Aspirin is ubiquitous, inexpensive, and mechanistically compelling in colorectal cancer (CRC), yet its clinical role has become more ambiguous with—notwithstanding—new data. Neutral phase III results in unselected disease, biomarker-restricted signals of benefit, and guideline shifts driven by bleeding risk have left clinicians without a consistent, setting-specific approach. This review fills that gap by translating the post-precision aspirin literature into a selection-first framework that specifies who may benefit, when off-trial use is reasonable versus discouraged, and why trial enrolment remains the preferred option in key areas of uncertainty. In prevention, aspirin produces modest reductions in adenoma recurrence in selected high-risk surveillance cohorts and shows delayed cancer-endpoint benefit in Lynch syndrome, highlighting that baseline CRC risk and time horizon are decisive. In older adults and lower-risk settings, the time-to-benefit often does not align with a higher, earlier bleeding liability. After curative-intent treatment, randomised trials now define clear boundaries: routine adjuvant aspirin in unselected resected CRC is not supported, whereas recurrence reduction appears confined to PI3K-pathway / PIK3CA-altered localised disease, supporting biomarker-restricted consideration and rigorous prospective validation. In metastatic CRC, existing signals are largely non-randomised and confounded, precluding anticancer-motivated prescribing outside prospective studies. Aspirin in CRC should be targeted rather than routine. The proposed clinical algorithm integrates setting, absolute risk, PI3K/PIK3CA status, bleeding/antithrombotic context, and expected time horizon to maximise net benefit while reducing avoidable harm, and to prioritise trial enrolment where equipoise persists.
BACKGROUND:Routine brain imaging screening (BIS) in patients with metastatic breast cancer (BC) without neurological symptoms is currently not recommended, as no survival/quality-of-life improvements have been demonstrated. We aimed to examine physicians and patients' attitudes and perceptions toward BIS. METHODS:International cross-sectional online survey for patients and physicians, distributed from May 2023 to February 2024. Patients with BC diagnosis were deemed eligible for patients' survey completion and BC-treating physicians were invited to fill the physicians' questionnaire. RESULTS:A total of 529 physicians from 50 countries (80 % European) responded, mostly medical oncologists (70 %) working in academic hospitals (53 %). Most physicians request BIS (65 %), mainly when extracranial progression occurs, especially for HER2+ and triple negative BC (TNBC). Among physicians never performing BIS (35 %), 91 % would in case of proved clinical benefit. A total of 545 patients from 14 European countries completed the questionnaire. Median age was 50 years, 86 % had metastatic BC, 51 % hormone receptor-positive (HR+)/HER2-negative, 30 % HER2-positive (HER2+) and 19 % TNBC. BM were diagnosed in 11.5 % patients with metastatic BC. 85 % patients would like to undergo BIS, especially younger ones (p = 0.02) and with HR-disease (p = 0.03), despite the uncertain clinical benefit. Notably, 91 % of patients would like to receive information regarding BM, while only 13 % of physicians routinely address the issue. CONCLUSIONS:These results underline the willingness of patients to know more about the prospects of BM development, in contrast to the lack of routine discussion of this topic by physicians. Further investigation is warranted to demonstrate the clinical utility of routine BIS.
Platinum and taxane chemotherapy is associated with the risk of hypersensitivity reactions (HSRs), which may require switching to less effective treatments. Desensitization to platinum and taxane HSRs can be used to complete chemotherapy according to the standard regimen. Therefore, we aimed to investigate the current management of HSRs to platinum and/or taxane chemotherapy in patients with gynecologic cancers. We conducted an online cross-sectional survey among gynecological and medical oncologists consisting of 33 questions. A total of 144 respondents completed the survey, and 133 respondents were included in the final analysis. Most participants were gynecologic oncologists (43.6%) and medical oncologists (33.8%), and 77.4% (n = 103) were involved in chemotherapy treatment. More than 73% of participants experienced >5 HSRs to platinum and taxane per year. Premedication and a new attempt with platinum or taxane chemotherapy were used in 84.8% and 92.5% of Grade 1–2 HSRs to platinum and taxane, respectively. In contrast, desensitization was used in 49.4% and 41.8% of Grade 3–4 HSRs to platinum and taxane, respectively. Most participants strongly emphasized the need to standardize the management of platinum and taxane HSRs in gynecologic cancer. Our study showed that HSRs in gynecologic cancer are common, but management is variable and the use of desensitization is low. In addition, the need for guidance on the management of platinum- and taxane-induced HSRs in gynecologic cancer was highlighted.
