Background: IgG concentrations in cerebrospinal fluid generally range from 20 to 45 mg/L. In multiple sclerosis immune reactions lead to intrathecal synthesis of specific IgGs that can be detected in biological fluid samples both quantitatively and qualitatively by isoelectric focusing of supplementary oligoclonal IgG bands. Method: A simple tool, using the MATLAB application, to facilitate and improve isoelectric focusing profile analysis is presented and evaluated in terms of its sensitivity, repeatability and reproducibility. A comparison between human readers and semi-automatic method has also been performed. Results: Results from the semi-automatic method were found to be equivalent or superior to generally employed laboratory methods. Repeatability analysis for semi-automatic processing yielded coefficients of variation (CVs) in the 3-7% range, and using a sample with an estimated IgG concentration of 200 mg/L, four bands were still visible after dilution to 5 mg/L, corresponding to band concentrations of 1.1-1.6 mg/L. Discordances between visual inspection and automatic analysis only appear at threshold levels for interpretation (the gray zone). Conclusion: The semi-automatic method has acceptable performance for routine implementation.
Detection of oligoclonal electrophoretic bands in cerebrospinal fluid (CSF) is an important diagnostic tool for Multiple Sclerosis (MS). Electrophoretic profiles are difficult to interpret due to low contrast and artefacts. A semi-automated method to ease analysis and to reduce subjectivity is presented. The method sequentially converts color images to grayscale, realigns bands, removes artifacts, then converts 2D images to a signal, before detecting, thresholding and editing peaks to optimize profiles. Such treated profiles (21 positive and 15 negative) are compared to ground truth analysis of an expert biologist. 16 profiles over 21 are well detected positive and 12 profiles over 15 are detected negative, results seem similar to inter-experts variability reported in literature.
Adaptive Filtering by Optimal Projection (AFOP) is an automatic method for reducing ocular and muscular artifacts on electro-encephalographic (EEG) recordings. This paper presents two additions to this method: an improvement of the stability of ocular artifact filtering and an adaptation of the method for filtering electrode artifacts. With these improvements, it is possible to reduce almost all the current types of artifacts, while preserving brain signals, particularly those characterising epilepsy. This generalised method consists of dividing the signal into several time-frequency windows, and in applying different spatial filters to each. Two steps are required to define one of these spatial filters: the first step consists of defining artifact spatial projection using the Common Spatial Pattern (CSP) method and the second consists of defining EEG spatial projection via regression. For this second step, a progressive orthogonalisation process is proposed to improve stability. This method has been tested on long-duration EEG recordings of epileptic patients. A neurologist quantified the ratio of removed artifacts and the ratio of preserved EEG. Among the 330 artifacted pages used for evaluation, readability was judged better for 78% of pages, equal for 20% of pages, and worse for 2%. Artifact amplitudes were reduced by 80% on average. At the same time, brain sources were preserved in amplitude from 70% to 95% depending on the type of waves (alpha, theta, delta, spikes, etc.). A blind comparison with manual Independent Component Analysis (ICA) was also realised. The results show that this method is competitive and useful for routine clinical practice. Objectives. - The Prostate Health Index (PHI) is calculated from concentrations of 3 different proteins: the total Prostate Specific Antigen (PSA), the free PSA and [-2]proPSA (p2PSA). We compared the PHI with 3 different markers: The total PSA, the free PSA and the free PSA/total PSA rate.Patients and methods. - When the total PSA was measured between 2 and 10 ng/mL, both the p2PSA and the free PSA immunoassays were performed. Statistical analysis searched a correlation between new (the p2PSA and the PHI) and usual (the total PSA, the free PSA and the free PSA/total PSA rate) markers.Results. - The p2PSA is correlated with the total PSA, the free PSA and the free PSA/total PSA rate but we observed a great dispersion. The PHI is not correlated with the total PSA neither with the free PSA nor with the free PSA/total PSA rate.Conclusion. - These results show that the PHI is an independent biomarker. The value of albuminuria determination is commonly accepted in the monitoring of renal function, particularly in diabetic patients. This may be achieved in different ways and using different assay kits. The aim of this study was to assess the reliability of immunoturbidimetric measurements by comparing the results obtained by two different methods in our laboratory. Therefore we measured albuminuria of 54 patients monitored for their diabetes, with normal to moderately high albuminuria, according to two methods: Microalbumin reagent (Beckman Coulter) on a Unicel DxC 600 (Beckman Coulter) and Albumin Turbiquant reagent (Dade Behring, Siemens) on a Turbitimer system (Dade Behring, Siemens). It was then possible to study the correlation between the two techniques. Our results suggest that although this correlation is excellent, the values differ significantly: the results observed with the Siemens method are higher by about 10%. It seems legitimate to think that a correction should be applied, depending on the used method. In addition, it appears that standardization is necessary, concerning analytical stage but although urine collection, pre-analytical stage or reporting and interpretation of results.
