Objective Despite significant savings with biosimilars, their negative perception can lead to the occurrence of a nocebo effect (NE), therefore we aimed to quantify the NE in inflammatory rheumatism after switching from adalimumab or etanercept originators to biosimilars. Methods This retrospective study was conducted in 4 hospitals in Normandy, France between January 2018 and July 2022. The study included patients with rheumatoid arthritis or spondyloarthritis in remission under adalimumab or etanercept originators before switching to biosimilars. The occurrence of a NE was considered in patients who did not maintain biosimilars at 12 months and who presented a subjective adverse event (AE). A comparative analysis of the quantitative data collected before and after switching was performed. The AE that led to biosimilar discontinuation was identified. Additional analyses were performed to identify potential risk factors for the occurrence of a NE. Results Among 183 patients included,13.1% presented a NE. Objective AEs were observed, including rheumatism reactivation (15.3%), intolerance (8.2%), infection (1.6%) and allergic reactions (0.5%). Morning stiffness duration was significantly different before and after the switch in the spondyloarthritis group (p=0.01). No risk factors were associated with the occurrence of a NE within the limits of the studied parameters. Conclusion The occurrence of a NE after switching to a biosimilar remains acceptable. It appears less frequent when the switch is supervised by the practitioner rather than being systematic (up to 33% in some countries). A shared medical decision seems to be essential in a subset of patients, which remains to be defined.
Introduction La pseudo-polyarthrite rhizomélique (PPR) est un des rhumatismes inflammatoires les plus fréquents chez le sujet âgé, caractérisée par un syndrome douloureux des ceintures scapulaires et pelviennes d’horaire inflammatoire. Le traitement standard repose sur les glucocorticoïdes (GC), mais leur utilisation prolongée peut entraîner des effets secondaires graves. L’efficacité des thérapies biologiques ciblant le récepteur de l’interleukine (IL)-6, tels que le tocilizumab (TCZ) et le sarilumab, a ouvert de nouvelles perspectives pour le traitement de la PPR [1] et la SFR a récemment proposé leur recours pour réaliser une épargne cortisonique [2]. L’objectif de notre travail était d’évaluer la proportion de patients sous anti-IL-6R ayant réalisé un sevrage total de corticothérapie à 6 et 12 mois de l’introduction d’un biomédicament anti-IL-6R. Patients et méthodes Les dossiers de patients diagnostiqués avec un rhumatisme inflammatoire des ceintures et traités par des anti-IL-6R dans quatre centres hospitaliers de l’ouest de la France entre janvier 2017 et décembre 2023 ont été analysés de manière rétrospective. Le critère d’évaluation principal était la proportion de patients ayant cessé les GC à 6 et 12 mois après l’initiation du traitement par anti-IL-6R. Les critères secondaires étaient la recherche de facteurs prédictifs de sevrage de corticothérapie et la tolérance des anti-IL-6R. Résultats Trente-six dossiers concernant des PPR traitées par anti-IL-6R ont été sélectionnés. La proportion de patients sevrés en GC était de 47,4 % à 6 mois (p=0,013) et 73 % à 12 mois (p<0,01) de l’initiation de l’anti-IL-6R, en monothérapie ou en association à un csDMARD chez 33 patients. Il y avait une tendance non significative à un moindre sevrage en corticothérapie si la PPR était ancienne (p=0,06). Les schémas thérapeutiques des anti-IL-6R ont été modifiés chez 19 patients (52,8 %) soit par une réduction des doses, soit un espacement progressif des injections ou encore par un relais d’injection intraveineuse à sous-cutanée. Treize patients (36,1 %) ont présenté des effets indésirables, les plus fréquents étant les infections et les leuconeutropénies, nécessitant l’arrêt du biomédicament pour quatre patients. Discussion L’emploi d’un anti-IL-6R permet une épargne cortisonique au cours de la première année chez une majorité de patients souffrant de PPR, confortant les recommandations établies par la SFR. L’évaluation des pratiques professionnelles montrent également des modalités d’utilisation des anti-IL-6R pour la PPR variables selon les objectifs de traitement et effets indésirables rencontrés, avec un profil de sécurité tolérable au sein de cette population âgée. La force de notre étude réside dans son caractère multicentrique ainsi que dans la taille importante de la cohorte. Des études randomisées sur l’efficacité des anti-IL-6R dans ces indications restent à envisager. Conclusion Un traitement par anti-IL-6R augmente l’épargne cortisonique complète chez des patients atteints de PPR au cours de la première année de traitement lors d’un suivi en vie réelle multicentrique.
