A 34-year-old man with hypercalcemia had a sestamibi scan that showed increased uptake that appeared to correspond with a 7 mm density in the mediastinum, adjacent to the aortic arch, on chest CT. This lesion was thought to be consistent with an ectopic parathyroid adenoma. Prior to surgical exploration, the patient was injected intravenously with 12.3 millicuries of Tc99m-sestamibi. The lesion identified preoperatively did not show abnormal radionuclide uptake using the intraoperative gamma probe, and was subsequently determined to be a hyperplastic lymph node. However, abnormal activity was localized to the thymus gland, from which an ectopic parathyroid adenoma was successfully excised. The patient recovered without incident. The use of the intraoperative gamma probe was critical in identifying and resecting the ectopic parathyroid adenoma in this patient, and in general, may reduce surgical time and reduce the morbidity and/or complications associated with surgical exploration.
Intussusception of the large bowel is a rare clinical entity. In adults, this pathology is usually associated with a malignant lead point and often requires operative management. Reported is the case of an 83-year-old female who was recently diagnosed with superficial bladder cancer (T1) treated by partial cystectomy. She presented 3 months post-operatively with an isolated mucosal metastasis of the transverse colon causing intussusception and large bowel obstruction. The patient was successfully treated by colonic resection with primary anastomosis. Histology was significant for a pedunculated sarcomatoid bladder carcinoma originating from the colonic mucosa with incomplete invasion of the bowel wall. An isolated mucosal metastasis of this variety has not been reported in the literature to date.
In this paper we illustrate the model-driven development approach applied to the user interface of an audiovisual search application, within the European project PHAROS. We show how conceptual modelling can capture the most complex features of an audio-visual Web search portal, which allows users to pose advanced queries over multimedia materials, access results of queries using multimodal and multi-channel interfaces, and customize the search experience by saving queries of interest in a personal profile, so that they can be exploited for asynchronous notification of new relevant audiovisual information. We show how model-driven development can help the generation of the code for sophisticated Rich Internet Application front-ends, typical of the multimedia portals of the future.
1. THE PHAROS PLATFORM AND DEMO PHAROS [1] is an Integrated Project aimed at building a platform for advanced audiovisual search applications. The Consortium comprises 12 partners from 9 European countries. PHAROS unbundles the functionalities of an audiovisual search engine into an open service-based ecosystem, where content can be submitted to customized analysis pipelines, third-party annotation components can be plugged-in, and content based search engines can be registered. PHAROS enables a variety of application scenarios, from content acquisition and enrichment, to annotation fusion, to multi-modal queries. Figure 1 shows the architecture of PHAROS, which supports two main process: Content Caption and Refinement (CCR) executes flow of operators on the captured content and produces XML metadata (subsequently indexed by a core XML search engine) and derived artifacts(used for similarity querying and result presentation); Query Execution and Result Presentation (QUIRP) accepts a user’s query (by keyword, by image similarity, by audio similarity, by video similarity), expands it with user’s profile and social information, brokers its execution on the registered search engines, and presents results in a Rich Internet Interface. Elastomeric nanofibers composed of cross-linked poly(2-hydroxyethyl methacrylate) are synthesized via a reactive electrospinning process. In this process, the photoinduced polymerization and cross-linking of polymers take place simultaneously during the electrospinning process. The produced nanofibers are mostly within the diameter range of 100-500 nm in the dry state. When they are immersed in aqueous solution, the nanofibers exhibit remarkably large elastic properties characteristic of bulk hydrogel materials.

This article analyzes recent state and federal court decisions on the effect of police department employee handbooks. Specifically, the questions of whether and when such manuals create an employment contract enforceable by the employee are discussed. The legal status of at-will employment doctrines in states and their interrelation with certain federal civil rights actions are examined. The situations in which police employee manuals have been used as bases for such actions are explored, along with factors considered by courts when deciding such suits. Trends in this area of law are identified, and steps the police executive can take to avoid adverse outcomes when drafting such manuals are suggested.
Economic impacts associated with whitewater recreation on the Klamath River in Oregon (USA) are estimated using a combination of primary expenditure data and the IMPLAN input-output system. Impact estimates are generated for a range of use levels. A ''naive'' model, which assumes that all expenditures in the destination area are related to the river, is distinguished from an ''adjusted'' model, which includes only expenditures that would be lost without the river. Primary data regarding substitution behavior indicate that estimates of total impact may be overstated if an adjusted model is not employed. Overall results show that whitewater recreation, even on a small scale, can help to diversify a region's economy.
Ces dernières années l'intéret s'est accru pour les expériences des immigrants après leur arrivée dans la societé nord‐américaine. Dans cet article, nous argumentons le fait que le logement, et plus spécifiquement le bail, est un sujet important bien que largement négligé en ce qui concerne la vie de l'immigrant dans la société canadienne. En utilisant des chiffres du recensement de 1986 et la structure des modèles log‐linéaires, nous examinons les baux des logements des immigrants dans les différentes régions du Canada. On argumente que les immigrants sont différenciés par la culture, le temps et l'espace en ce qui concerne le bail, avec des immigrants plus récents, particuliérement ceux qui arrivent des pays en voie de développement, qui ont des taux de propriété particulierement plus bas.In recent years interest has grown in the post‐arrival experiences of immigrants in North American society. In this paper we argue that housing, and more specifically tenure, is an important, though largely neglected, issue with respect to immigrant life in Canadian society. Using data from the 1986 census and a log‐linear modelling framework, we examine housing tenure among immigrants in the regions of Canada. It is argued that immigrants are a culturally, temporally and spatially differentiated sub‐population in regard to tenure, with more recent immigrants, especially those from the Developing World, having substantially lower rates of homeownership.
