Although the frequency and magnitude of disasters vary between countries, it is evident that disasters will continue to affect us, making preparedness crucial for effective response. Disasters exert immense pressure on health care systems, highlighting the need for strengthened institutional readiness. The knowledge and skills acquired by anaesthesiologists during training equip them to assume leadership roles in disaster management. In this review, we aim at emphasizing the vital role of anaesthesiologists in disaster preparedness and response. Being prepared for disasters reflects a community’s resilience. Drawing from recent global experiences, in this paper, we highlight the role of anaesthesiologists in disaster preparedness and response and the importance of integrating anaesthesiologists into institutional disaster response plans. Their active involvement in disaster preparedness and management enhances patient safety, optimizes resource use, and improves outcomes during crises. Anaesthesiologists must advocate for involvement in disaster planning and take a leadership role in disaster management to strengthen health care systems and promote community resilience in the face of future disasters.
Assessing soil quality in industrial coal-mining regions is critical for ensuring environmental sustainability and public health. This study provides a comprehensive assessment of heavy metal (HM) contamination, ecological risk, and source apportionment in the Zonguldak province of Turkey. By utilizing systematic soil sampling, the research integrates statistical evaluation and spatial mapping with multiple contamination indices, including the Enrichment Factor (EF), Geo-accumulation Index (Igeo), Pollution Load Index (PLI), and Potential Ecological Risk Index (PERI). Results revealed that mean concentrations of Co, Cu, Hg, Cd, Zn, and Ni significantly exceeded crustal and global soil averages. Furthermore, high coefficients of variation for elements such as Cu and Zn suggested significant anthropogenic influences. Spatial analysis, supported by Igeo and EF indices, identified critical pollution hotspots near the urban centers of Ereğli, Alaplı, and Zonguldak, as well as the Gülüç River basin. The overall ecological assessment indicated moderate ecological risk (PERI) across most of the study area, with Hg posing the highest individual risk based on average Er values. PMF analysis identified five potential sources contributing to heavy metal enrichment: coal mining and coal-based emissions, geogenic origins, industrial processes, agricultural activities, and mixed sources. This integrated approach establishes a robust scientific baseline for the effective management of heavy metal pollution in coal-dependent industrial landscapes.
Toxic substances in seafood can accumulate in consumers and cause serious health problems. The purpose of this research was to evaluate the levels of 11 essential and toxic elements (arsenic, copper, mercury, zinc, iron, cadmium, chromium, lead, nickel, selenium and titanium) in nail samples from fishermen who frequently consumed seafood and control groups who consumed them less frequently. This study was conducted in the Marmara Sea coastal cities of Istanbul, Kocaeli, Tekirda & gbreve; and Yalova. 352 individuals were enrolled, 263 fishermen and 89 controls. A questionnaire was administered to the individuals, then fingernail samples were collected. Nail samples were digested by the addition of HNO3 and H2O2, then the element levels was measured by ICP-MS. Zinc and iron levels in fishermen's nails were higher than in controls. Positive correlations were observed between levels of arsenic, mercury, iron, cadmium, chromium, lead, nickel, selenium, and titanium in the fishermen's nails and monthly seafood consumption. Mean nail zinc and copper levels were lower than expected in both groups. Other elements were at expected levels. In conclusion, consuming seafood caught in this region does not result in element exposure sufficient to represent a risk, but may increase in the burdens of some elements.
Objective: The aim of this study is to evaluate the role of mothers' democratic, authoritarian, permissive, and overprotective parenting styles in childhood obesity and mothers' perception of their children's body weight. Method: The research was cross-sectional in design, and a total of 94 children aged between two and six years were included, together with their mothers. Mothers were administered the Parental Attitude Scale, and children's Body Mass Index (BMI) was classified using age- gender-specific tables of the International Obesity Task Force (IOTF). The effect of mothers' parental attitudes on their children's BMI groups was evaluated. However, mothers matched their child's appearance to the appearance they thought corresponded to the age and gender- specific weight plot and mothers' weight perceptions were evaluated. Results: The prevalence of overweight-obesity among children was found to be 19.2%. There wasn't statistically difference between democratic, authoritarian, overprotective and permissive parenting styles and BMI categories of children (p=0.819, p=0.878, p=0.319, p=0.494). The mothers of overweight-obese children perceived their children as significantly overweight compared to other children (p<0.001), they associated their overweight-obese children with the normal body weight imagery. Mothers of children with a normal body weight also selected thinner child images to represent their own child. Conclusions: Mothers of children with normal body weight and overweight-obese BMI, perceived their children's body weight as thinner than it actually was. It's recommended that mothers' perceptions of their children's body weight be assessed in well-child clinics. In cases where these perceptions are inaccurate, mothers should be informed and provided with educational materials.
