Introduction En raison du micro-environnement immunitaire spécifique du foie, la présence de métastases hépatiques pourrait diminuer l’efficacité des inhibiteurs de point de contrôle immunitaire (IPCIs). Les résultats de la littérature concernant l’efficacité de ces derniers chez les patients porteurs de carcinome bronchique non à petites cellules (CBNPC) avec métastases hépatiques ne sont pas unanimes. Méthodes Cette étude rétrospective bicentrique inclue des patients avec CBNPC stade IV, EGFR, ALK et ROS-1 sauvages, consécutivement pris en charge en 1ère ligne de traitement entre janvier 2019 et décembre 2023 par IPCI, seul ou en association à de la chimiothérapie, dans deux services d’oncologie thoracique en CHU. Les données ont été censurées au 1er mars 2024. Les caractéristiques clinicopathologiques initiales, le taux de réponse objective (TRO) systémique et hépatique, le taux de contrôle de la maladie (TCM), et les survies globales (SG) et sans progression (SSP) en vraie vie ont été comparées selon le statut métastatique hépatique. Résultats 212 patients ont été inclus, avec un âge moyen de 61,9 ans (SD±9,74), 66 % d’hommes, 76 % d’histologie non-épidermoïde et 26 (12 %) avec métastases hépatiques (MH+). La majorité des patients a bénéficié d’un traitement par IPCI+chimiothérapie (71 %), contre 29 % par IPCI seul. Les caractéristiques initiales des patients n’étaient pas différentes entre les MH+ et MH− à l’exception d’un nombre de sites métastatiques supérieur chez les MH+. Le TRO systémique n’était pas différent entre les deux groupes : MH+ 50 % vs MH− 57 %, p=0,5 ; de même que le TCM : MH+ 62 % vs MH −72 %, p=0,3. La médiane de SSP (3,0 mois vs 6,1 mois, HR 1,38, IC 95 0,87–2,18) et de SG (10,1 mois vs 16,0 mois, HR 1,32, IC 95 0,77–2,24) n’étaient pas différents entre les deux groupes. Chez les patients avec métastases hépatiques, le TRO des lésions hépatiques était inférieur (21 %) au TRO systémique (50 %). Conclusion L’efficacité de l’immunothérapie en 1ère ligne thérapeutique n’était pas diminuée chez les patients présentant des CBNPC stade IV métastatiques au foie comparé aux patients indemnes de lésions hépatiques. Cependant, à l’échelle lésionnelle, les métastases hépatiques étaient moins susceptibles de présenter une réponse tumorale sous traitement.
A pilot study has revealed a significant correlation between high levels of circulating HGF and poor response to ICIs therapy in late-stage NSCLC patients treated with ICIs. This study aimed to evaluate the prognostic value and role of the HGF/MET pathway in first-line immunotherapy treatment in advanced NSCLC patients. We included 82 consecutive patients, 49 treated with immunotherapy and 33 treated with chemotherapy alone as a group control. We analysed the plasma levels of HGF by ELISA. Immunohistochemistry was used to assess the expression levels of PD-L1, MET, and CD8+ T-Cell infiltration. The contribution of HGF/MET to NSCLC response to ICIs was further investigated by culturing peripheral blood mononuclear cells (PBMC)stimulated with 10 μg/mL phytohemagglutinin (PHA) and/or pembrolizumab (20 nM). The cells were cultured with recombinant HGF or co-cultured with NSCLC patients-derived explants.in the presence of cMET inhibitors Crizotinib or SGX. The immune response was quantified through CD8+ activation and INFγ production. Correlation tests demonstrated that lower HGF levels were associated with a good response to ICIs (RR=2.143; p=0.004) and low progression (RR=0; p=0.01). Patients with low HGF levels (< Q1) at treatment initiation had longer progression-free survival (PFS) (841 vs 136.5 days; HR = 0.26 95% CI 0.1292-0.5411; p=0.0003) and longer overall survival (OS) (median not reached vs. 489 days; HR 0.36; 95% CI 0.1505-0.8408; p=0.02). Furthermore, when activated PBMCs were cultured in the presence of recombinant HGF or on NSCLC monolayer, the activation/proliferation of CD3+CD8+ lymphocytes, was inhibited, even in the presence of pembrolizumab. The addition of HGF/MET inhibitors (Crizotinib and SGX) suppressed the immunosuppressive properties of recombinant HGF or NSCLC, stimulating INFγ production. Our study found that high levels of HGF in the peripheral blood are associated with poor response to ICIs, disease progression, and shorter survival in patients with advanced NSCLC. Furthermore, we showed through in vitro experiments that inhibiting the HGF/MET signaling pathway could be a promising therapeutic approach to enhance the efficacy of ICIs in this patient population.
