Background and Aim: The classification of weight gain during pregnancy and the somatic classification of neonates according to birth weight and duration of pregnancy can be done using percentile values. We aimed to compare such classifications using percentiles of the overall study population with classifications using percentiles that were calculated taking account of maternal height and weight. Material and Methods: Using data from the German Perinatal Survey (1995-2000, over 2.2 million singleton pregnancies) we classified weight gain during pregnancy as low (< 10th percentile), high (> 90th percentile), or medium (10th-90th percentile). Neonates were classified by birth weight as small for gestational age (SGA, < 10th percentile), large for gestational age (LGA, > 90th percentile), or appropriate for gestational age (AGA, 10th-90th percentile). Classifications were performed for 12 groups of women and their neonates formed according to maternal height and weight, either with the percentiles calculated from the total study population or with group-specific percentiles. Results: Using percentiles of the total study population there was large variability between the 12 groups in the proportions with low and high weight gain and in the proportions of SGA and LGA neonates. The variability was much lower when group-specific percentiles were used. Conclusions: Classifications of maternal weight gain during pregnancy and birth weight differ substantially, depending on whether percentiles calculated from the total study population or group-specific percentiles are used. The impact of using percentiles that take account of maternal anthropometric parameters for the medical care and health of neonates needs to be elucidated in future research.
There is a linear relationship between maternal height and birth weight. For each 1 cm increase in maternal height, birth weight increases by 16.7 g. Birth weight percentiles should be calculated by taking maternal height into account. We present birth weight percentile values for girls and boys born after 23-43 completed weeks of gestation for 5 maternal height groups. With these percentiles "genetically" small and "genetically" large, but healthy, neonates can be classified more adequately. The calculations are based on data of about 2.2 million singleton pregnancies from the German Perinatal Survey of 1995-2000.
Background and Aim: Smoking during pregnancy has a negative influence on prenatal growth. We investigated the relationship between the number of cigarettes smoked per day and anthropometric measurements of the neonate.Material and Methods: Datasets of 508926 singleton pregnancies were available (441409 of which included information on maternal smoking) from the German Perinatal Survey of 1998-2000. We analysed the birth weight, length, and head circumference as well as the neonatal growth indices weight/length, weight/head circumference, body mass index, and length/head circumference. Comparisons were made between the children of non-smoking women and those of women smoking 1-5, 6-10, 11-15, 16-20, 21-25, 26-30, or > 30 cigarettes per day (ANOVA, post-hoc tests according to Bonferroni). Analyses were performed for the total study population and for a restricted sample (deliveries after 37-41 completed weeks of gestation by women with a height >= 157 cm and a weight >= 54 kg) to limit confounding variables.Results: Analyses of birth weight, length, head circumference, weight/length, weight/head circumference, and body mass index revealed highly significant decreases (p < 0.001) in all smoking groups compared with non-smokers. There were monotonic decreases in these neonatal growth parameters with increasing smoking over the range 0-20 cigarettes per day. For more than 20 cigarettes per day "dose dependence" was less convincing but group sizes were relatively small. The growth index length/head circumference was not suitable to demonstrate a "dose-dependent" effect of smoking.Conclusions: Because smoking has a "dose-dependent" effect on a number of parameters of foetal growth, any reduction in cigarette consumption during pregnancy should be encouraged.
BACKGROUND AND AIM: Maternal smoking and body mass index (BMI) affect the somatic classifications of neonates based on birth weight, length, and head circumference. Here we investigate the somatic classifications of neonates based on weight-for-length and Rohrer's ponderal index. MATERIAL AND METHODS: Singleton neonates (n = 433 643; data from the German perinatal survey of 1998–2000) were classified as small, appropriate or large for gestational age (SGA, AGA, LGA) based on the 10th and 90th population percentiles of weight-for-length (weight/length) and ponderal index (weight/length 3 ). Maternal BMI was classified as underweight (< 18.5 kg/m 2 ), normal weight (18.5–24.99 kg/m 2 ), overweight (25–29.99 kg/m 2 ), or obese (≥ 30 kg/m 2 ). Smoking during pregnancy was categorized as 0, 1–7, 8–14, or ≥ 15 cigarettes per day. The somatic classification of neonates was compared between different maternal BMI and smoking groups. RESULTS: Classifying neonates by weight-for-length we found that the SGA rate was greater for underweight women (16.3 %) and smaller for overweight (7.5 %) and obese women (7.0 %) compared with the normal weight reference population (9.9 %, all data for non-smokers). Furthermore, SGA rates increased with increasing cigarette consumption. The combination of smoking ≥ 15 cigarettes per day and being underweight was associated with a very high SGA rate of 35.7 %. Classifying neonates according to ponderal index showed qualitatively similar but smaller changes: here smoking ≥ 15 cigarettes per day and being underweight was associated with an SGA rate of 14.4 %. CONCLUSIONS: The somatic classification of neonates according to weight-for-length illustrates the effects of maternal BMI and smoking more clearly than the classification according to the ponderal index.

