Décrire les caractéristiques sociodémographiques, cliniques et paracliniques des patients ayant un œdème papillaire unilatéral consultant aux urgences ophtalmologiques.Il s’agit d’une étude rétrospective observationnelle des patients avec un œdème papillaire unilatéral, consultant aux urgences ophtalmologiques du centre hospitalo-universitaire de Bordeaux, entre le 1er octobre 2008 et le 31 octobre 2009.Nous avons inclus 52 patients avec un œdème papillaire unilatéral. On comptait 61,5 % de femmes (n = 32) et 38,5 % d’hommes (n = 20). La moyenne d’âge était de 53,5 ans (σ = 22,9 ans), avec un âge plus élevé pour les hommes que pour les femmes (61,1 ans vs 48,7 ans avec p = 0,05). Le diagnostic retenu était une neuropathie optique ischémique antérieure aiguë pour 27 patients, une neuropathie optique inflammatoire pour 17 patients, une hypertension intracrânienne pour trois patients et un « pseudo-œdème papillaire » pour deux patients. Quant aux trois patients restants, le diagnostic étiologique précis de l’œdème papillaire n’a pas été fait.Cette étude a évalué la prise en charge des patients atteints d’un œdème papillaire unilatéral consultant aux urgences ophtalmologiques. Elle aurait ainsi aidé à l’amélioration de nos pratiques.To study the epidemiologic and clinical characteristics of patients with unilateral optic disc edema diagnosed in the emergency room (ER).Retrospective observational study of all cases of unilateral optic disc edema seen in the ophthalmology ER of Bordeaux University Hospital, France, between October 1, 2008 and October 31, 2009.Fifty-two patients with unilateral optic disc edema were studied, including 61.5% women (n = 32) and 38.5% men (n = 20). Mean age was 53.5 years (±22.9). Male patients were older than female (61.1 years vs 48.7 years, P = 0.05). Final diagnosis was anterior ischemic optic neuropathy (27 cases), papillitis (17 cases), papilledema (three cases), and pseudoedema (two cases). No etiologies were identified for the remaining three patients.Our study discusses the diagnostic approach for patients presenting with unilateral optic disc edema; this discussion may help to improve clinical practice.
La chirurgie abdominale altère la fonction respiratoire, et les complications pulmonaires sont une des premières causes de morbidité et de mortalité postopératoires. Après une analyse de l'incidence et des facteurs de risque des complications respiratoires, cette revue de la littérature examine les moyens de leur prévention et de leur traitement. L'évaluation préopératoire est surtout clinique et les épreuves fonctionnelles respiratoires (EFR) ne sont pas indispensables. La prédiction des complications est difficile. Certains risques comme l'âge ou le type de chirurgie sont inévitables. Souvent le temps de préparation est trop court pour améliorer l'état nutritionnel. Mais les désordres métaboliques doivent être corrigés et la consommation de tabac arrêtée 8 semaines avant l'intervention pour obtenir un réel bénéfice. Pour les patients atteints de bronchopneumopathie chronique obstructive (BPCO), l'évaluation sera faite, si nécessaire, par le pneumologue, le kinésithérapeute, le chirurgien et l'anesthésiste. En l'absence d'examen préalable, des EFR avec examen des gaz du sang seront faites pour quantifier le trouble et évaluer les effets d'un traitement. Un patient ne devrait pas être récusé sur des critères spirométriques. La préparation, débutée au moins 24 à 48 heures avant l'intervention, comprend des antibiotiques, des bêta-2-agonistes, des anticholinergiques en aérosol, voire de la théophylline et de la kinésithérapie respiratoire. Le traitement doit continuer 3 à 5 jours en phase postopératoire. En règle générale, l'anesthésie entraîne des complications précoces, et la chirurgie des complications tardives. Les interventions par voie de cœlioscopie permettent de diminuer la morbidité postopératoire, surtout chez les patients sans antécédent respiratoire. Il n'a pas été démontré que l'anesthésie locorégionale, ou une technique particulière d'analgésie postopératoire étaient capables de diminuer les complications respiratoires. Après l'intervention, l'oxygène améliore la SaO2 mais ne prévient pas les épisodes de désaturation dus aux apnées obstructives. Les différentes techniques de kinésithérapie sont équivalentes, mais la ventilation artificielle à pression positive intermittente donne lieu à de sérieux effets secondaires et doit être abandonnée. La kinésithérapie et la spirométrie incitative sont simples, non invasives et indiquées pour traiter l'encombrement bronchique et les atélectasies. La respiration en pression positive continue est efficace contre les atélectasies. Pour les patients atteints de BPCO (stade II ou III), une prise en charge en milieu de soins intensifs