Our results suggest a heavy burden of unexplained dyspnea on physical and mental health. Expert centers may help in the diagnosis and thus enable earlier symptomatic management.
Introduction L’hypertension pulmonaire (HTP) est une complication des pneumopathies interstitielles diffuses (PID) fibrosantes dont elle aggrave le pronostic. Les données concernant l’effet des traitements de l’HTP dans cette indication sont limitées. Nous avons évalué l’effet des traitements de l’HTP sur la mortalité des patients avec HTP et PID fibrosante. Méthodes Dans cette étude de cohorte rétrospective monocentrique nous avons inclus tous les patients avec un diagnostic d’HTP du groupe 3 confirmé par cathétérisme cardiaque droit et une PID fibrosante pris en charge au CHU de Lille entre mai 2004 et avril 2024. Les sarcoïdoses, sclérodermies et PID kystiques ont été exclues. L’objectif principal était d’évaluer l’effet du traitement de l’HTP sur la mortalité. Le critère de jugement principal était la mortalité toute cause. L’analyse statistique a comporté des tests de Log-Rank et des courbes de Kaplan Meier en univarié et un modèle de Cox en multivarié. Les différences entre les groupes ont été analysées avec des tests U de Mann-Whitney ou des tests t de Student. Résultats Nous avons inclus 75 patients, 45 (60 %) hommes, d’âge médian 68,5ans (IQR : 62–74), dont 22 (29,3 %) avec une fibrose pulmonaire idiopathique (FPI), 18 (24 %) avec une pneumopathie d’hypersensibilité fibrosante (PHS-F) et 11 (14,7 %) avec une pneumopathie interstitielle non spécifique (PINS) idiopathique (Figure 1). La capacité vitale forcée (CVF) médiane était à 73 % de la théorique (IQR : 59–90), la capacité de diffusion du monoxyde de carbone (DLCO) à 27 % de la théorique (IQR : 21–35) et les RVP à 6,1 UW (IQR : 4,4–9,7). À partir du diagnostic d’HTP, la médiane de survie était de 14,7 mois (IC 95 % : 9,4–20,6). Au total, 26 patients (33,3 %) étaient traités parmi lesquels 22 ont reçu du sildénafil. Dans le groupe traité, il y avait plus fréquemment des RVP>5UW (n=25, 96 % vs n=24, 49 %, p<0,01), une classe NYHA III-IV (n=22, 88 % vs n=27, 64 %, p=0,05), une CVF>70 % (n=17, 65,4 % vs n=20, 44,4 %, p=0,02) et une réduction de la distance moyenne parcourue au TM6 (194,4±101,4m vs 294,7±120,1m, p<0,01).La prise d’un traitement de l’HTP était associée à une diminution de la mortalité toute cause (HR 0,52 [0,30–0,91], p<0,01) en analyse univariée et multivariée après ajustement sur l’âge, le sexe, la CVF<70 % et les RVP>5UW (HR 0,31 [0,15–0,67], p<0,01) (n=69). Les autres facteurs associés à la mortalité toute cause en analyse multivariée étaient la CVF<70 % (HR 2 [1,0–3,8], p=0,04) et les RVP>5UW (HR 3,4 [1,7–7], p<0,01) (n=69). Conclusion L’administration d’un traitement de l’HTP chez les patients présentant une PID fibrosante avec HTP est associée à une diminution de la mortalité toute cause bien que les patients traités semblaient plus sévères.
FLNA pathogenic variants lead to various congenital malformations. Pulmonary manifestations were reported in early age. In adults, few cases of emphysema were published. We aimed to assess the frequency and characteristics of emphysema in adults with a pathogenic or probably pathogenic variant in FLNA. We conducted a transversal single-center study. Adults with FLNA pathogenic variant followed in Lille University Hospital were identified among the French rare disease database BaMaRa®. They attended medical examination, non-contrast chest CT-scan and pulmonary function tests (PFT). We collected medical history, tobacco smoking status, occupational and domestic exposures. When emphysema was identified, the PTPN6 gene was sequenced and alpha1antitrypsin deficiency searched. The primary objective of the study was to assess the frequency of emphysema. Secondary objectives were to evaluate emphysema type, distribution and extension, lung function in case of emphysema and the frequency of unexplained emphysema. Among the 29 patients identified, 10 met non-inclusion criteria, 4 declined, 5 could not be reached and so 10 patients were included. Introduction L’asthme touche principalement les patients en âge de travailler et peut avoir un impact défavorable sur l’activité professionnelle. Dans l’asthme sévère, peu d’études ont évalué l’effet potentiel des biothérapies sur la productivité au travail.Les objectifs de l’étude étaient d’évaluer l’évolution de la productivité au travail et les facteurs associés dans l’asthme sévère après six mois de biothérapie. Méthode Nous avons mené une étude de cohorte prospective, non contrôlée, multicentrique (16 centres) française. Les patients asthmatiques sévères éligibles à une biothérapie et ayant une activité professionnelle ont été inclus. Les patients ont débuté une biothérapie lors de la visite d’inclusion (visite 1), puis ont été réévalués après six mois de traitement (visite 2).Les critères de jugement ont été évalués à l’aide du questionnaire Work Productivity and Activity Impairment (WPAI). Le critère de jugement principal est le pourcentage de