Fenestration is the standard treatment for hepatic cysts, but recurrence remains a concern. We report the case of a 73-year-old woman who presented with leg edema. Computed tomography revealed a 140-mm hepatic cyst extending from segments 8 to 1, compressing the inferior vena cava (IVC). Preoperative 3D simulation using artificial intelligence-assisted imaging software (SYNAPSE VINCENT) demonstrated that the exposed area of the cyst wall not covered by liver parenchyma was limited to the space part of the caudate lobe behind the hepatoduodenal ligament. Laparoscopic fenestration was performed with intraoperative indocyanine green (ICG) guidance to avoid injury to the liver and biliary tract while maximizing the fenestration area. The patient experienced no postoperative complications, and no symptomatic recurrence was observed during 6 months of follow-up. This case highlights the utility of preoperative 3D simulation for fenestration site selection in a challenging hepatic cyst.
PURPOSE:Aortic calcification is associated with arteriosclerosis and is often seen in patients undergoing hepatectomy. However, its impact on post-operative outcomes after major hepatectomy is still unclear. METHODS:From July, 2015 to December, 2022, 127 patients who underwent resection of three or more adjacent liver segments (major hepatectomy) were retrospectively reviewed. Aortic calcification at the origin of celiac artery was assessed on pre-operative abdominal CT scan images. pPerioperative factors and postoperative outcomes were compared between patients with and without aortic calcification. Uni-variable and multi-variable analyses were performed to identify risk factors of posthepatectomy liver failure (PHLF). RESULTS:Aortic calcification at the origin of celiac artery was observed in 62 (48.8%) of 127 patients. Those with aortic calcification were significantly older and had a higher incidence of hypertension, as a comorbidity, compared to those without. Furthermore, incidences of both post-operative liver failure and other complications were significantly higher among patients with aortic calcification, who also had a longer hospital stay. Multivariable logistic analysis identified aortic calcification and longer operation time as independent risk factors of PHLF. Additionally, stenosis of the celiac artery also impacted the development of PHLF. CONCLUSION:These findings indicate that aortic calcification at the origin of celiac artery is associated with advanced age and may be a risk factor of PHLF following major hepatectomy.
Annular pancreas is a rare congenital anatomical anomaly, in which the pancreatic parenchyma surrounds the descending duodenum. Generally, annular pancreas is diagnosed on the basis of symptoms associated with complications of peptic ulcer, pancreatitis, cholelithiasis, and rarely, malignant tumors. Herein, we report an 84-year-old man for whom, during hospitalization for a urinary tract infection, pancreatic cystic lesions and an annular pancreas were noted incidentally on computed tomography. These findings led to a diagnosis of intraductal papillary mucinous neoplasms on further examination. He safely underwent robot-assisted pylorus-preserving pancreatoduodenectomy, with an operative time of 478 min and blood loss of 37 g. He was discharged on postoperative day 8 without postoperative complications. In conclusion, it is important to note that, in this case, intraductal papillary mucinous neoplasms were detected before they became malignant, and minimally invasive surgery was performed safely despite the anatomical anomaly of an annular pancreas.
INTRODUCTION:Gallbladder bed detachment (GBB-D) in laparoscopic cholecystectomy (LC) lacks a standardized technique. We developed butterfly traction (BT) to perform optimal GBB-D. This study evaluates its effectiveness and potential for standardization. METHODS:BT aligns the dissection axis with the surgeon's dominant hand by positioning the gallbladder upright and using a left-sided lateral approach. This improves right-side mobility, enabling horizontal traction and avoiding axis interference. BT has been adopted in all LCs at our institution since July 2023. Among 118 LCs performed from 2019 to 2024, three open conversion cases and 13 bailout cases were excluded. Operative outcomes in the remaining patients were compared between the BT group (n = 50) and the nonstandardized (NS) group (n = 52). RESULTS:No significant differences were observed in age, gender, comorbidities, or preoperative gallbladder drainage between groups. However, the BT group had more surgeons with ≤ 5 years of experience (52% vs. 29%, p = 0.017). Operative times were comparable (p = 0.363), but bile contamination due to gallbladder perforation during GBB-D was significantly lower in the BT group (4% vs. 17%, p = 0.030), as was blood loss (0 vs. 1 g, p = 0.003). BT was particularly effective in higher difficulty cases (Grade B/C from Tokyo Guidelines 2018), with shorter operative times (95 vs. 116 min, p = 0.095), less blood loss (0 vs. 2 g, p < 0.001), and fewer perforations (6% vs. 24%, p = 0.046). CONCLUSIONS:BT improved surgical outcomes and minimized bile contamination, even when performed by less experienced surgeons. It shows strong potential as a standard technique for GBB-D in LC.
