Distinguishing between kidney stones and phleboliths can constitute a diagnostic challenge in patients undergoing unenhanced low-dose CT (LDCT) for acute flank pain. We sought to investigate the accuracy of radiomics and a machine-learning classifier in differentiating between kidney stones and phleboliths on LDCT. Radiomics features were extracted following a semi-automatic segmentation of kidney stones and phleboliths for two independent consecutive cohorts of patients undergoing LDCT for acute flank pain. Radiomics features from the first cohort of patients (n = 369) were ultimately used to train a machine-learning model designed to distinguish kidney stones (n = 211) from phleboliths (n = 201). Classification performance was assessed on the second independent cohort (i.e., testing set) (kidney stones n = 24; phleboliths n = 23) using positive and negative predictive values (PPV and NPV), area under the receiver operating curves (AUC), and permutation testing. Our machine-learning classification model trained on radiomics features achieved an overall accuracy of 85.1% on the independent testing set, with an AUC of 0.902, PPV of 81.5%, and NPV of 90.0%. Classification accuracy was significantly better than chance on permutation testing (p < 0.05, permutation p value). Radiomics and machine learning enable accurate differentiation between kidney stones and phleboliths on LDCT in patients presenting with acute flank pain. • Combining a machine-learning algorithm with radiomics features extracted for abdominopelvic calcification on LDCT offers a highly accurate method for discriminating phleboliths from kidney stones. • Our radiomics and machine-learning model proved robust for CT acquisition and reconstruction protocol when tested in comparison with an external independent cohort of patients with acute flank pain. • The high performance of the radiomics-based automatic classification model in differentiating phleboliths from kidney stones indicates its potential as a future diagnostic tool for equivocal abdominopelvic calcifications in the setting of suspected renal colic.
OBJECTIVE To assess long term functional and safety follow-up data after 80-W GreenLight photoselective vaporization (GL PV) of the prostate and transurethral resection of the prostate (TURP). MATERIALS AND METHODS Prospective randomized trial at a single tertiary referral center (Geneva, Switzerland). Patients were recruited in the outpatient clinic if they met the criteria for surgical treatment of benign prostatic obstruction. At baseline, 238 patients were treated either with the 80-W GL PV or monopolar TURP. After 5 years, data were available from 105 patients: 44 GL PV patients and 61 TURP patients. The primary outcome measure was the International Prostate Symptom Score (IPSS). Secondary outcome measures included maximum urinary flow rate (Qmax), postvoidal residual (PVR) and reoperation rate. Statistical analyses were performed using Stata 14 (StataCorp). RESULTS After 5 years of follow-up, mean improvements in International Prostate Symptom Score, postvoidal residual and maximum urinary flow rate were similar in both groups. The re-treatment rate was 14.3% in the GL PV group vs 11.9% in the TURP group (P = .9). CONCLUSION Noninferiority of the GL PV to TURP was confirmed in all functional and safety outcomes at 5-year follow-up. GL-PV could be a safe surgical alternative for patients suffering from benign prostatic obstruction. (c) 2018 Published by Elsevier Inc.
Patients with rectal cancer have an increased risk of developing sexual disorders. These dysfunctions are caused by the disease itself (negative psychological impact, nerve compression in the pelvis), as well as by the treatments (radiotherapy, chemotherapy and surgery). Most sexual disorders are due to the surgery and can be attributed to injuries of the retroperitoneal nerves. Sexuality assessment of patients before and after treatment, a precise knowledge of the pelvic anatomy by the surgeon, and an appropriate care by specialists (gynaecologists, urologists and sexologists) are essential to reduce the risk of sexual disorders and to minimize their impact on overall quality of life.
