Les conséquences secondaires à la radiochimiothérapie curative dans les cancers des VADS sont d’ordre physique et psychosocial, incluant une diminution de la tolérance à l’effort et de la qualité de vie. Le patient rentre alors dans un cercle vicieux du déconditionnement qui, in fine, majore sa vulnérabilité aux traitements et dégrade sa tolérance à l’effort et sa qualité de vie. Les activités physiques adaptées (APA) et une prise en charge nutritionnelle individualisée sont des stratégies cohérentes pour inverser ce cercle vicieux. Soixante-dix patients avec un cancer des VADS non métastatique étaient randomisés dans le groupe « APA » ou « contrôle » entre juillet 2012 et novembre 2015 dans 4 centres de soins régionaux. Le groupe « APA » participait à des séances de 55 minutes d’ElectroStimulation (ES) du quadriceps, 3 fois par semaine durant toute la durée du traitement, suivi de 3 mois d’ergocycle à domicile (3 séances par semaine de 45 minutes). Les deux groupes bénéficiaient d’un suivi nutritionnel selon les recommandations [Nutrition clinique et métabolisme 2012 ;26(4) :149–164]. Des évaluations ont été réalisées avant traitement (V0), à la fin du traitement oncologique (V1) et 3 mois après la fin du traitement (V2). Les mesures portaient sur : 1) la tolérance à l’effort (temps d’endurance), 2) le nombre de pas par jour (mesures actimétriques), 3) la qualité de vie (SF-36, QLQC30). On considérait comme succès (critère d’efficacité principal) un patient qui augmentait son taux d’endurance initial de 40 % ou plus à V2. En moyenne, l’observance aux séances d’ES était de 87 % et pour l’ergocycle de 51 %. L’intervention APA améliorait la tolérance à l’effort d’au moins 40 % chez 68 % des patients (IC95 % [48–84) vs 27 % (IC95 % [16–48]) dans les groupes « APA » vs « contrôle » respectivement (p ≤ 0,05)[T.S1].[PB2]. L’évolution médiane du temps d’endurance entre V0 et V2 était de +27 % vs −43 % dans les groupes « APA » vs « contrôle » respectivement (p = 0,001). Des effets significatifs étaient observés sur le nombre de pas moyen réalisé par jour : 4050 (V0) à 4096 (V2) dans le groupe « APA » vs 4010 (V0) à 3257 (V2) dans le groupe « contrôle » (p = 0,003). Les analyses de qualité de vie n’objectivaient aucun effet positif ou négatif. L’étude NUTRIMOUV a permis de montrer qu’une intervention APA couplée à une prise en charge nutritionnelle adaptée était faisable dans cette population. De plus, cette prise en charge améliorait la tolérance à l’effort sans modification de la qualité de vie. Les effets observés étaient principalement en lien avec l’utilisation de l’ergocycle. En effet, l’ES pendant le traitement n’avait pas d’impact positif sur le temps d’endurance des patients. Une étude de phase III comparative serait justifiée pour valider ces résultats.
INTRODUCTION:The aim of our study was to investigate whether exclusive home-based training was feasible and effective in COPD patients and if patients have a persistent improvement in their level of physical activity after this intervention. METHODS:One hundred COPD patients (FEV1=42.6% predicted) first underwent 25 weekly sessions of supervised cycle ergometry training, followed by one year of monthly supervised follow-up. Six minutes walking test, endurance test, BODE index and activity monitoring were performed before, after and one year after inclusion. RESULTS:About 80% of the patients completed the program. They improved their exercise tolerance and their daily physical activity level, even one year after inclusion. However, more severe patients did not maintain these benefits. Daily physical activity quantity was also decreased in these patients one year after inclusion. CONCLUSION:This home-based exercise training program is feasible and effective. Improvements are sustainable for the majority of patients. However, severe patients were not able to maintain these benefits. Other exercise training strategies may have to be considered in these patients.

Some of the pathophysiological mechanisms involved in this O-2 desaturation could be modified by individualized exercise training as part of a pulmonary rehabilitation programme. The aim of this study was to determine the effect of pulmonary rehabilitation on O-2 desaturation exhibited by COPD patients during a 6MWT.Methods Twenty COPD patients (FEV1 = 61.1 +/- 3.2% predicted) who exhibited 02 desaturation before rehabilitation (mean 7.3 +/- 0.7% with a mean duration of 5.3 +/- 0.1 min) participated. They performed four weeks of RP including individualized whole-body exercise training achieving a mean 9.3 +/- 0.27 hours per week of exercise tailored to their ventilatory threshold.Results Dyspnoea at the end of the test, ventilatory threshold and FEV, were retained as correlates of desaturation before rehabilitation. After rehabilitation, 6MWT distance increased (p < 0.01) with reduced dyspnoea (p < 0.05). Two sub-groups were identified: persistent desaturaters (DS, n = 13) and non-desaturers group (NDS, n = 7). There were no baseline differences between the two groups. After rehabilitation only the persistent desaturaters showed a significant increase in distance achieved during 6MWT associated with a reduced dysp nea (p < 0.05). This group showed a mean 02 desaturation equal to 8.1 +/- 0.9% which persisted to 5 +/- 0.3 min. A tendency to a lower dyspnoea at the end of 6MWT performed before rehabilitation was observed in NDS compared with DS (p < 0.058).Conclusion It seems that responses to a pulmonary rehabilitation programme including individualized exercise training could act on O-2 desaturation. Indeed 7 of 20 (35%) COPD patients exhibiting O-2 desaturation during a 6MWT showed no O-2 desaturation after rehabilitation programme while 13 on 20 (65%) do it.