e13662 Background: The integration of digital technology has significantly transformed clinical practice, especially in oncology. ONCOassist, an application that integrates content from the NCCN Templates offers professionals quick and up-to-date access to information crucial for cancer treatment. Methods: To analyze and determine whether factors such as the Human Development Index (HDI) or Gross National Income (GNI) influence the adoption of the NCCN Templates, an in-application survey was distributed to health professionals who accessed NCCN content. The survey asked two questions: (1) would respondents use a specific NCCN protocol for a current patient (2) if not, the reasons why they did not utilize it. Finally, how the acceptance of the NCCN Templates was related to HDI or GNI and specialty was analyzed. Results: The survey was responded to by 2522 health professionals from 122 countries. The NCCN Templates were widely accepted (80%) across different geographic and economic contexts, indicating their global applicability and trust. Generally, GNI is categorized by high income, upper middle income, lower middle income, and low income, while HDI includes very high, high, medium, and low income. For the lowest levels of GNI and HDI, there were few responses (1% by GNI and 8% by HDI), and for purposes of this abstract, they were combined with the next highest level, lower middle income, and medium HDI. Using GNI, the rates of using the NCCN Templates in countries with high income (72%) were lower than those with upper-middle income (83%) and lower middle/low income (86%). In a similar inverse tendency, based on HDI, the utilization rates of the NCCN Templates were 70%, 83%, and 86% in countries with very-high, high, and medium/low HDI, respectively. When stratified by specialty, the absolute numbers showed that most respondents belonged to the field of the breast (36%), followed by gastrointestinal (28%) and lung cancers (10%). However, when we analyzed percentages, we observed that the decision-making process regarding protocol adoption was consistent among specialties despite the variance in specialist population size. When we analyzed the reasons for not utilizing NCCN Templates, “Other reasons” was the primary response (51%). Among the remaining answers, using specific local protocols was the primary reason (47% of the remaining), followed by the lack of drug availability (18% of the remaining) and reimbursement issues (18% of the remaining). Conclusions: There was a general acceptance of the NCCN Templates among healthcare professionals globally. The willingness to utilize NCCN Templates content was not strongly determined by the HDI or GNI, suggesting a perception of their utility beyond economic barriers. The protocol has the potential to be used in daily clinical practice, improving decision-making.
Background Access to novel combination therapies for multiple myeloma (MM) remains limited in Europe partly due to health technology assessment (HTA) processes and formal reimbursement mechanisms.1,2 To address issues of access barriers, Sanofi convened OncoCollective (OC), a multi-disciplinary stakeholder collaborative of global experts, to explore solutions. Methodology The OC Novel Combination Therapy Working Group developed an illustrative case study to map two hypothetical patient journeys in a specific European country (Sweden), a journey that faces barriers to receiving novel combination treatment for MM versus one with fewer barriers.3−5 The group reviewed existing outcomes data to estimate the potential impact on progression-free survival (PFS) to explore if improved access to novel combination therapies could significantly improve patient outcomes and quality of life.3−5 Results The illustrative patient journeys demonstrated that an optimized therapeutic course with few barriers to combination therapy enabled more advanced care and extended PFS across multiple lines of therapy. In contrast, a journey facing limited access to combination therapy could result in suboptimal treatment sequencing and shorter PFS. Conclusion These findings highlight that current healthcare systems may not be equipped to adequately handle the value of combination therapies, and different reimbursement requirements may create inequalities in MM across countries or regions within the same country. Solutions proposed by the OC include a) advocating for mandatory national guidelines, b) encouraging real-world evidence collection and methodologically sound modelling studies, c) appealing for healthcare funding as an investment, and d) working towards a holistic vision of clinical benefit leading to meaningful improvements in outcomes for all patients with MM. Introduction The addition of taxanes to anthracycline-based chemotherapy is considered standard of care in the treatment of breast cancer. However, there are insufficient data regarding the safety of taxanes during pregnancy. The aim of this study was to describe the incidence of obstetric and neonatal adverse events associated with the use of taxane-containing chemotherapy regimens for the treatment of breast cancer during pregnancy.Methods This is a multicenter, international cohort study of breast cancer patients treated with taxanes during pregnancy. A descriptive analysis was undertaken to synthetize available data.Results A total of 103 patients were included, most of whom were treated with paclitaxel and anthracyclines given in sequence during gestation (90.1%). The median gestational age at taxane initiation was 28 weeks (range = 12-37 weeks). Grade 3-4 adverse events were reported in 7 of 103 (6.8%) patients. The most common reported obstetric complications were intrauterine growth restriction (n = 8 of 94, 8.5%) and preterm premature rupture of membranes (n = 5 of 94, 5.3%). The live birth rate was 92 of 94 (97.9%), and the median gestational age at delivery was 37 weeks (range = 32-40 weeks). Admission to an intensive care unit was reported in 14 of 88 (15.9%) neonates, and 17 of 70 (24.3%) live births resulted in small for gestational age neonates. Congenital malformations were reported in 2 of 93 (2.2%).Conclusion Obstetric and neonatal outcomes after taxane exposure during pregnancy were generally favorable and did not seem to differ from those reported in the literature with standard anthracycline-based regimens. This study supports the use of taxanes during gestation when clinically indicated.