Background: Maternal height and weight are important determinants of perinatal outcomes. Height and weight can be combined in the measure of body mass index (BMI). We aimed to investigate the utility of maternal BMI as a predictor of perinatal outcomes.Materials and Methods: Based on data collected between 1995 and 2000 as part of the German perinatal survey, we examined singleton pregnancies of women with BMIs of 18, 24, or 30. We compared preterm birth rate, birth weight, and the somatic classification of neonates as small, appropriate, or large for gestational age (SGA, AGA, LGA) for women with heights of 150 cm and 180 cm for each BMI.Results: For women with a BMI of 18 (24; 30) and a height of 150 cm, the preterm birth rate was 13.9% (9.1%; 12.5%); for women with the same BMI and a height of 180 cm the preterm birth rate was 12.1% (6.1%; 4.4%). Birth weight for women with a BMI of 18 (24; 30) and a height of 150 cm was 2 889 g (3 170 g; 3 147 g); for women with the same BMI and a height of 180 cm it was 3 314 g (3 629 g; 3 753 g). The LGA rate for women with a BMI of 18 (24; 30) and a height of 150 cm was 2.1% (5.2%; 5.2%); for women with the same BMI and a height of 180 cm it was 7.7% (20.5%; 27.7%).Conclusions: There is considerable variability in perinatal outcomes between women with the same BMI but different heights. This limits the utility of BMI as a predictor of perinatal outcomes.

Several randomized controlled trials (RCTs) have investigated the prophylactic use of probiotics in preterm infants aimed at reducing the rate of necrotising enterocolitis (NEC). There are 4 meta-analyses on this subject. 2 more RCTs have been published since these meta-analyses were completed. Each meta-analysis, as well as the 2 recent RCTs, document reduced rates of NEC and mortality with the use of prophylactic probiotics. We calculated meta-analyses based on 3 approaches: A - RCTs common to all meta-analyses, B - RCTs ever accounted for in a meta-analysis but not common to all, and C - the 2 recent RCTs. The 3 subgroups yield similar results, with an overall reduction in the relative risk (RR) of NEC (Bell > or =2) to 0.35 (95% CI 0.23-0.55) and of mortality to 0.41 (0.28-0.60). NEC rates and mortalities in the dominant RCTs are in the range reported from North American and European networks. Best results appear to be achieved with probiotics based on 2 or more probiotic species and/or with a combination of Bifidusbacterium spp. and Lactobacillus acidophilus. No unwanted side effects have been reported among 1 117 infants randomized to receive probiotics. We conclude that probiotics are safe and beneficial in preterm infants at risk for NEC.
Background Low birth weight is associated with an increased risk of cardiovascular events in later life. Insulin resistance is a key finding in adult patients with cardiovascular diseases. The neonatal phenotype of an individual with insulin resistance might be low birth weight, as insulin influences fetal growth. The renin–angiotensin–aldosterone system has been associated with cardiovascular disease and insulin resistance. We analyzed whether fetal polymorphisms of the angiotensinogen (AGT) and angiotensin-converting enzyme genes influence birth weight and/or fetal total glycated hemoglobin (fTGH), a surrogate parameter of fetal insulin resistance at birth. Method In 1132 white women delivering singletons, neonatal umbilical blood samples and clinical data of the mothers and newborns were obtained. Newborns were genotyped with respect to the AGT M235T and angiotensin-converting enzyme insertion/deletion polymorphism. Results The AGT M235T TT polymorphism is associated with reduced birth weight (TT: 3288 g versus TM + MM: 3435 g, P < 0.05). Furthermore, newborns with a high percentage of fTGH (>6.5%) are more likely to have the TT genotype than those with normal fTGH (≤6.5%, P < 0.05). With higher cutoffs for fTGH, the significance increases to P less than 0.005. No association was seen between these parameters and the fetal angiotensin-converting enzyme insertion/deletion phenotype. Conclusion The fetal AGT M235T polymorphism is associated with low birth weight and elevated fetal fTGH at birth. Previous findings show that elevated fetal fTGH correlates with low birth weight and that higher activity of the renin–angiotensin–aldosterone system is an independent risk factor for the development of diabetes mellitus and coronary artery disease. Therefore, our data are supportive of the fetal insulin hypothesis.
