Context Childhood obesity increases the risk of chronic disease in adulthood. Objective To construct an early predictive model for a growth trajectory that is highly related to childhood overweight/obesity. Design Prospective cohort study. Settings Shanghai Birth Cohort (SBC) and US Collaborative Perinatal Project (CPP). Participants A total of 848 mother-child pairs in the SBC (2013-2016) and 22 691 pairs in the CPP (1959-1965) with 2- and 7-year follow-up, respectively. Main Outcome Measures A high-risk postnatal growth trajectory intimately associated with childhood overweight/obesity and its predictive model. Results We demonstrated that the shifts of postnatal body mass index (BMI) percentile had been completed around 1 year of age and identified a high-risk growth trajectory that was closely related to overweight/obesity [odds ratio 6.5 (95% CI 5.9, 7.2)] at 7 years old. Children with this trajectory presented with a consistent BMI around the 85th percentile after the age of 1 year. It could be recognized early after birth using a predictive model with 4 metabolites (tyrosine, glycine, octenoylcarnitine, and stearoylcarnitine), combined with sex, birth weight, and maternal prepregnancy BMI. The model had an area under the receiver operating characteristic curve of 0.869 (95% CI 0.779, 0.932), a sensitivity of 83.3% (95% CI 51.6%, 97.9%), and a specificity of 81.1% (95% CI 70.3%, 89.3%) in the validation data set. Conclusion Children with postnatal high-risk growth trajectories were significantly associated with subsequent overweight/obesity at 7 years old. Metabolite profiles at birth combined with clinical measures were able to predict at-risk children before overweight/obesity occurrence.

This study aimed to characterize the clinical and epidemiological features of airway FBs in a pediatric Chinese population. A retrospective review of medical records of children aged 0 to 14 years who attended with a diagnosis of FB aspiration the Shanghai Children's Hospital between January 2013 and December 2017 was carried out. Descriptive analysis was used to assess patient's demographics, clinical, radiographic, bronchoscopic findings, time to presentation, and characteristics of the FBs. Among the 200 patients included in the study, 92% were under 3 years of age, with a peak incidence of FB aspiration occurring between 1 and 2 years old. The male to female ratio was about 2.6:1. Twenty-three percent of the patients were admitted within 24 hours of the event, 40% within 1 week, 30% within 1 month, and 7% more than 1 month after aspiration. The most common presenting symptoms of laryngotracheal FBs were cough, dyspnea, and wheezing; those of bronchial FBs were cough, decreased air entry, and wheezing. Chest X-ray was normal in four-fifths of the children with laryngotracheal FBs, whereas most common abnormal X-ray findings in children with bronchial FBs were mediastinal shift, obstructive emphysema, and pneumonia. There was a trend that in children younger than 2 years FBs were more frequently found in the left bronchus, whereas in older children FBs were more frequently found in the right bronchus. Ninety-three percent of the removed FBs were organic materials such as food items and the most frequently aspirated FBs were peanuts. Flexible bronchoscopy was performed in 82.5% of the patients, while rigid bronchoscopy or direct laryngoscopy in 17.5% of the patients. Four patients were subjected to thoracic surgery and 1 died during rigid bronchoscopy due to acute respiratory failure. FBs is a frequent pathology among Chinese children. Tracheobronchial FBs should be strongly suspected in young children who have sudden onset of cough and wheezing episode, even when physical and radiographic evidence is absent.
Previous studies suggested an association between female fertility and body size, but most of these studies were from Western countries and focused mainly on obesity. This study investigated the association between preconception body mass index (BMI) and time to pregnancy (TTP) in women planning to conceive from Shanghai, China. A total of 1,182 women aged 24-46 years were recruited from the Shanghai Birth Cohort between 2013 and 2015 and were followed up for 12 months. Preconception BMI was categorized as underweight, normal weight, and overweight/obesity according to the Chinese classification of BMI. Fecundability (FOR) and infertility (IOR) odds ratios were estimated using Cox (n = 1,092) and Logistic (n = 820) regression models, respectively. We found no differences in fecundability between underweight and overweight/obese women and normal-weight women. Furthermore, underweight and overweight/obese women did not have a higher risk of infertility compared with normal-weight women. Our findings suggest that non-optimal preconception BMI does not appear to influence female fecundability and infertility in Chinese women. These results should be interpreted with caution as they may be applicable only to women with demographic and anthropometric characteristics similar to our study population. Our findings need to be confirmed in other populations.