Les malformations de l’appareil urinaire sont multiples. Souvent ces anomalies entraînent des obstructions chroniques de la voie urinaire et la formation de calculs. La duplication urétérale est l’anomalie la plus fréquente touchant 1 % de la population. Nous rapportons la prise en charge chirurgicale laparoscopique d’une anomalie de trajet avec un passage urétéral rétrocave (prévalence 0,07 %) associée à une importante dilatation pyélocalicielle et des calculs. Patiente de 25 ans présentant des douleurs du flanc droit. Le bilan radiologique met en évidence un trajet urétéral rétrocave provoquant une importante dilatation pyélocalicielle d’amont associée à de multiples calculs de stase. L’indication à un traitement en 2 temps est posé, à savoir un décroisement urétéral laparoscopique sous assistance robotisée avec urétéroplastie, suivi d’une urétérorénoscopie souple afin d’extraire les calculs de stase. Disparition complète des douleurs avec une voie urinaire bien ouverte à 6 semaines postopératoire, visualisée lors de l’urétérorénoscopie. Le trajet urétéral rétrocave est une malformation rare qui peut être prise en charge par abord laparoscopique. Dans ce cas, nous avons pu confirmer par urétérorénoscopie la bonne ouverture urétérale post urétéroplastie laparoscopique.

Les patients atteints d’un cancer du rectum presentent un risque accru de developper ou d’aggraver des troubles de la sexualite. Ces dysfonctions sont causees par la maladie (impact psychologique negatif, compression nerveuse dans le petit bassin) d’une part, et par les traitements (radio, chimiotherapie et chirurgie) d’autre part. Parmi ceux-ci, la chirurgie est responsable de la plupart des troubles sexuels, par lesion des plexus neurovegetatifs. Une evaluation de la sexualite des patients avant et apres traitement, une connaissance precise de l’anatomie du petit bassin par le chirurgien ainsi qu’une prise en charge adaptee par des specialistes (gynecologues, urologues, sexologues) constituent les etapes essentielles afin de diminuer le risque de survenue de troubles sexuels et leur impact sur la qualite de vie globale des patients.

Introduction: The study aimed to evaluate 3 different modalities of transrectal ultrasound (TRUS)-guided prostate biopsies (PBs; 2D-, 3D- and targeted 3D-TRUS with fusion to MRI - T3D). Primary end point was the detection rate of prostate cancer (PC). Secondary end point was the detection rate of insignificant PC according to the Epstein criteria. Patients and Methods: Inclusion of 284 subsequent patients who underwent 2D-, 3D- or T3D PB from 2011 to 2015. All patients having PB for initial PC detection with a serum prostate-specific antigen value ≤20 ng/ml were included. Patients with T4 and/or clinical and/or radiological metastatic disease, so as these under active surveillance were excluded. Results: Patients with T3D PB had a significantly higher detection rate of PC (58 vs. 19% for 2D and 38% for 3D biopsies; p = 0.001), with no difference in Gleason score distribution (p = 0.644), as well as detection rate of low-risk cancers (p = 0.914). Main predictive factor for positive biopsies was the technique used, with respectively a 3- and 8-fold higher detection rate in the 3D- and T3D group. For T3D-PB, there was a significant correlation between radiological cancer suspicion (Prostate Imaging Reporting and Data System Score) and cancer detection rate (p = 0.02). Conclusions: T3D PB should be preferred over 2D PB and 3D PB in patients with suspected PC as it improves the cancer detection rate.
OBJECTIVE To report our experience with robot-assisted ureteral anastomosis for kidney graft. Kidney graft complex ureteral strictures or symptomatic vesicoureteral reflux may require complex reconstruction. This is classically done through an open surgical access, which adds to the morbidity of kidney transplantation. The da Vinci robot enables performance of complex laparoscopic procedures and may hence be used for such reconstructions. PATIENTS AND METHODS We retrospectively reviewed all patients undergoing robotic surgical revision for stricture or reflux disease over a 3-year period. Contemporary patients who underwent open surgery were used as a control group. RESULTS Ten patients underwent a robotic attempt, of whom 4 needed conversion to open surgery. Seven patients underwent an open surgery. Preoperative demographics were similar in both groups. The median operative time was 293 minutes, with a shorter operative time in the open group. The group of patients who could be completed robotically had a significantly lower postoperative length of stay (5 vs 9 