Intrauterine devices (IUDs) are highly effective, long-term methods of contraception. Although evidence of a direct association between IUD use and pelvic inflammatory disease (PID) is scarce, concerns about PID related to IUDs use has limited their use throughout the world. We designed this study to examine the effect of IUDs on PID. For the study, we recruited 200 participants from among women who requested an IUD as a means of contraception. The IUDs were removed 36 months later or in case of PID. No PID cases were recorded during the follow-up period. Prior to IUD insertion, 121 women (60.5%) had symptoms and/or signs of lower genital tract infection, whereas during the follow-up period 179 women (89.5%) had symptoms and/or signs of lower genital tract infection. The Papanicolaou smears were negative for Actinomyces throughout the study period. Also, cultures for sexualy transmitted disease microorganisms were negative throughout the study period. Following IUD removal, 189 IUD cultures (94.5%) were positive. The bacterial flora of the removed IUDs consisted of common aerobic and anaerobic microorganisms that do not account for PID. The most common microorganisms identified were Staphylococcus coagulase negative, Eschericia coli, and Enterococcus faecalis. IUDs are a very effective and safe method of contraception if potential recipients are selected carefully. Culture of the removed IUDs and therapeutic management of women with positive cultures are not recommended when women are asymptomatic for PID.
The effectiveness of intracervical prostaglandin E2 (PGE2) was studied in 96 primigravidas with unfavorable cervix and need for induction of labor. Group A consisted of 49 oligohydramnios and group B of 47 cases with normal amniotic fluid volume (controls). A single dose of 0.5 mg PGE2 gel was applied intracervically and several parameters were recorded during the next hours. The mean number of uterine contractions increased during the first 2 h in both groups, decreased during the next one and did not change significantly afterwards. The mean fetal heart rate (FHR) in group A decreased during the first 2 h and then increased, staying always within normal limits. The mean FHR in group B increased for 2 h, decreased during the next hour and did not change significantly afterwards. No significant differences were found between the two groups regarding mode of delivery, Apgar score and neonatal acidosis. Intracervical PGE2 appeared to effectively stimulate cervical ripening and labor induction in oligohydramnios, without causing side effects to the uterus and fetus.
Tamoxifen is a non-steroid antiestrogen widely used as adjuvant therapy in breast cancer. It has been associated with a variety of endometrial and/or myometrial changes. We propose a method for the diagnostic approach of these changes using vaginoscopic hysteroscopy.
Three cases of breast cancer during pregnancy and lactation which were referred to the Obstetrics and Gynecology Department of loannina University Hospital during the period 1990-1997 are presented. Diagnosis and management of these cases are discussed and a strength protocol is suggested to identify new cases of breast cancer during pregnancy and lactation.
The purpose of this study was to evaluate the effect of tamoxifen therapy on the endometrium by transvaginal color Doppler sonography and on lipid profile focusing on lipoprotein (a) [Lp(a)] levels. Seventy-five postmenopausal breast cancer patients were examined by transvaginal color Doppler sonography and serum Lp(a) levels. Lipid parameters were measured after overnight fasting. Forty of the patients were treated with tamoxifen (20-30 mg/day) for at least 1 year. The remaining 35 patients did not receive tamoxifen and were used as controls. Statistical analysis was performed using t-test and Mann-Whitney U-test (Systat version 5.0). The patients receiving tamoxifen had significantly thicker endometrium (7.9 +/- 3.6 mm) compared to the control group (4.5 +/- 1.8 mm) (p < or = 0.001). The mean pulsatility index and resistance index of the uterine arteries in the tamoxifen group were 2.063 +/- 0.49 and 0.83 +/- 0.07, respectively, and were significantly lower than those of the control group (2.69 +/- 0.16 and 0.88 +/- 0.02) (p < 0.001). In addition, tamoxifen decreased total cholesterol (p < 0.001) and low-density lipoprotein cholesterol (p <0.001) and apolipoprotein B (p < 0.05) significantly. Tamoxifen also increased high density lipoprotein cholesterol (p < 0.05) and apolipoprotein A-I (p < 0.05). These results indicate that tamoxifen stimulates the endometrium and acts as an anti-atherogenic agent in postmenopausal women.
Thirty-two pregnant women with small-for-gestational-age (SGA) fetuses and 45 pregnant women with appropriate-for-gestional-age (AGA) fetuses (controls) were recruited after the 32nd week of gestation. Blood samples were collected for estimation of somatomedin-C (SMC/IGF-I) and hPL in the maternal serum and in the umbilical cord serum. The systolic/diastolic (S/D) ratio of the umbilical artery was also recorded. The results showed somatomedin-C and hPL levels in the maternal serum and in the umbilical cord to be significantly decreased and the Doppler S/D ratio to be significantly increased in the SGA group. In this group, using the multivariable regression analysis, we found significant correlations between maternal hPL, somatomedin-C, Doppler S/D ratio and birth weight.
OBJECTIVE:To evaluate a laparoscopic technique of cornual resection using the harmonic scalpel.METHODS:Four patients with unruptured interstitial pregnancies were treated laparoscopically using the harmonic scalpel at Ioannina University Hospital.RESULTS:There were no failures in the technique in any of our patients.CONCLUSION:Our study suggests that interstitial pregnancies of a maximum gestational age of 7-8 weeks and sac diameter less than 4-5 cm may be treated laparoscopically.
Hysteroscopy is an effective technique for the identification of intrauterine: abnormalities in women with abnormal uterine bleeding. We evaluated a new method - vaginoscopic hysteroscopy -on an outpatient basis for its diagnostic accuracy and its acceptance by patients.