Ob die vorbeugende Gabe von Probiotika die Inzidenz der Nekrotisierenden Enterokolitis (NEC) Frühgeborener senkt, wurde in mehreren randomisierten kontrollierten Studien (RCTs) untersucht. Die Studien wurden in vier Metaanalysen zusammengefasst, zwei weitere RCTs wurden nach Abschluss der Metaanalysen publiziert. Die Metaanalysen und die zwei jüngsten RCTs zeigen eine verringerte NEC-Inzidenz und Mortalität durch die prophylaktische Probiotikagabe. Wir haben mögliche Faktoren wie Studienqualität und -größe, Geburtsgewicht, Ernährung, NEC-Inzidenz und Mortalität sowie die Anzahl der verwendeten Probiotika mittels Subgruppenanalysen untersucht. Alle Ansätze zeigten ähnliche Ergebnisse. Zusammengefasst ergab sich eine Reduktion des relativen Risikos (RR) für die NEC (Bell-Stadium ≥2) auf 0,35 (95% CI 0,23–0,55) und für die Mortalität auf 0,41 (0,28–0,60). Die in den Studien mit hoher Qualität beschriebenen Raten für NEC und Mortalität liegen im Bereich dessen, was aus nordamerikanischen und europäischen Netzwerken bekannt ist. Die besten Ergebnisse wurden in RCTs erzielt, die mehr als einen Keim, meist eine Kombination aus Bifidusbacterium sp. und Lactobacillus acidophilus verwendeten. Bei keinem der 1102 mit Probiotika behandelten Frühgeborenen wurden unerwünschte Wirkungen beschrieben. Wir folgern aus den analysierten Daten, dass Probiotika für Frühgeborene mit einem Risiko für NEC sicher und nützlich sind.
INTRODUCTION:Maternal body mass index has an impact on maternal and fetal pregnancy outcome. An increased maternal BMI is known to be associated with admission of the newborn to a neonatal care unit. The reasons and impact of this admission on fetal outcome, however, are unknown so far.OBJECTIVE:The aim of our study was to investigate the impact of maternal BMI on maternal and fetal pregnancy outcome with special focus on the children admitted to a neonatal care unit.METHODS:A cohort of 2049 non-diabetic mothers giving birth in the Charite university hospital was prospectively studied. The impact of maternal BMI on maternal and fetal outcome parameters was tested using multivariate regression analysis. Outcome of children admitted to a neonatal ward (n = 505) was analysed.RESULTS:Increased maternal BMI was associated with an increased risk for hypertensive complications, peripheral edema, caesarean section, fetal macrosomia and admission of the newborn to a neonatal care unit, whereas decreased BMI was associated with preterm birth and lower birthweight. In the neonatal ward children from obese mothers are characterized by hypoglycaemia. They need less oxygen, and exhibit a shorter stay on the neonatal ward compared to children from normal weight mothers, whereas children from underweight mothers are characterized by lower umbilical blood pH and increased incidence of death corresponding to increased prevalence of preterm birth.CONCLUSION:Pregnancy outcome is worst in babies from mothers with low body mass index as compared to healthy weight mothers with respect to increased incidence of preterm birth, lower birth weight and increased neonate mortality on the neonatal ward. We demonstrate that the increased risk for neonatal admission in children from obese mothers does not necessarily indicate severe fetal impairment.
