BACKGROUND:Non-small cell lung cancer (NSCLC) is the leading cause of cancer-related death globally, with stage IA NSCLC presenting a unique opportunity for curative interventions. The efficacy of minimally invasive surgery (MIS) vs. stereotactic ablative radiotherapy (SABR) remains debated due to limited direct comparative data. METHODS:This multicenter observational study analyzed data from 1014 patients diagnosed with clinical stage IA NSCLC (2015-2021) and treated with MIS (including VATS and RATS) or SABR. After propensity score matching, 234 patients (117 per group) were included. Matching balanced age, ECOG performance status, Charlson Comorbidity Index, lung function, and histological subtype to compare overall survival (OS), freedom from recurrence (FFR), and recurrence rates. RESULTS:The mean follow-up was 35 months for MIS and 33 months for SABR. The matched cohort (n = 234) showed superior 5-year locoregional control (LRC) rates for MIS (93%) vs. SABR (88%). FFS at 2 and 5 years was higher for MIS (93.5% and 90.3%) than SABR (82.1% and 77.9%; P = .010). OS was significantly higher in MIS, with a hazard ratio of 1.60 (95% CI: 1.11-2.31). Early mortality rates were 2.6% for MIS at 30 and 90 days. SABR exhibited no 30-day mortality and a 90-day rate of 1.7%. CONCLUSION:MIS is associated with higher OS, LRC and FFR, but higher treatment-related mortality compared to SABR for stage IA NSCLC. This study supports the preference for surgical interventions where feasible. While future randomized controlled trials may provide more insights, this observational study contributes valuable evidence to guide clinical decision-making.
BACKGROUND:Stereotactic body radiotherapy (SBRT) is increasingly used to treat lung metastasis (LM) in patients with soft tissue sarcoma (STS) and bone sarcoma (BS). METHODS:This retrospective study evaluated the outcomes of patients with BS and STS treated with SBRT for LM between 2010 and 2023. RESULTS:We enrolled 102 patients (51 each with STS and BS), of whom 71 were males and 31 were females (median age, 40 years; range, 11-81 years). At diagnosis, 76 and 26 patients had localized and metastatic disease, respectively, with a median of 4 recurrences (range, 1-12). Before SBRT, 75 patients received chemotherapy and 52 underwent surgery for LM, with 276 nodules treated with SBRT (median dose, 48 Gy; range, 40-52). Local control of irradiated LM was 86% at 1 year and 78% at 2 years. By 31 December 2023 (median follow-up, 4.8 years), 60 patients had died and 42 survived (20 without ongoing disease). From the first LM relapse, the median overall survival (OS) was 4.8 years and the 5-year OS was 49% (95% confidence interval, 39-60%), with no difference between STS and BS; the median OS was 2.9 years and the 5-year OS was 36% after SBRT. Chemotherapy before SBRT was a negative prognostic factor by multivariate analysis. CONCLUSIONS:Long-term follow-up shows that SBRT as part of a multimodal treatment approach has reasonable survival rates in patients with LM due to sarcoma. Compared with historical results using only surgery and chemotherapy, SBRT has improved the 5-year OS.
Although the gold standard for stage I lung cancer is surgery, stereotactic body radiotherapy (SBRT) is a valid alternative, particularly in frail patients. The aim of this study is to compare SBRT to minimally invasive surgery (MIS) in a propensity-matched score cohort of patients from two Institutional high-volume Centers.
PURPOSE:The results in terms of side effects vary among the published accelerated partial-breast irradiation (APBI) studies. Here, we report the 5-year results for cosmetic outcomes and toxicity of the IRMA trial.METHODS:We ran this randomized phase III trial in 35 centers. Women with stage I-IIA breast cancer treated with breast-conserving surgery, age ≥ 49 years, were randomly assigned 1:1 to receive either whole-breast irradiation (WBI) or external beam radiation therapy APBI (38.5 Gy/10 fraction twice daily). Patients and investigators were not masked to treatment allocation. The primary end point was ipsilateral breast tumor recurrence. We hereby present the analysis of the secondary outcomes, cosmesis, and normal tissue toxicity. All side effects were graded with the Radiation Therapy Oncology Group/European Organisation for Research and Treatment of Cancer Radiation Morbidity Scoring Schema. Analysis was performed with both intention-to-treat and as-treated approaches.RESULTS:Between March 2007 and March 2019, 3,309 patients were randomly assigned to 1,657 WBI and 1,652 APBI; 3,225 patients comprised the intention-to-treat population (1,623 WBI and 1,602 APBI). At a median follow-up of 5.6 (interquartile range, 4.0-8.4) years, adverse cosmesis in the APBI patients was higher than that in the WBI patients at 3 years (12.7% v 9.2%; P = .009) and at 5 years (14% v 9.8%; P = .012). Late soft tissue toxicity (grade ≥ 3: 2.8% APBI v 1% WBI, P < .0001) and late bone toxicity (grade ≥ 3: 1.1% APBI v 0% WBI, P < .0001) were significantly higher in the APBI arm. There were no significant differences in late skin and lung toxicities.CONCLUSION:External beam radiation therapy-APBI with a twice-daily IRMA schedule was associated with increased rates of late moderate soft tissue and bone toxicities, with a slight decrease in patient-reported cosmetic outcomes at 5 years when compared with WBI, although overall toxicity was in an acceptable range.
The role of consolidation radiotherapy (RT) for bulky lesions is controversial in patients with advanced-stage Hodgkin lymphoma who achieve complete metabolic response (CMR) after doxorubicin, bleomycin, vinblastine, dacarbazine (ABVD)-based chemotherapy. We present the final results of the Fondazione Italiana Linfomi HD0801 trial, which investigated the potential benefit of RT in that setting. In this phase 3 randomized study, patients with a bulky lesion at baseline (a mass with largest diameter $5 cm) who have CMR after 2 and 6 ABVD cycles were randomly assigned 1:1 to RT vs observation (OBS) with a primary endpoint of event-free survival (EFS) at 2 years. The sample size was calculated estimating an EFS improvement for RT of 20% (from 60% to 80%). The secondary end point was progression-free survival (PFS). One hundred sixteen patients met the inclusion criteria and were randomly assigned to RT or OBS. Intention-to-treat (ITT) analysis showed a 2-year EFS of 87.8% vs 85.8% for RT vs OBS (hazard ratio [HR], 1.5; 95% confidence interval [CI], 0.6-3.5; P = .34). At 2 years, ITT-PFS was 91.3% vs 85.8% (HR, 1.2; 95% CI, 0.5-3; P = .7). Patients in CMR randomly assigned to OBS had a good outcome, and the primary end point of a 20% benefit in EFS for RT was not met. However, the sample size was underpowered to detect a benefit of 10% or less, keeping open the question of a potential, more limited role of RT in this setting.