Background: Changes in certain fine specificities (FS) of ACPA (anti-citrullinated peptide antibodies) and anti-CarP (anti-carbamylated peptide antibodies), whether in the form of reduced antibody titres or negative seroconversion, could be correlated with response to treatment in rheumatoid arthritis (RA). Only one study has focused on the overall level of ACPA, showing that a reduction in titres occurred with anti-lymphocyte drugs such as rituximab and abatacept (ABA) but not with anti-cytokine drugs, in correlation with clinical response [1]. However, the impact of these treatments on the FS of ACPA and anti-CarP has not been evaluated. Objectives: The aim of this study was to analyse changes in the FS titre of 7 IgG and 7 IgA ACPA and anti-CarP antibodies under ABA in a cohort of a biologic naïve RA patients and to determine whether this change in the autoimmune repertoire was related to the prognosis of RA in patients with this treatment. Methods: A multiplexed ALBIA (addressable laser beads immunoassay) immunoassay for the detection of six fine specificities of ACPA (immunodominant epitopes) and one fine specificity of anti-CarP identified by our team [2], was developed. IgG and IgA fine specificities titres at the initiation date of ABA (M0) and at 6 months (M6) of treatment were assessed in a cohort of 59 patients with active ACPA+ RA (DAS-28 > 3.2) treated with methotrexate + ABA. Clinical and biological data for disease activity and systemic inflammation were collected at M0 and M6 of ABA treatment. Linear models explaining the DAS-28 at M6 by the DAS-28 at M0, the percentile of the ACPA level at M0 and the percentile of the fine specificity were used to assess the prognostic value of each fine specificity; a Bonferroni correction (n=14 tests) was applied. Results: 59 patients were included. The median age (interquartile range [IQR]) was 59 (IQR: 46; 64) years, with 42 (79.2%) women. The median ACPA titre was 916 IU (IQR: 346; 1719). Only IgG-class anti-ENO1 titres measured at M0 were significantly associated with prognosis, with an estimated effect of -0.17 (95% CI -0.27 to -0.07, p=0.03 after Bonferroni) DAS-28 points at M6 per additional decile of anti-ENO1 IgG dosage, in the linear model adjusted for ACPA rate at M0 and DAS-28 at M0. The mean (standard deviation [SD]) number of positive fine specificities (cumulative IgA and IgG) (≥ detection limit) was estimated to be 3.08 (3.52) at M0 with a mean (SD) decrease of -0.97 (3.58) at M6. In a linear model explaining DAS-28 at M6 by the number of positive fine specificities at M0, DAS-28 at M0 and the decrease in the number of fine specificities between M0 and M6, the latter effect was estimated at -0.05 (95% CI: -0.16 to +0.06, p=0.36 without Bonferroni) DAS-28 points at M6 for an additional fine specificity negativation between M0 and M6. Conclusion: These results suggest that anti-ENO1 IgG fine specificity may have a prognostic impact in patients treated with ABA, although no conclusions can be made for the other fine specificities; this result remains to be confirmed in a larger population. Introduction L’ostéoporose est une pathologie fréquente caractérisée par une fragilité généralisée du squelette osseux entrainant un sur risque fracturaire. Cette pathologie est un problème de santé publique grandissant dans une population vieillissante. Pour améliorer sa prise en charge, des parcours de soins coordonnés pluridisciplinaires ont été créés telles que les filières fractures. Les filières fractures permettent un meilleur dépistage des patients, du taux d’instauration de traitement anti-ostéoporotique et une diminution de la mortalité. L’objectif de ce travail était de décrire et d’évaluer l’instauration d’une filière fracture au sein du CHU de Rouen. Matériels et méthodes Nous avons conduit une étude observationnelle rétrospective de février 2023 à février 2024 au sein du Chu de Rouen concernant l’ensemble des patients inclus dans la filière fracture. Nous avons analysé les caractéristiques démographiques de notre population ainsi qu’évalué les critères de qualité de notre filière selon les standards émis par l’International Osteoporosis Foundation (Fig. 1, Fig. 2). Résultats 523 patients entre 50 et 80 ans avec une fracture nouvellement diagnostiquée ont été identifiés au CHU de Rouen. Après exclusion, 389 patients (74,38 %) ont été inclus dans la filière fracture. Parmi eux, 121 patients (31,10 %) ont été reçu en consultation et 74 patients (19,02 %) présentaient une ostéoporose et ont eu une indication à l’instauration d’un traitement anti-ostéoporotique. Concernant les patients venus à la consultation, 98 (80,99 %) étaient des femmes d’environ 64,99 ans (±8,4 années). Les fractures les plus fréquentes étaient le poignet (33,88 %) et