PURPOSE:We aimed to evaluate the accessibility and equity of automated external defibrillators (AEDs) in areas with a high concentration of older adults and to propose evidence-based policy directions using geographic information systems (GIS). METHODS:For the secondary analysis, multiple datasets were integrated from the Korea's National Geographic Information Institute, the Ministry of the Interior and Safety, the Suwon Fire Department, and the Suwon Southern Fire Station, including continuous digital topographic maps, demographic statistics, AED locations, road networks, and out-of-hospital cardiac arrest incidence data. The clustering and spatial heterogeneity of AED deployment were examined using kernel density estimation and spatial autocorrelation analysis. Service areas reflecting the walking speeds of older adults were delineated, and equity was assessed using the Gini coefficient. A two-step floating catchment area analysis was used to provide supply-to-demand ratios and accessibility indices. RESULTS:AEDs exhibited clustered patterns centered on public locations, and several areas were identified as potential service blind spots, revealing inequities at the neighborhood level. Several AED sites were insufficient for the local older-adult population, and only a subset of areas demonstrated relatively high accessibility indices. CONCLUSION:As the population age structure shifts toward older age groups, AED policy criteria should be redesigned to reflect the mobility and accessibility of older adults. Implementing mixed relocation strategies that incorporate age-friendly criteria, conducting periodic policy performance evaluations, and enhancing AED visibility and tailored education that accounts for the cognitive characteristics of older adults will contribute to improving the health of older populations.
BackgroundGiven the increasing emphasis on infection control, a comprehensive analysis of nursing research is essential for mapping prevalent scholarly themes, thereby identifying the thematic priorities and evolving knowledge structures that underpin optimized infection control strategies.ObjectiveThis study aimed to identify meaningful concepts and trends in infection control and expand the scope of research with new insights.MethodsThis exploratory study utilized Latent Dirichlet Allocation topic modeling to identify research trends over time. A total of 2,651 papers published between 1974 and 2022 were retrieved from nine databases, including PubMed, CINAHL, and Scopus.ResultsTopic modeling identified four core themes, led by epidemiological investigation at 33.82% and infection control leadership at 24.06%, both of which emerged as hot topics with significantly increasing trends. Hygiene management of vulnerable subjects at 22.06% and transmission route blocking at 20.06% were identified as Cold topics, showing a significant decline in research focus over the study period.ConclusionsThis study elucidates the multidimensional knowledge structure of infection control research in nursing at both micro and macro levels. The findings suggest that enhancing infection control implementation may be supported by addressing organizational factors, including leadership and professional supervision. Furthermore, these results underscore the potential for integrating epidemiological evidence into multifaceted nursing practices to strengthen infection control systems.
Background:Korea has one of the highest suicide rates among OECD countries, ranking first for several consecutive years, with a rate of approximately 24 deaths per 100,000 population. Understanding the association between prescription drugs and suicide attempts is crucial for developing effective suicide prevention and management strategies. This study analyzed Health Insurance Review and Assessment Service data to identify medical events preceding suicide attempts and to construct a classification model of prescribed medications using decision tree analysis. Methods:This secondary data study analyzed 1264 adult suicide attempters aged 20 to 59 years who were recruited from a pilot project for postmanagement in emergency departments conducted from March 2021 to March 2023, which may limit the generalizability of the findings. Claims data from the Health Insurance Review and Assessment Service were extracted using the specific item codes, and the drug details were obtained from the Ministry of Food and Drug Safety's pharmaceutical classification table 100-890. Results:Of the total subjects, 65% were female and 35% male, with the highest proportion aged 20-29 years. Medications prescribed prior to suicide attempts were analyzed according to age-specific prescription patterns, with a focus on the types and classes of drugs prescribed. The classification model revealed age-specific patterns, with psychotropic medications emerging as major predictors across all age groups. These models included psychotropic medications, digestive ulcer drugs, antipyretic and analgesic agents, anticonvulsants, and respiratory system drugs, with respiratory medications emerging as the most crucial variable. In the 50s, psychotropic medications-particularly hypnotics and sedatives-appeared in all four pathways. Among these, zolpidem was most commonly prescribed for insomnia, underscoring the strong link between sleep disturbance and suicide risk. Of particular concern, nearly nine out of 10 suicide attempters were prescribed at least one medication carrying suicide-related warnings or contraindications, as noted by the Ministry of Food and Drug Safety and the U.S. Food and Drug Administration. Conclusion:The results highlight differences in healthcare utilization and prescribed medication patterns by age group among suicide attempters. Implications for nursing management:These results can be used as foundational data for considering suicide risk during nursing assessment and diagnosis of general and psychiatric patients and developing specialized nursing strategies and management.
