Introducción. Trypanosoma cruzi, agente causal de la enfermedad de Chagas, exhibe una sustancial heterogeneidad fenotípica y genotípica que puede influir en las variaciones epidemiológicas y clínicas de la enfermedad, así como en la sensibilidad a los fármacos utilizados en el tratamiento.Objetivo. Evaluar la sensibilidad in vitro al benznidazol, el nifurtimox y el posaconazol de 40 cepas clonadas de T. cruzi de Paraguay, con distintos genotipos, huéspedes y localidades de origen.Materiales y métodos. En su estado epimastigote, los parásitos se incubaron en medio de cultivo LIT (Liver Infusion Tryptose) con diferentes concentraciones de cada fármaco en ensayos por triplicado. El grado de sensibilidad se estimó a partir de las concentraciones inhibitorias del 50 y el 90% (IC50 e IC90) y se obtuvieron los valores promedio y la desviación estándar de cada cepa y fármaco. La significación estadística entre grupos se determinó mediante análisis de varianzas con el test no paramétrico de Wilcoxon/Kruskal-Wallis y valores de p<0,05.Resultados. Se observó un amplio rango de respuesta a los fármacos. Se identificaron dos grupos de parásitos (A y B) con diferencias significativas en la sensibilidad al benznidazol (p<0,0001), y tres grupos (A, B, C) en cuanto a la sensibilidad al nifurtimox y el posaconazol (p<0,0001).Conclusiones. En general, las cepas fueron más sensibles al nifurtimox que al benznidazol y el posaconazol. Estas diferencias evidencian la heterogeneidad de las poblaciones de T. cruzi que circulan en Paraguay, lo que debe considerarse en el tratamiento y el seguimiento de las personas afectadas.
La enfermedad de Chagas es hasta hoy día endémica en el Chaco paraguayo, perjudicando principalmente a las poblaciones rurales o de menores recursos económicos, representando así las comunidades indígenas algunas de las poblaciones más vulnerables. El objetivo de este trabajo fue determinar la presencia de triatominos vectores de Trypanosoma cruzi en las comunidades indígenas Ayoreo Totobiegosode Chaidi y Aocojadi del Alto Paraguay. Se evaluaron características de las viviendas y su relación con la permanencia de vectores. La búsqueda de vectores se realizó en el intradomicilio y peridomicilio de las viviendas, y mediante la instalación de una trampa de luz en un ambiente silvestre adyacente a una de las comunidades. Para la búsqueda de flagelados fueron examinadas al microscopio óptico las heces de los triatominos colectados. Se realizó la técnica de xenocultivo para los insectos positivos. Fueron encontrados e identificados triatominos tanto en los ambientes domésticos como en el ambiente silvestre en la zona de las comunidades del Pueblo Ayoreo Totobiegosode del Alto Paraguay: Chaidi y Aocojadi, durante el periodo de colecta del 2016. Los triatominos identificados en este estudio, Triatoma sordida y Triatoma guasayana, son propios de ambientes silvestres, aunque durante ciertas épocas del año actúan como especies intrusivas invadiendo las áreas domésticas. Se detectó la infección natural con T. cruzi-like en un ejemplar proveniente de la comunidad Chaidi, sin descartar la posibilidad de que el parásito se encuentre circulando en Aocojadi. La investigación contribuye a considerar el riesgo de exposición a la enfermedad que tiene la población local, debido a que hasta el momento de este estudio no se cuenta con información sobre la presencia o ausencia de circulación de T. cruzi en estas comunidades.
SUMMARY Active Trypanosoma cruzi transmission persists in the Gran Chaco region, which is considered hyperendemic for Chagas disease. Understanding domestic and sylvatic transmission cycles and therefore the relationship between vectors and mammalian hosts is crucial to designing and implementing improved effective control strategies. Here we describe the species of triatomine vectors and the sylvatic mammal reservoirs of T. cruzi , in different localities of the Paraguayan and Bolivian Chaco. We identify the T. cruzi genotypes discrete typing units (DTUs) and provide a map of their geographical distribution. A total of 1044 triatomines and 138 sylvatic mammals were captured. Five per cent of the triatomines were microscopically positive for T. cruzi (55 Triatoma infestans from Paraguay and one sylvatic Triatoma guasayana from Bolivia) and 17 animals (12·3%) comprising eight of 28 (28·5%) Dasypus novemcinctus , four of 27 (14·8%) Euphractus sexcinctus , three of 64 (4·7%) Chaetophractus spp. and two of 14 (14·3%) Didelphis albiventris . The most common DTU infecting domestic triatomine bugs was TcV (64%), followed by TcVI (28%), TcII (6·5%) and TcIII (1·5%). TcIII was overwhelmingly associated with armadillo species. We confirm the primary role of T. infestans in domestic transmission, armadillo species as the principal sylvatic hosts of TcIII, and consider the potential risk of TcIII as an agent of Chagas disease in the Chaco.