Introduction/Background Exposure to platinum and taxane-based chemotherapy is associated with the risk of developing hypersensitivity reactions (HSRs), which may necessitate the use of less effective treatments to avoid anaphylaxis. Desensitisation to platinum and taxane HSRs can be used to complete chemotherapy according to the standard regimen; therefore, this study investigated the current management of HSRs to platinum and/or taxane chemotherapy in patients with gynaecological cancers. Methodology We conducted a cross-sectional survey among gynaecological and medical oncologist consisting of 33 questions. SurveyMonkey software was used to create and distribute the questionnaire. The survey was posted online on the ENYGO and Oncoalert social media channels and data were collected from 1) members of ENYGO and ESGO and 2)ESGO/ENYGO followers. Results A total of 144 respondents completed the survey and 133 respondents were included in the final analysis. The gender of the participants was balanced with 54.9% female and 45.1% male with an average age of 38 years. The majority of participants were gynaecological oncologists (43.6%) and medical oncologists (33.8%) working mainly in a university hospital (57.1%) and 77.4%(n=103) were involved in chemotherapy treatment. Of these, 79 participants administered platinum and 67 participants taxane-based chemotherapy. The majority of participants experienced more than 5 HSRs to platinum and taxane per year (73.4% and 73.1%, respectively). Pre-medication with antihistamines/steroids and a new attempt at standard infusion with platinum or taxane chemotherapy were used in 84.8% and 92.5% of Grade 1–2 HSRs to platinum and taxane, respectively. In contrast, tolerance induction was used in 49.4% and 41.8% of Grade 3–4 HSRs to platinum and taxane, respectively. Tolerance induction was mainly performed by medical oncologists,allergists and gynaecological oncologists. Conclusion The majority of participants strongly emphasised the need to standardise the management of platinum and taxane HSR in gynaecological cancer and to develop international guidelines, regardless of their involvement in chemotherapy treatment. BACKGROUND:Immune checkpoint-inhibitors (ICIs) are changing outcomes in different cancer settings, notably for patients with non-small-cell lung cancer (NSCLC). There are, however, still important gaps of evidence for clinical practice when using these novel treatments. In this study, we assessed physicians' opinion and experience on challenges for clinical practice with ICIs monotherapy in NSCLC.METHODS:A survey was conducted on experienced physicians treating patients with NSCLC with ICIs. Two rounds of pilot tests were carried out for validation among a group of experts. Topics under analysis were in relation to treatment of elderly populations, performance status, brain metastases, use of steroids or antibiotics, the effects of gut microbiome, autoimmune diseases, human immunodeficiency virus infection, solid organ transplants, use of anti-programmed cell death protein 1 versus anti-programmed death-ligand 1 drugs, atypical tumour responses, predictors of response, duration of treatment and a final open question on additional relevant challenges.RESULTS:Two hundred and twenty-one answers were collected, including 106 (48%) valid answers from experts for final analysis (physicians who have treated at least 20 patients with NSCLC with ICIs). The vast majority agreed that the selected topics in this study are important challenges ahead and more evidence is needed. Moreover, predictors of response, treating brain metastasis, shorter duration of treatment, the effects of gut microbiome and concomitant use of steroids were voted the most important topics to be further addressed in prospective clinical research.CONCLUSIONS:This survey contributed to understanding which are the main challenges for clinical practice with ICIs monotherapy in NSCLC. It can also contribute to guide further clinical research, considering the opinions and experience of those who regularly treat NSCLC patients with ICIs.