OBJECTIVES:To examine the significance of tumor budding as a prognostic factor of resected pancreatic ductal adenocarcinoma (PDAC) specimens after preoperative chemoradiotherapy (CRT). MATERIALS AND METHODS:Among 162 PDAC patients who underwent pancreatectomy after gemcitabine and S1-based CRT from 2012 to 2019, 131 were evaluated for tumor budding. Tumor buds were counted at the invasive front, where the degree of budding was the greatest (hematoxylin and eosin staining, ×20 magnification). Overall survival (OS), disease-specific survival (DSS), and recurrence-free survival (RFS) were compared between the patients without tumor budding (non-TB group) and those with tumor buddings (TB group). Multivariate Cox proportional hazards analysis was conducted to examine the significance of tumor budding as a prognostic factor. RESULTS:OS, DSS, and RFS (median survival time) of the non-TB group were significantly longer than those of the TB group (OS: 50.7 vs 27.5 months, P = 0.014; DSS: 63.3 vs 33.0 months, P = 0.014; RFS: 20.3 vs 11.3 months, P = 0.028). Multivariate analysis identified adjuvant chemotherapy ( P = 0.003) as a favorable prognostic factor of OS and tumor budding ( P = 0.023) as an adverse prognostic factor of DSS. CONCLUSIONS:This study revealed that the presence of tumor budding was an independent adverse prognostic factor in PDAC patients resected after gemcitabine and S1-based CRT.
Laparoscopic liver resection (LLR) is a surgical procedure with varying degrees of difficulty depending on tumor status and surgical technique. Therefore, we aimed to evaluate the relationship between surgical difficulty levels and outcomes of LLR, particularly portal vein thrombosis (PVT). We performed LLRs in 214 patients between January 2009 and December 2022. Among them, 200 patients who underwent pure LLR were allocated into three groups according to the Institut Mutualiste Montsouris (IMM) classification: Group I (n = 152), Group II (n = 26), and Group III (n = 22). The perioperative outcomes were also compared; risk factors for postoperative complications were evaluated. The operation time was significantly longer (p < 0.001) and intraoperative blood loss was significantly higher (p < 0.001) in Group III than in Groups I and II. PURPOSE:Although laparoscopic pancreaticoduodenectomy (LPD) and robot-assisted pancreaticoduodenectomy (RPD) are gradually gaining popularity, their advantages over open pancreaticoduodenectomy (OPD) remain controversial. This study aimed to compare the short-term outcomes of OPD, LPD, and RPD to elucidate the advantages and disadvantages of each procedure. METHODS:We retrospectively analyzed 16 LPD, 43 RPD, and 36 OPD procedures performed at a single center between April 2020 and May 2024. Clinical data, including operative time, estimated blood loss, postoperative complications, length of hospital stay, and hospitalization costs, were retrospectively collected and analyzed. RESULTS:RPD demonstrated a significantly longer operative time (553 min) than OPD (446 min) and LPD (453 min) but a significantly lower estimated blood loss than OPD (150 mL vs. 400 mL, p < .001). Postoperative complication rates (Clavien-Dindo grade ≥3) were lower for RPD (24.4%) than those for OPD (50.0%) and LPD (68.8%). RPD also showed a significantly lower rate of clinically relevant postoperative pancreatic fistula (14.6% vs. 38.9% for OPD and 43.8% for LPD) and a shorter duration of hospitalization (11 vs. 28 days for OPD and 21 days for LPD, p < .001). Hospitalization costs were higher for RPD (20 109 USD) than for OPD (18 487 USD, p < .001), with LPD (20 496 USD) and RPD costs being similar. CONCLUSIONS:RPD appears to offer advantages in terms of reduced blood loss and postoperative complications and shortened hospital stay despite longer operative times and higher hospitalization costs. Therefore, RPD may be a more beneficial approach than OPD or LPD in pancreatic surgery.