l’extrémité supérieure de l’humérus (18,18 %). Le taux de non-réponse à l’appel téléphonique était de 42,42 % et le taux de refus de consultation de 21,85 %. Le délai moyen entre l’épisode de fracture et la consultation était de 66jours. Discussion L’évaluation de notre filière fracture rejoint des niveaux moyens concernant le délai entre la survenue de la fracture et la consultation ainsi que pour le taux d’instauration d’un traitement anti-ostéoporotique selon les critères de l’International Osteoporosis Foundation. Le faible taux d’évaluation en consultation est comparable à celui d’autres études dans la première année d’instauration d’une filière mais reste cependant à perfectionner. Conclusion Cette étude met en évidence l’impact bénéfique de l’instauration d’une filière fracture au CHU de Rouen sur la prise en charge de l’ostéoporose mais souligne également les améliorations futures à réaliser comme l’amélioration de l’identification des patients et du taux de participation en consultation. Une prise en charge conjointe avec le service de gériatrie sera instaurée par la suite pour la prévention des chutes.
Introduction Les hématopoïèses clonales sont des anomalies hématologiques dont la fréquence augmente avec l’âge, touchant 10 à 30 % de la population après 70 ans. Ces atteintes des cellules progénitrices de la moelle peuvent être de signification indéterminée (CHIP), induire des cytopénies (CCUS) ou en lien avec une myélodysplasie. Plusieurs cohortes de patients présentant ces anomalies hématologiques ont mis en évidence des manifestations dysimmunitaires, notamment articulaires (jusqu’à 70 %). Toutefois, ces cohortes sont antérieures à la description du syndrome VEXAS où depuis, la mutation UBA1 fait l’objet d’une recherche systématique devant ces tableaux cliniques au prognostic sévère [1]. L’objectif de cette étude est de décrire les manifestations articulaires associées aux hématopoïèses clonales et de comparer celles non VEXAS aux patients atteints de VEXAS. Patients et méthodes Les données de patients issues du Groupement hospitalier Caux-Maritime, des centres hospitaliers Elbeuf-Louviers-Val de Reuil et Eure-Seine, du Groupe hospitalier du Havre, du centre Henri Becquerel (Rouen) et du CHU de Rouen ont été étudiées. Les critères d’inclusion sont la recherche d’une mutation UBA1 entre décembre 2019 et janvier 2023, la présence d’une hématopoïèse clonale parmi CHIP, CCUS, VEXAS et/ou myélodysplasie associée des atteintes cliniques et/ou biologiques dysimmunitaires (Fig. 1, Fig. 2). Résultats Parmi les 67 patients ayant bénéficié d’une recherche de mutation UBA1 entre décembre 2019 et janvier 2023, 30 patients présentaient une anomalie hématologique associée des atteintes cliniques et/ou biologiques dysimmunitaires. Parmi ces 30 patients, 29 patients disposaient de données analysables. Seize patients (55,2 %) ne présentaient pas de mutation UBA1 (non-VEXAS) et 13 patients (44,8 %) présentaient la mutation UBA1 (VEXAS). Douze (75 %) patients non-VEXAS présentaient une atteinte articulaire (arthralgie ou arthrite), contre 4 (30,8 %) dans le groupe VEXAS (Odds-ratio : 6,75, valeur de p : 0,022). Quatre (25 %) patients du groupe non-VEXAS présentaient une arthrite. Trois des quatre analyses de liquide articulaire étaient disponibles : le liquide articulaire était composé principalement de neutrophiles pour 2 patients et de lymphocytes pour un patient. Aucun patient n’a présenté d’arthrite dans le groupe VEXAS. Il n’y avait pas d’atteinte axiale dans notre cohorte. L’atteinte articulaire était associée à une moindre fréquence d’atteinte pulmonaire (Odds-ratio : 0,06 et valeur de p : 0,007) et à un score IPPS-R plus élevé (IPSS-R intermédiaire à élevé : 37,5 % contre 0 %). Parmi les mutations les plus associées aux hématopoïèses clonales avec manifestations auto-immunes [2], des mutations SRSF2 étaient retrouvées chez 2 patients avec atteintes articulaires contre 0 sans, des mutations TET2 chez 3 patients avec atteintes articulaires contre 2 sans, et des mutations IDH2 chez 1 patient avec atteinte articulaire contre 1 sans. Nous n’avons pas retrouvé de mutation IDH1 dans notre cohorte. Les mutations NRAS et ASXL1 étaient les mutations les plus fréquemment associées avec un phénotype articulaire (toutes deux présentes chez 3 patients avec atteintes articulaires, aucun patient sans). Conclusion Ce travail a permis de mettre en évidence une atteinte articulaire plus fréquente, avec plus d’arthrite, chez les patients atteints d’hématopoïèse clonale avec manifestation dysimmunitaire non liées au syndrome VEXAS en comparaison avec ceux atteints du syndrome VEXAS. La spécificité des mutations pouvant provoquer des atteintes articulaires nécessitera la réalisation d’analyses sur une cohorte plus large.