To verify the occurrence of natural Trypanosoma cruzi infection in non-human primates from a rural endemic area of the east region of Paraguay, xenodiagnosis was performed in 35 animals belonging to two species. For genotyping and T. cruzi discrete typing unit (DTU) assignment, a combination of four markers was used, including amplification products of the small (18S) and large (24Sα) subunits of ribosomal ribonucleic acid gene, the intergenic region of mini-exon gene and the heat shock protein 60 Eco-RV polymerase chain reaction-restriction fragment length polymorphism (HSP60/EcoRV-PCR-RFLP). One specimen of Sapajus cay was found positive and infected by the DTU TcII. This result constitutes the first record of natural T. cruzi infection in a sylvatic monkey in Paraguay, harbouring a DTU associated with severe Chagas disease in humans.
El articulo trata de reflejar las vivencias que me embarcaron en la aventura de ensenar y como las mujeres que me rodearon desde la infancia hasta la edad adulta me senalaron con su ejemplo el camino a seguir. Ellas fueron claves en la formacion de muchas mujeres que quedaron marcadas por esas grandes de la pedagogia y que deben su vocacion a lo que ellas les inculcaron. Maria Goyri, Jimena Menendez Pidal, Maria Elena Gomez-Moreno, Carmen Garcia del Diestro, Josefina Aldecoa... y otros muchos nombres que pertenecen a la historia pedagogica de este pais y no pueden ser olvidados. En este articulo he tratado de reflejar la huella que esas mujeres y otras muchas del entorno familiar o social dejaron en una generacion como la mia. Ese testimonio es el que quiero que permanezca y por ello he escrito estas paginas que tienen mas de memoria biografica que de trabajo de investigacion o analisis.
Triatoma sordida is widely distributed throughout the Chaco and the Eastern Region of Paraguay. It is associated to palm trees and artificial ecotopes located in peridomestic environments. The aim of this work was to determine genetic and morphometric variability and feeding behavior among population of T. sordida captured in domicile and peridomicile areas of Paraguay. Feeding contents and levels of genetic and morphometric variation were determined in 124 T. sordida from domicile and peridomicile populations of San Pedro and Paraguarí departments of the Eastern Region and Boquerón and Presidente Hayes departments of the Western region using Double Diffusion Gel, random amplified polymorphic DNA (RAPD), and head and wings morphometry. Morphometric analysis revealed isolation of populations by geographic region and larger size in triatomine populations from the Western Region. RAPD showed no specific patterns for domicile and peridomicile populations. The estimator of diversity (F ST; 0.08) and high gene flow obtained (N m; 5.7) did not allow the establishment of genetic differentiation within the same region. The blood meal source showed that poultry feeding was 38% of host preferences, and human blood was the second feeding preference (24%) in the insects from the Eastern Region while poultry feeding was predominant in those from the Western Region (30%). This work showed homogeneity between T. sordida populations of the same region and between domicile and peridomicile. The genetic diversity was determined among T. sordida populations of both geographical regions suggesting differentiation associated to eco-geographical isolation by distance. It is important to notice that pattern feedings were different between the two regions. Further studies should be focused on how phenetic and genetic variations could be related to the adaptation capacity of these triatomine populations to domicile, increasing their vector potentiality in the transmission of Chagas disease.