présentéisme, défini par une présence au travail alors que l’absence au travail ou l’aménagement du lieu de travail seraient justifiés. Les critères de jugement secondaires sont les pourcentages d’absentéisme (absence au travail due à l’asthme) et de perte de productivité au travail.Le contrôle de l’asthme a été évalué par le questionnaire Asthma Control Questionnaire 6 (ACQ-6), la qualité de vie liée à l’asthme par l’Asthma Quality of Life Questionnaire (AQLQ) et l’impact des symptômes ORL par le Sino-Nasal Outcome Test 22 (SNOT-22).Pour répondre à l’objectif principal, l’évolution du présentéisme entre V1 et V2 a été évaluée à l’aide d’un test de Student apparié. Résultats Au total, 128 patients ont été inclus. Après 6 mois de suivi, on observe une diminution significative du présentéisme (moyenne [écart-type]=34 [26] % à V1 vs 18,2 [25,2] % à V2, p<0,001) et de la perte de productivité au travail (35,8 [28,2] % vs 18,8 [26,7] %, p<0,001). Il n’a pas été observé de différence concernant l’absentéisme (12,4 [29,2] % vs 9,1 [27,6], p=0,35). On observe également une amélioration du contrôle de l’asthme, de la dyspnée, des paramètres fonctionnels respiratoires et une diminution du nombre d’exacerbations sévères et de la dose cumulée de corticoïdes oraux. À l’inclusion, le présentéisme élevé était associé à un mauvais contrôle de l’asthme (odd ratio [intervalle de confiance à 95 %]=31,9 [6,3 ; 584,6] pour les patients non contrôlés vs patients contrôlés), à un faible niveau de qualité de vie (OR=2,6 [1,6 ; 4,7] pour une diminution d’un point d’AQLQ), et à un faible niveau d’étude (comparé à un niveau d’études supérieures : OR=3,6 [1,4 ; 9,7] pour les patients sans diplôme, OR=0,9 [0,3 ; 2,4] pour les patients avec un baccalauréat).Nous avons observé des corrélations significatives entre l’évolution du présentéisme et l’évolution des scores ACQ-6 (rho [intervalle de confiance à 95 %]=0,64 [0,5 ; 0,74]), AQLQ (−0,54 [−0,68 ; −0,37]) SNOT-22 (0,44 [0,25 ; 0,61]). Conclusions Les résultats suggèrent, sous réserve de l’absence de groupe contrôle, que le recours aux biothérapies pourrait améliorer le présentéisme et la productivité au travail. L’évolution du présentéisme est notamment corrélée au contrôle de l’asthme et aux symptômes ORL.
Introduction La réadaptation respiratoire (RR) est majoritairement proposée en hospitalisation à temps complet ou partiel, plus rarement à domicile, et récemment en téléréadaptation. Sachant que moins de 10 % des patients atteints de BPCO hospitalisés pour une exacerbation auront accès à une RR, les formats hybrides, associant le présentiel et le distanciel, semblent intéressants à développer afin d’augmenter l’offre de soins. L’objectif de cette étude observationnelle était d’évaluer la faisabilité et l’efficacité d’un programme de RR hybride à domicile (Article 51 RR TéléDom) chez des patients atteints de BPCO sortis d’hospitalisation pour exacerbation. Méthodes La RR était composée de 4 séances supervisées en présentiel à domicile et 4 séances supervisées en distanciel, à raison d’une séance par semaine pendant 8 semaines. L’intervention incluant du réentrainement à l’effort, de l’ETP et un accompagnement psychosocial et motivationnel, était mis en place par un référent unique (care manager), à la suite d’un bilan éducatif initial réalisé en présentiel à domicile. La dyspnée (mMRC, questionnaire Dyspnea-12), la fatigue (FAS), la qualité de vie (CAT), l’anxiété et dépression (HAD), la tolérance à l’effort (TS6) et la capacité fonctionnelle (TUG) ont été évalués au début et à la fin de la RR. Résultats De janvier 2022 à décembre 2023 (évaluation intermédiaire), 306 patients ont été inclus dans l’expérimentation RR TéléDom : 32 (10,4 %) refusaient d’emblée la RR et 16 (5,2 %) la refusaient après le bilan éducatif, 82 (26,8 %) ont refusé RR TéléDom (pas d’internet, refus de la visio, troubles auditif et/ou visuel) mais accepté de réaliser le programme traditionnel de 8 visites supervisées en présentiel à domicile (RR Dom) et 176 (57,5 %) patients ont réalisé RR TéléDom. Le groupe RR TéléDom était initialement plus jeune (64 vs 70ans), avait une meilleure capacité fonctionnelle (TUG : 7,9 vs 11,9 secondes) et un score mMRC (3,0 vs 3,3 points) plus faible que le groupe RR Dom. En moyenne, les patients débutaient la RR 21±8jours après la sortie de l’hôpital. L’ensemble des paramètres évalués était amélioré dans le groupe RR TéléDom (p<0,01), tandis que le groupe RR Dom n’améliorait pas la fatigue (p=0,098), l’anxiété (p=0,512) et le TS6 (p=0,063). À l’exception de l’anxiété (p=0,023), l’évolution des paramètres dans le temps était similaire entre les groupes. Conclusion Ces résultats préliminaires suggèrent qu’un format hybride de RR à domicile est : (i) faisable et efficace à court terme chez des patients sortis d’hospitalisation pour exacerbation de leur BPCO ; (ii) mais non adapté à tous les patients. La poursuite de cette expérimentation devra s’attacher à définir des typologies de patients pour lesquels le format hybride est pertinent et efficace.