Epidemiological studies have reported association of urinary 3-phenoxybenzoic acid (3-PBA), a major metabolite of pyrethroid insecticides (PYRs), with respiratory disease. However, knowledge regarding its effect on pulmonary function in susceptible children is limited. This study aimed to assess the associations between environmental 3-PBA concentrations and pulmonary function in children aged 6-17 years. Using data on 1174 children aged 6-17 years from the U.S. National Health and Nutrition Examination Survey (NHANES) 2007-2012, the exposure to PYRs was assessed by measuring urinary 3-PBA concentrations and pulmonary function was assessed by spirometry. Multivariable linear regression and generalized linear models (GLMs) were used to examine the associations between 3-PBA concentrations and pulmonary function in children, controlling for confounders. We found that 3-PBA concentrations were inversely associated with forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC), and peak expiratory flow (PEF) in the pediatric population (p-trends < 0.05). When stratified by age (6-10 and 11-17 years) and gender (boys and girls), the adverse effects of PYR exposures on pulmonary function were more pronounced among boys aged 11e17 years. Among this age group, 3-PBA concentrations were negatively associated with FEV1, FVC, forced expiratory flow between 25% and 75% of FVC (FEF25-75%), and PEF. However, among children aged 6-10 years, no associations were found between 3-PBA concentrations and any of the pulmonary function measures, in either boys or girls. Our findings suggest that environmental PYR exposures may adversely affect children's pulmonary function, with the strongest associations among 11-17 years old boys. (C) 2020 Elsevier Ltd. Objective · To evaluate the levels of environmental pollutants including lead, mercury, organophosphorus pesticides (OPs), perfluoroalkyl and polyfluoroalkyl substances (PFASs) and triclosan (TCS) and further analyze the correlation between these pollutants in pregnant women.Methods · Pregnant women were recruited from the Laizhou Wan Birth Cohort (LWBC) in Shandong from September 2010 to December 2013. A total of 149 pregnant women were finally enrolled who completed questionnaires and provided sufficient biological samples for pollutants measurement including blood lead, blood mercury, urinary metabolites of OPs[dimethylphosphate (DMP), dimethylthiophosphate (DMTP), diethylphosphate (DEP), diethylthiophosphate (DETP), etc.], serum perfluorooctanoic acid (PFOA) and perfluorooctane sulfonate (PFOS), as well as urinary TCS. Spearman correlation analysis and cosine cluster analysis were used to explore the correlation between pollutants. Results · The detection rates of lead, DMP, PFOA and PFOS were all 100.0%. And the detection rates of mercury, DMTP, DEP, DETP and TCS were 89.3%, 81.2%, 97.3%, 96.6% and 59.1%, respectively. The median and range of concentrations for lead, mercury, PFOA, PFOS and TCS were 28.40 (11.30–65.70) μg/L, 0.85 (<LOD–10.98) μg/L, 39.54 (1.16–273.68) μg/L, 4.56 (0.55–15.38) μg/L, 0.58 (<LOD–58.01) μg/g, respectively. The median and range of concentrations for DMP, DMTP, DEP and DETP were 36.33 (0.55–1 331.04) μg/g, 2.65 (<LOD–128.84) μg/g, 14.70 (<LOD–585.05) μg/g, 1.84 (<LOD–86.21) μg/g, respectively. The concentrations of DMP and DEP were generally higher than those in developed countries. The concentration of PFOA was much higher than those in foreign studies, while the concentrations of PFOS and TCS were relatively lower. Correlation analysis and cosine cluster analysis revealed that mercury was positively correlated with PFOA (r=0.36, P=0.000) and PFOS (r=0.42, P=0.000). Conclusion · The population in LWBC is widely exposed to multiple pollutants and there are certain correlations between mercury and PFASs, suggesting that attention should be paid to emerging pollutants besides traditional ones.

