Les tumeurs neuroendocrines du pancréas sont des tumeurs rares avec une incidence estimées à 1/100000 habitants. Nous rapportons le cas d'un carcinome neuroendocrine bien différencié du pancréas chez un patient de 55 ans, diabétique sous traitement admis dans notre structure pour un syndrome tumoral de l'hypochondre droit. Le traitement a consisté en une duodéno-pancréatectomie céphalique.
L'invagination intestinale aiguë (IIA) secondaire à une cause inhabituelle est une complication rare et une urgence abdominale. Elle pose un problème diagnostique et thérapeutique. Nous rapportons le cas d'un patient âgé de 45 ans, qui présentait une sténose bulbaire peptique résistante au traitement médical. Le patient a été opéré avec présence en per opératoire d'un estomac de stase, en amont d'une sténose bulbaire infranchissable. Une jéjunostomie d'alimentation à la Witzel a été réalisée vu l’état général très altéré du patient. L’évolution était marquée par l'installation d'une invagination sur sonde de jéjunostomie ayant nécessité une intervention chirurgicale avec des suites simples. L'amélioration de la prise en charge et du pronostic de l'IIA secondaire à une cause inhabituelle nécessitant un diagnostic précoce. Le traitement de cette forme particulière d'invagination est presque exclusivement basé sur la chirurgie.
Spontaneous cholecystocutaneous fistula is a rare complication of chronic calculous cholecystitis, because currently gallstones are diagnosed and treated at an early stage. This occurrence is possible even if it seems actually to be rare. We report a case of spontaneous cholecystocutaneous fistula of Anterior Abdominal Wall with subcutaneous gallstones admitted in the medical oncology unit of HASSAN II University Hospital. An 64-year-old woman, presented with a large subcutaneous abscess in the right subcostal area with swelling. An abdominal computed tomography scan showed a subcutaneous gallstone and communication between the abscess and the gallbladder. Cholecystectomy was performed and the abdominal wall abscess was drained externally. This case report demonstrates that maintaining a high degree of suspicion of this rare entity is helpful in achieving correct preoperative diagnosis, and that computed tomography scan should be performed in all cases of unexplained abdominal wall suppuration.
Le diverticule de Meckel est une anomalie congénitale résultant d'une résorption incomplète du canal omphalo-mésentérique, pouvant se compliquer dans 4% des cas. Il s'agit d'une patiente âgée de 48 ans, sans antécédent pathologique, admise aux urgences pour des douleurs abdominales généralisées à point de départ hypogastrique. L'examen clinique trouvait une sensibilité abdominale généralisée chez une patiente fébrile à 38,7 c. Le bilan biologique avait montré une hyperleucocytose à 18000/mm3, avec une CRP à 130 mg/l. L'échographie avait objectivé un épanchement intra péritonéal de moyenne abondance. Un complément scannographique avait mis en évidence une énorme collection rétro péritonéale refoulant le colon gauche et l'intestin grêle, avec une distension grêlique (A). La patiente a été opérée par laparotomie médiane, avec découverte d'un volumineux abcès rétro péritonéal en rapport avec un diverticule de Meckel situé à 90 cm de la valvule iléo caecal, et perforé au niveau de sa pointe dans le rétro péritoine (B). On a réalisé une résection grêlique emportant le diverticule avec double stomie grêlogrêlique à la Bouilly-Walkman associée à une aspiration de l'abcès puis lavage et drainage péritonéal large (C). Les suites post opératoires ont été simples, avec rétablissement de la continuité digestive après deux mois. L'étude anatomo pathologique de la pièce de résection est revenue en faveur d'une diverticulite Chronique sans signe de malignité.
Few cases of Liposarcoma of the spermatic cord have been reported in the literature. This rare tumor represents 7% of malignant tumors of the spermatic cord. We report a case of an unusual liposarcoma of the spermatic cord, with respect to its sclerosing variant, occurring in a healthy looking 42 years old man. A wide local excision of the tumor was performed. Adjuvant radiotherapy may sometimes be effective on local recurrence. Even though this tumor evolves slowly, a prolonged follow up is required because of the risk of late recurrence.
