Szumy uszne u dzieci występują dużo częściej niż są rozpoznawane, ponieważ dzieci nie potrafią w sposób jasny wyrazić swoich dolegliwości. Na podstawie analizy dostępnego piśmiennictwa, zaleceń Brytyjskiego Towarzystwa Audiologicznego (British Society of Audiology) przygotowanego we współpracy z grupą roboczą (Pediatric Tinnitus Working Group) oraz doświadczeń własnych przedstawiono charakterystykę dziecięcych szumów usznych, ich wpływ na różne aspekty życia pacjentów pediatrycznych oraz zalecenia diagnostyczne. Ponadto omówiono sposoby postępowania i terapii szumów usznych u dzieci.
Objective: This cross-sectional study was designed to add to the emerging empirical literature characterizing the psychiatric and audiologic features of misophonia. Because most research to date has not compared misophonia to clinical control groups, the present study used both participants who did not report any sound intolerance problems and a clinical control group of participants with auditory over-responsivity not formally meeting criteria for a diagnosis of misophonia using proposed diagnostic criteria by Schroeder et al. (2013). Severity of misophonia symptoms, frequency of current or lifetime psychiatric disorders, loudness discomfort, and hearing loss were compared across groups. Methods: Structured interviews, questionnaires, and objective measures of audiologic functioning were administered to a sample of adult participants (N = 156). Measures included an interviewer-rated diagnostic assessment of misophonia, the MisoQuest, (Siepsiak et al., 2020), M.I.(NI)-I-. (Sheehan et al., 1998), loudness discomfort level (LDL), and hearing loss (PTA). Results: Group differences in misophonia symptom severity among all three groups were observed: FWelch (2,50.57) = 149.92, p < .001, n2 = 0.64, validating group assignment. Psychiatric disorders were significantly more frequent in the misophonia group (71%) than in the auditory over-responsivity group (40%) and control group (40%): X-2 (2, N - 142) - 14.3; p - .001; V - 0.317. A wide range of psychiatric disorders were observed in the misophonia group, (e.g., major depressive episode, suicidality and panic disorder were the most common). There were no significant differences between groups with regards to audiologic functioning. Conclusion: Misophonia co-occurs with a variety of psychiatric disorders but does not appear to be associated with loudness discomfort or hearing impairments.
Background Tinnitus is a phantom auditory sensation in the absence of an external stimulus. It is accompanied by a broad range of negative emotional symptoms and a significantly lower quality of life. So far, there is no cure for tinnitus, although various treatment options have been tried. One of them is mobile technology employing dedicated apps based on sound therapy. The apps can be managed by the patient and tailored according to their needs. Objective The study aims to assess the effect of a mobile app that generates background sounds on the severity of tinnitus. Methods The study involved 68 adults who had chronic tinnitus. Participants were divided into a study group (44 patients) and a control group (24 patients). For 6 months those in the study group used a free mobile app that enriched the sound environment with a background sound. Participants were instructed to use the app for at least 30 minutes a day using their preferred sound. The participants in the control group did not use the app. Subjective changes in the day-to-day functioning of both groups were evaluated using the Tinnitus Handicap Inventory (THI) questionnaire, a visual analog scale, and a user survey. Results After 3 months of using the app, the THI global score significantly decreased (P<.001) in the study group, decreasing again at 6 months (P<.001). The largest improvements were observed in the emotional and catastrophic reactions subscales. A clinically important change in the THI was reported by 39% of the study group (17/44). Almost 90% of the study participants (39/44) chose environmental sounds to listen to, the most popular being rain and ocean waves. In the control group, tinnitus severity did not change over 3 or 6 months. Conclusions Although the participants still experienced limitations caused by tinnitus, the advantage of the app was that it led to lower negative emotions and thus reduced overall tinnitus severity. It is worth considering whether a mobile app might be incorporated into the management of tinnitus in a professional setting.

W ciągu ostatnich kilkudziesięciu lat wiele badań poświęcono częstości występowania szumów usznych, o czym świadczy liczba publikacji dotyczących tego zagadnienia. W niniejszym opracowaniu przedstawiono wyniki tych badań oraz szacunkowe przewidywania na temat nowych przypadków, które pojawią się w przyszłości. Dokładne poznanie skali występowania szumów usznych w populacji osób dorosłych wydaje się istotne, ponieważ liczba pacjentów zgłaszających się po pomoc z powodu dokuczliwych szumów usznych z roku na rok wzrasta. Może to wskazywać, że szumy uszne stają się istotnym problemem ze względu na potrzebę zapewnienia kompleksowej diagnostyki i leczenia pacjentom cierpiącym z ich powodu.