Immune checkpoint inhibitors (ICIs) are now widely used in monotherapy or in combination with chemotherapy in advanced non-small cell lung cancer (NSCLC), and in combination with chemotherapy in advanced small-cell lung cancer (SCLC). However, few data are currently available concerning the acquired resistance mechanisms to ICIs in lung cancer. There is therefore a need for systematic prospective collection of tumor samples for histological and molecular analyses at the time of progression. REBIMMUNE trial is a prospective, bicentric trial (2 academic centers), that includes consecutive patients with advanced NSCLC or SCLC treated with ICIs (alone or in combination with chemotherapy) experiencing tumor progression while on ICIs. After signature of a consent form, the patients have a new tumor biopsy on a progressive tumor site. On these samples, we will perform various immunohistochemistry tests, including PTEN, Beta-2-microglobuline (B2M), Beta-catenin (Wnt pathway activation), CD8 and PD-L1 expressions. We will also perform targeted molecular screening on PTEN, JAK and B2M genes along with NGS panels. We will compare these results with those performed on corresponding diagnosis samples, and correlate them with clinical outcomes (response rate, progression-free survival, overall survival). Le nivolumab est un inhibiteur de point de contrôle immunitaire (ICI) prescrit dès la deuxième ligne dans les cancers bronchiques non à petites cellules (CBNPC) de stade avancé. Des études ont suggéré que le microbiote intestinal et l'administration d'une antibiothérapie précoce (AP) pouvaient moduler l'efficacité des ICI. Les objectifs de notre étude sont d'évaluer l'impact d'une AP (dans les deux mois précédents jusqu'au mois suivant le début du traitement) sur l'efficacité du nivolumab et la valeur prédictive du taux de citrulline plasmatique (marqueur de la barrière intestinale) et du microbiome sanguin. Les patients consécutifs traités par nivolumab pour un CBNPC avancé en deuxième ligne ou plus entre juillet 2014 et décembre 2017 ont été inclus. Les taux de citrulline plasmatique ont été mesurés aux mois (M) 0, M2, M4 et M6. L'analyse du microbiome plasmatique a été faite par séquençage des régions variables V3–V4 du gène ARN 16S bactérien à M0. Parmi les 72 patients inclus, 30 (42 %) ont reçu une AP. La survie globale (SG) était significativement plus longue chez les patients sans AP (médiane de 13,4 mois) que ceux avec AP (5,1 mois, p = 0,03). La survie sans progression (SSP) n'était pas significativement modifiée selon l'utilisation d'une AP (médianes respectives 3,3 et de 2,75 mois, p = 0,249). Des échantillons de plasma ont été prélevés chez 36 patients. Les taux de citrulline plasmatique à M0 étaient associés aux taux de réponse (52 % avec citrulline ≥ 20 μM vs 20 % avec citrulline < 20 μM, p = 0,084), au bénéfice clinique (nivolumab ≥ 6 mois) (56 % avec citrulline ≥ 20 μM vs 0 % avec citrulline < 20 μM, p = 0,002). La SSP médiane était de 7,9 mois (citrulline ≥ 20 μM) vs 1,6 mois (citrulline < 20 μM) (p < 0,0001), et la SG médiane était respectivement non-atteinte vs 2,2 mois (p < 0,0001). Les patients traités par AP tendaient à avoir des taux plus faibles de citrulline que ceux sans AP : 17 μM (IQR 15–30) vs 31 μM (IQR 21,3–40,5) (p = 0,06). Concernant le microbiome à M0, la présence de certaines familles bactériennes était associée à la réponse et au bénéfice clinique (Peptostreptococcae, Paludibaculum, Lewinella) et d'autres à la progression sous nivolumab (Gemmatimonadaceae). Une AP est associée à des taux faibles de citrulline et à une diminution de la SG des patients atteints d'un CBNPC traités par nivolumab. La citrulline plasmatique ainsi que la composition du microbiome sanguin pourraient être des marqueurs prédictifs de l'efficacité du nivolumab.