La fistule appendiculo-cutanée est une communication anormale entre le tube digestif et la peau à travers l'appendice. Elle représente une complication rare de la perforation appendiculaire au cours de l'appendicite et une urgence chirurgicale. Nous rapportons le cas d'un patient âgé de 48 ans, admis aux urgences pour des douleurs fébriles de la fosse iliaque droite, avec issu de pus à travers un orifice situé au niveau du flanc droit. L'examen clinique avait mis en évidence un orifice de 0,5 cm de diamètre situé au niveau du flanc droit donnant issu à de pus verdâtre, avec une défense de la fosse iliaque, chez un patient fébril à 38,2C. Le bilan biologique avait montré une hyperleucocytose à 15000/mm3, avec une CRP à 65 mg/l. Une échographie abdominale a été en faveur d'une appendicite aigue avec un abcès pariétal en regard. Le scanner abdomino-pelvien avait révélé un aspect d'appendicite aigue compliquée d'une fistule appendiculo-cutanée et d'un abcès pariétal en regard (A). Le patient a été opéré par laparotomie médiane avec découverte d'une appendicite aiguë suppurée avec un appendice de 7 cm de long, fistulisé à la peau au niveau du flanc droit, siège d'un abcès pariétal d'environ 20 cc de pus prélevé (B). Le geste a consisté à une appendicectomie rétrograde après déconnexion de la fistule et un drainage de l'abcès. Les suites post opératoires ont été simples et le patient est sorti de l'hôpital à j+4. L'examen anatomo-pathologique de la pièce opératoire est revenu en faveur d'une appendicite aiguë suppurée sans signes de malignité.
Le volvulus du cæcum suite à une tumeur appendiculaire est une complication rare et une urgence chirurgicale dont l'origine est multifactorielle. Nous rapportons le cas d'un patient âgé de 59 ans, admis aux urgences pour un syndrome occlusif associé à des douleurs de la fosse iliaque droite. L'examen clinique avait objectivé une masse sensible de la fosse iliaque droite. Le scanner abdomino-pelvien avait révélé un aspect de volvulus caecal associé à une masse tissulaire latéro-caecale interne (A). Le patient a été opéré par laparotomie médiane avec découverte d'un volvulus du cæcum sur un processus appendiculaire (B), d'où la réalisation d'une hémi colectomie droite avec anastomose iléo colique termino-terminale. L'examen anatomo pathologique de la pièce opératoire est revenu en faveur d'un adénocarcinome mucineux de l'appendice, classé Pt3N1Mx. Le patient a bénéficié d'une chimiothérapie adjuvante, et après un recul de 12 mois, il est en bon contrôle clinico radiologique.L'intérêt de ce cas clinique est double, d'une part en raison de la rareté de cette pathologie et d'autre part dans la cause du volvulus du cæcum.