Background: This study aimed to assess the safety and efficacy of gemcitabine plus S-1-based chemoradiotherapy (GS-CRT) among patients with locally advanced pancreatic ductal adenocarcinoma (PDAC), especially among those with unresectable locally advanced (UR-LA) cases. Methods: A total of 351 consecutive PDAC patients were enrolled and prognostic predictors of disease-specific survival (DSS) were identified. Results: The treatment completion rate was 98.9% and Grade 3 or higher adverse events occurred in 181 cases (51.6%). Among 319 re-evaluated patients, pancreatectomy was performed in 184 (57.7%). Based on resectability, the 5-year DSS rates for the entire cohort were 39.6% (R), 43.8% (BR-PV), 21.2% (BR-A) and 13.3% (UR-LA), while the predictors of DSS were performance status (PS), hemoglobin (Hb) level, celiac artery (CA) involvement of ≥180 degrees and JPS 8th T category. In the resected cases, the predictors of DSS were preoperative PS, preoperative CA19-9 level, preoperative JPS-T factor, degree of histological response and adjuvant chemotherapy. In UR-LA resected patients, preoperative prognostic nutritional index (PNI), absence of pathological venous invasion and adjuvant chemotherapy were predictors of DSS. Conclusions: Even though Grade 3 or higher adverse events were encountered in about half of the cases, they were uneventfully managed. Therefore, GS-CRT is safe and highly tolerable with potential to improve patients' prognosis. Preoperative PS, CA19-9 levels and histological response are important prognostic factors, as well as adjuvant therapy. In UR-LA patients, prognostic nutritional index (PNI) and adjuvant chemotherapy were important for curative intent surgery.

This study investigates the influence of solution treatment of Shirasu-based materials on their adsorption ability of carbon dioxide. Various concentrations of calcined Shirasu-based materials were treated in aqueous ammonia solution, and the adsorption ability of the treated materials significantly increased with decreasing the concentration. The material with the highest adsorption ability (0.41 mmol-CO2g-1) shows comparable ability to conventional solid absorbent materials. From the result of DRIFT spectra of the samples after adsorption of carbon dioxide, it is suggested that carbon dioxide molecules are physically adsorbed on bridging protonic acid sites and single protonic site of the treated Shirasu-based materials, and the adsorption amount of carbon dioxide on both the sites increases with decreasing the concentration of the sample treated in aqueous ammonia solution. The amount of adsorbed water on Shirasu-based materials also influences the adsorption ability to carbon dioxide, and the ability tends to increase with decreasing the amount of adsorbed bulk-like water.
OBJECTIVE:Pancreatic stump closure in minimally invasive distal pancreatectomy (DP) commonly utilizes staplers due to its simplicity; however, postoperative pancreatic fistula (POPF) remains the most frequent complication. We have developed a novel stump closure technique using a transpancreatic mattress suture with a polyglycolic acid sheet (TP method) under robotic DP. This study aims to evaluate the efficacy of the TP method. MATERIALS AND METHODS:This study included 145 cases of pure minimally invasive DP performed between February 2011 and July 2024: 34 robotic (R) and 97 laparoscopic (Lap). Surgical outcomes were compared across 3 groups based on the stump closure method: 18 cases using the TP method (TP group; R: 18), 22 with hand-sewn closure in a fish-mouth manner (FM group; Lap: 22), and 91 with a reinforced stapler (S group; R: 16, Lap: 75). Logistic regression analysis was employed to identify risk factors for POPF. RESULTS:POPF occurred in 20 of 131 cases (grade B: 19, C: 1, 15.3%). The TP group exhibited the lowest POPF rate (TP vs FM vs S: 5.6% vs 27.3% vs 14.3%). Multivariate analysis identified pancreatic thickness ≥12 mm as an independent risk factor. For thickness <12 mm, no significant differences in POPF rates were observed (TP vs FM vs S: 0% vs 25.0% vs 5.6%). However, for thickness ≥12 mm, the TP group had a significantly lower POPF rate compared with the S group (TP vs FM vs S: 7.7% vs 28.6% vs 47.4%). CONCLUSION:The TP method is superior to stapler closure in preventing POPF, especially in cases with pancreatic thickness of ≥12 mm.
The efficacy of preoperative chemoradiotherapy (CRT) for anatomically resectable pancreatic adenocarcinoma (R-PDAC) remains contentious. This study aims to elucidate the treatment outcomes of preoperative CRT for R-PDAC and to identify prognostic factors. This retrospective study included 109 R-PDAC patients treated with gemcitabine- or S-1 plus gemcitabine-based preoperative CRT from February 2005 to April 2023. Cox proportional hazards regression was employed to identify factors associated with worse overall survival (OS). Among the 109 cases, 90 patients (82.6%) underwent curative-intent resection following CRT. The median OS for the entire cohort was 36.5 months, significantly longer in resected cases than in unresected cases (40.6 vs 11.4 months). Multivariate analysis identified pretreatment serum levels of carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) and the Eastern Cooperative Oncology Group performance status ≥1 as independent prognostic factors. When these factors were scored (risk score 0-3) using pre-CRT serum CA19-9 level (≥640 U/mL) and CEA (≥6.1 ng/mL), OS for risk scores 1 (n = 41), 2 (n = 20) or 3 (n = 6) was significantly shorter than for risk score 0 (n = 42) (score 0 vs 1 vs 2 vs 3: 49.1 vs 33.8 vs 16.1 vs 16.8 months). The presence of portal vein invasion on imaging and post-CRT serum CA 19-9 level (≥111 U/mL) were independent prognostic factors in resected cases. Biological factors, including serum levels of CA19-9 and CEA, along with conditional factor of Eastern Cooperative Oncology Group performance status ≥1, were identified as independent prognostic factors for R-PDAC patients treated with preoperative CRT. Preoperative CRT is considered effective for cases lacking these risk factors.