Le nivolumab (Nivo) est un anticorps anti-PD1 utilisé en deuxième ligne de traitement dans les cancers bronchiques non à petites cellules (CBNPC) de stade avancé. La sélection des patients susceptibles de bénéficier d'une réponse prolongée demeure une question centrale. L'objectif de ce travail est d'explorer l'intérêt du PD-L1 plasmatique (PD-L1p) comme nouveau biomarqueur potentiel. Il s'agit d'une étude monocentrique réalisée dans un centre hospitalo universitaire. Des prélèvements de plasma ont été réalisés de manière prospective au cours du suivi de patients atteints de CBNPC de stade avancé traités par Nivo. Les dosages de PD-L1p ont été réalisés par technique ELISA (clone 28–8) au moment du diagnostic, lors de l'initiation du Nivo et lors du premier (1er) bilan d'évaluation tumorale (à 2 mois). Les taux de PD-L1p ont été évalués en fonction de la réponse tumorale et du bénéfice clinique (maladie contrôlée sous Nivo pendant plus de 6 mois). La survie sans progression (SSP) et la survie globale (SG) ont également été évaluées en fonction des taux de PD-L1p. Quarante et un patients traités par Nivo entre juillet 2015 et mai 2017 ont été inclus. Il s'agissait essentiellement d'hommes (66 %), fumeurs (88 %), avec un âge médian de 66 ans et traités pour un adénocarcinome (63 %). Aucun impact des taux au diagnostic ou lors de l'initiation du Nivo n'a été montré. Les patients avec bénéfice clinique (n = 12) avaient un taux de PD-L1p au 1er bilan d'évaluation tumorale plus bas que les patients sans bénéfice clinique : médiane de 27,9 pg/mL vs 50,9 pg/mL (p = 0,033). Un taux de PD-L1p supérieur à 27,9 pg/mL au 1er bilan d'évaluation était associé à une SSP plus courte (2,3 mois [> 27,9 pg/mL] vs non atteinte (NA) [< 27,9 pg/mL], p = 0,024). Le taux de réponse objective était de 27 % en cas d'augmentation du PD-L1p (plus de 5 %) entre le début du Nivo et le 1er bilan versus 61 % en l'absence d'augmentation de PD-L1p, p = 0,077). L'augmentation du taux de PD-L1p entre le début du Nivo et le 1er bilan était associé à une SSP médiane plus courte : 2,1 mois (augmentation de PD-L1p) versus 5,5 mois (pas d'augmentation de PD-L1p) (p = 0,022). Aucun impact sur la SG n'a montré. Le seuil de 27,9 pg/mL au 1er bilan d'évaluation permettait de prédire le bénéfice clinique à 6 mois avec une valeur prédictive positive de 77 % et une valeur prédictive négative de 86 %. Ces résultats suggèrent un rôle potentiel du dosage de PD-L1p dans la prédiction de la réponse, de la SSP et du bénéfice clinique au Nivo.
