OBJECTIVES:A multicenter study to compare results of BRAHMS Kryptor PCT with those obtained using four BRAHMS-partnered procalcitonin (PCT) automated immunoassays (DiaSorin Liaison, BioMérieux Vidas, Roche Cobas E601 and Siemens Advia Centaur) and the Diazyme immunotubidimetric assay implemented on four clinical chemistry platforms (Abbott Architect c16000, Siemens Advia 2400, Roche Cobas C501 and Beckman Coulter AU5800). DESIGN AND METHODS:One hundred serum samples from in-patients with PCT values between 0.10 and 58.7 ng/mL were divided into aliquots and tested with the nine different reagents and analyzers. BRAHMS PCT Kryptor results were used as reference. RESULTS:Compared to BRAHMS PCT Kryptor, significant differences in results were observed on Vidas, Advia Centaur, Architect, Cobas C501 and AU5800. However, the correlation coeffiecients (r) with BRAHMS PCT Kryptor were between 0.899 and 0.988. The mean bias was less than ±1.02 ng/mL, except for Vidas (2.70 ng/mL). The agreement at three clinically relevant cut-offs was optimal: between 83-98% at 0.50 ng/mL, 90-97% at 2.0 ng/mL, and 98% at 10 ng/mL. The comparison of Diazyme PCT across the four clinical chemistry analyzers yielded high correlation coefficients (r between 0.952 and 0.976), a mean bias less than ±0.9 ng/mL, acceptable agreement at 0.5 ng/mL (>82%), and high concordance at the 2.0 ng/mL (>97%) and 10 ng/mL (>98%) cut-offs. CONCLUSIONS:The methods and applications evaluated in this multicenter study are aligned with BRAHMS PCT Kryptor and can be used for predicting the risk of progression to systemic inflammation in patients with bacterial infections using the conventional PCT diagnostic thresholds.
Cardiac troponin I (cTnI) is a powerful tool for diagnosis of acute myocardial infarction (AMI). This necessitates the availability of highly specific, sensitive, and precise assays thoroughly validated in well-designed studies. We describe the performance characteristics of the TOSOH AIA-Pack next-generation cTnI immunoassay evaluated at six institutions. The protocol consisted of following sections evaluating analytical and preanalytical aspects: calibration stability, antibody specificity, detection limit and imprecision, linearity, method compa- rison, sample stability, and anticoagulant interference. Reference values, the 10% CV cutoff for detecting myocardial necrosis, and the AIA cTnI release kinetics after AMI were also defined. The minimum detectable cTnI concentration was 0.038 µg/L. Linearity of response was demonstrated along the entire dynamic range of the assay. The cTnI concentration corresponding to a total CV of 10% was 0.13 µg/L. The assay completely recovered troponin complex, but produced a response to free cTnI that was half in comparison to complexed cTnI. Values in heparin and EDTA plasma were on average ∼15% and ~13% higher than in matched serum, respectively. cTnI, measured in 120 healthy individuals, was always undetectable. When compared in the early phase after AMI (≤ 18 h after onset), results of AIA and ACS:180, but not of AIA and Dimension, showed a significant, time-dependent bias. The AIA cTnI assay revealed acceptable analytical performance making it suitable for clinical applications. SUMMARY La Troponina I cardiaca (cTnI) è uno strumento fondamentale per la diagnosi di infarto acuto del miocardio (IMA); per questo devono essere disponibili metodi di determinazione altamente specifici, sensibili e precisi, convalidati in studi rigorosamente definiti. Qui vengono descritte le caratteristiche del metodo AIA-Pack 2∧ generazione valutate in uno studio multicentrico che ha coinvolto sei laboratori. Il protocollo di valutazione, riguardante aspetti analitici e pre-analitici, è stato articolato nelle seguenti sezioni: stabilità della calibrazione, specificità dell'anticorpo, limite di rivelabilità e imprecisione, linearità, confronto metodi, stabilità del campione e interferenza da anticoagulanti. Inoltre sono stati definiti i valori di riferimento, il cut-off al 10 CV% per rilevare la necrosi miocardica e la cinetica di rilascio della cTnI dopo IMA. La concentrazione minima misurabile di cTnI è risultata 0.038 µg/L. La linearità della risposta è stata dimostrata per tutto l'intero range dinamico del saggio. La concentrazione di cTnI misurabile con una imprecisione del 10 % (CV totale) è risultata 0.13 µg/L. Si è osservato che il metodo recupera completamente la troponina complessata mentre la risposta alla forma libera di cTnI è circa la metà rispetto a quella complessata. I valori di cTnI in campioni di plasma (prelevati con eparina e con EDTA) sono risultati più elevati rispettivamente del 15% e del 13% rispetto al siero. La cTnI in 120 individui sani è risultata sempre non misurabile. I valori di cTnI nella fase precoce dopo IMA ( ≤ 18 ore dall'evento) misurati con AIA-Pack e con ACS:180 mostrano un bias significativo tempo-dipendente; questo non si osserva confrontando le misure AIA-Pack con quelle Dade Dimension.Le prestazioni analitiche del metodo cTnI AIA-Pack si sono dimostrate accettabili e adatte alle applicazioni cliniche.
