To make safer the triple test, based on maternal serum levels of AFP, uE3 and hCG: it allows the computing of a likelihood ratio which modifies age-related Down syndrome (DS) risk in the early second trimester. It is the most common screening method, but it is not very safe because of 10% false positive results to reach a sensitivity 85%. The integrated test is a two-stage method: it uses measurements obtained during the first trimester (nuchal translucency by ultrasound and pregnancy-associated plasma protein A (PAPP-A), in maternal serum) and in early mid-trimester (triple test) together with maternal age to provide a single estimate of DS risk. In our hospital since January 2000 the triple test has been complemented by the integrated test. It became a successful reality in 2003, when a larger number of obstetric sonographers accepted any kind of external quality assessment. From the start of the program to December 2006 we evaluated women who attended 18 maternal care units where information was given and written consent signed. Ultrasound examination was performed for dating by CRL and NT was measured, blood was sampled for PAPP-A, a second blood sample for the triple test at 15 weeks was booked and then collected. Our laboratory provided risk estimate using the Wallac Auto Delfia platform for maternal serum determinations and LMS Alpha software for computing. The integrated test population consisted of 37 250 singleton pregnancies, all delivered; median maternal age was 32 years; cut off was 1/350 at term, under the national level, so there were 1222 positive results; ascertainment of DS cases was complete and included 55 cases. False positive rate 3.1%; detection rate 85.4% at mid-trimester; OAPR 1/25; OANR 1/4502. Also 19 trisomy-18 cases and 13 open spina bifida cases were detected after a positive result. The integrated test was a safer option in our prospective experience, which is one of the largest in the world.