Recent data suggest that gut microbiota and antibiotic use affect the efficacy of immune checkpoint inhibitors. We aimed to evaluate the predictive role of blood microbiota, plasma citrulline (marker of the intestinal barrier), and the impact of early (from 2 months before until 1 month after beginning of nivolumab) and late (after 1 month of nivolumab) antibiotic use on nivolumab efficacy in NSCLC patients. We included consecutive patients with advanced NSCLC treated with nivolumab between July 2014 and December 2017. Plasma tests were taken prospectively during treatment. The microbial population present in the plasma samples was determined using sequencing of variable regions of the 16S rRNA bacterial gene at M0. Plasma citrulline concentrations were evaluated by ion exchange chromatography at month (M) 0, M2, M4 and M6 of nivolumab. Seventy-two patients were included (male: 62%; smokers: 87%; adenocarcinoma: 63%). Early use of antibiotics (EUA) (n=28/72) was associated with poor overall survival (OS) (median 5.1 months, versus 13.4 months without EUA, p=0.03), whereas later use of antibiotic during treatment had no significant effect. Thirty-six patients (50%) had serial plasma samples available for analyses. The composition of blood microbiota at M0 was associated with tumor response and long-term benefit of nivolumab, with a significant impact of Paludibacilum, Gemmatimonadaceae and Nocardioides. Patients with long-term benefit of nivolumab had significantly higher plasma citrulline concentrations than other patients, at M0, M2 and M4, and maintained high citrulline concentrations at M6. Median progression-free survival (PFS) and OS for patients with citrulline ≥20μM at M0 were 7.9 months and not reached, versus 1.6 months (p<0.0001) and 2.2 months (p<0.0001) for patients with citrulline <20μM, respectively. Patients with EUA had lower median citrulline concentrations at M0 (17μM [IQR 15-30] versus 31μM [IQR 21.3-40.5]; p=0.06). This study confirms the negative impact of EUA during nivolumab treatment, and suggests that plasma evaluation of the intestinal barrier and blood microbiota may help to predict the outcome of NSCLC patients treated with nivolumab.
The key pathophysiological feature of chronic obstructive pulmonary disease (COPD) is an abnormal inflammatory bronchial reaction after inhalation of toxic substances. The priority is the avoidance of such toxic inhalations, but the use of anti-inflammatory drugs also seems appropriate, especially corticosteroids that are the sole anti-inflammatory drug available for this purpose in France. The risks associated with the prolonged use of these parenteral drugs are well known. Inhalation is therefore the optimal route, but inhaled drugs may also lead to adverse consequences. In COPD, there is an inhaled corticosteroids overuse, and a non-satisfactory respect of the guidelines. Consequently, their withdrawal should be considered. We reviewed seven clinical studies dealing with inhaled corticosteroids withdrawal in patients with COPD and found that included populations were heterogenous with different concomitant treatments. In non-frequent exacerbators receiving inhaled corticosteroids outside the recommendations, withdrawal appears to be safe under a well-managed bronchodilator treatment. In patients with severe COPD and frequent exacerbations, the risk of acute respiratory event is low when they receive concomitant optimal inhaled bronchodilators. However, other risks may be observed (declining lung function, quality of life) and a discussion of each case should be performed, especially in case of COPD and asthma overlap.