Narząd słuchu odgrywa szczególną rolę u osób wykorzystujących słuch w pracy zawodowej (muzyków, reżyserów dźwięku). Nagłe pogorszenie słuchu często występujące niezależnie lub równolegle z szumami usznymi może im znacznie utrudniać wykonywanie pracy. Pogorszenie progu słyszenia o co najmniej 30 dB dla trzech sąsiadujących częstotliwości oktawowych tonów słyszalnych stanowi obecnie kryterium rozpoznania nagłej głuchoty. U osób zawodowo posługujących się słuchem już dużo mniejszy ubytek słuchu, często dla częstotliwości spoza zakresu oznaczanego w standardowej audiometrii tonalnej, może powodować znaczny dyskomfort w pracy zawodowej. Dlatego konieczne jest zwrócenie szczególnej uwagi na tę grupę zawodową pod kątem ustalenia rekomendowanych procedur postępowania w sytuacji nagłego pogorszenia słuchu, nawet jeśli niedosłuch nie spełnia kryteriów nagłej głuchoty. Zasadne wydaje się również opracowanie odrębnych standardów dla tej grupy zawodowej.
Zmniejszona tolerancja na dźwięki jest ogólnym określeniem używanym do opisania obniżonej tolerancji codziennych dźwięków, które większość ludzi uważa za znośne. Dźwięki te to często dźwięki wysokie lub dźwięki, które pojawiają się nagle (dźwięki impulsowe). Przykładem takich dźwięków są np. brzęk naczyń i sztućców, szczekanie psów czy hałas drogowy. Obniżona tolerancja na dźwięki to zaburzenie percepcji słuchowej, która może wystąpić jako mizofonia lub nadwrażliwość słuchowa. Wytyczne krajowych i światowych towarzystw otolaryngologicznych dotyczące tego schorzenia odnoszą się głównie do osób dorosłych, pomijając istnienie tego problemu wśród dzieci. Celem pracy była analiza dostępnego piśmiennictwa na temat obniżonej tolerancji na dźwięki, która przez autorów jest różnie definiowana. Praca ma charakter poglądowy, a jej wyniki są oparte na przeglądzie piśmiennictwa polsko- i anglojęzycznego, zgromadzonego w bazach: PubMed, Web of Science, Cochrane Library, MEDLINE oraz EMBASE. Wyszukiwanie przeprowadzono za pomocą słów kluczowych: decreased sound tolerance, misophonia, hyperacusis. Ramy czasowe ograniczono do lat 2000–2018. Przedmiotem zainteresowania autorów były prace na temat rozpowszechnienia, definicji oraz potencjalnych mechanizmów powstawania obniżonej tolerancji dźwiękowej, które przedstawiono zwłaszcza pod kątem istniejących związków z procesami fizjologicznymi i neurofizjologicznymi. Porozumienie w sprawie stosowania standardowych kryteriów diagnostycznych stanowiłoby istotny krok naprzód zarówno w przypadku praktyki klinicznej, jak i badań naukowych. Przyszłe badania powinny obejmować epidemiologię oraz utworzenie narzędzi diagnostycznych. Ponadto, badania nad współistniejącymi schorzeniami i strategiami terapii psychologicznej mogłyby pomóc w zidentyfikowaniu mechanizmów leżących u podstaw leczenia i potencjalnie prowadzić do opracowania skutecznej metody leczenia.
Szumy uszne u dzieci wykazują pewne podobieństwo do szumów usznych występujących u osób dorosłych. Różnice w odczuwaniu szumów usznych polegają na tym, że dzieci rzadko samoistnie zgłaszają szumy uszne, natomiast zapytane potrafią opowiedzieć o swojej dolegliwości. Dziecięce szumy uszne zawsze wymagają diagnostyki w celu wykluczenia zmian organicznych oraz również dlatego, że mogą wpływać negatywnie na jakość życia młodego pacjenta, powodując problemy ze snem, koncentracją i być przyczyną gorszych wyników w nauce. W pracy przedstawiono przyjazny dziecku i jego rodzicom schemat postępowania diagnostycznego i terapeutycznego w przypadku dziecięcych szumów usznych.
The 12th International Conference organized by the Tinnitus Research Initiative took place on 17–19.05.2019 in Taipei. The conference gathered about 200 participants from around the world and was devoted entirely to the subject of tinnitus. The conference was interdisciplinary, with otolaryngologists, audiologists, scientists, psychologists, and physiotherapists. The program was divided into thematic blocks including epidemiology, animal tinnitus models, diagnostics, electrophysiology, clinical trial methodology, the use of hearing aids and cochlear implants in tinnitus therapy, auditory hypersensitivity, and genetics.
