Hepatocellular carcinoma (HCC), the predominant subtype of primary liver cancer, remains as a globally lethal malignancy with limited therapeutic options. Despite therapeutic efficacy of small-molecule inhibitors, the majority of HCC patients are still incurable due to suboptimal response rate and serious systemic toxicity. Panobinostat (PANO), a potent histone deacetylase (HDAC) small-molecule inhibitor, has established clinical efficacy in hematological malignancies. However, clinical application of PANO in solid tumors (e.g., HCC) is significantly hindered by in vivo delivery barriers. In the present study, we show that PANO triggers cell cycle arrest and apoptosis in HCC cells, demonstrating its therapeutic potential for this malignancy. We also develop a nanocarrier using folic acid-targeted polyethylene glycol-modified poly(lactic-co-glycolic acid) to facilitate in vitro and in vivo administration of PANO. In mice with orthotopically allografted HCC, the resultant PANO-loaded nanoformulation achieves potent antitumor efficacy, characterized by significant tumor cell death, remarkable tumor regression, and prolonged animal survival. These findings demonstrate a promising regimen based on nanodelivery of PANO for HCC therapy.
右佐匹克隆是外消旋体佐匹克隆的S异构体,为第3 代非苯二氮(艹卓)类镇静催眠药,具有口服吸收快、生物利用度较高和半衰期适中等药动学特征.作为美国FDA首个批准用于治疗成人短暂性和慢性失眠的催眠药,右佐匹克隆可同时兼顾改善入睡及睡眠维持障碍,不仅对原发性失眠具有显著疗效,对多种共病性失眠也有较好的治疗作用,同时其不良反应发生率低,患者普遍耐受良好,在长期应用中不产生药物耐受性、依赖性及反跳性失眠等,是一种安全有效的催眠药.本文就右佐匹克隆的药学特征及临床应用研究进行综述.
目的 探讨注射用复方三维B(Ⅱ)联合匹多莫德分散片治疗带状疱疹神经痛的临床效果.方法 回顾性分析2020年5月—2021年9月期间就诊于吉林大学第二医院的胸段带状疱疹后神经痛患者128例的临床资料,按照随机数表法将其分为观察组和对照组,每组64例,对照组患者仅采用匹多莫德分散片进行治疗,观察组则采取注射用复方三维B(Ⅱ)联合匹多莫德分散片治疗方案.对比两组患者的治疗效果及不良反应.结果 治疗后1 d、7 d、1个月以及3个月观察组患者的VAS评分均比对照组低,差异有统计学意义(P<0.05).观察组治疗有效率为98.44%,明显高于对照组的87.50%,差异有统计学意义(χ2=4.303,P<0.05).治疗后观察组患者的烧灼感、瘙痒感评分低于对照组,差异有统计学意义(P<0.05).结论 注射用复方三维B(Ⅱ)联合匹多莫德分散片治疗方案可有效改善胸段带状疱疹后神经痛患者的疼痛程度,缓解临床症状,提升治疗有效率.
目的 探讨参芎葡萄糖注射液治疗冠心病心绞痛的临床效果.方法 回顾性分析2020年7-8月期间来吉林大学第二医院就诊的108例冠心病心绞痛患者的临床资料,按照患者接受治疗方法分为对照组与观察组两组,各54例.对照组基础治疗,观察组在对照组治疗基础上给予参芎葡萄糖注射液,对两组患者的症状缓解情况及治疗效果.结果 观察组冠心病心绞痛患者的有效率为98.15%,高于对照组的85.19%,差异有统计学意义(χ2=4.363,P=0.036).治疗后,观察组冠心病心绞痛患者病情发作持续时间要短于对照组,发作频率低于对照组,差异有统计学意义(P<0.001).结论 参芎葡萄糖注射液可有效改善冠心病心绞痛患者的临床症状,减少心绞痛发作次数,提升治疗有效率.