Introduction/Background Social media is a strong tool to raise awareness and share scientific output in gynaecologic oncology. Social media posts vary in terms of output (visual, written, mixed) and target audience (users' age, language, patient versus caretakers versus healthcare professionals and geographic region). Cervical cancer (CxCa) is the most common gynecologic malignancy affecting women in low- and middle-income countries. However, literature regarding the role of Instagram in CxCa is lacking. In this study, we aim to investigate the post content pertaining to CxCa on Instagram. Methodology We retrieved the volume of posts related to the hashtag #cervicalcancer using Instagram's search feature October 2010 and May 5th, 2023. We recorded and described the number of posts in regard to content, likes, and users. Using the Instagram algorithm, we examined the top two posts related to the hashtags described. We read the posts qualitatively and interpreted them to describe their content. Results Our query resulted in 248,707 posts. For the top two posts identified, the focus was on the 'battle' and patients' lives with cancer. Figure 1 is based on the top Instagram post (likes=503). It is a word cloud of comments; it shows a mix of sadness and caring with a focus on the need for awareness of the disease. We defined it as 'story of battle against #cervicalcancer and need of awareness for prompt diagnosis'. Conclusion We identified that the main focus pertaining to CxCa on Instagram is the patient and family perspective of living with and battling this disease. Disclosures None
The cyclin-dependent kinase 4 and 6 inhibitors (CDK4/6i) have become the standard of care for hormone receptor-positive (HR + ) and human epidermal growth factor receptor 2-negative (HER2-) metastatic breast cancer, improving survival outcomes compared to endocrine therapy alone. Abemaciclib and ribociclib, in combination with endocrine therapy, have demonstrated significant benefits in invasive disease-free survival for high-risk HR+/HER2-early breast cancer patients. Each CDK4/6i-palbociclib, ribociclib, and abemaci-clib-exhibits distinct toxicity profiles. Radiation therapy (RT) can be delivered with a palliative or ablative intent, particularly using stereotactic body radiation therapy for oligometastatic or oligoprogressive disease. However, pivotal randomized trials lack information on concomitant CDK4/6i and RT, and existing preclinical and clinical data on the potential combined toxicities are limited and conflicting. As part of a broader effort to establish international consensus recommendations for integrating RT and targeted agents in breast cancer treatment, we conducted a systematic review and meta-analysis to evaluate the safety profile of combining CDK4/6i with palliative and ablative RT in both metastatic and early breast cancer settings.
Introduction/Background Instagram is a powerful platform to raise awareness of various health issues. In recent years, this has been observed for gynaecological cancer awareness topics. Due to its visual nature and large outreach potential, Instagram provides a platform for educational exchange, health promotion, and building communities. Nevertheless, evidence of its use and its impact in raising awareness for vaginal cancer is still lacking. We aimed to identify the use of Instagram to raise awareness for vaginal cancer and the volume and type of posts that are used in relation to this relevant albeit rare gynaecological cancer. Methodology We identified the volume of posts related to specific hashtags by using the Instagram search feature. We retrieved results from the following hashtags from October 2010 to May 5th, 2023: #vaginalcancer and #vaginalcancerawareness. We recorded and described the number of posts. Using the Instagram algorithm, we examined the top two posts related to the hashtags described. These were then thematically analysed to identify content. If a post received replies, these were analysed using a word cloud to identify the keywords being used. Results We identified 11,832 and 293 posts on Instagram relating to #vaginalcancer and #vaginalcancerawareness, respectively. The main theme of the posts was eliminating stigma and embarrassment and promoting donations for raising awareness (table 1). Conclusion Instagram is reported to have over one billion active users each month. It is increasingly used to raise health awareness, and there is an increasing trend within gynaecological cancers. Women are using Instagram to