days), quicker return to normal food intake (postoperative day 1 vs 3), and quicker control of pain without opiates (postoperative day 1 vs 4) than the converted or open group. Morbidity was comparable with 1 late Clavien IIIb complication in each subgroup (open, converted, and robotic group). After a median follow-up of 43 months, renal function was stable and there were no recurrent graft infections. CONCLUSION Robotic ureteral reconstruction for kidney graft patients is feasible and efficient, and offers the classical advantages of minimally invasive surgery with outcomes comparable with open series. La nécessité d’effectuer un curage ganglionnaire (CG) lors d’une prostatectomie radicale (PR) reste controversée pour les patients à faible risque d’invasion ganglionnaire (IG). Le but de l’étude est d’évaluer s’il y a un bénéfice à effectuer un CG chez les patients avec un faible risque d’IG selon les tables de Partin (TP). Dans notre institution, de février 2006 à juin 2013, un CG a été réalisé uniquement chez les patients présentant un risque d’IG > 5 %. Depuis juin 2013, il est systématique. Évaluation rétrospective de 384 sur 570 patients ayant subi une PR laparoscopique robotique dans notre institution, de février 2006 à avril 2016. Jusqu’à juin 2013, 220 sur 415 patients (53 %) n’ont pas reçu de CG, en raison d’un faible risque d’IG selon les TP. Ces 220 patients ont été divisés en deux groupes. Le 1er se composait de 201 patients avec un risque moyen d’IG < 5 % selon les TP et une valeur supérieure de l’éventail de risque < 5 %. Le 2e se composait de 19 patients avec un risque moyen < 5 % selon les TP et une valeur supérieure de l’éventail de risque ≥ 5. Le groupe 2 avait un taux significativement supérieur de récidive ou de PSA postopératoire non négatif (26 vs 6 patients, 33 % vs 13 %, p 0,018). De juin 2013 à avril 2016, tous les patients (n = 164) ont subi une PR avec un CG systématique. Parmi eux, 121 patients n’auraient pas du recevoir un curage selon les critères utilisés précédemment. Ces 121 patients ont été divisés en deux groupes comme expliqué ci-dessus. Le 1er groupe comptait 91 patients et le 2e 30 patients. Le groupe 2 avait un taux de ganglions métastatiques significativement majeur (2 vs 0, valeur de p 0,009). Les TP permettent une sélection correcte des patients devant subir un CG, si la valeur supérieure de l’éventail de risque est < 5 %. En tenant compte uniquement de la valeur moyenne, sans considérer la fourchette de risque, on prend un risque de manquer des ganglions métastatiques.
NPRA ; son indication ne cesse de s’étendre à des tumeurs à la fois plus grandes et plus difficiles d’accès. L’objectif de cette étude est d’évaluer la sécurité et les issues oncologiques de la NPRA pour des tumeurs pT1b/pT2a versus pT1a. Analyse rétrospective de toutes les tumeurs rénales traitées par NPRA de décembre 2009 à mai 2016 au sein de la même institution. Quatre-vingt-dix-huit patients étaient opérés par NPRA. Quatre patients (1 dans le groupe pT1a (1,3 %) et 3 dans le groupe pT1b/pT2a (15 %) étaient convertis par laparotomie) retirés de l’analyse. La taille, l’âge, le temps d’ischémie chaude, le score de PADUA, les pertes sanguines, le temps opératoire, les complications postopératoires, les marges chirurgicales et la durée de séjour étaient évalués. Les images préopératoires étaient révisées pour calculer le score de PADUA. Le stade tumoral était déterminé en fonction des résultats pathologiques définitifs. 79 % (n = 74) patients dans le groupe pT1a et 21 % (n = 20) dans le groupe pT1b/pT2a. Aucune différence significative concernant la taille tumorale, l’âge, le temps d’ischémie, les pertes sanguines et le taux de complications postopératoires entre les deux groupes (cf. Tableau 1). Score de PADUA était sensiblement inférieur dans le groupe pT1a que dans pT1b/pT2a (p < 0,003). La durée opératoire moyenne était significativement plus courte dans le groupe pT1a (254 ± 86 minutes vs. 300 ± 61 minutes ; p = 0,005). Onze pour cent des pT1a avaient une marge chirurgicale positive, alors que tous les pT1b/pT2a étaient réséqués en marge saine (p = 0,12). Le temps de séjour moyen avait tendance à être plus court dans le groupe pT1a mais sans signification statistique (4,4 ± 1,8 vs 5 ± 1,9 jours ; p = 0,11). NPRA permet une morbidité périopératoire minimale, une sécurité sur le plan chirurgical et oncologique pour les cancers rénaux pT1b/pT2 techniquement accessibles ; même si le temps opératoire est plus long par rapport au pT1a. Notre étude montre que les patients peuvent bénéficier d’une approche chirurgicale minimale invasive pour les pT1b/pT2a.