L'électroencéphalogramme (EEG) présente un regain d'intérêt dans le diagnostic des démences depuis l'introduction des méthodes mathématiques d'analyse. La plupart des études réalisées n'analyse que la maladie d'Alzheimer (MA). Le but de ce travail est de déterminer si l'analyse linéaire classique et non linéaire de l'EEG issue de la théorie du chaos est pertinente pour différencier les démences entre elles. 156 patients présentant une démence (50 MA, 36 démences vasculaires (Dva), 42 démences mixtes (DM), 13 démences à corps de Lewy) ou un Mild Cognitive Impairment (n=15) bénéficièrent d'un EEG et de tests neuropsychologiques. Il fut réalisé une analyse spectrale de puissance de l'EEG ainsi qu'une analyse non-linéaire à partir de la méthode des récurrences proposée par Eckmann. Des analyses discriminantes ont été calculées pour classifier les différents types de démence. Les variables EEG retenues et celles issues du bilan neuropsychologique ont été choisies en fonction des analyses de variance entre groupes. Les analyses discriminantes établies permettaient de discriminer à 97,3% les Dva des DCL ; à 89,1% les DM des DCL ; à 87,3% les MA des Dva ; à 84,9% les Dva des DM ; à 78,9% les MA des Dva et à 62,5% les MA des DM. Les variables EEG les plus intéressantes pour l'analyse discriminante étaient issues de l'analyse linéaire. Le seul paramètre non linéaire utilisé était l'indice de Shannon. Les tests neuropsychologiques retenus étaient le rappel libre total en mémoire épisodique verbale et l'épreuve de dénomination de Bachy. Seuls des paramètres EEG ressortaient et ont donc étaient utilisés pour distinguer les DCL des autres démences. L'analyse linéaire et non linéaire de l'EEG semble donc particulièrement intéressante pour diagnostiquer la DCL. Le pourcentage de patients bien classés était majoré par l'utilisation conjointe de variables neuropsychologique et EEG pour les autres démences. La démence la moins bien discriminée des autres est la DM. Cela peut être expliqué par sa variabilité clinique et l'insuffisance des critères diagnostiques actuels. L'analyse linéaire et non linéaire de l'EEG représente un outil pour le diagnostic étiologique des démences.
Purpose. - To evaluate screening for diabetic retinopathy, using three-field nonmydriatic fundus photography, combined with telemedicine, in two hospitals.Methods. - Patients from the Endocrinology Department of Saint Philibert Hospital were screened consecutively from March 2007 to September 2008. Three 450 digital images per eye examination were taken using a nonmydriatic fundus camera and sent by intranet to the Department of Ophthalmology of Saint Vincent de Paul Hospital for grading by two ophthalmologists.Results. - A total of 1147 patients (52% mates, 48% females) were screened. The mean age was 60 years (range, 19-92 years). Most patients had type 2 diabetes (n = 1000; 90%). The mean HbA1c was 8%. Microalbuminuria was detected in 239 patients (20.8%). Of these 1147 patients, 521 (45%) said they had never had a fundus examination before. Fundus photography of at least one eye could not be graded in 160 patients (14%). Diabetic retinopathy was detected in 187 patients (19%). Suspected macular edema (hard exudates within one disc diameter of the macular center) was found in 38 patients (3.8%). Referral to an ophthalmologist was required in 347 patients (30%) for diabetic retinopathy and unreadable photographs. During the period of the study, 107 patients had both three-field nonmydriatic photography and dilated funduscopy, which was used as reference. Agreement between the two readers was 0.97.Conclusions. - Nonmydriatic photography combined with telemedicine is useful for screening for diabetic retinopathy and identifies patients requiring further complete eye examination. Le but de ce travail est de déterminer si l'analyse linéaire et non linéaire de l'EEG est corrélée avec les différents tests neuropsychologiques dans les différentes démences. 156 patients présentant une démence (50 MA, 36 démences vasculaires, 42 démences mixtes, 13 démences à corps de Lewy) ou un Mild Cognitive Impairment (n=15) bénéficièrent d'un EEG et de tests neuropsychologiques (MMSE, échelle de MATTIS, épreuve de Gröber et Buschke, figure de Rey, dénomination de Bachy, empans directs et inverses, test de barrage de chiffres, Trail Making Test, fluences catégorielles et littérales, praxies réflexives). Il fut mené une analyse spectrale de puissance de l'EEG ainsi qu'une analyse non-linéaire à partir de la méthode des récurrences proposée par Eckmann. L'analyse linéaire et non linéaire de l'EEG a permis d'établir des corrélations très nombreuses non seulement avec les tests évaluant l'efficience cognitive globale (MMS et échelle de MATTIS) mais