The Annual Report on the Results of Treatment in Carcinoma of the Uterus, Vagina, and Ovary has many years' experience in the calculation of therapeutic results in the treatment of malignant tumors of the female pelvis. Many important factors have to be taken into consideration. Much weight has been laid down on the setting up of exact definitions of the classification and staging of malignant tumors of the female genital tract. Collaboration in the Annual Report has been of great educational value in the keeping of correct and comparable therapeutic statistics.

Die Entwicklung der Therapie bei einer Krankheit, wie sie auch sei. erfordert die Möglichkeit, die erreichten Erfolge kritisch beurteilen zu können. Die Internationale Krebsgesellschaft betont die Bedeutung einer einheitlichen Terminologie, einer exakten Klassifikation und einer klinischen Stadieneinteilung vor Einleitung der Therapie des einzelnen malignen Neoplasmas, nicht nur um Ergebnisse verschiedener Krankenhäuser beurteilen zu können, sondern auch um die Behandlung des einzelnen Falles zu projektieren. Die internationale Stadieneinteilung und die T.N.M.-Klassifikation des Collumcarcinoms hat sich in dieser Beziehung als vorteilhaft erwiesen. Die Unterteilung des Stadium I in IA und IB wurde von manchen Kollegen kritisiert, aber ich glaube, daß diese Einteilung sowohl von statistischem wie von therapeutischem Gesichtspunkt von Belang ist. Niemand will behaupten, daß es möglich ist, eine scharfe Grenze zwischen IA und IB ziehen zu können, aber die Erfahrung lehrt auch, daß manchmal die Beurteilung, ob das atypische Epithel in der Cervix invasiv oder nicht invasiv wächst, für einen Pathologen mit großer Erfahrung schwer sein mag. Die routinemäßige Anwendung der exfoliativen Cytologie und der Kolposkopie als Suchmethoden hat die Anzahl der diagnostizierten Fälle von präinvasivem und frühinvasivem Collumcarcinom in den letzten Jahren enorm gesteigert.

Evaluation of treatment methods is possible only by comparing therapeutic results. Consequently, it is of vital importance that international agreement is reached on nomenclature and on a uniform staging of all cases of ovarian cancer. The purpose of clinical staging is to facilitate the accurate, concise description of the apparent extent of the neoplasm.
Ovarian cancer is not an entity but a group of diseases. Studies of the results of treatment must be based on homogenous groups of tumours and not on mixtures of histologically and biologically different tumour types. Furthermore, there still exists a considerable degree of confusion as to which turnouts should be considered as true carcinomas. It is the lack of generally accepted definitions of the various histological forms that has resulted in the extreme differences in long-term results given by various authors. The importance of establishing a standardized, internationally accepted classification of ovarian turnouts, so that the results of different clinics may be accurately compared, is obvious.
Malignant neoplasms of the ovary are rather common. In 1958, for instance, 890 new cases of invasive carcinoma of the cervix, and 765 of ovarian cancer were diagnosed in Sweden. We all share the opinion that, in general, the prognosis is poor. Unfortunately, symptoms of ovarian carcinoma are often absent and these neoplasms are frequently not discovered until the growth has extended to surrounding organs or has metastasized. The treatment becomes progressively less effective with increasing anatomic extent of the disease.
Surgery is the keystone in the treatment of ovarian carcinoma. In his brilliant way Parsons (1962) has discussed the surgical approach. Brunschwig has described the value of superradical operations in another paper in this book. The question remains: is simple oophorectomy ever proper therapy?

Bei dem Ermessen der Wirkung verschiedener Behandlungsmethoden wäre es wünschenswert, die Energieabsorption im bestrahlten Gewebe zu beurteilen. Deshalb streben wir danach die Dosis in rad anzugeben.
A description has been given in detail of 3,484 cases of cervical carcinoma treated by intracavitary radium and orthovoltage x-ray therapy at the Radiumhemmet during the years 1949 through 1957. A 5 year relative apparent recovery rate of 54.2 per cent has been attained.