Hintergrund: Obwohl vier Metaanalysen (1–4) zeigen, dass Probiotika NEC-Rate und Mortalität von Frühgeborenen senken, fehlt bisher eine allgemeine Empfehlung für ihren Einsatz. Die Datenlage hat sich 2008 durch eine weitere große randomisierte Multizenterstudie (RCT) mit 2×217 Patienten verbessert (5). Wir nehmen dies zum Anlass für eine überarbeitete Metaanalyse. Methoden: Stringenter Ansatz (A): Berücksichtigung der vier RCTs, die von keiner der bisherigen Metaanalysen ausgeschlossen wurden. Heterogener Ansatz (B): Berücksichtigung aller RCTs, die wenigstens in einer Metaanalyse berücksichtigt sind. Berechnungen für beide Ansätze jeweils mit und ohne Einschluss des jüngsten RCT. Ergebnisse (A): Unter Probiotika, verglichen mit Placebo, Abfall der NEC-Inzidenz (≥IIa) von 5.4 auf 1.7% (RR 0.32 [0.16–0.64]) und der Mortalität von 9.6 auf 4.6% (RR 0.48 [0.31–0.75]). Effektgrößen mit Einschluss der jüngsten Studie ähnlich (NEC: RR 0.31[0.17–0.55], Mortalität: RR 0.44 [0.29–0.68]), Irrtumswahrscheinlichkeit jedoch weit unter 0.1% (p<0.0001/p=0.0002). Ergebnisse (B): Abfall der NEC-Rate von 5.5 auf 2.4% (RR 0.41 [0.24–0.70]) und der Mortalität von 9.3 auf 4.3% (RR 0.47 [0.30–0.73]). Effektgrößen mit Einschluss der jüngsten Studie ähnlich (NEC: RR 0.38 [0.23–0.61], Mortalität: RR 0.44 [0.29–0.66]), Irrtumswahrscheinlichkeit weit unter 0.1% (p<0.0001/p=0.0001). Zusammenfassung: Die aktuelle Studie untermauert die Wirksamkeit und Sicherheit der Probiotikaprophylaxe. Für die NEC ergibt sich eine NNT von 29, für die Mortalität 22. Unter Behandlung von 1011 Kindern mit Probiotika wurde kein Sepsisfall mit einem Probiotikumkeim beobachtet, aber ca. 39 Todesfälle vermieden.
Hintergrund: NEC bleibt eine bedrohliche Erkrankung für VLBWs. Wir untersuchten den Effekt der Aktivierung des Transskriptionsfaktors PPARγ in unreifem Darm. Für PPARγ sind neben der Wirkung als Insulinsensitizer protektive Effekte im Rahmen von oxidativen Stress beschrieben. Dies zeigte sich in Zelltypen verschiedener Gewebe und bei Patienten mit Colitis ulcerosa. Methoden: Die Darmschädigung wurde bei neugeborenen Ratten durch Hyperkapnie/Reoxygenierung (H/R) induziert. Gut barrier failure wurde als erhöhte Permeabilität für Dextran, Apoptpose mittels Caspase 3 Aktivität erfasst. PPARγ wurde durch Pioglitazon nur aktiviert oder gleichzeitig mit einem PPARγ Inhibitor gehemmt. Die Expression der induzierbaren NO-Synthase (iNOS) sowie von Galektin–3 wurde mittels real time-PCR oder Westernblot gemessen. Ergebnisse: In Voruntersuchungen fanden wir gut barrier failure ausschließlich im unreifen Darm. Dieser Effekt wurde durch Pioglitazon gehemmt. Gleichzeitig sahen wir vermehrte Galektin–3 und verminderte iNOS Expression. Beide Effekte wurden durch PPARγ Hemmung verhindert. Die Reduktion der H/R-induzierten Apoptose ließ sich durch PPARγ Hemmung nicht reproduzieren. Schlussfolgerung: Für Galektin–3 ist eine antiinflammatorische Wirkung, die Stabilisierung der tight junctions und die Fixierung von Rezeptoren in Zellmembranen beschrieben. Unsere Ergebnisse zeigen, dass Pioglitazon den unreifen Darm vor H/R-induziertem gut barrier failure schützt. Wir vermuten, dass dieser Effekt duch die erhöhte Galektin–3 Expression vermittelt wird. Der Schutz vor Apoptose wird evtl. PPARγ-unabhängig vermittelt.