Phenomenon: Physician immigration from other countries is increasing as developed countries continue to be desirable destinations for physicians; however, the determinants of Turkish physicians' migration decisions are still unclear. Despite its wide coverage in the media and among physicians in Türkiye, and being the subject of much debate, there is insufficient data to justify this attention. With this study, we aimed to investigate the tendency of senior medical students in Türkiye to pursue their professional careers abroad and its related factors. Approach: This cross-sectional study involved 9881 senior medical students from 39 different medical schools in Türkiye in 2022. Besides participants' migration decision, we evaluated the push and pull factors related to working, social environment and lifestyle in Türkiye and abroad, medical school education inadequacy, and personal insufficiencies, as well as the socioeconomic variables that may affect the decision to migrate abroad. The analyses were carried out with a participation rate of at least 50%. Findings: Of the medical students, 70.7% had emigration intentions. Approximately 60% of those want to stay abroad permanently, and 61.5% of them took initiatives such as learning a foreign language abroad (54.5%) and taking relevant exams (18.9%). Those who wanted to work in the field of Research & Development were 1.37 (95% CI: 1.22-1.54) times more likely to emigrate. The push factor that was related to emigration intention was the "working conditions in the country" (OR: 1.89, 95% CI: 1.56-2.28) whereas the "social environment/lifestyle abroad" was the mere pull factor for the tendency of emigration (OR: 1.73, 95% CI: 1.45-2.06). In addition, the quality problem in medical schools also had a significant impact on students' decisions (OR: 2.20, 95% CI: 1.83-2.65). Insights: Although the percentage of those who want to emigrate "definitely" was at the same level as in the other developing countries, the tendency to migrate "permanently" was higher in Türkiye. Improving working conditions in the country and increasing the quality of medical faculties seem vital in preventing the migration of physicians.
Toxic chemicals from polluted seas can enter the human body through seafood consumption and cause health problems. The aim of this study was to evaluate the levels of selected heavy metals and trace elements among fishermen who frequently consumed seafood and controls who consumed seafood less frequently in four provinces on the shores of the Sea of Marmara, which is heavily polluted by industrial activities. Fourteen elements (antimony, arsenic, cadmium, chromium, copper, iron, lead, manganese, mercury, nickel, selenium, strontium, vanadium, and zinc) were analyzed in hair samples using the inductively coupled plasma-mass spectrometer method. Levels of arsenic (0.147 ± 0.067 µg/g vs. 0.129 ± 0.070 µg/g, p = 0.025), chromium (0.327 ± 0.096 µg/g vs. 0.269 ± 0.116 µg/g, p < 0.01), nickel (0.469 ± 0.339 µg/g vs. 0.403 ± 0.368 µg/g, p = 0.015), strontium (1.987 ± 1.241 µg/g vs. 1.468 ± 1.190 µg/g, p < 0.01), and zinc (103.3 ± 43.1 µg/g vs. 92.7 ± 37.4 µg/g, p = 0.047) were higher in the fisherman group than in the control group. No difference was found between the groups in terms of other elements. The findings suggest that heavy metal-trace element contamination in the Sea of Marmara may increase the exposure levels of individuals to some chemicals through seafood consumption.
Objective: Technological advances increased prolonged life expectancy of the terminal patients, who had end-stage diseases. End-of-life care in intensive care units (ICU) has increased with the rise in admissions of terminal patients to ICU. Our aims in this study were to determine the prevalence of terminal patients, and to find the reasons for potentially inappropriate treatments in ICUs. Materials and Methods: It was nationwide, multicenter, point prevalence and observational study. All adult patients, who stayed more than 48 h in the ICU, were enrolled. All patients were recorded on an electronic case record form, consisting of data on patient demographics, treatments, family participation and mini survey for physicians. The study was conducted on October 15, 2018 with a follow-up for 30 days. Results: Of 1127 patients 286 (25%) ICU patients were diagnosed as terminal patients by ICU physicians depending on primary physician statement. Terminal patients relatives requests and physicians legal concerns reduced end-of-life care quality. Terminal patients had significantly increased usage of mechanical ventilation, inotropic drugs, and poor end-of-life care quality (p<0.001). Fifty-four percent of the terminal patients didn’t have any end-of-life decisions at discharge. Half of the terminal patient relatives requested the full code. Without legal concerns, most of the physicians would apply do not resuscitate (86%), withhold (77%) and withdraw (53%) to terminal patients at the end-of-life. Conclusion: Terminal patients occupy an important place in the ICU. To increase the quality of terminal patients’ end-of-life care in the ICU, advanced care planning and legal arrangements should be conducted properly.
Purpose: This study aimed to investigate the effects of different general anesthesia procedures on postoperative olfactory functions and olfactory memory. Materials and Methods: This prospective study was conducted with 97 patients. Patients were divided into three groups based on anesthesia induction and maintenance technique: Group P (induction and maintenance with propofol), Group PS (induction with propofol, maintenance with sevoflurane), and Group S (induction with sevoflurane, maintenance with sevoflurane). Butanol threshold and olfactory identification tests were administered 30 minutes (min) before the operation (T1) and 30 min (T2), 8 hours (h) (T3) and 24 h (T4) after the operation. Results: Butanol threshold values were increased at the T2 time point compared to baseline in all groups, which returned to baseline values at T3 only in Group P. There was a significant difference between Group P and Group S in terms of butanol threshold values at all time points except T1. When olfactory identification increased at T3 and T4 compared to baseline in Group P, there was a significant difference between T2, T3, and T4 time points in Group S, and between T2 and T3 time points in Group PS as compared to Group P. Conclusion: Propofol only causes a temporary impairment in olfactory functions in the early period and does not alter olfactory memory.