et une ventilation mécanique prolongée sont parfois nécessaires.Abdominal surgery, especially upper abdominal surgical procedures are known to adversely affect pulmonary function. Pulmonary complications are the most frequent cause of postoperative morbidity and mortality. This review article aimed to analyse the incidence and risk factors for postoperative pulmonary morbidity and their prevention. The most important means for preoperative assessment is the clinical examination; pulmonary function tests (spirometry) are not reliably predictive for postoperative pulmonary complications. Age, type of surgical procedure, smoking and nutritional state have all been identified as potential predictors for postoperative complications. However, usually there is not enough preoperative time available to obtain beneficial effects of stopping smoking and improvement of nutritional state. In patients with COPD, a preoperative multidisciplinary evaluation including the primary care physician, pulmonologist/intensivist, anesthesiologist and surgeon is required. Consensus as to preoperative physiologic state, therapeutic preparation, and postoperative management is essential. Simple spirometry and arterial blood gas analysis are indicated in patients exhibiting symptoms of obstructive airway disease. There are no values that contra-indicate an essential surgical procedure. Smoking should stop at least 8 weeks preoperatively. Preoperative therapy for elective surgery with antibiotics, beta2-agonist, or anticholinergic bronchodilator aerosols, as well as training in cough and lung expansion techniques should begin at least 24 to 48 hours preoperatively. Postoperative therapy should be continued for 3 to 5 days. Usually, anaesthesia is responsible for early complications, whereas surgical procedures are often associated with delayed morbidity. Laparoscopic procedures are recommended, as postoperative morbidity and hospital stay seem reduced in patients without COPD. Regional anaesthesia is given as having less adverse effects on pulmonary function than general anaesthesia. However, for unknown reasons these benefits are not associated with a decrease in postoperative respiratory complications. Moreover, the quality or the type of postoperative analgesia does not influence postoperative respiratory morbidity. Postoperatively, oxygen administration increases SaO2, but cannot abolish desaturation due to obstructive apnea. The various techniques of physiotherapy (chest physiotherapy, incentive spirometry, continuous positive airway pressure breathing) seem to be equivalent in efficacy; but intermittent positive pressure breathing has no advantages, compared with the other treatments and could even be deleterious. Chest physiotherapy and incentive spirometry are the most practical methods available for decreasing secretion contents of airways, whereas continuous positive airway pressure breathing is efficient on atelectasis. In stage II or III COPD patients, admission in a intensive therapy unit and prolonged mechanical ventilation may be required.
INTRODUCTION:Severe persistent asthma represents a major and costly public health issue. There is evidence that long-term treatment with omalizumab might have disease-modifying activity but data on the consequences of discontinuing treatment after a positive response are limited. The purpose of this study was to investigate-in real-life prescribing conditions-what happens when omalizumab is discontinued in patients with severe, persistent allergic asthma who have responded well to omalizumab treatment. METHODS:An observational, descriptive, cross-sectional, retrospective study to establish the time to loss of asthma control after the discontinuation of courses of omalizumab treatment of varying duration. RESULTS:24 lung specialists reviewed data from 61 responder patients who had discontinued omalizumab after a mean duration of 22.7 ± 13.1 [range: 2.5; 59.5] months of treatment. Loss of asthma control was documented in 34 patients (55.7%) with a median interval between discontinuation and loss of control of 13.0 months (mean 20.4 ± 2.6 [95% CI: 8.3-28.1]). No correlation was detected between time to loss of control and duration of treatment, although control tended to be sustained for longer in patients whose response had been classified as "excellent" as opposed to "good" (median: 17.0 vs. 12.8 months; NS). DISCUSSION:The discontinuation of omalizumab was not associated with any rebound effect or exacerbation of the disease, and control was sustained throughout the follow-up period of at least 6 months in nearly half of all patients, including all of those who had been treated for 3.5 years or more. After the reintroduction of omalizumab, 4 out of 20 patients did not respond again.