OBJECTIVE To provide ideas and reference for the treatment and pharmaceutical care of infective endocarditis(IE) caused by Micrococcus luteus complicated with severe pneumonia. METHODS The clinical pharmacist participated in the treatment of a patient with IE caused by M. luteus complicated with severe pneumonia; all anti-infective treatment plans were agreed upon after the doctor invited the clinical pharmacist for consultation. After the implementation of the plan, the clinical pharmacist conducted pharmaceutical care of effectiveness and safety for the plan, including adopting suitable drug, adjusting the dose of vancomycin by using parameters such as steady-state valley concentration and creatinine clearance rate, monitoring renal function and adverse drug reactions. RESULTS IE caused by M. luteus was cured after surgery and full treatment with anti-bacterial drugs, the severe pneumonia was improved, and the decline of renal function caused by drugs and the primary disease were recovered; clinical pharmacists had ensured the effect of anti-infection treatment by assisting in the formulation of treatment plans and the implementation of pharmaceutical care, avoiding further renal damage and solving the problem of cefoperazone sulbactamrelated drug fever. CONCLUSIONS IE caused by M. luteus is relatively serious, and the treatment drug can be vancomycin and rifampicin. During the treatment, it is necessary to monitor the renal function, and adjust the dose of vancomycin or change other drugs; anti-infection pharmaceutical care provided by clinical pharmacists can guarantee the effectiveness and safety of antiinfection plan, and avoid the occurrence of severe adverse drug reactions.
目的 探讨注射用二羟丙茶碱联合异丙托溴铵治疗喘息型支气管炎急性发作的效果及安全性.方法回顾性分析2021年8月—2022年8月期间就诊于吉林大学第二医院的116例支气管炎患者的临床资料,根据不同的治疗方法分为对照组与观察组,各58例.入院后两组患者均进行基础治疗,对照组患者在此基础之上给予异丙托溴铵治疗,观察组则给予注射用二羟丙茶碱联合异丙托溴铵治疗,对比分析两组患者的治疗效果及安全性.结果 观察组治疗总有效率为 98.27%,低于对照组的 86.20%,差异有统计学意义(χ2= 4.336,P=0.037).治疗后,观察组炎症指标、血气指标优于对照组,差异有统计学意义(P<0.05).观察组并发症发生率为1.72%,低于对照组的13.79%,差异有统计学意义(χ2=4.336,P=0.037).结论 针对喘息型支气管炎急性发作的患者,注射用二羟丙茶碱联合异丙托溴铵治疗可有效改善临床症状,提高治疗有效率,且具有较好的安全性,临床应用效果较好.
目的 探讨琥珀酰明胶注射液治疗低血容量休克所致急性心力衰竭疗效探讨及安全性.方法 回顾性分析2021年4月—2022年4月在吉林大学第二医接受治疗的低血容量休克所致急性心力衰竭110例,按照患者接受治疗方法分为对照组与观察组,每组55例.对照组患者入院后给予常规急救治疗方案,观察组则在此基础之上给予琥珀酰明胶注射液,对两组患者的治疗效果以及治疗安全性进行对比.结果 对照组治疗有效率为85.45%,低于观察组的98.18%,差异有统计学意义(χ2=4.356,P<0.05);观察组患者的并发症发生率总为0.00%,低于对照组的7.27%,差异有统计学意义(P<0.05).结论 琥珀酰明胶注射液在低血容量休克所致急性心力衰竭治疗中获得了较好的应用效果,其可有效改善患者的心功能,且治疗安全性较高,临床应用效果较好.
目的 探究分析注射用烟酸在动脉硬化性闭塞症患者中的实际应用价值.方法 回顾性分析2020年3月—2021年3月吉林大学第二医院接收并治疗的138例动脉硬化性闭塞症患者的临床资料,按照疗方法分成对照组(n=69)和观察组(n=69),前者采取西洛他唑片进行治疗,后者在此基础上采用注射用烟酸进行治疗,对比分析两组临床治疗效果、血流动力学指标变化情况、血脂水平.结果 治疗后,观察组的临床治疗总有效率95.65%明显高于对照组的85.51%,差异有统计学意义(χ2=4.161,P=0.041).观察组右侧胫后与右侧足背的动脉收缩峰值速度快于对照组,HDL水平高于对照组,差异有统计学意义(t=2.336、2.957、3.332,P<0.05).结论 注射用烟酸可进一步改善动脉硬化性闭塞症患者血脂和血流动力学指标,应用效果显著.
目的 分析iRoot SP在慢性根尖周炎疾病治疗中的应用效果.方法 便利选取2020年1月—2021年11月长春市口腔医院收治的慢性根尖周炎患者130例为研究对象,随机分为观察组与对照组,各65例.对照组采用树脂类的根充糊剂冷侧压充根管的方式治疗,观察组患者在对照组治疗的基础上采用iRoot SP治疗,对比两组患者疼痛程度、炎性因子及术后1年疗效.结果 两组治疗后1 d的疼痛程度比较,差异无统计学意义(P>0.05),观察组治疗后3、7 d的疼痛评分均低于对照组,差异有统计学意义(P<0.05).治疗前两组患者炎性因子水平对比,差异无统计学意义(P>0.05),治疗后观察组患者炎性因子水平均低于对照组,差异有统计学意义(P<0.05).观察组总有效率98.46%高于对照组的80.00%,差异有统计学意义(χ2=9.686,P<0.05).结论 iRoot SP在慢性根尖周炎患者临床治疗中收到了良好的效果,可有效缓解慢性根尖周炎疾病患者的疼痛感,降低炎性因子水平,还可有效改善基牙相关指标,在临床中具有较高的应用价值.