Objective To analyse in routine practice the efficacy of targeted therapies on joint involvement of patients with rheumatoid arthritis/systemic sclerosis (RA/SSc) overlap syndrome. Methods This was a retrospective analysis of medical records of two academic centres over a 10-year period. Joint response to targeted therapies was measured according to EULAR criteria based on Disease Activity Score (DAS)-28. In addition, changes in CRP level and glucocorticoid consumption were recorded. Results Nineteen patients were included. Methotrexate (n=11) and hydroxychloroquine (n=4) were the most used first-line treatments. Targeted therapies were frequently used (n=14). Tocilizumab was the most selected therapy (n=8), then rituximab (n=5), abatacept and anti-tumour necrosis factor (n=4). Twenty-one treatment sequences were assessed, including 18 with EULAR response criteria. Responses were "good" or "moderate" in 100% (4/4) of patients treated with abatacept, 80% (4/5) with rituximab, 40% (2/5) with tocilizumab, and 25% (1/4) with anti-TNF. T and B lymphocyte-targeted therapies (abatacept, rituximab) resulted more frequently in a "good" or "moderate" response compared to cytokine inhibitors (tocilizumab, etanercept, infliximab) with a significant decrease in DAS-28 at 6 months (-1.75; p=0.016) and a trend to a lower consumption of glucocorticoids. Conclusion In patients with RA/SSc overlap syndrome refractory to conventional synthetic-DMARDs, T and B lymphocyte-targeted therapies seem to be a promising therapeutic option to control joint activity.
Background: Extra-articular manifestations occurred in 40% of rheumatoid arthritis (RA) patients. Bronchopulmonary involvement is frequent and is associated with significant morbidity and mortality. High resolution-computed tomography (HR-CT) resulted as the most sensitive tool available to assess it [1]. Levels of autoantibodies, including rheumatoid factors (RF) and anti-cyclic citrullinated peptides (anti-CCP), are associated with lung damage, even subclinical [2,3]. However, there are no data about the potential link between the different isotypes of RF, notably IgA and lung manifestations, while the production of such autoantibodies occurs early in the preclinical phase of the disease. Objectives: To analyze the association between IgA RF and bronchopulmonary involvement in RA patients, considered alone or in combination with other autoantibodies. Methods: This retrospective monocentric cohort study included RA patients with IgA RF tested between 2009 and 2019. All data were collected from medical records. Patients' HR-CT have been analyzed by two radiologists to differentiate between parenchymal and bronchial lung damage. All discordant interpretations were adjudicated by an expert pulmonologist. Statistical analysis was carried out using an exact Fisher's test, supplemented by an adjusted logistic regression model. Results: Out of 2384 IgA RF assays performed, 183 patients were included in the study and separated into 2 groups according to IgA RF status (75 positive and 108 negative). The mean age of patients was 61 ±16 years and 72.1% were female. IgA RF+ patients were mostly comprised of male smokers, undergoing biologic treatments, and presented elevated levels of both RF IgM and anti-CCP. Overall lung damage was observed in 20.2% of the patients, from whom 14.5% and 8.7% had parenchymal and bronchial damage, respectively. No correlation was found between overall lung damage and IgA RF positive status (p = 0.19). While there was an association between concomitant positivity of anti-CCP and RF, whatever the isotype IgA or IgM, and parenchymal involvement, it was not found after adjustment. Older age and male gender seemed to be the only risk factors for overall lung damage, and age being the only factor associated to parenchymal damage. Conclusion: Our study did not show an association between the positivity/titers of IgA RF and bronchopulmonary involvement of RA. Until