share their experiences and build communities. However, considering the enormous outreach this platform provides, its use relating to vaginal cancer remains suboptimal. As social media platforms also gain attention for gynaecologic cancer awareness, it is important that the medical community is aware of this and becomes active in sharing relevant, evidence-based information and updates. Introduction/Background Human Papillomavirus (HPV) vaccination plays a major role in prevention of HPV-related malignancies. Most importantly, related to cervical cancer elimination initiatives. Social media channels may be an effective tool in spreading information about preventive measures. The aim of this study was to assess the type of content published on Instagram regarding HPV vaccinations under the hashtag #hpvvaccination. Methodology We evaluated the use of Instagram's search feature to identify most popular posts related to the hashtag #HPVvaccination. The data were collected between the launch of Instagram (October 2010) and May 5th, 2023. We recorded and described the number of posts. Using the Instagram algorithm, we examined the top posts related to the hashtag #HPVvaccination. These were then thematically analysed to identify content of the post. popularity was based on several parameters, including the number of likes and comments of the post. Results Our query resulted in a total of 3,211 posts, including #HPVvaccination. The most popular post under the hashtag #HPVvaccination was related to HPV awareness post by the Eve Appeal highlighting how HPV can affect men and women. This post received 632 likes and 24 comments. The second most popular post with was published by the Singapore Cancer Society and included a picture of a roadshow providing information on possible methods of obtaining free HPV vaccinations. This post received 40 likes and no comments. Conclusion The most popular posts with the #hpvvaccination hashtag were published by a charity funding research and awareness regarding early diagnosis and prevention of HPV-related cancer and by a cancer society. Both posts were prepared in order to increase awareness about prevention of the disease. Disclosures None
e13075 Background: Over the past years, the treatment landscape of breast cancer (BC) has significantly evolved. Several new anticancer agents entered clinical use. Among them CDK4/6i have revolutionised both curative and advanced BC scenarios. The availability of new drugs raises the crucial question on how to integrate them with local and regional treatments. Strong levels of evidence on the safety profile of CDK4/6i in combination with different RT techniques or schedules are currently missing. We aim to systematically review and summarise the published data on the safety profile of palliative and ablative RT combined with CDK4/6i in early and metastatic BC settings. Methods: A literature search (PUBMED/MedLine, Embase, and Cochrane databases) was conducted to identify original studies on the safety profile of the combination of CDK4/6i with RT. This systematic review and meta-analysis project is part of the ESTRO Guidelines Committee consensus recommendations on integration of RT with targeted treatments for breast cancer, an initiative developed by a multidisciplinary Core Group and Expert Panel of medical, clinical, radiation oncologists, preclinical specialists, and patient advocates. Results: Eleven articles finally met all eligible criteria and were included in the meta-analysis. Data could not be collected in the adjuvant setting due to lack of data in the current literature. Pooling data of the included studies show a moderate incidence of hematological toxicities grade 3+ (21.0 %) and a very low-grade 3+ non-hematological toxicity were observed. Conclusions: The safety profile of CK4/6i and RT combination is acceptable. However, international consensus guidelines regarding the safety issue of CDK4/6i and RT combination for breast cancer are warranted.
Social media platforms like Facebook and Twitter have revolutionized rare disease research and have catalyzed the connection among patients with rare cancers.A new study from the Germ Cell Tumor Survivor Sisters Facebook group provides evidence of the utility of naturally forming patient groups assisting researchers with developing the evidence base for care and supporting those living with the disease.Such studies are the first steps in rare disease research powered by empowered patients by solving the zebra rare disease puzzle through social media.