PD-LID, a complication of L-Dopa treatment, is associated with inability to perform daily living activities, reduction in quality of life and increase in health care costs. Limited data are currently available on the burden of this disease. LIDIA describes medical and non-medical resource utilization and economic impact of PD-LID patients from payer and society perspective according to LID severity: mild (mAIMS score<8), moderate (mAIMS [8-12]) and severe (mAIMS>12) patients. This observational, prospective, longitudinal, multicentre 6 month-study was conducted by French neurologists specialized in Parkinson’s disease (PD). PD patients taking L-dopa for at least 3 years were included, with PD-LID present for more than 3 months and for more than 25% of the time. Thirty-three neurologists included 186 patients (mean 68 years old, 52% women) with mean mAIMS score at 10. PD-LID patients were mainly followed by their neurologists (96%) and general practitioner (GP) (84%) with more non-medical follow-up for severe patients. Almost 60% had at least one biological or radiological examination, only 31% required transport. All patients received L-Dopa (including 32% long-acting), and 69% dopaminergic agonists. Only 21% had at least one hospitalization and over 40% needed home layout and mobility equipment. Most patients (70%) were assisted by a caregiver (24h/week), 43% required external assistance (6h/week), 22% received an invalidity allowance. Total costs represent 35117€, 33640€ and 43739€ in respectively mild, moderate and severe patients (on average per patient/year). Non-medical costs (transportations, paid external help, caregivers time, mobility equipment, home layout) are the major cost drivers (80%), including caregiver help (70%), followed by medical (consultations, treatments, hospitalizations)(15%) and indirect costs (allowances and work stoppages)(5%). The LIDIA study confirms that in this disabling disease, the economic burden is mainly supported by the patient family, with the payer part being 23%. Economic burden is greater in patients with severe dyskinesia.
Le but de ce travail était d’étudier la couverture vaccinale grippale et pneumococcique des patients admis dans un SSR de pneumologie.Entre septembre 2010 et août 2011, nous avons mené une étude observationnelle prospective sur les patients admis dans notre établissement. La recherche d’une vaccination antigrippale et antipneumococcique était systématiquement effectuée grâce à un questionnaire standardisé.Sur 476 patients admis dans notre établissement, 246 avaient une BPCO, 175 avaient eu une intervention chirurgicale thoracique et 55 patients avaient une affection respiratoire chronique différente de la BPCO. L’âge moyen des patients était de 67 ans (60–76) et le sex-ratio de 1,6 (291 H et 185 F). Parmi la population cible de la vaccination antigrippale, la couverture vaccinale était de 73 %. La principale raison de l’absence de vaccination contre la grippe était un refus du patient (59 %). Parmi la population cible de la vaccination antipneumococcique, la couverture vaccinale était de 53 %. La principale raison de l’absence de vaccination contre le pneumocoque était l’absence de proposition de ce vaccin par un médecin (92,5 %).La couverture vaccinale était faible surtout celle contre le pneumocoque. Les SRR de pneumologie sont des sites stratégiques pour améliorer la couverture vaccinale.The aim of this work was to study the vaccination coverage against influenza and pneumococcus in patients admitted to a pulmonary care department.Between September 2010 and August 2011, we conducted a prospective observational study of patients admitted to our institution. A history of vaccination against influenza and pneumococcus was sought systematically using a standardized questionnaire.Of 476 patients admitted to the pulmonary service at our institution, 246 had COPD, 175 had undergone thoracic surgery and 55 had a chronic respiratory disease other than COPD. The average age of our patients was 67 years (60–76) and the sex-ratio was 1.6 (291 M and 185 F). Amongst the target population for influenza vaccination, coverage was 73%. The main reason for patients not to have been vaccinated against influenza was patient refusal or intolerance (59%). Amongst the target population for antipneumococcal vaccination, the coverage was 53%. The main reason for the lack of vaccination against pneumococcus was that no offer of vaccination had been made by a physician (92.5%).Vaccination coverage was low, in particular for pneumococcus. Pulmonary departments are strategic sites which could take action to systematically improve vaccination coverage.
Les troubles mnésiques, plus particulièrement ceux de la mémoire épisodique verbale (MEV), constituent un motif fréquent de plainte cognitive chez les patients atteints de sclérose en plaques (SEP). Outre la gêne occasionnée dans la vie quotidienne, ces troubles peuvent avoir un impact sur les activités professionnelles. L'évaluation neuropsychologique des patients doit donc comporter des tests explorant la MEV, de façon à décrire le degré d'atteinte des différents processus cognitifs contribuant à la MEV et, le cas échéant, pouvoir proposer des stratégies rééducatives adaptées. L'objectif de cet article est de proposer une revue critique de la littérature portant sur les capacités de MEV dans la SEP. Cette revue permettra de présenter les références conceptuelles et les caractéristiques psychométriques des principaux tests de MEV appliqués dans cette pathologie (épreuves isolées ou intégrées à des batteries cognitives plus globales développées pour la SEP). Dans un second temps, nous proposons un inventaire, par processus cognitifs, des travaux réalisés dans cette affection. Cette approche permettra d'aborder les limites de chaque conception et les éventuelles ambiguïtés terminologiques. Les contributions de ces travaux aux connaissances sur les troubles mnésiques de la SEP seront précisées, ainsi que l'impact des caractéristiques de la maladie (formes de SEP, durée d'évolution, EDSS).Memory impairment, especially verbal episodic memory (VEM), represents a common ground for cognitive complaint in patients with multiple sclerosis (MS). Beyond the difficulty caused in daily life, these deficits may impact on occupational activities. Neuropsychological assessment of these patients has to include VEM tests, to describe the level of dysfunction of the different processes contributing to VEM and, if required, to guide adapted cognitive rehabilitation. The objective of the present paper is to propose a critique review of the literature on VEM abilities in MS. This review will present the conceptual references and the psychometric characteristics of the main VEM tests applied in MS (isolated tests or included within more general batteries developed specifically for MS). In a second phase, we propose an inventory of work on MS presented as a function of the cognitive processes involved. This approach provides an approach to the limitations of each conception and possible terminological ambiguities. Contributions to knowledge of MS memory impairments will be clarified, as well as the impact of the disease characteristics (MS forms, disease duration, EDSS).