e14726 Background: Biliary tract cancers (BTC) are relatively rare (3% of digestive cancers), they mainly affect older women and are located in the gallbladder in about 70% of the cases. Methods: In this retrospective study, we reviewed 63 cases of BTC admitted in the medical oncology unit of HASSAN II university center between January 2007 and December 2011. The aim of this study is to define epidemiologic, diagnostic and therapeutic aspects of this disease in a Moroccan population. Results: In our study, BTC account for 4,9% of digestive cancers. The mean age of our patients was 58,2 years ranged from 32 to 81 years, female gender represent 69%. The tumors were located in the gallbladder (GB) in 70% of the cases divided into adenocarcinoma (79%) and squamous cell carcinoma (21%). The other locations were hilar cholangiocarcinoma (24%) and intrahepatic cholangiocarcinoma (6%). Magnetic resonance cholangiographic was performed (n=15) and suspected cancer in all cases. Incidental finding of GB cancer at surgery was recorded in 21 patients: intraoperatively in 8 cases or after pathological study in 13 cases. Staging of the disease was divided into localized stage (7%), locally advanced stage (13%) and metastatic stage (80%). A complete surgery in GB cancer was performed in 13 patients, a palliative surgery was done in 20 patients: cholecystectomy (n=7) and biliary drainage (n=13). For metastatic patients (n=44), a palliative chemotherapy was administred as following: combination of cisplatin and gemcitabin (n=18), gemcitabin (n=7), combination of cisplatin and 5 fluoro uracile (n=5), combination of oxaliplatin and gemcitabin (n=3), capecitabine (n=3), FUFOL Mayo clinic (n=2) and combination of capecitabine and oxaliplatin (n=2). While 9% of the patients received supportive care because of their comorbidities and the deterioration of the performans status and liver blood tests. Conclusions: Because of the silent evolution of BTC, the diagnosis is therefore delayed, the prognosis is poor and treatment is usually palliative. Our results are consistent with those of the literature concerning the epidemiological finding and diagnosis aspects.A multidisciplinary approach is recommended to improve the prognosis of patients with BTC.
ABSTRACT Introduction The management of colorectal cancer in the elderly patients is becoming a major problem of public health. The principles of treatment of colorectal cancer in patients aged 70 years old and over are the same as those of younger ones, but therapeutic indications should be adjusted according to physiological age, general condition, and the existence of co morbidities. Methods In this retrospective study, we reviewed 62 cases of colorectal cancer in patient aged 70 years old and over admitted in the medical oncology unit of HASSAN II university hospital between January 2009 and January 2012. The aim of this study is to determinate epidemiologic, clinical and therapeutic aspects of this disease in the elderly Moroccan population. Results 62 cases of colorectal cancer were diagnosed in patients aged 70 years old and over representing 27, 5% of all colorectal cancers. 70% of our patients are aged between 70 and 74 years old (n= 43), 20% between 75 and 79 years old, and 10% 80 years old and over. 20% of cases had antecedent of cardiovascular disease, and 8 % had a history of cancer family. The average time of diagnosis was 7.3 months, the clinical symptoms were mainly rectal bleeding (53%), abdominal pain (43%). 37 % of the patients were metastatic at the diagnosis (n=23), sites of metastasis were liver in 10 cases (43%), lung in 7cases (30%), peritoneum in 6 cases (26%), lymph nodes in 5 cases (21%). 20% of patients (n=13) presented a stage III disease, stage II was noted in 35% (n=22), and stage I in 6% (n=4). Left resection of the colon was performed in 24 % of cases, right hemi-colectomy was done in 22%, surgery of rectum was performed in 20%, and metastatic liver resection was done in 5% of all metastatic patients. Preoperative concomitant chemo radiotherapy was performed in 13% for lower and middle rectum. Adjuvant chemotherapy based on Capecitabine alone was administrated in 12 cases, and XELOX in 4 cases, with 25% of diarrhea (n=4), 18 % of hand and foot syndrome (n=3), 18% of non-febrile neutropenia (n=3), and 12% of peripheral neuropathy (n=2). 11% of our patients received metastatic chemotherapy based on XELERI+BEVACIZUMAB (n=7) with 2 cases of diarrhea grade 2, 11% patients received chemotherapy based on XELOX with one death secondary to thrombocytopenia grade 4 after first cycle, and 3% chemotherapy with CAPECITABINE alone. We opted for no treatment in 11% of patients (n=7): Cardiac failure in four cases, 2 cases because of deterioration of performance status, and terminal renal failure in one case. Conclusion All patients with colorectal cancer aged 70 years old and over should receive the most appropriate treatment thought to be effective and safe, according to their biological age and co morbidities. Geriatric assessment is essential in the care for elderly patients with colorectal cancer.