目的 分析治疗先天性缺牙时应用正畸+口腔修复方案的治疗效果.方法 随机选取2021年1月—2022年12月长春市964医院南湖院区90例先天性缺牙患者进行分组研究,分组方法为随机信封法.参照组(n=45)治疗方案为口腔种植或是固定桥修复;观察组(n=45)治疗方案为正畸+口腔修复.对两组临床疗效进行比较.结果 观察组患者治疗有效率为97.78%,高于对照组的80.00%,差异有统计学意义(χ2=7.200,P<0.05).治疗前,两组患者咀嚼功能、语言功能、美学修复评分比较,差异无统计学意义(P>0.05);治疗后,观察组患者咀嚼功能、语言功能、美学修复评分高于参照组,差异有统计学意义(P<0.05).观察组患者咀嚼及语言功能恢复时间短于参照组,差异有统计学意义(P<0.05).结论 应用正畸+口腔修复治疗先天性缺牙疗效确切且恢复用时更短、美观度更高.
目的:分析二类精神药品住院使用情况,为各科室合理用药提供用药依据.方法:使用院内药品信息管理系统,导出某院2022 年 8~11 月住院药房二类精神药品使用数据,指标包括药品名称、规格、使用量、使用频次、销售金额、患者年龄、药品单价及使用时间等.根据各药品限定日剂量(DDD),分析销售金额及排序、用药人群性别及年龄分布、用药频度(DDDs)及排序、限定日费用(DDC)及排序、药物利用指数(DUI)及序号比(B/A值)等指标分析用药情况.结果:共提取 11 种二类精神药品信息,注射剂使用占比 81%.销售金额排名第一的是地佐辛注射剂.DDDs排名第一的是咪达唑仑注射剂.枸橼酸咖啡因注射剂、丁丙诺啡透皮贴、喷他佐辛注射剂及地佐辛注射剂DDC较大,价格较高.氨酚羟考酮片、咪达唑仑注射剂、右佐匹克隆片及艾司唑仑片的DUI值远大于 1.0.51~70 岁患者用药以镇静催眠药物居多,31~40 岁患者以镇痛药物居多.结论:临床药师可从各年龄段患者用药特点入手,提供个体化用药指导.医疗机构应加大处方审查和点评力度,减少不规范处方出现频次,对高处方量药物重点监控,避免药物滥用.
目的 分析老年急性脑梗死患者在治疗时使用复方曲肽注射液联合依达拉奉治疗的治疗效果以及作用机制.方法 方便抽取2021年5月—2022年7月吉林大学第二医院收治的急性脑梗死老年患者130例为研究对象,采用随机信封法分为两组,采取常规治疗+依达拉奉治疗的65例患者为对照组,在上述治疗方案的基础上增加使用复方曲肽注射液治疗的65例患者为观察组,对两组患者血管内皮指标、美国国家卫生院卒中量表(NIHSS)评分、改良Rankin量表(mRs)评分、治疗有效率、不良反应发生率进行对比.结果 治疗后观察组患者可溶性血栓调节蛋白(sTM)、D-二聚体(D-D)、内皮素-1(ET-1)以及可溶性髓样细胞触发受体样转录因子-1(sTLT-1)检测结果、NIHSS量表以及mRs量表评分结果均低于对照组,差异有统计学意义(P<0.05).治疗后观察组急性脑梗死患者总有效率达96.92%,高于对照组的80.00%,差异有统计学意义(χ2= 9.118,P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05).结论 急性脑梗死病情危重,在常规治疗的基础上联合使用复方曲肽注射液和依达拉奉可提高总体疗效.
右佐匹克隆是外消旋体佐匹克隆的S异构体,是具有环吡咯酮结构的非苯二氮卓类镇静催眠药.不同于苯二氮卓类药物,右佐匹克隆可能通过作用于GABAA受体的某些α亚基而发挥药理作用,因此副作用更小.口服吸收快、生物利用度较高和半衰期适中等药动学特征同样是右佐匹克隆发挥其优越性的关键因素.本研究就右佐匹克隆的药理学及药动学研究展开阐述.