Patients with Alzheimer’s disease (AD) who deteriorate rapidly are likely to have a poorer prognosis. There is a clear need for a clinical assessment fool to defect, such a decline in newly diagnosed patients.
L'impossibilité d'atteindre la posologie optimale du traitement en raison d'une intolérance digestive limite l'efficacité des inhibiteurs de la cholinestérase (IChE). L'objectif principal de cette étude ouverte multicentrique, suivie 24 semaines, était de confirmer l'efficience clinique d'exelon transdermique en déterminant le pourcentage de patients pouvant atteindre et maintenir la posologie maximale de 9.5 mg/24h. Les patients présentant une MA probable (MMSE 10-26), étaient traités par exelon forme transdermique à la dose initiale de 4.6 mg/24h, de novo ou en substitution d'un traitement oral par IChE (en raison d'une insuffisance de bénéfice thérapeutique, d'une intolérance ou d'une impossibilité d'atteindre la dose optimale). Le MMSE, les 4-IADL, la mini-Zarit et la CGI étaient recueillis en fin d'étude ainsi qu'un questionnaire d'observance et d'acceptabilité du traitement pour les patients antérieurement traités par une forme orale. 218 patients (87 hommes,131 femmes) ayant une MA probable, d'âge moyen 78.7 ans et de MMS moyen 19.1, ont été inclus par 76 investigateurs.128 patients (58.7%) avaient bénéficié d'un traitement oral antérieur par IChE. 163 patients (74.8%) ont atteint la posologie optimale, et 154 d'entre eux (70.6%) l'ont maintenue au moins 8 semaines. 151 patients (72.6%) ont été considérés comme stabilisés ou améliorés (Impression Clinique du Changement/CGI) en fin étude, tandis que le score moyen du MMSE restait stable à 19.2 (+ 0.4 pour les patients "de novo"). Les événements indésirables les plus fréquents ont été les réactions cutanées (50 patients, 22.9%), les nausées (10 patients, 4.6%), les vomissements et les diarrhées (8 patients, 3.7%). 37 patients (16.9%) ont arrêté l'étude avant son terme pour effets indésirables dont 24 (11%) pour effets cutanés, 5 (2.3%) pour chutes, 3 (1.4%) pour vomissements et 1 (0.5%) pour nausées. Cette étude confirme que la bonne tolérance digestive d'exelon dispositif transdermique permet à plus des 2/3 des patients d'atteindre la posologie optimale de traitement alors que ce n'était le cas que pour au plus 40% d'entre eux dans des études similaires avec la forme orale.
Levodopa is the gold standard drug for the symptomatic control of Parkinson’s disease (PD). However, long‐term treatment with conventional formulations [levodopa and a dopa decarboxylase inhibitor (DDCI)], is associated with re‐emergence of symptoms because of wearing‐off and dyskinesia. Treatment with levodopa/DDCI and entacapone extends the half‐life of levodopa, avoiding deep troughs in levodopa plasma levels and providing more continuous delivery of levodopa to the brain. In this open‐label, retrospective, observational study we investigated the effects of levodopa/DDCI and entacapone therapy in 800 PD patients with motor fluctuations. Levodopa/DDCI and entacapone treatment was assessed as good/very good in improving motor fluctuations (64%) and activities of daily living (ADL; 62%). The therapeutic utility was considered to be good/very good in 70% of cases. Moreover, there was a reduction in levodopa dose in 20% of patients. Neurologists preferred levodopa/DDCI and entacapone compared with increasing levodopa dosage, dose‐fractionation or addition of a dopamine agonist (63%, 29% and 23% of patients respectively). Reasons included achieving more continuous dopaminergic stimulation (40%), reducing motor fluctuations (54%) and improving ADL (41%). This analysis reveals the preference of neurologists for levodopa/DDCI and entacapone over conventional levodopa‐modification strategies for the effective treatment of PD motor fluctuations in clinical practice.