Les pneumonies aiguës communautaires (PAC) sont une cause fréquente de prescription antibiotique chez les patients hospitalisés. La plupart des recommandations préconisent 7 à 14 jours de traitement antibiotique, mais cette durée ne repose pas sur des preuves univoques. Nous étudions la possibilité de réduire la durée de traitement de cette pathologie à 3 jours chez les patients ayant une réponse clinique précoce au traitement antibiotique. Essai randomisé double insu de non-infériorité avec 2 groupes parallèles comparant 3 et 8 jours de traitement par bétalactamines. La randomisation est réalisée à j3 chez les patients présentant les critères de stabilité. Les critères d’inclusion sont : à j0, température > 38 °C, signes cliniques de PAC, nouvel infiltrat radiologique, et une réponse clinique, définie par l’obtention des critères de stabilité, au traitement à j3. Les critères de non-inclusion sont : bithérapie d’antibiotique, pneumonie d’inhalation, pneumonie à germes intracellulaires, immunodépression, allergie aux pénicillines. Les patients sont suivis pendant 30 jours, avec pour objectif principal la guérison à j15. Actuellement, 195 patients ont été inclus, l’âge moyen est de 68,0 ± 18,3 ans et le sex-ratio de 1,4. Les principales comorbidités sont : bronchite chronique obstructive (n = 47 ; 24,1 %), insuffisance cardiaque (n = 37 ; 19,0 %), diabète (n = 36 ; 18,5 %), athérosclérose (n = 26 ; 13,3 %), pathologie cérébro-vasculaire (n = 18 ; 9,2 %), insuffisance rénale chronique (n = 13 ; 6,7 %), néoplasie (n = 5 ; 2,6 %), hépatopathie (n = 4 ; 2,1 %). Le score pneumonia severe index (PSI) avant inclusion était en moyenne 78,9 ± 31,2 (15–181) et la température moyenne de 38,9 ± 0,6 °C. La durée moyenne de séjour hospitalier était 6 ± 5,5 jours (0–44, médiane : 4 jours). Au cours du suivi, le principal effet indésirable était les diarrhées (n = 17) dont 1 épisode d’infection à clostridium difficile. Une analyse de sécurité conduite par un comité indépendant a revu les 131 premiers patients inclus, 19 ont présenté un effet indésirable grave et 3 sont décédés. Le taux de guérison global était de 91,6 % avec 11 échecs dont 2 décès. Le comité indépendant a conclu à la sécurité de l’essai et à sa poursuite. Trois jours de traitement antibiotique avec une bétalactamine au cours des PAC semblent efficaces dans cette analyse préliminaire. Ces résultats permettraient de réduire considérablement l’exposition antibiotique et de participer à la lutte contre la résistance bactérienne.
Introduction. - Miliary brain metastases are a rare form of brain metastatic lesions.Case report. - We report the case of a 58-year-old patient with lung adenocarcinoma and an EGFR mutation, who had metastatic lesions in the bones, pleura and pericardia at the time of diagnosis. The patient was treated with tyrosine kinase inhibitor. A few months later, he presented with progressive neuropsychiatric symptoms, which were attributed to miliary brain metastases based on the radiological pattern (micronodules, some of which were calcified) and the elimination of alternative possible diagnoses. Despite tumour stability in the thorax and metastatic sites other than the brain, his neurological condition deteriorated, even after cerebral radiotherapy, leading to his death eight months after the diagnosis of lung cancer.Conclusion. - Miliary brain metastases are a rare form of brain metastases with unusual clinical presentation. The diagnosis is based on the radiological pattern of cerebral miliary dissemination, with sometimes calcified tumor nodules. Introduction. Clinical trials have provided some evidence of a favorable effect of inhaled corticosteroids on the frequency of exacerbations and on the quality of life of patients with chronic obstructive pulmonary disease (COPD). In contrast, ICS have little or no impact on lung function decline and on mortality.State of the art. Inhaled corticosteroids are recommended only in a minority of COPD patients, those with severe disease and repeated exacerbations and probably those with the COPD and asthma overlap syndrome. However, surveys indicate that these drugs are inappropriately prescribed in a large population of patients with COPD. Overtreatment with inhaled corticosteroids exposes these patients to an increased risk of potentially severe side-effects such as pneumonia, osteoporosis, and oropharyngeal candidiasis. Moreover, it represents a major waste of health-care spending.Conclusion. Primary care physicians as well as pulmonologists should be better aware of the benefits as well as the side-effects and costs of inhaled corticosteroids. (C) 2016 SPLF.