OBJECTIVE:The primary objective was to reveal the impact of social media ambassadors and the collaboration between the European Society of Gynaecological Oncology (ESGO) and the OncoAlert Network on Twitter during the ESGO 2022 Congress by comparing it with the ESGO 2021 Congress. We also aimed to share our experience on how to organize a social media ambassador program and evaluate the potential benefits for the society and the ambassadors.METHODS:We defined the impact as promoting the congress, sharing the knowledge, change in follower count, and change in tweet, retweet, and reply counts. We used the Academic Track Twitter Application Programming Interface to retrieve data from ESGO 2021 and ESGO 2022. We used the keywords of ESGO2021 and ESGO2022 to retrieve data for each of the conferences. The time range in our study captured interactions from before, during, and after conferences. We collected the ambassadors', ESGO's, and the European Network of Young Gynae Oncologists' (ENYGO's) follower data on Twitter from November 2021 to November 2022 for comparative analysis.RESULTS:There was a 7.23-fold increase in the use of the official congress hashtag in 2022 compared with 2021. Compared with #ESGO2021 data, the main interventions of the Social Media Ambassadors and OncoAlert partnership determined 7.79-, 17.36-, 5.50-, 10.58-, and 8.50-fold increases with #ESGO2022 data in the mentions, mentions in retweet, tweet, retweet, and replies, respectively. Similarly, all other most commonly used hashtags in the top 10 list indicated a range from 2.56- to 7.00-fold increase. Compared to the ESGO 2021 congress month, ESGO and the majority (83.3%, n=5) of ambassadors gained more followers during ESGO 2022 congress month.CONCLUSIONS:An official social media ambassadors program and collaboration with influential accounts in the field of interest are beneficial for congress-related engagement on a social media platform (Twitter). Individuals participating in the program can also benefit from gaining higher visibility among specific audience.
Introduction/Background Social media represents a strong instrument to share and divulge information targeting people of different ages and backgrounds. Little is known regarding how social media may impact patients' understanding and acceptance of their disease, large-scale screening and health care improvement. Due to the rising incidence of endometrial cancer globally, we aimed to understand the extent of posts regarding this disease on Instagram, one of the most used social media platforms. Methodology We collected and analysed the number of Instagram posts for the following hashtags: #uterinecancer, #endometrialcancer, and #wombcancer from the launch of Instagram (October 2010) to May 5th, 2023. Moreover, using the Instagram algorithm, we highlighted the two top posts for each used hashtag. Results We identified 83.670 posts related to endometrial cancer. More specifically, 48.597 posts for #uterinecancer, 28.234 posts for #endometrialcancer and 6.839 posts for #wombcancer. The most rated post on endometrial cancer belonged to a scientific research alliance providing an informative description of the disease, whereas the second top post consisted of a cancer-related nutritional advices' advertisement for patients. Figure 1 summarises the most frequent words gathered from the top post comments. It shows that endometrial cancer posts are usually linked to everyday life factors according to patients' point of view: hormonal changes, stressful life, hormonal therapy, and childbearing. Conclusion In conclusion, Instagram may be considered a crucial social media platform for patients with endometrial cancer to obtain information regarding their disease. Disclosures None
Introduction/Background Social media is an increasingly powerful tool to raise awareness of gynaecological cancers and share scientific output in the field of gynaecologic oncology. Recent studies on Twitter revealed its impact on raising awareness regarding gynaecological cancers and dissemination of scientific knowledge. Through the use of images, powerful patient stories, and varied infographics, charities and the wider public and patients are raising awareness and support for gynaecological cancers on Instagram. However, the literature on the role of Instagram in gynaecological cancers is lacking. We aim to investigate the volume and content of posts regarding ovarian cancer (OC) on Instagram. Methodology We retrieved the results on #ovariancancer Instagram query between October 2010 and May 5, 2023. The study made use of Instagram's search feature to identify posts related to the aforementioned hashtag and this was recorded. Instagram algorithm determines and shows a number of 'top' posts. Our study examined the top five posts related to the hashtag in question. These were thematically analysed to identify content of the post. Results Our query resulted in 375,072 posts. Among the top five posts during the study period, the likes ranged from 2838 to 48. Figure 1 is based on the top Instagram post, which included 26 comments. Moreover, the number of comments among the top five posts ranged from 4 to 202. The descriptions of top posts were awareness, patient education by a patient with OC, a patient sharing her struggles, OC treatment of side effects, and promotion of OC research with a patient story, respectively. It is a word cloud of comments; it shows positivity, love, praying, and hope within the comments section. Conclusion Instagram is a platform mainly used by patients and advocates to raise awareness regarding OC. There is a potential for medical professionals and societies to increase presence on Instagram. Disclosures None
SUMMARY:Social media (SoMe) platforms have the ability to strengthen the oncology community, leading to intellectual connections that with time develop into friendships. SoMe has immense potential in all areas of medicine, and SoMe in oncology is proof of this, raising awareness about clinical trials, promoting cancer prevention techniques, amplifying oncology information, enabling diverse viewpoints into conversations, as well as educating colleagues regardless of geography.