The study addressed the efficacy of 12 weeks of yoga training in treating 25 patients with chronic tinnitus. Ten of the patients underwent magnetic resonance imaging (MRI) before and after training. A control group comprised 13 people reporting chronic tinnitus. All participants were assessed with the Tinnitus Functional Index. After the 12-week yoga course, the areas considered to have benefited most were the sense of control of tinnitus, sleep, quality of life, and intrusiveness. The MRI studies indicated that connections in the white matter of the motor cortex appeared to be stronger as a result of training. Yoga training has good potential to improve the daily functioning of patients with chronic tinnitus and can be considered a promising supporting method for tinnitus treatment.
BackgroundWhen measuring the treatment effect in tinnitus with multi-item outcome instruments, it is crucial for both clinical and research purposes to take into consideration clinical importance of the outcome scores. The aim of the present study is to determine minimal important change (MIC) in tinnitus which is clinically meaningful to patients with otosclerosis.MethodsThe study population was 95 patients with otosclerosis, suffering from tinnitus. They completed the Tinnitus Functional Index before stapedotomy and 3months after the surgery. The minimal important change was estimated with the Clinical Global Impression Scale as the external criterion (anchor). The mean change method and the receiver operating characteristic (ROC) method were used to determine minimal important change in tinnitus sensation.ResultsThe improvement in tinnitus after stapedotomy was reported by 69.4% of the patients with otosclerosis. Minimal important change in tinnitus was estimated as reduction of 8.8 points in the Tinnitus Functional Index.ConclusionsThe anchor-based approach using an external criterion (anchor) allows to determine change in tinnitus sensation which is meaningful to patients after stapedotomy. The value of 8.8 points in Tinnitus Functional Index could be used as benchmark of stapedotomy effectiveness in otosclerosis patients suffering from tinnitus. Hearing difficulties comorbid with tinnitus could affect the perception of tinnitus change.
Cochlear implantation is becoming the standard treatment in patients with severe to profound sensorineural hearing loss. Moreover, tinnitus is a symptom that is highly connected with hearing impairment. The purpose of the study was to assess the influence of CI on tinnitus distress using the Tinnitus and Hearing Survey (THS), Tinnitus Handicap Inventory (THI) and Tinnitus Functional Index (TFI). Twenty adult patients were included in this study with unilateral cochlear implantation. Preand postoperative tinnitus perception was evaluated, before, during implant activation visit and 1 month before activation. Of the approached patients, 15 completed the full sets of questionnaire and 5 only Tinnitus and Hearing Survey (THS). Before implantation, the tinnitus prevalence was 75% (15 of 20) in the whole study group. Prior to implantation, the total result THI was 51.6 (SD=22.5) and TFI score was 41.6 (SD=15.3). Postoperatively, the THI scores decreased to 39.3 (SD=27.1) and TFI score reduced to 34.6 (SD=23.5). Moreover, results were analyzed using the Paired Sample t-test. The level of significance was set at p<0.05. In conclusion, in present study, cochlear implant improves hearing threshold and can significant reduced tinnitus distress. INTRODUCTION:Tinnitus in children is still rarely mentioned in medical literature. The prevalence of pediatric tinnitus has been reported to range between 3-37% and higher in children with hearing loss.OBJECTIVES:The objective of this study was to estimate the prevalence of the tinnitus in 7 years old children and to conduct statistical analysis of the relation between tinnitus and hearing loss.MATERIAL AND METHODS:In this study 60 212 children and their parents were asked about the presence of tinnitus by answering a questionnaire at home. Additionally, children were asked if they can hear noise in their ears or head, before hand hearing screening test at school was conducted. The hearing screening test was performed using screening audiometer with air conduction.RESULTS:The results showed that tinnitus was present in group of 8164 children (13.6%). Between children directly asked for tinnitus, 33% mentioned of having it. Over 75% of children did not complained of tinnitus spontaneously to parents. Children with hearing loss reported tinnitus twice more comparing to children with normal hearing.CONCLUSIONS:Tinnitus is frequent complain among 7 years old children. Children with hearing loss reported tinnitus twice more comparing to children with normal hearing. It is recommended to include to a questionnaire an inquiry about the presence of tinnitus during hearing screening tests performed recently more frequently at primary schools for early detection and diagnosis of tinnitus.