Each developed country faces an aging population. Increase of age rises the risk to develop urologic pathology especially in men. Routine investigation and diagnostic of urologic pathology important, however must take into account the patient in his globality (including age, comorbidities, drug medication as well as sociocultural environment). The aim of this article is to help the general practitioner in taking decisions with frequent uro-geriatric symptoms. Bladder catheterization remains an invasive treatment and indication should be regularly re-evaluated. Urinary infections must be distinguished from bacterial colonisations, with some exceptions, don't need any specific treatment. We will resume two algorithms on the management of hematuria and the management of prostate cancer suitable for the elderly.
L’injection sous-muqueuse d’agents de comblement est une option minimalement invasive pour le traitement de l’incontinence urinaire type stress, dépendant toutefois des caractéristiques biophysiques de ces agents. Nous avons comparé deux nouveaux produits avec des agents déjà validés. Les objectifs principaux étaient l’évaluation de l’injection, les réactions macroscopiques de la muqueuse, la migration des agents et l’effet de comblement sur une durée totale de 12 mois. Injections endoscopiques randomisées de 4 agents différents dans la sous-muqueuse vésicale de truies. Les nouveaux agents testés étaient l’Embosphere™ (EMB) et une solution de Polymerpolylactilique (PLA) qui précipite après injection. Ils étaient comparés au Deflux™ (DEF) et au Macroplastique™ (MAC). Au total, 3 injections dans 8 porcs à j0 correspondant à 6 injections de chaque produit. L’injection et l’effet de comblement ont été évalués sur une échelle de 0 à 3. Des cystoscopies de contrôle ont permis le suivi macroscopique à 3, 6 et 12 mois. Des cystectomies et des mesures de volume des agents ont été réalisées à 12 mois à l’aide d’un étrier. L’injection était plus facile pour le PLA et les EMB (3) suivi du DEF (1,83) et du MAC (1), p < 0,05. Le score moyen de comblement était plus important pour le MAC (2,8) suivi des EMB (2,5), DEF (2,2) et PLA (1,8) ; p = 0,05. Les volumétries montrent des résultats similaires avec 1144 mm3 pour le MAC, 755 mm3, pour les EMB, 420 mm3, pour le PLA et 370 mm3 pour le DEF. Aucune modification majeure de la muqueuse vésicale n’a été observée pour les EMB. Une inflammation de la muqueuse et une migration de produit est suspectée avec le DEF. En revanche, le MAC et le PLA ont provoqué des réactions de la muqueuse (inflammation, érosion, nécrose) dans 100 % des cas. Des signes de migration ont été observés dans un cas d’injection de PLA. Les agents les plus efficaces en termes de capacité de comblement et de biocompatibilité étaient les EMB et le DEF. Ces résultats doivent être vérifiés par les examens histologiques en cours.