In Paraguay, the enzootic transmission of Trypanosoma cruzi has not been well determined. Therefore, several aspects of the transmission dynamics of this parasite in the sylvatic environment are still unknown, such as naturally infected reservoir species, circulating strains and degree of interaction with the domestic cycle. The studies performed until now in the wild environment of endemic areas on Chagas disease (Chaco and San Pedro) have identified five species as natural reservoirs of T. cruzi: Didelphis azarae, Monodelphis domestica, Dasypus novemcinctus, Euphractus sexcinctus y Chaetophractus spp. Among them, Dasypus novemcinctus stands out for its high infection prevalence of around 50%. In the domestic environment, dogs infected by this parasite have been reported in higher proportion than cats, cattle and pigs.
Se evaluó la sensibilidad in vitro al benznidazol y al extracto de hojas de la planta Zanthoxylum chiloperone de diez cepas del Trypanosoma cruzi. Se utilizaron formas epimastigotas de las cepas, pertenecientes a diferentes linajes, diferente procedencia geográfica, incluidas tres de Paraguay, y aisladas de hospederos distintos, abarcando humanos, triatominos y animales silvestres. El grado de sensibilidad a la droga y al extracto vegetal fue estimado por el porcentaje de lisis de los parásitos a las 24 y 48 horas de exposición. Se observó un amplio rango de variación en el porcentaje de lisis entre las cepas, de 22,2% a 90,8% a la concentración del benznidazol de 200 μg/mL, con el hallazgo de una cepa con sensibilidad nula al mismo. Con respecto al extracto vegetal, tres concentraciones fueron testadas, de 500, 700 y 900 μg/mL, observándose variaciones en los porcentajes de lisis entre las cepas con las dos primeras concentraciones, entre un 2,1% y 100%, con lisis total en todas las cepas a la concentración de 900 μg/mL. No se observó asociación entre la diversidad de respuesta a los compuestos y la clasificación de las cepas ni con su origen geográfico y tipo de hospedero. Estas diferencias observadas resaltan la heterogeneidad de las poblaciones naturales del T. cruzi, aspecto importante a tener en cuenta en los estudios de sensibilidad a quimioterapéuticos y en los tamizajes primarios de nuevos antichagásicos. Así mismo, se destaca la eficacia del extracto vegetal sobre diferentes cepas de este parásito.
Trypanosoma cruzi populations circulating in Paraguay have shown a great genetic diversity, which has been evaluated by different methodologies and techniques. Several genotypes of this parasite have been identified infecting a diversity of hosts, including patients, triatomines and wild and domestic animals from different regions. The genetic diversity observed in the isolates of our country could imply differences in the clinical manifestations of Chagas disease, as well as differences in their biological characteristics such as grade of infectivity, virulence, pathogenicity, antigenic capacity and susceptibility or resistance to trypanocidal drugs. Up to now, these aspects have been little explored and evaluated in Paraguayan strains. The isolation and identification of T. cruzi groups and subgroups that circulate in the different transmission cycles in a determined geographic region are still relevant because they allow knowing the dynamics of parasite transmission and evaluating the introduction risk from wild foci.
Background: Chagas disease prevention critically depends on keeping houses free of triatomine vectors.Insecticide spraying is very effective, but re-infestation of treated dwellings is commonplace.Early detection-elimination of reinfestation foci is key to long-term control; however, all available vector-detection methods have low sensitivity.Chemicallybaited traps are widely used in vector and pest control-surveillance systems; here, we test this approach for Triatoma spp.detection under field conditions in the Gran Chaco.Methodology/Principal Findings: Using a repeated-sampling approach and logistic models that explicitly take detection failures into account, we simultaneously estimate vector occurrence and detection probabilities.We then model detection probabilities (conditioned on vector occurrence) as a function of trapping system to measure the effect of chemical baits.We find a positive effect of baits after three (odds ratio [OR] 5.10; 95% confidence interval [CI 95 ] 2.59-10.04)and six months (OR 2.20, CI 95 1.04-4.65).Detection probabilities are estimated at p<0.40-0.50 for baited and at just p<0.15 for control traps.Bait effect is very strong on T. infestans (three-month assessment: OR 12.30, CI 95 4.44-34.10;p<0.64), whereas T. sordida is captured with similar frequency in baited and unbaited traps.Conclusions/Significance: Chemically-baited traps hold promise for T. infestans surveillance; the sensitivity of the system at detecting small re-infestation foci rises from 12.5% to 63.6% when traps are baited with semiochemicals.Accounting for imperfect detection, infestation is estimated at 26% (CI 95 16-40) after three and 20% (CI 95 11-34) after six months.In the same assessments, traps detected infestation in 14% and 8.5% of dwellings, whereas timed manual searches (the standard approach) did so in just 1.4% of dwellings only in the first survey.Since infestation rates are the main indicator used for decision-making in control programs, the approach we present may help improve T. infestans surveillance and control program management.