The pathogenesis of chronic rhinosinusitis (CRS) is largely unknown, but accumulating evidence supports the role of the airway epithelium in its pathophysiology. In our study here, we evaluated whether epidermal growth factor (EGF) regulates a hypoxia‐inducible factor 1α (HIF‐1α)–microRNA‐21 (miR‐21)–aquaporin 4 (AQP4) axis in nasal epithelial cells from CRS patients. We found that, compared with normal sinus mucosa, EGF, HIF‐1α, and miR‐21 were upregulated and AQP4 was downregulated in sinus mucosa from patients with CRS and in a CRS mouse model. It was established that EGF upregulated HIF‐1α and miR‐21 expression, that HIF‐1α regulated miR‐21 transcription, and that the AQP4 gene was a target of miR‐21. Knockdown of EGF and HIF‐1α mRNAs and of miR‐21, or overexpression of AQP4 mRNA, inhibited proliferation and promoted apoptosis of hypoxia‐exposed human nasal epithelial cells, effects that were associated with reduced levels of α‐SMA, fibronectin, and vimentin, as well as promoted caspase‐3 activity and E‐cadherin levels. In the mouse CRS model, EGF elevation increased in vivo production of inflammatory IL‐4 and IFN‐γ to promote CRS, which was reversed by AQP4 elevation. Collectively, EGF upregulates HIF‐1α and miR‐21 expression to inhibit AQP4 expression, thereby promoting the proliferation of nasal epithelial cells and the development of CRS.
Background Chronic rhinosinusitis (CRS) is characterized by persistent symptomatic inflammation of the nasal passage and sinus mucosa. Various microRNAs (miRs) have been implicated in CRS. Hence, the current study was conducted to explore the effect of microRNA-761 (miR-761) on remodeling of nasal mucosa and epithelial-mesenchymal transition (EMT). Methods Bioinformatics analysis was initially performed to predict the differentially expressed genes (DEGs) associated with CRS. Gene targeting relationship between miR-761 and lipocalin 2 (LCN2) was analyzed by bioinformatics analysis and verified using dual-luciferase reporter gene assay. Histopathological analyses of the nasal mucosa tissues were conducted via hematoxylin-eosin (HE) and alcian blue (AB)-periodic acid Schiff (PAS) staining. ELISA was employed to determine the IL-8 and MMP-9 levels. To define downstream pathway of miR-761, levels of proteins related to LCN2/Twist1 signaling pathway were assessed. Additionally, the effects of miR-761 on EMT, proliferation, and apoptosis were determined. Results LCN2 was highly expressed in CRS. LCN2 was a target of miR-761. miR-761 overexpression or LCN2 silencing decreased IL-8 and MMP-9 levels and morphological changes in nasal epithelial tissue from CRS mice. Overexpressed miR-761 or silenced LCN2 decreased the expression of LCN2 and Twist1, indicating LCN2/Twist1 signaling pathway was inactivated. Moreover, miR-761 overexpression or LCN2 silencing reduced the expression of N-cadherin and vimentin, while increased that of E-cadherin, suggesting inhibition of EMT. Furthermore, miR-761 overexpression or LCN2 silencing promoted cell proliferation and inhibited cell apoptosis in CRS. Conclusion Taken together, miR-761 suppressed the remodeling of nasal mucosa through inhibition of LCN2 and the LCN2/Twist1 signaling pathway.