aussi avec chaque test neuropsychologique. Plus le test neuropsychologique et/ou l'efficience cognitive globale était altéré et plus l'EEG était ralenti et contraint. Les corrélations les plus nombreuses le furent avec l'analyse linéaire (73,6%) et principalement avec le rapport des fréquences lentes (thêta et delta) sur les rapides (alpha et bêta) avec une discrète prédominance pour l'hémisphère gauche et les régions fronto-temporales. Les aires cérébrales les plus corrélées aux tests sont les régions fronto-temporales (52%), pariéto-occipitales (24%), de la ligne médiane (15%) puis centrales (15%). Chaque test neuropsychologique était corrélé à des altérations EEG situées sur de nombreuses électrodes sans réelle prédominance régionale témoignant de la mise en jeu d'un grand nombre de réseaux neuronaux impliquant différents lobes cérébraux lors de la réalisation d'une tâche cognitive particulière. La mise en évidence de nombreuses corrélations entre les tests neuropsychologiques et les paramètres d'analyse linéaire et non linéaire de l'EEG conforte la pertinence de l'utilisation de l'EEG pour le diagnostic et le suivi des démences.

OBJECTIVES:The aims of this study were to determine the performance of each variable, to define the optimal diagnostic thresholds and to determine the relative value of assaying chromogranin A (CgA).DESIGN:Prospective study.METHODS:Two groups of patients were studied: a control group of 71 patients and a group of 63 patients with a histologically-proven pheochromocytoma (52 pheochromocytomas and 14 paragangliomas). Fourteen of the patients had a family history of the disease. Eleven variables were assayed in each patient, i.e. the plasma and urinary concentrations of amines and their derivatives, and the CgA serum concentration.RESULTS:The study of the control group showed that all the serum assays gave false positive results (from 6 to 23%), as did four of the six urinary assays (from 2.9 to 12.3%). The areas under the receiver operating characteristic curves varied from 0.689 to 0.992. The variables relating to the epinephrine pathway were significantly less expressed in the hereditary diseases than in the sporadic cases. The diagnostic thresholds of the three most efficient variables have been raised.CONCLUSIONS:Plasma determinations of metanephrines are now an easy and convenient tool for the diagnosis of pheochromocytoma. However, in our study the best specificity was obtained with the urinary tests rather than with the plasma assays while the highest sensitivities were for the normetanephrine assays. The assay of CgA was highly efficient in diagnosing pheochromocytomas in the absence of renal insufficiency. By combining it with fractionated metanephrine assays, the sensitivities of the latter were increased.
Depuis l'application des méthodes mathématiques non-linéaires à l'analyse dynamique de l'EEG, leur intérêt n'a cessé de croître en épileptologie notamment dans la perspective de détection anticipée de crises. Le but de ce travail est, en recourant à ces méthodes, de suivre longitudinalement dans le temps les niveaux moyens de la dynamique de l'EEG afin d'établir des corrélations avec l'état clinique et l'évolution de l'épilepsie. Dix patients présentant une épilepsie partielle avérée furent suivis cliniquement sur plusieurs années afin de repérer les périodes de stabilité et d'évolutivité de l'affection. Sur la même période l'EEG était enregistré régulièrement. Pour chaque électrode fut menée une analyse spectrale de puissance ainsi qu'une analyse non-linéaire à partir de la méthode des récurrences proposée par Eckmann. Cette dernière permit de calculer 3 indices dynamiques de l'EEG : Indice de Shannon, Pourcentage de déterminisme et Ratio. Chez les patients dont la comitialité était cliniquement stable, il fut noté une stabilité dans le temps des indices dynamiques de l'EEG. En revanche, pour ceux présentant des périodes avec majoration des crises furent observées des variations significatives des niveaux moyens des indices dynamiques non-linéaires. Chez un patient ayant bénéficié d'une chirurgie de l'épilepsie ces indices se maintinrent stables sur plusieurs années. Cette étude met donc en évidence une corrélation existant entre l'évolutivité de la maladie épileptique et la dynamique de l'EEG appréciée à partir des indices non-linéaires. La signification de ces changements dynamiques est discutée. Ces résultats préliminaires sont à confirmer sur une population plus vaste ainsi que pour les différentes formes d'épilepsie généralisée. La mise en évidence d'un lien entre la dynamique de l'EEG et l'évolutivité clinique de l'épilepsie confirme son caractère dynamique ainsi que l'intérêt des travaux portant sur la détection anticipée des crises.
