The impact of ileostomy on intestinal microflora and bacterial translocation has been investigated in 21 adults rats. Animals were divided in three groups: Group I, control, group II, sham underwent laparotomy alone, and Group IIII, laparotomy and terminal ileostomy. 7 days later, animals were killed and different intestinal segments, and organs were harvested for bacteriological analysis. The number of colony forming units for intestinal samples was expressed per intestinal content. All results showed a normal distribution and were expressed as mean standard error. Unpaired Student's t-test was used. Ileostomy induced dysmicrobims, the microflora imbalance was particularly observed in ileum and caecum. Bacterial translocation to kidneys, spleen and liver occurred in control group. Laparotomy and particularly ileostomy significantly increased the bacterial translocation. Ileostomy induced a dysmicrobism by disruption of the microbial ecology balance of the gastro-intestinal tract and significantly increased bacterial translocation.

We investigated the action of amoxicillin-clavulanic acid (Augmentin)(R) an in vitro dynamic model of human gastrointestinal microbial ecosystem. The composition of the fecal microflora from three volunteers was inoculated in the system and compared with data obtained from human patients. 750mg/93.25mgx2/day of the amoxycillin/clavunate were added daily for 3 days. The effect on the microbial ecology was evaluated before administration, during treatment at day 3, and 5 days after the administration was stopped. The treatment induced a decrease in bifidobacteria and lactobacilli and increase in enterobacteria and Clostridium sp in all reactors (p < 0.05). The number tended to restore to their original levels after the administration was stopped. Bacteroides sp was not affected by the treatment in the ileum and descending colon, but significantly increased in transverse and descending colon (p < 0.05). The count did not found the original level after the treatment. Total aerobe and anaerobe bacteria were slightly influenced but remained relatively unchanging over time during the treatment. In conclusion, Augmentin (R) clearly altered the microbial community. These findings were similar and confirmed by in vivo studies (clinical situations).