Its cure simultaneously with RP remains a concern with respect to the type of technique, and the risk to infect an eventual mesh The aim of this study was to evaluate the feasability, morbidity and efficacy of combined inguinal hernia synthetic mesh repair during robot assisted radical prostatectomy (RARP). METHODS 526 medical records of patients who underwent transperitoneal RARP from 2006 to 2015 were reviewed. Demographics and peri-operative datas were analyzed for two groups (RARP alone vs RARP and hernia repair). Pre-operative iv cefazoline and gentamycine was given to all patients. Cure was performed using a polyester mesh (Parietex - Covidien, New Haven, USA), placing the porous side against the wall for an efficient tissue integration, and the smooth side facing the structures on which tissular attachment had to be limited. The mesh was cut in a rectangle of approximately 10x8 cm or 18x8 cm for respectively unilateral and bilateral cures, and was anchored with absorbable staples (Absorbatack – Covidien), so as non absorbable sutures around the iliac vessels. Early complications were classified using Clavien-Dindo classification. Hernia recurrence was assessed during regular follow-up, or self-reported by patients. RESULTS 49 patients (9 %) had a hernia repair associated with RARP (35 unilateral and 14 bilateral). Median age, BMI and ASA score were the same in both groups. Total operative time was 278 min for RARP vs 290 min for combined procedure. Post-operative length of stay (median 4 days) and blood loss were not affected by the combined procedure. There were no mesh infections nor migrations. Complication rate was identical in both groups. After a median follow-up of 44 months (IQR 17-86), 46 patients (94 %) were hernia-free, while 3 (6 %) presented a hernia recurrence. CONCLUSIONS Combined inguinal hernia repair with a synthetic mesh during RARP is a feasible and efficient procedure which added no morbidity to standard RARP. Placing the mesh during an operation with urine spillage led to no infections. Efficacy after median-term follow-up is similar to laparoscopic hernia repair alone. Due to its length and its small diameter, the ureter is exposed to a high obstructive risk which may be ascribed to extremely variable pathologies. Because of a remarkably active peristalsis, the clinical consequence is acute if the obstacle suddenly settles. The radiological sign of appeal is the pyelocalyceal dilatation, which is widely listed in the Western medical system, in consideration of the abundance of the practiced imaging. From the acute situation to the fortuitous discovery, its understanding must be further investigated because of the immediate symptomatic potential impact such as renal colic, which can be associated with sepsis, as well as possible long-term sequelae on renal function. This article aims to help the primary care physician to initiate its diagnosis and treatment.
Les biopsies ciblées par fusion des images d’IRM multiparamétrique (IRMmp) et d’échographie transrectale augmentent le rendement diagnostic des foyers tumoraux identifiés par IRMmp. Néanmoins, des biopsies randomisées sont toujours effectuées en même temps que les biopsies ciblées. Notre but est de déterminer l’efficacité des biopsies de fusion combinées aux biopsies randomisées dans la détection des foyers significatifs et dans la prédiction du score de Gleason. Analyse rétrospective des patients ayant bénéficié d’une IRMmp, de biopsies et d’une prostatectomie radicale dans notre institution en 2014. L’IRMmp était effectuée sur une machine 3 T avec antenne endorectale et lue par 2 uro-radiologues seniors. Les biopsies ont été pratiquées par des urologues expérimentés sur l’Urostation Koelis. Tous les foyers tumoraux ont ensuite été caractérisés sur des pièces de prostatectomie radicale par 1 uro-pathologiste dédié. Les foyers significatifs ont été définis comme tout volume supérieur à 0,5 mL, tout score de Gleason ≥ 7 ou tout foyer avec extension extraprostatique. Sur les 25 spécimens opératoires, 62 foyers ont été recensés (2,48/pièce) dont 40 (65 %) significatifs. L’IRMmp avait identifié 31 des 40 foyers (78 %) et les biopsies randomisées et ciblées 39 (97 %). Si l’on considère le foyer index (score de Gleason le plus élevé, plus haut volume ou extension extraprostatique), le taux de détection par l’IRMmp était de 84 % (21/25). L’ajout des biopsies randomisées augmentait ce taux à 96 % (24/25). Une corrélation exacte entre le Gleason bioptique et le Gleason définitif a été observée pour 84 % des foyers index. Huit pour cent des biopsies présentaient un score de Gleason supérieur au Gleason définitif du foyer, et 8 % un score inférieur. Vingt-deux pour cent des foyers significatifs et 16 % des foyers index sont à risque d’omission par biopsies ciblées des seules lésions identifiées à l’IRMmp. Ces résultats plaident en défaveur de l’adoption des biopsies ciblées seules, notamment dans le contexte d’un traitement focal/zonal. Les biopsies combinées permettent une excellente détection des foyers significatifs et une bonne prédiction de leur score de Gleason définitif.
Urothelial carcinomas are the fourth most common tumors in men. Upper tract urinary carcinomas (UTUCs) are uncommon and represent only 5e10% of urothelial carcinomas. 1 Metastatic testicular cancers are rare and primary tumor sources are the prostate, lung, and gastrointestinal tract. We report the first case of testicular metastasis 2 years after initial curative surgery for a high-grade UTUC, all other reported cases weren't proceed by curative surgery. 3 (C) 2016 The Authors.