OBJECTIVE:Accumulating studies have revealed that microRNAs (miRs) play a critical role in the development and progression of nasopharyngeal carcinoma (NPC), which is a disease with a remarkable racial and geographical distribution. In our study, through the alteration in the expression of microRNA-185 (miR-185) in NPC cells by microarray-based gene expression profiling, we subsequently evaluated its ability to influence NPC cells and associated mechanism. METHODS:The expressions of miR-185 and HOXC6 in NPC and paracancerous tissues collected from patients with NPC were detected. The CNE-2 cells with the lowest miR-185 among the five NPC cell lines (CNE-1, CNE-2, HNE-1, HNE-2, and 5-8F) were selected and transfected with a series of mimic or inhibitor of miR-185, or shRNA-against HOXC6. The Kaplan-Meier method was used to analyze the survival of patients. Besides, the reverse transcription quantitative polymerase chain reaction (RT-qPCR) and western blot analysis were used to determine the levels of related genes/proteins. By means of cell counting kit-8 (CCK-8) assay, transwell assay, flow cytometry, and AO staining, the influences miR-185 has on the processes associated with NPC, including cell proliferation, invasion, apoptosis and autophagy were evaluated. RESULTS:NPC was observed to decrease miR-185 but increase HOXC6. Dual luciferase reporter gene assay demonstrated that HOXC6 is a target gene of miR-185. Increased mRNA and protein levels of Bax, caspase-3, LC3 and Beclin1 and reduced levels of HOXC6, TGF-β1, mTOR, Cyclin D1, PCNA, Bcl-2 were found by overexpression of miR-185. High expression of miR-185 and low expression of HOXC6 had longer survival time of NPC patients. Overexpressed miR-185 enhanced cell apoptosis and autophagy, and reduced cell proliferation and invasion, while miR-185 inhibitor was observed to have induced effects on the CNE-2 cells. CONCLUSION:Overall, the data show that miR-185 could negatively target HOXC6 to suppress cell proliferation, promotes apoptosis and autophagy through inhibiting TGF-β1/mTOR axis in NPC. Thus, miR-185 is useful strategy for the treatment of NPC.
目的 构建抗CD133单链抗体(scFV)/白喉毒素DAB389融合蛋白,在大肠杆菌里表达,并鉴定及纯化.方法 采用PCR的方法,扩增融合基因DAB389-Linker-CD133 (scFV),并插入到原核表达载体pET28a中,构建了重组表达载体pET28a-DAB389-CD133,融合蛋白在BL21 (DE3)中表达,表达产物经SDS-PAGE鉴定,通过Ni柱纯化目的蛋白,获得了纯度达95%的DAB389-Linker--CD133 (scFV)融合蛋白.Western blot鉴定蛋白活性.流式细胞仪检测能否与CD133抗原特异性结合.结果 扩增的片段与理论值一致,序列分析正确;纯化得到纯度为95%的重组蛋白.Western blot分析能特异性地与抗白喉抗体结合.流式细胞仪检测能与CD133抗原特异性结合.结论 成功构建了抗CD133单链抗体(scFV)/白喉毒素DAB389融合蛋白,具有结合抗原和免疫毒素双重活性,为进一步靶向治疗奠定了基础.
BACKGROUND:The aim of the study was to investigate the effect associated with the protein expression of VEGF, JAK2 and STAT3 on the clinicopathologic characteristics and prognosis in the development and progression of nasopharyngeal carcinoma (NPC).METHODS:Fifty NPC patients in addition to 20 patients with chronic nasopharyngitis (CNP) were recruited for the purposes of the study. Western blotting and immunohistochemistry methods were employed to evaluate the protein expressions of JAK2, STAT3 and VEGF in the NPC and CNP tissues, with their respective correlations with the clinicopathologic characteristics of NPC patients subsequently analyzed. Spearman's rank correlation coefficient and Kaplan-Meier method were conducted to evaluate the respective correlations of JAK2, STAT3 and VEGF with NPC as well as the survival rates of patients with NPC. Cox regression analyses was performed in determine the prognostic NPC factors.RESULTS:Compared with the CNP tissues, the NPC tissues exhibited elevated levels of JAK2, STAT3 and VEGF which were subsequently determined to share a positive correlation with T stages, lymph node metastasis (LNM), N stages and clinical stages, while a negative correlation with survival rates were observed in the NPC patients. Positive correlations between the expressions of JAK2, STAT3 and VEGF were detected among the NPC tissues. NPC patients survival time with negative expressions of JAK2, STAT3 and VEGF were observed to be longer than that of NPC patients with positive expressions of JAK2, STAT3 and VEGF. T stage, LNM, N stage, clinical stage. The expressions of JAK2, STAT3 and VEGF were discovered to be independent risk factors associated with the prognosis of patients with NPC.CONCLUSION:The results obtained from the present study support the notion that higher expressions of JAK2, STAT3 and VEGF may be correlated with the clinicopathologic characteristics and prognosis of patients suffering from NPC.
OBJECTIVE To establish a safety surveillance model of post marketing drugs based on supervision by clinical pharmacists.METHODS Pharmacist dominating and nurse assisting safety surveillance model was established,and Naranjo adverse reaction probability scale (APS) was utilized to evaluate adverse reactions.RESULTS By working mode composed of pharmacists and nurses,the surveillance was better conducted.The results of surveillances were real and reliable as assessed in clinical practices.CONCLUSION This drug safety surveillance mode can ensure integrity,authenticity and accuracy of clinical data.