Background: Previous studies suggested that thyroid function in the first trimester of gestation played an important role in fetal growth. However, limited clinical data are available to support this relationship in the second trimester of gestation. Objective: We examined the relationship of maternal thyroid function parameters in early second trimester of gestation with birth outcomes in China. Methods: Participating 5016 mother-infant pairs were recruited from the Songjiang District Maternity and Infant Hospital, Shanghai, China, between July and December 2016. Linear regression assessed the associations of thyroid hormones with birthweight. Logistic regression tested the correlations between thyroid hormones and low birthweight, macrosomia, small for gestational age , and large for gestational age infants. Maternal serum TSH, TT3, TT4, FT3, and FT4 concentrations were measured in 16-20 weeks of gestation. Results: TT3 and FT3 were positively associated with birthweight, while TT4 and FT4 were negatively associated with birthweight, respectively. Furthermore, higher TT3 and FT3 were associated with increased risks of LGA infants (OR = 1.48, 95% CI: 1.15-1.9; OR = 1.22, 95% CI: 1.01-1.46), respectively. Higher TT3 was associated with an increased risk of macrosomic infants (OR = 1.35, 95% CI: 1.04-1.74). In contrast, higher FT4 was associated with decreased risks of LGA (OR = 0.87, 95% CI: 0.81-0.93) and macrosomic infants (OR = 0.90, 95% CI: 0.84-0.96), respectively. No associations were found between TSH and any of the outcomes. Conclusions: TT3 and FT3 were positively associated with fetal growth, while TT4 and FT4 were negatively associated with fetal growth. Our findings suggested that thyroid function in early second trimester of gestation is a potential risk factor for abnormal fetal growth.
Background: Several epidemiological studies suggest that prenatal exposure to BPA may interfere with the neurodevelopment of pre-school and school-age children. However, a limited number of studies are available for effects during children at a younger age, especially in China. Methods: Based on Laizhou Wan Birth Cohort (LWBC), BPA concentrations were measured in urine among 506 pregnant women during their hospital admission for delivery and neurodevelopment of their children was assessed using the Gesell Development Schedules at 12 months (n = 368) and 24 months (n = 296). Linear regression and generalized linear models were used to analyze the association between prenatal BPA exposure and the children's developmental quotient scores (DQs). Results: The median of maternal BPA concentration was 0.48 mu g/L or 1.05 mu g/g creatinine. Maternal BPA concentrations were adversely associated with children DQs at 12 months of age, with a 10-fold increase in prenatal BPA concentrations correlated to 1.43-point decrease in DQs in the adaptive domain (beta = -1.43; 95% CI: -2.30 to -0.56, p = 0.001). When stratified by gender, prenatal BPA concentrations were adversely associated with the adaptive domain DQs among boys (p-trend = 0.012) and girls (p-trend = 0.028) and the social domain DQs (p-trend = 0.019) only among girls. At 24 months of age, the significant adverse association was only found in the language domain among girls (beta = -1.69; 95% CI: -3.23 to -0.15, p = 0.032). Conclusion: Based on a Chinese population, we found potential impacts of prenatal BPA exposure on childhood neurodevelopment at 12 and 24 months of age, especially among girls.
目的·了解上海市松江区妇女孕期增重(gestational weight gain,GWG)情况,探讨GWG与妊娠结局之间的关系.方法·共收集2016年7月-12月在上海市松江区妇幼保健院产检并住院分娩的妇女4 884例,根据2009年美国医学研究所(Institute of Medicine,IOM)制定的GWG范围分为GWG过少组、GWG正常组和GWG过多组,分析GWG对新生儿出生体质量、孕龄、新生儿评分以及不良妊娠结局如早产、小于胎龄儿(small-for-gestational age,SGA)、大于胎龄儿(large-for-gestational age,LGA)、巨大儿的影响.结果·GWG过少者占25.7%,GWG过多者占33.3%.新生儿出生体质量随着GWG的增加而增加(β=24.17,95% CI为21.90 ~ 26.45,P=0.000);GWG对于发生SGA是保护因素(OR=0.89,95% CI为0.87 ~ 0.91,P=0.000),对于发生LGA(OR=1.12,95% CI为1.09~ 1.14,P=0.000)及巨大儿(OR=1.14,95% CI为1.11 ~ 1.16,P=0.000)是危险因素.孕龄随着GWG增加而延长(β=0.03,95% CI为0.03 ~ 0.04,P=0.000);GWG是早产发生的保护因素(OR=0.91,95% CI为0.88 ~ 0.95,P=0.000).相对于GWG正常组,GWG过少组更易发生SGA(OR=1.54,95% CI为1.22~ 1.94,P=0.000)和早产(OR=1.53,95% CI为1.03 ~ 2.26,P=0.034),GWG过多组则更易于发生LGA(OR=2.20,95% CI为1.71 ~ 2.84,P=0.000)和巨大儿(OR=2.41,95% CI为1.86 ~ 3.11,P=0.000).结论·孕期保持适宜的体质量增加对改善妊娠结局有重要意义,需重视对孕产期体质量的管理,保障母婴健康.
