Abstract Study question Is there an impact of CFTR (cystic fibrosis transmembrane-conductance regulator) gene mutation type and/or general health condition of men with CF and the ICSI outcomes? Summary answer Neither genetic severity nor clinical severity seems to have a negative impact on the biological or clinical ICSI issues for cystic fibrosis men patients. What is known already CFTR protein regulates electrolyte and fluid transport in many tissues with exocrine function, including male reproductive tract. Mutation of CFTR gene causes CF, which affects the function of several organs, and impairs male fertility. CF is generally associated to an obstructive azoospermia because of bilateral absence of vas deferens and seminal vesicles degeneration. CFTR protein is detected in human fetus at the early developmental stages and highly expressed in testis and epididymis. CFTR provides establishment of specific fluid environment for germ cell differentiation and maturation. According to literature, it seems likely that CFTR mutations affect sperm quality and embryo development. Study design, size, duration This cohort study was conducted in the Assisted Reproduction Center of an university hospital on 52 patients, for whom a CF has been diagnosed and monitored in the same hospital. The CF diagnosis was based on CFTR genetic tests and clinical symptoms. All men were azoospermic and underwent on a microscopic epididymal sperm aspiration (MESA) and/or a testicular sperm extraction (TESE). 52 couples underwent in total 107 ICSI with frozen sperm between 1999 and 2019. Participants/materials, setting, methods Clinical data- one year preceding surgical sperm collection and freeze - related to the severity of the CF were collected: respiratory spirometry data, Pseudomonas aeruginosa colonization, number of antibiotic treatment, BMI and Pancreatic insufficiency, as well as CFTR mutations. Linear regression tests (fisher and χ²) were carried out to establish or not correlation between biological and clinical ICSI outcomes and on the other hand genotype and/ or each clinical parameter. Main results and the role of chance The mean age of patients at sperm retrieval was 31.3 years [21-55]. Mean BMI was 21.3. Patients with severe genotype represented 67.8% and likely (67.3%) with external pancreatic deficiency. P.aeroginosa colonization was revealed in 30% of cases and 45.6% patients received at least one IV antibiotic treatment. Respiratory function has been impaired in 18 cases, with a maximal expiratory volume per second < 40%. Spermatozoa were found and frozen for all patients after surgical sperm retrieval (20 MESA and 32 MESA + TESE). The mean number of epididymal motile progressive spermatozoa was satisfactory (10.2±18.83 million). Women mean age was 30 years at ICSI and female infertility was associated in 15.4%. Fertilization and cleavage rates were 66% both. Mean number of transferred and/or frozen embryos per attempt was 3. Implantation rate was 20%. A total of 50 pregnancies and 44 live births were obtained (35.5% of cumulative clinical pregnancy rate per ICSI and 29.1% of live birth rate per transfer). Fertilization rate, absence of pregnancy or absence of live birth were not affected by the genetic severity (p = 0.63, p = 0.46 and p = 0.16 respectively). Likewise, and after several univariate analyses there was no statistically significant correlation between clinical severity and ICSI outcomes. Limitations, reasons for caution Practices of assisted reproduction have constantly evolved during the study period, this concerns the controlled ovarian stimulation protocols, IVF techniques, equipment and culture media used. Our study needs to be completed by a multivariate analyse Wider implications of the findings Regarding to better quality of life of CF patients nowadays and to our results- with no significant correlation between genetic and/or clinical severity and ICSI outcomes, surgical sperm collection can be proposed only in case of a conceptional project or before transplantation and immunosuppressive agents in order to perform ICSI. L’efficacité de l’ivacaftor, un modulateur de la protéine CFTR, chez les patients atteints de mucoviscidose (MV) a été démontrée dans des essais randomisés contrôlés de phase 3. L’étude BRIO est une étude observationnelle évaluant l’efficacité de l’ivacaftor chez des patients atteints de MV et porteurs de mutations répondant à l’ivacaftor, en France. Cette analyse décrit l’impact de l’ivacaftor sur l’utilisation des ressources en santé (URS) chez des patients de plus de 6 ans. L’étude BRIO est une étude observationnelle et multicentrique en cours dans 33 centres. Les patients atteints de MV, déjà traités ou nouvellement traités par ivacaftor sont éligibles. Les données des patients sont recueillies de manière prospective au cours des deux ans suivant l’inclusion et de manière rétrospective au cours des 12 mois précédant le début de l’ivacaftor et jusqu’à leur inclusion dans l’étude. L’analyse décrit l’URS : les hospitalisations, toutes causes confondues, et l’utilisation d’antibiotiques (voie orale/IV/inhalée) pour exacerbation respiratoire (ER) dans les 12 