BACKGROUND: In our previous report on Turkish COVID-19 patients requiring intensive care, the 24 patients in a single ICU were elderly and mortality was high. We extended our analysis to include patients admitted to ten ICUs. OBJECTIVES: Report the demographics, clinical features, imaging findings, comorbidities, and outcomes in COVID-19 patients. DESIGN: Retrospective. SETTING: Intensive care unit. PATIENTS AND METHODS: The study includes patients with clinical and radiological confirmed or laboratory-confirmed COVID-19 infection who were admitted to ten ICUs between 15 March and 30 June 2020. MAIN OUTCOME MEASURES: Clinical outcomes, therapies, and death during hospitalization SAMPLE SIZE: 974, including 571 males (58%). RESULTS: The median age (range) was 72 (21–101) years for patients who died (n=632, 64.9%) and 70 (16–99) years for patients who lived (n=342, 35.2%) (P<.001). APACHE scores, and SOFA scores were higher in patients who died than in those who survived (P<.001, both comparisons). Respiratory failure was the most common cause of hospitalization (82.5%), and respiratory failure on admission was associated with death (P=.013). Most (n=719, 73.8%) underwent invasive mechanical ventilation therapy. CONCLUSIONS: The majority of patients admitted to the ICU with a diagnosis of COVID-19 require respiratory support. LIMITATIONS: Although the Turkish Ministry of Health made recommendations for the treatment of COVID-19 patients, patient management may not have been identical in all ten units. CONFLICT OF INTEREST: None.

Selected heavy metal-trace element (Ag, As, Ba, Cd, Cu, Hg, Pb, Sb, Se, Sr, and V) levels were determined by the ICP-MS method in whole-blood samples of fishermen and control group who accommodate in four provinces of the Marmara Sea. Mercury (1.267 ± 1.061 µg/L to 0.796 ± 0.853 µg/L) and lead (17.8 ± 9.0 µg/L to 12.0 ± 6.83 µg/L) levels were higher in the fishermen group than that of control group (p < 0.001 for both). There was no difference between the fishermen group and the control group in terms of whole-blood levels of other elements. Total monthly fish consumption was 9340.4 gr in the fishermen group and 326.4 gr in the control group, and the difference between the groups was significant (p < 0.001). There was no difference between the groups in terms of having amalgam dental filling (p > 0.05). The results suggest that consuming high amounts of sea products caught from the Marmara Sea is a source for some heavy metals such as mercury and lead, which poses a public health risk. Unlike the control group, the positive correlation between arsenic, copper, and strontium levels and age in fishermen can also be evaluated as an indicator of chronic exposure.
Anaesthesiology is an extremely stressful and risky branch of medicine. New techniques, new procedures, and innovations in anaesthesia increase the responsibilities and obligations of anaesthesiologists day by day. Operating rooms and intensive care units, which are the working environment of anaesthesiologists, are considered to be an unhealthy workplace. Anaesthesiologists are exposed to various potential physical, chemical, biological, ergonomic, and psychosocial risk factors and hazards in their work environments. The occupational risks anaesthesiologists are exposed to threaten their health and may cause their professional performance to decline. This article aimed to raise awareness about the occupational risks, hazards, and precautions in anaesthesiology practice.
Rapid industrialization and excessive human population growth may cause deterioration in marine water quality and biodiversity. Heavy metals are one of the most common pollutants in the seas and marine organisms, including demersal and pelagic fish, can accumulate them from the environment. Assessment of the ecological risk of heavy metals from fish has an important role in public health. In this study, some heavy metal (Pb, Cd, As, Cr, Hg, Cu, Zn, and Fe) concentrations were determined in the muscle tissues of two commonly consumed fish species, Trachurus mediterraneus (Mediterranean horse mackerel) and Merlangius merlangus (Whiting), which are the fifth (14,222 tons/year) and sixth (6814 tons/year) highest commercial catches of marine fish species in Turkey, respectively. Heavy metal concentrations of samples collected from four sites (Adalar, İzmit Bay, Yalova, and Tekirdağ) in the Marmara Sea were determined using ICP-MS. Fish samples caught at Yalova station were found to have the highest heavy metal concentrations. According to the World Health Organization (WHO), in terms of the mean values, only As, and Cr were higher than permissible limits in T. mediterraneus, while Pb, Cd, As, and Cr were higher in M. merlangus. Arsenic concentrations were higher than maximum limits in both T. mediterraneus and M. merlangus. The estimated weekly intake (EWI) was calculated to assess the potential health impact. The EWI for arsenic, cadmium, lead, and mercury for some sites was above the provisional tolerable weekly intake.