Depuis près de dix ans, la thérapie antirétrovirale (ARV) a amélioré le pronostic de l’infection au VIH-1 et a démontré un meilleur contrôle de l’excrétion virale en réduisant la charge virale dans le sperme. Cependant, les traitements par les inhibiteurs nucléosidiques de la transcriptase inverse (INTI) ont pu révéler d’importants effets indésirables. La majeure partie de ces effets semble associée à un mécanisme commun : une altération de l’activité mitochondriale. Depuis l’application de la prise en charge des couples sérodifférents, dont l’homme est infecté par le VIH-1 et sous thérapie avec INTI, de nombreux résultats dans la littérature, tels que paramètres spermatiques et issue des grossesses, ont attiré l’attention des équipes de recherche. Bien que ces études suggèrent un effet des INTI sur certains paramètres du sperme, les mécanismes responsables de ces effets sont peu explorés. Les INTI possèdent une forte affinité pour la transcriptase inverse du VIH-1. Comme tous les INTI ne sont pas seulement inhibiteurs de la transcriptase inverse mais aussi inhibiteurs de l’ADN polymérase ß et γ, plusieurs effets toxiques peuvent être envisagés. Cette spécificité n’est cependant pas absolue et des incorporations « accidentelles » d’INTI peuvent être faites sur l’ADN génomique des spermatozoïdes. Une seule étude sur le génome des spermatozoïdes de patients sous thérapie INTI a été publiée sans résultats concluants. Récemment, des travaux ont suggéré que l’exposition aux INTI pouvait induire une altération de l’activité des mitochondries de spermatozoïdes. Les INTI sont connus pour induire une augmentation de la génération de substances oxygénées réactives qui favorise la dégradation du potentiel trans-membranaire (Δψm) de la mitochondrie. Cette perte du Δψm peut induire le relargage de facteurs spécifiques de l’apoptose, par exemple le cytochrome c de la mitochondrie, qui initie l’apoptose. La fragmentation de l’ADN des spermatozoïdes, associée à l’apoptose, a été rapportée comme une cause possible de fausses couches spontanées à répétition. Si l’incorporation des INTI a été rapportée dans l’ADN des cellules somatiques, l’absence de données sur l’ADN génomique du spermatozoïde justifie que soit mise en place une étude sur les effets de l’exposition paternelle aux INTI sur le matériel génomique du gamète mâle, notamment en raison de son implication dans le développement du zygote après la fécondation.For the last ten years, antiretroviral therapy (ARV) has improved the prognosis in HIV-1 infection and showed a better control of the viral excretion by reducing viral shedding in semen. However, nucleoside analogues reverse transcriptase inhibitors (NRTI) therapy reported important adverse effects. Most of these side effects observed seem to be linked with a common mechanism: mitochondrial activity alteration. Since the introduction of protocols for HIV-1 serodiscordant couples, with male infected partners under NRTI therapy, many results in the literature such as: semen characteristics and pregnancies, drew the attention of research teams. Many studies have suggested that NRTI has an affect on semen parameters, but proposed mechanisms of these effects have rarely been discussed. NRTI have a great affinity for the reverse transcriptase of HIV-1. Because many NRTI are not only inhibitors of reverse transcriptase but also inhibitors of the DNA polymerase β and γ, several toxic effects can be considered. Nevertheless, this specificity is not absolute and “accidental” incorporations of NRTI can occur on genomic sperm DNA. Only one study on genomic sperm DNA with patients under NRTI therapy was published without concluding results. Recently, studies have suggested that NRTI exposure could induce an alteration on mitochondrial energy-generating ability of spermatozoa. NRTI are known to induce an increase in the generation of reactive oxygen species, which results in the degradation of mitochondrial transmembrane potential (Δψm). This loss of Δψm can tend to release some specific apoptosis factors, such as cytochrome c, that initiates programmed cell death. Sperm DNA fragmentation, associated to apoptosis, was reported as a possible cause of recurrent pregnancy loss. If the incorporation of NRTI was reported in genomic DNA of somatic cells, the absence of data on the genomic sperm DNA justifies further studies concerning the effects of paternal exposure to NRTI on the genomic material of the male gamete, in particular because of its implication in the zygote development after fertilization.