mois pré- et post-début de l’ivacaftor. L’analyse statistique a utilisé un modèle binomial négatif ajusté selon la durée dans l’étude sur les 12 mois (référence : date de début de l’ivacaftor). Sur 107 patients inclus, 100 ont reçu au moins 12 mois de traitement par ivacaftor. À l’inclusion, l’âge moyen (écart-type) était de 21,1 (14,3) ans et 56 % des patients étaient de sexe masculin. Le taux d’hospitalisation, toutes causes confondues, par patients-années (PA) a significativement diminué dans les 12 mois post- par rapport aux 12 mois pré-début de l’ivacaftor (rapport des taux : 0,40 ; intervalle de confiance [IC] à 95 % : 0,26–0,61). Le nombre de jours d’hospitalisation par PA a diminué de 5,3 jours avant l’ivacaftor à 2,5 jours après le début de l’ivacaftor (rapport des taux : 0,46 ; IC 95 % : 0,22–0,95). Le nombre de traitements antibiotiques par PA pour les ER a baissé dans les 12 mois post- par rapport aux 12 mois pré-ivacaftor (rapport des taux : 0,47 ; IC 95 % : 0,32–0,68). Le nombre de jours de traitements d’antibiotiques en PA a baissé de 20,9 jours à 11,4 jours pré-/post-ivacaftor (rapport des taux : 0,54 ; IC 95 % : 0,40–0,72). Le traitement des patients atteints de MV par l’ivacaftor en vie réelle en France a entraîné une réduction de l’URS : diminution des hospitalisations toutes causes confondues et des traitements antibiotiques pour ER. L'analyse décrit le VEMS et l'indice de masse corporelle (IMC) durant les 12 premiers mois sous ivacaftor, les exacerbations respiratoires (ER) durant les 12 mois pré- et post- début de l'ivacaftor, et les cas de décès et de greffe pulmonaire. L'analyse statistique a été réalisée par modèle mixte (mesures répétées pour les variables continues) et binomial négatif pour les ER (référence : date de début de l'ivacaftor). Sur 107 patients inclus, 100 ont reçu au moins 12 mois de traitement par ivacaftor ; 62 % des patients étaient porteurs d'une mutation G551D sur au moins un allèle, et 56 % étaient de sexe masculin. À l'inclusion, l'âge moyen (écart-type) était de 21,1 (14,3) ans, avec 56 (52 %) des patients âgés de moins de 18 ans. Les améliorations des VEMS, IMC (patients ≥ 20 ans) et z-score de l'IMC (patients < 20 ans) étaient significatives sur les 12 premiers mois de traitement (Tableau 1). Cent quatre ER (49 patients) ont été rapportées lors des 12 mois pré-ivacaftor et 54 ER (36 patients) lors des 12 mois post-début de l'ivacaftor, avec réduction significative des ER par patient-année, post-ivacaftor (0,51) versus pré-ivacaftor (0,97) (ratio des fréquences : 0,53 ; intervalle de confiance [IC] à 95 % : 0,40–0,69). Aucun décès n'est survenu et une greffe pulmonaire a été rapportée. L'ivacaftor a été arrêté chez 2 patients en raison de leur inclusion dans une étude interventionnelle. Aucun nouveau signal de sécurité n'a été noté et 68/73 des événements indésirables rapportés ont été jugés non liés à l'ivacaftor. À l'instar des précédents essais de phase 3, l'utilisation de l'ivacaftor en vie réelle chez des patients atteints de MV confirme une amélioration significative du VEMS, de l'IMC et de la fréquence des ER. The efficacy of ivacaftor in cystic fibrosis (CF) patients has been demonstrated in Phase 3 randomized-controlled trials. BRIO is a real-world study assessing effectiveness of ivacaftor in patients with CF and ivacaftor-responsive mutations in France. This interim analysis describes the impact of ivacaftor on healthcare resource utilization (HCRU) in patients ≥6 years. BRIO is an ongoing observational, non-interventional multi-centre study. Of 55 CF centers contacted, 33 participated. Ivacaftor-naive and -treated patients were eligible. Patient data are collected prospectively for two years after enrolment, and retrospectively for 12 months preceding initiation of ivacaftor, and up to enrolment in the study. This interim analysis describes HCRU, including all-cause hospitalisation and antibiotics use (oral/IV/inhaled) due to pulmonary exacerbations (PEx), for 12 months pre-ivacaftor initiation and 12 months post-ivacaftor initiation. HCRU analysis used a negative binomial model adjusted for time on study in the 12-month period, with baseline defined as ivacaftor initiation date. Of 107 enrolled patients, 100 received ≥12 months treatment with ivacaftor. At baseline, mean (SD) age was 21.1 (14.3) years, 56% were male. All-cause hospitalisations showed a substantial reduction per patient year (PPY) in 12 months post-ivacaftor initiation vs 12 months pre-ivacaftor initiation (rate ratio [RR] 0.40; 95% CI: 0.26–0.61). Corresponding hospitalisation days PPY were reduced from 5.3 days pre-ivacaftor to 2.5 days post-ivacaftor (RR 0.46; 95% CI: 0.22–0.95). Number of unique antibiotics PPY for treatment of PEx was also reduced 12 months post-ivacaftor vs 12 months pre-ivacaftor (RR 0.47; 95% CI: 0.32–0.68). Corresponding days of antibiotic use PPY declined from 20.9 days pre-ivacaftor to 11.4 days post-ivacaftor (RR: 0.54; 95% CI: 0.40–0.72). Treatment of CF patients with ivacaftor in a real-world setting in France led to substantial reduction in HCRU, as evidenced by decreased all-cause hospitalisations and antibiotics use due to PEx.