BACKGROUND:Despite the advancement of the medical environment, health-related infections continue to increase. The study was to explore the knowledge structure of infection control nursing research using keywords and networks. It was also to identify theoretical groundings regarding the current infection control nursing theory and presented the area and level of future theory. METHODS:A quantitative content analysis design was employed. Using text network analysis, keywords were derived from the abstracts of 2,651 studies conducted from 1974 to 2022, and centrality and network connection structure analyses were used to identify the keywords' structure and characteristics. RESULTS:In the frequency of occurrence, vaccine is the most frequent, followed by health personnel, influenza, and glove. The connection structure analyzed using weights that the terms influenza and vaccine appeared simultaneously, followed by injury and needle, hepatitis and vaccine, immunization and vaccine, and injury and needlestick injury in order. The words health personnel, vaccine, and glove, which all showed high centrality, indicate that they are keywords studied in relation to various words in infection control nursing research. CONCLUSION:This study confirmed that keywords revealed through text network analysis were medical workers, vaccines, and gloves. Vaccine was closely related to terms such as influenza, hepatitis, and healthcare provider, while the term gloves was closely related to respiratory protection, injury and needles, and healthcare provider. Infection control for healthcare providers is based on evidence-based knowledge and practices, and quality evaluation of the evidence and verification of effectiveness are required. Background Research for the development of nursing education strategies to enhance the competency of the nursing students on disaster safety are needed. This study aimed to identify the types of perceptions on disaster safety in nursing students, and to analyze and describe the characteristics of each type of disaster safety perception of nursing students in South Korea. Methods An exploratory study design applying Q methodology, a research method designed to study subjectivity. Participants were 30 nursing students in their 20s who are living in C city. This P-set was selected to best reveal the disaster safety awareness of nursing students. Participants provided their subjective viewpoints by sorting 30 statements into a grid. Analyses involved correlation and factor analysis. The study was carried out from June to December, 2020. Results In this study, four types of disaster safety awareness of nursing students were uncovered. The characteristics of each type were confirmed as follows: Type 1 was national responsibility, type 2 was individual responsibility, type 3 was preparedness-oriented, and type 4 was education-oriented. Conclusion This study shows that the types of perceptions on disaster safety in Korean nursing students were national responsibility, individual responsibility, preparedness-oriented, and education-oriented. The findings from this study can be implied as fundamental data in nursing education of disaster safety.
Background:Attention is drawn to the subjective health status and quality of life of older adult single-person households, whose number is gradually increasing as factors including low fertility, increased life expectancy, aging, and household miniaturization interact.Objective:The study was to identify predictors that affect the quality of life of single-person households aged 65 years or older and living in South Korea.Methods:A secondary data analysis design was used. Data included physical, mental, social, and demographic characteristics, subjective health status, and quality of life parameters of 1,029 older adult single-person households surveyed by the Korea Health Panel in 2019. For analysis, the predictive model was evaluated using split-sample validation and the ROC curve. The area under the curve after the decision tree analysis was calculated. Final nodes predicting the quality of life of older adult single-person households were derived.Results:Significant predictors were identified in this order: subjective health status, chronic disease, income, and age. Subjective health status was the most important factor influencing quality of life (△ p < 0.001, x2 = 151.774). The first combination that perceived high quality of life of older adult single-person households was the case of high subjective health status and no chronic disease, followed by the case of high subjective health status, presence of chronic disease, and high income.Conclusion:This study confirmed that subjective health status and chronic disease are essential factors for quality of life among the four related indicators of quality of life presented by the OECD. In nursing practice, nurses need to pay attention the factors influencing quality of life of older adult single-person households. Especially, nursing practice for older adult single-person households needs to be focused on improving subjective health status and on relieving chronic disease.
Purpose: The aim of this study was to identify awareness and competency for Multi-Drug Resistant Organisms (MDRO) infection control in nursing students with experience of clinical practice. Methods: This cross-sectional descriptive study was conducted from March 2019 to May 2019 by including 231 nursing students in four nursing schools located in Seoul, Gyeonggi-do and Chungcheongnam-do. The data were collected using self-report questionnaires. Results: The awareness and the competency for Carbapenem-Resistant Enterobacteriaceae (CRE) infection control were lower than that of Methicillin-Resistant Staphylococcus aureus (MRSA). The agreement between the awareness and the competency of MDRO infection control in participants was low with regard to isolation, contact precautions, and disinfection for MRSA. Also, it was low with respect to disinfection, isolation, contact precautions, and carrier identification for CRE. The awareness and the competency of MDRO infection control exhibited significant positive correlation. Conclusion: The infection control competency is required to prevent MDRO infection. In order to enhance the infection control competency, it is important to raise awareness about MDRO infection control by providing education based on the guidelines and the principles of infection control.