The follow-up of premature rupture of membranes requires an hospitalization. The aim of this work was to evaluate the interest of the Prom-Test in the action to be taken in the case of a suspicion of premature rupture of membranes, a negative result allowing to not hospitalize the patient. 105 tests have been carried out producing 73 negative results, 23 positive and 9 doubtful. The childbirth, started or not, occurred much more quickly when the test is positive. Only 13 of the 73 negative tests resulted in a hospitalization including 11 for beginning of delivery. A correct use of the test can prevent unnecessary days of hospitalization.
L’EEG a été utilisé dans la sclérose en plaques (SEP) à partir de l’analyse visuelle puis spectrale et enfin de la cohérence. Nous proposons de recourir ici aux méthodes mathématiques d’analyse non linéaire. L’objectif est d’utiliser l’analyse dynamique du signal EEG afin de déceler des modifications précoces présentes dès la première poussée de SEP. L’étude est aussi menée dans les autres formes évolutives de l’affection. La méthode de représentation des récurrences (Eckmann 1987) a été appliquée aux 17 voies d’EEG après une analyse en composantes principales (ACP). Elle a permis le calcul des indices suivants. Shannon, Pourcentage de Déterminisme après ACP et indices interhémisphériques (Delta Shannon, Delta déterminisme). L’étude prospective a comparé 245 patients : 124 formes rémittentes (R) (dont 36 patients avec une seule poussée), 93 secondairement progressifs (SP), 28 primairement progressives (PP), à un groupe témoin apparié. Pour tous les paramètres non linéaires étudiés, le groupe SEP diffère très significativement du groupe témoin. Il existe au moins un paramètre anormal chez 38,4 p. 100 des patients. La fréquence des anomalies ne diffère pas dans les trois formes évolutives. En revanche, dans les formes R évoluant depuis moins d’un an, il existe 50 p. 100 d’anomalies. Le taux de paramètres interhémisphériques anormaux, qui est de 17,1 p. 100 toutes formes confondues, atteint 37,5 p. 100 chez ces patients du groupe R de moins d’un an d’évolution. Les paramètres non linéaires de l’EEG sont anormaux chez 38,4 p. 100 des SEP. Ces modifications sont en faveur d’une diminution de la complexité de la dynamique telle qu’elle a été observée dans d’autres atteintes cognitives. Fait notable, elles surviennent très précocement et sont plus fréquentes (50 p. 100) chez les patients dont la SEP évolue depuis moins d’un an avec une seule poussée. Chez eux, les indices interhémisphériques sont aussi plus souvent altérés. Ces perturbations de la dynamique de l’EEG en faveur d’une atteinte interhémisphérique survenant dès la première poussée vont dans le sens d’anomalies très précoces de la substance blanche du corps calleux (Audoin, 2007).
OBJECTIVES:Hyperhomocysteinaemia is associated with cardiovascular events in nondiabetic individuals. The present study was aimed to explore the implication of hyperhomocysteinemia in development of cardiovascular events in patients with type 2 diabetes.DESIGN:A total of 185 patients with type 2 diabetes (115 women and 70 men, 30 to 93 years of age) have been included consecutively in the ambulatory unit at the Saint-Philibert Hospital. For each patient the concentration of homocysteine, cholesterol and triglyceride levels and HbA1c have been measured. In the studied population, 121 patients presented cardiovascular events (myocardial infarctus, peripheric arteriopathy, cerebrovascular accident).RESULTS:The patients with cardiovascular events were older, the concentration of homocysteine and creatinine were higher. The plasma homocysteine levels adjusted for age and creatinine levels were higher in patients with cardiovascular events than in patients without cardiovascular events (15.4 +/- 3.52 micromol/L and 13.13 +/- 2.26 micromol/L respectively; p = 2. 10(-5)).CONCLUSIONS:Hyperhomocysteinemia is an independant risk factor for cardiovascular events in type 2 diabetes, independent of age and renal function.