PURPOSE:The Tokyo Guidelines 2018 introduced the Surgical Difficulty Score (TGDS18) to assess laparoscopic cholecystectomy (LC) difficulty based on intraoperative findings. This study aimed to predict surgical difficulty preoperatively using clinical factors correlated with TGDS18. METHODS:Of 369 LC cases for cholecystitis (Jan 2014-Jul 2024), 106 with operative video data were analyzed. Multivariate analysis of 69 with preoperative CT (≤14 days) evaluated the association between preoperative clinical findings and TGDS18 sub-scores (around the gallbladder, Calot's triangle, gallbladder bed, additional findings, unrelated to inflammation). RESULTS:TGDS18 was positively correlated with operative time, blood loss, and hospital stay (all p < .001). Patients undergoing subtotal cholecystectomy had higher TGDS18 scores (median 20, p < .001). Six preoperative findings strongly associated with TGDS18 sub-scores were identified: calcified stone in cystic duct, TG18 Grade ≥2, preoperative gallbladder drainage, urgent operation, pericholecystic inflammation, and age-adjusted Charlson comorbidity index ≥7. The rate of subtotal cholecystectomy increased with the number of findings linked to the "Calot's triangle" sub-score-cystic duct stone and TG18 Grade ≥2. (0% with no findings, 8% with one finding, and 23% with both, p = .009). Similarly, the risk of cholecystectomy requiring the posterior wall left can be predicted by the number of clinical findings related to the 'Gallbladder bed' sub-score (p = .009). CONCLUSIONS:The clinical findings linked to TGDS18 allow tailored preoperative strategies for acute cholecystitis.
PurposeWe proposed a novel morphological classification for intraductal papillary neoplasm of the bile duct (IPNB) and evaluated its association with postoperative prognosis.MethodsForty-two IPNB patients who underwent surgical resection were classified morphologically into three types-branched (n = 10), main duct (n = 26), and mixed (n = 6)-based on preoperative imaging features indicating cystic and/or bile duct involvement. Among them, 32 patients with evaluable specimens were further categorized pathologically into Type 1 (n = 10) and Type 2 (n = 22). Patient characteristics and postoperative outcomes were analyzed.ResultsIntraepithelial neoplasia was more frequently observed in the branched type, whereas invasive carcinoma predominated in the main duct type. In the mixed type, a half of patients involved both intra- and extrahepatic bile ducts, and this type also showed the highest incidence of residual tumor. The mixed type had the poorest 5-year postoperative survival rate (50%), compared to 90% in the main duct type and 100% in the branched type. It also exhibited the highest 5-year recurrence rate (62%). Among IPNB patients with associated invasive carcinoma, tumor infiltration beyond the bile duct wall (p < 0.001) and lymph node metastasis (p = 0.021) were significantly associated with poor prognosis, whereas the anatomical extent of the lesion (intrahepatic, extrahepatic, or both) was not. Morphological classification was significantly correlated with pathological subtypes: the branched type was predominant in Type 1 (60%), while the main duct type predominated in Type 2 (64%) (p = 0.039).ConclusionsOur novel morphological classification of IPNB correlates with postoperative prognosis and may assist in preoperative planning of surgical strategies for IPNB patients.