L'observance des antibiotiques inhalés dans la mucoviscidose est un enjeu majeur pour le pronostic, au regard du rôle des infections à Pseudomonas aeruginosa. Les formes poudres de manipulation plus simple que les nébulisations font espérer une meilleure observance (Fig. 1). L'étude réalisée chez 126 patients recrutés au sein de 42 CRCM de France métropolitaine a évalué celle-ci par le score de Morisky (un score > 2 définit un patient observant) calculé sur deux cures d'antibiotiques inhalés à 12 mois d'intervalle, la première sous forme nébulisée (NEB) et la seconde sous forme poudre TOBI Podhaler (POD). Quatre-vingt-huit patients ont été analysés. À l'inclusion, 50 % [39,3–60,7 %] des patients étaient observants sous NEB (Morisky NEB = 2,5 ± 1,2), et 59,6 % [45,3–72,4] l'étaient à 12 mois sous POD (Morisky POD = 2,7 ± 1,0), soit une différence de 9,6 %. L'analyse par tranche d'âge montre une meilleure observance chez les enfants et adolescents et un effet POD plus marqué dans ces tranches d'âge (Fig. 1). La fréquence des évènements indésirables a été faible et leur nature conforme à celle décrite dans le RCP. Il existe une meilleure observance du traitement avec POD (59,6 % de patients observants) comparativement à celle décrite lors de l'inclusion avec les formes nébulisées (50,0 % de patients observants). L'observance des antibiotiques inhalés est meilleure chez les enfants et adolescents que chez les adultes.
Since the discovery of chloride secretion by the Cystic Fibrosis Transport regulator CFTR in 1983, and CFTR gene in 1989, knowledge about CFTR synthesis, maturation, intracellular transfer and function has dramatically expanded. These discoveries have led to the distribution of CF mutations into 6 classes with different pathophysiological mechanisms. In this article we will explore the state of art on CFTR synthesis and its chloride secretion function. We will then explore the consequences of the 6 classes of mutations on CFTR protein function and we will describe the new therapeutic developments aiming at correcting these defects.

Le traitement de la mucoviscidose est resté longtemps symptomatique. Des thérapeutiques ciblant la protéine CFTR défectueuse commencent à être disponibles. Les résultats de la thérapie génique restent modestes mais une étude clinique récente montre un bénéfice sur le VEMS. Les avancées concernent surtout les thérapies ciblant la protéine CFTR. Une amélioration respiratoire importante (gain de VEMS d’environ 10 % et baisse des exacerbations respiratoires) a été obtenue sous ivacaftor, potentiateur de CFTR, chez les patients porteurs de mutations gating, aboutissant à l’obtention d’une Autorisation de mise sur le marché (en 2012 pour la mutation G551D et en 2015 pour des mutations plus rares). Chez les patients homozygotes phe508del, l’association de l’ivacaftor à un correcteur de CFTR (lumacaftor) permet une amélioration respiratoire plus modeste. L’efficacité de l’ataluren chez des patients porteurs de mutations non-sens est en cours d’évaluation. De nouveaux correcteurs et potentiateurs de CFTR sont en développement. Ils pourraient changer le cours de la maladie mais les enjeux médicoéconomiques ne doivent pas être sous-estimés. L’ivacaftor peut être prescrit chez les patients porteurs d’une mutation de classe 3 à partir de l’âge de 6 ans. Prochainement, l’Orkambi® sera disponible pour les patients homozygotes phe508del à partir de 12 ans. The treatment of cystic fibrosis has been symptom-based for a number of years. New therapies that aim to improve CFTR protein function are now emerging. The results of gene therapy has been modest but a recent clinical trial shows a positive effect on FEV1. Recent research has focused primarily on CFTR protein function. Significant respiratory improvement (an average 10% FEV1 increase and a decrease in the frequency of exacerbations) has been achieved with ivacaftor, a CFTR potentiator, in patients with gating mutations, resulting in its marketing authorization (in 2012 for the G551D mutation and in 2015 for rarer mutations). In phe508del homozygous patients, the combination of ivacaftor with a CFTR corrector (lumacaftor) has also led to respiratory improvement, albeit less impressive. The effectiveness of ataluren in patients with nonsense mutations is being evaluated. New CFTR correctors and potentiators are being developed. CFTR protein therapy could change the course of the disease but cost/effectiveness issues should not be overlooked. Ivacaftor can be prescribed in CF patients with a class 3 mutation from the age of 6 years. The Orkambi® will soon be available for homozygous phe508del patients from the age of 12 years.