Die Pathophysiologie der Nekrotisierenden Enterokolitis bleibt unklar: einzig die Unreife bleibt ein gesicherter Risikofaktor. Um entwicklungsabhängige Unterschiede aufzuzeigen, verglichen wir im Tiermodell die Reaktion auf Hyperkapnie/Reoxygenierung (H/R) bei Ratten verschiedenen Reifegrades. Nachdem für CO, einem Produkt des Enzyms Hämoxygenase-1 (HO-1), ein protektiver Effekt im Enterokolitismodell gezeigt werden konnte, untersuchten wir die Expression der HO-1 in unserem Modell. In der vorliegenden Studie soll der Einfluss der Änderung der Expression der HO-1 auf die Ausprägung einer intestinalen Entzündungsreaktion sowie einen Schaden an der Darmwand untersucht werden. Methoden: Wir induzierten experimentelle Enterokolitis durch H/R in neugeborenen-gestillten Ratten. Die Aktivierung bzw. Hemmung der Hämoxygenase-1 erfolgte durch eine einmalige intraperitoneale Gabe vom Hemin bzw. Zink-Protoporphyrin-IX, jeweils 4 Stunden vor H/R. Unphysiologische Permeabilitätserhöhung der Darmwand (gut barrier failure) zeigten wir durch die intestinale Aufnahme von FITC-markiertem Dextran. Die Expression von Hämoxygenase-1 (HO-1) und iNOS untersuchten wir mittels quantitativer PCR und Western blot Analyse, die Aktivität der Caspase-3 in einem kolorimetrischem Assay. Ergebnisse: Im Darm der reifen Tiere sahen wir einen frühen Anstieg der HO-1-Expression, ein gut barrier failure sowie Aktivierung der Apoptose blieb aus. Bei den unreifen Tieren blieb der frühe Anstieg der HO-1-Expression aus: es kam sowohl zur Expressionssteigerung der iNOS, zur Aktivierung der Caspase-3, als auch zum gut barrier failure. Nach Gabe von Hemin sahen wir früh vermehrt HO-1-Expression gefolgt von einer signifikanten Reduktion des gut barrier failures (auf das Niveau der Kontrolltiere ohne H/R). Gleichzeitig war kein Anstieg der iNOS-Expression mehr nachweisbar. Die Hemmung der HO-1-Aktivität führte zu einem signifikanten Anstieg des gut barrier failures und der Capsase-3-Aktivität. Schlussfolgerung: Unsere Ergebnisse zeigen erstmals einen kausalen Zusammenhang: Unreife führt zu verminderter und geringerer Expression der HO-1, dies zur Aktivierung inflammatorischer Prozesse mit der Folge des gut barrier failures im unreifen Darm nach Hyperkapnie/Reoxygenierung. Der HO-1 Expression kommt somit eine Schlüsselrolle in der Pathologie von Entzündungsreaktionen im unreifen Darm zu und kann helfen, die unverstandene Pathologie der Nekrotisierenden Enterokolitis zu verstehen.
A genetic association study was conducted to assess whether genetically determined alterations of the nitric oxide system are associated with clinical markers of pre-eclampsia. A large number of Caucasian women were consecutively included after delivery and genotyped for the endothelial nitric oxide synthase gene (NOS3) polymorphisms G894T, T789C (n=1502) and intron 4a/b (n=2186). There are no significant differences in mean blood pressure (BP), protein excretion or new-onset peripheral oedema between any of the genotypes over the course of pregnancy. Neither particular haplotypes nor the combined presence of any two alleles is associated with those markers of pre-eclampsia. The maternal polymorphisms do not seem to influence fetal growth, birth weight or the incidence of congenital malformations. We demonstrate in a large Caucasian population that maternal polymorphisms of the NOS3 gene are not related to clinical markers of pre-eclampsia. The functional relevance of the NOS3 variants alone does not seem to be strong enough to affect BP regulation during pregnancy.
Einleitung: Bacillus cereus ist eine seltene, aber potentiell pathogene Species bei systemischen Infektionen von Frühgeborenen. Wir berichten über ein Frühgeborenes mit schwerster B. cereus- Sepsis, das erfolgreich mit Meropenem, Fosfomycin und Gentamicin behandelt wurde, und sich nach Entfernen der zentralen Katheter stabilisierte.
Hintergrund: Trotz des Einsatzes von Erythropoetin besteht bei mehr als 85% aller Frühgeborenen <1000g nach den derzeit gültigen Transfusionsindikationen noch immer die Notwendigkeit mindestens einer Transfusion (Tx) während des stationären Aufenthaltes. Derzeit existiert kein Parameter, welcher die Indikation für eine Transfusion objektiviert.
Nach wie vor bleibt Frühgeburtlichkeit als Hauptrisikofaktor der Nekrotisierenden Enterokolitis (NEC), die zu den bedrohlichsten Notfällen in der Neonatologie zählt, unverstanden. Die vorliegende Studie untersucht die Unreife in der intestinalen Inflammationsreaktion auf biochemischer bzw. molekularbiologischer Basis.