This study clarified the influence of the splenic vein (SV) on delayed gastric emptying (DGE) in patients who underwent pancreaticoduodenectomy (PD) combined with portal vein (PV) and superior mesenteric vein (SMV) resections. A total of 147 patients who underwent open subtotal stomach-preserving pancreaticoduodenectomy (SSPPD) with PV/SMV resection were included in this study and classified into two groups: SV resection (n = 101) and preservation (n = 46). The incidence of DGE was compared between groups. Univariate and multivariate analyses were used to identify the risk factors for DGE. Hepatic ischemia-reperfusion injury causes liver damage during surgery. In hepatic ischemia-reperfusion injury, the blood coagulation cascade is activated, causing microcirculatory incompetence and cellular injury. Coagulation factor Xa (FXa)- protease-activated receptor (PAR)-2 signaling activates inflammatory reactions and the cytoprotective effect of FXa inhibitor in several organs. However, no studies have elucidated the significance of FXa inhibition on hepatic ischemia-reperfusion injury. The present study elucidated the treatment effect of an FXa inhibitor, edoxaban, on hepatic ischemia-reperfusion injury, focusing on FXa-PAR-2 signaling. A 60 min hepatic partial-warm ischemia-reperfusion injury mouse model and a hypoxia-reoxygenation model of hepatic sinusoidal endothelial cells were used. Ischemia-reperfusion injury mice and hepatic sinusoidal endothelial cells were treated and pretreated, respectively with or without edoxaban. They were incubated during hypoxia/reoxygenation in vitro. Cell signaling was evaluated using the PAR-2 knockdown model. In ischemia-reperfusion injury mice, edoxaban treatment significantly attenuated fibrin deposition in the sinusoids and liver histological damage and resulted in both anti-inflammatory and antiapoptotic effects. Hepatic ischemia-reperfusion injury upregulated PAR-2 generation and enhanced extracellular signal-regulated kinase 1/2 (ERK 1/2) activation; however, edoxaban treatment reduced PAR-2 generation and suppressed ERK 1/2 activation in vivo. In the hypoxia/reoxygenation model of sinusoidal endothelial cells, hypoxia/reoxygenation stress increased FXa generation and induced cytotoxic effects. Edoxaban protected sinusoidal endothelial cells from hypoxia/reoxygenation stress and reduced ERK 1/2 activation. PAR-2 knockdown in the sinusoidal endothelial cells ameliorated hypoxia/reoxygenation stress-induced cytotoxicity and suppressed ERK 1/2 phosphorylation. Thus, edoxaban ameliorated hepatic ischemia-reperfusion injury in mice by protecting against micro-thrombosis in sinusoids and suppressing FXa-PAR-2-induced inflammation in the sinusoidal endothelial cells.
Achieving margin-negative resection is crucial in treatment of solid pseudopapillary neoplasm (SPN) of the pancreas, while preserving the spleen during distal pancreatectomy is highly desirable in pediatric cases. Laparoscopic Warshaw procedure (Lap-WT) is invaluable when tumor involvement in splenic vessels complicates preservation. However, the feasibility of Lap-WT in pediatric patients remains contentious. This study presents the clinical outcomes of three pediatric SPN cases who underwent Lap-WT. The median age was 10 years, with a median tumor size of 50 mm. Lap-WT demonstrated successful outcomes with a median operation time of 311 min and blood loss of 12 mL. No postoperative complications occurred, with a median length of hospital stay of 8 days. Long-term follow-up showed mild thrombocytopenia and increased spleen volume in two cases, perigastric varices in one, with no bleeding complications. No instances of tumor recurrence were observed. Lap-WT emerges as a feasible approach for pediatric SPN, ensuring spleen preservation without compromising oncological outcomes.
Background: Few studies have investigated the remnant hepatic functional reserve before hepatectomy by calculating the functional remnant liver rate (RLR) using threedimensional computed tomography (3D-CT)/technetium-99m-diethylenetriaminepentaacetic acid galactosyl human serum albumin (99mTc-GSA) single-photon emission CT (SPECT) fusion imaging. We aimed to preoperatively evaluate the predictive value of functional remnant liver rate (RLR) and indocyanine green (ICG) disappearance rate (KICG) in determining the occurrence of posthepatectomy liver failure (PHLF). Summary of Background Data: The conventional method of volumetric rem-KICG calculated from remnant liver volume and the KICG is difficult to accurately reflect heterogenous remnant liver function. Methods: In total, 106 patients who underwent major hepatectomy were retrospectively analysed. Of these, 24 (22.6%) developed clinically relevant PHLF grades B/C. We examined the ICG retention rate at 15 min (ICGR15) and KICG and constructed a 3DCT/ 99mTc-GSA SPECT fusion image to calculate the volumetric RLR, functional RLR, volumetric rem-KICG, and functional rem-KICG. Results: The multivariate analysis showed functional rem-KICG as the strongest independent risk factor for PHLF grade B/C. The functional-to-volumetric RLR ratios in the patients with portal vein obstruction and/or tumor volume of ≥500 mL was significantly higher. The volumetric rem-KICG determined that hepatectomy was unsafe in 7 patients, whereas the functional rem-KICG determined that it was unsafe in 3 patients. Conclusions: Functional rem-KICG was more reliable than volumetric rem-KICG in predicting clinically relevant PHLF grade B/C, as the resected side’s hepatic status highly influenced the function of the remnant liver. This finding could lead to a wider application of this technique.