A finde evaluar la acción terapéutica dei benznidazol en nifios en edad escolar (de 6 a 12 afios)con Ttypanosoma cruzi, se estudiaron 20 nií’ios con serologia positiva para la enfermedad de Chagas. Los nifios eran procedentes de dos compaflias dei Departamento de Paraguarí, situadas en un área endémica dei Paraguay. Antes de iniciar ei tratarniento ias viviendas de estas zonas fueron evaiuadas entomológicarnente y posteriormente rodadas con insecticida. Antes dei tratamiento estos 20 niflos fueron sometidos a un examen clínico y laboratonai de control que incluyó paiasiternia directa, xenodiagnástico y confirmacián setológíca por ELISA e lnmunoflorescencia Indirecta (lFl). Se realizaron determinaciones hematolágicas y bioquímicas antes y a los 20 dias de iniciado ei tratarniento para permitir detectar ia aparición de efectos colaterales durante ei rnismo. En ei estudio basai, ei 1 5% (3/20) de ias muestras tuvo parasitemia directa positiva, y eu 55% (11/20) de los nifios se obtuvo xenodiagnástico positivo. Todos fueron tratados con benznid.azoi: 5-7 mg/kg de peso por dia durante 60 dias. Las evaiuaciones seroiógicas y parasitoiógicas se uealizumn a los 6, 12, 18 y 24 meses después de haber finalizado ei tratarniento. No se observaron variaciones entre ias determinaciones hematológicas y bioquímicas basales y ias realizadas a los 20 dias. Los resultados parasitológicos ai afio y a los 2 afios después deI tratamiento resuitaron negativos en ei 100% de las muestras. Con respecto a Ia serologia, ai comparar la densidad óptica dei test de ELISA, pareando ias muestras dei estudio basai, ai afio y a los 2 afios de finalizado ei tratamiento, en los 12 nifios que no migraron, se observó una diferencia estadísticamente significativa entre los valores dei basal y ei primer afio (p <0.0047), y entre ei basal y ei segundo afio (p <0.001), No hubo diferencia significativa (p <0.1659) ai comparar los títulos entre ei primero y segundo afio. Se concluye que hubo disminución significativa de la concentración de anticuerpos dos afios después dei tratamiento en los 12 niflos que no migraron.
The recent invasion of water hyacinth Eichhornia crassipes (C. Mart.) Solms (1883) in the Guadiana River Basin (Spain) is described and the distribution of this Amazonian floating plant is analyzed from a geobotanical and chorological perspective. Georeferenced locations of invasion in Spain and Portugal are presented and the relative growth rate (RGR) and doubling time (DT) indexes defined by Gopal (1987) were calculated. The sexual reproductive cycles were determined in order to evaluate the invasive capacity at these latitudes. Predictive models of the plant's potential distribution in the Guadiana River were constructed based on expert knowledge and using a Geographic Information System, on the basis of the water's physico-chemical parameters. Given the size of the potential area of distribution, it seems necessary to make provision for an important increase and subsequent maintenance in the level of the means and logistics targeted at the prevention and control of this weed.
Trypanosoma cruzi, the causative agent of Chagas disease, has at least two principal intraspecific subdivisions, T. cruzi I (TCI) and T. cruzi II (TCII), the latter containing up to five subgroups (a-e). Whilst it is known that TCI predominates from the Amazon basin northwards and TCII to the South, where the disease is considered to be clinically more severe, the precise clinical and evolutionary significance of these divisions remains enigmatic. Here, we present compelling evidence of an association between TCI and opossums (Didelphis), and TCII and armadillos, on the basis of key new findings from the Paraguayan Chaco region, together with a comprehensive analysis of historical data. We suggest that the distinct arboreal and terrestrial ecologies, respectively, of these mammal hosts provide a persuasive explanation for the extant T. cruzi intraspecific diversity in South America, and for separate origins of Chagas disease in northern South America and in the southern cone countries.