now, despite conflicting results, only high anti-CCP titers, and potentially presence of anti-carbamylated protein antibodies, have been found to be associated with risk of lung damage. Background: Female spondyloarthritis (SpA) has distinctive features compared with male SpA, and 18 to 28% of these SpAs occur in the post-partum period [1]. Post-partum is associated with musculoskeletal remodelling and IL-17 secreting cells activation, two key factors in the development of SpA [2]. We hypothesized that post-partum SpA might manifest as predominant enthesitic involvement and be less dependent on HLA-B27 and TNFa. Objectives: To investigate the characteristics of SpA occurring in the post-partum period and their response to anti-TNFa. Methods: We performed́ a case-control, retrospective, observational, monocentric study using Rouen University Hospital's data warehouse. Dates of delivery and symptom onset were available for 154 SpA patients. Characteristics were compared between our cases (SpA occurring in the post-partum period) and controls (SpA occurring remote from delivery). Next, data from the French National Health Data System (NHDS) database were cross-referenced with the national birth register to identify SpA that were reported in the post-partum period. Data from January 1, 2017 to January 1, 2021 were analysed. Results: We We identified 58 cases and 96 controls. Post-partum SpA were proportionally less associated with HLA-B27 (37.3% vs. 52.3%, p-value: 0.040), had more anterior chest wall pain (74.1% vs. 54.7%, p-value: 0.008) and psoriasis (37.9% vs. 16.8%, p-value: 0.006). They were just as likely as controls to have a biological inflammatory syndrome at the time of diagnosis, but less so at the time of introduction of the first bDMARD (22.2% vs. 48.1%, p-value: 0.003). Regarding sacro-iliac joints, there was less radiographic damage (4% vs. 18.6%, p-value: 0.009) and MRI sacro-iliitis (30.6% vs. 47.4%, p-value: 0.033) in post-partum SpA. Anti-TNFa retention rate in the post-partum group was 45.6% at 12 months, versus 71.2% in our controls (Odd ratio: 2.56 [95% CI: 1.34 - 4.95]). Multivariate survival analysis found a hazard ratio for bDMARD discontinuation 2.43 (95% CI: 1.62 - 3.65) within the first two years of treatment. In the NHDS, 423 patients were identified as post-partum SpA between 2017 and 2021 and they received 560 anti-TNFa. In these women, retention rate of anti-TNFa was low with 51% (95% CI: 0.47 – 0.55) of patients still taking anti-TNFa 6 months after the initiation and 30% (95% CI: 0.26 – 0.34) after 12 months. Conclusion: Our study highlighted a phenotype singular to post-partum SpA with more anterior chest wall pain, more psoriasis and less association with HLA-B27 and less MRI damage. While the non-radiographic SpA retention rate for anti-TNFa is 73% [3], the rates of 45.6% on the post-partum SpA from the retrospective cohort and 30% from the NHDS suggest a poorer response to these treatments. Introduction La double positivité ACPA (anti-protéines citrullinées)/anti-CarP (anti-protéines carbamylées) semble être associée à un pronostic plus sombre de la polyarthrite rhumatoïde (PR). Les cibles antigéniques in situ des anti-CarP étant encore inconnues, du sérum de veau fœtal (SVF) carbamylé a été jusqu’à présent utilisé comme cible pour les dosages ELISA. Les antigènes cibles des anti-CarP pourraient être différents selon le statut ACPA des patients. Ainsi, Jones et coll. ont montré que chez des patients ACPA-, les anti-CarP reconnaissaient préférentiellement la chaine β du fibrinogène carbamylé [1]. Nos précédents travaux ont permis d’identifier la chaine ϒ du fibrinogène comme un antigène spécifique de la réponse anticorps anti-fibrinogène carbamylé (ACa-Fib). Nous avons montré que la présence des ACa-Fib IgG semblait associée à une inflammation systémique tandis que le pronostic structural sombre restait associé aux ACPA chez les patients de la cohorte régionale VerA [2]. Truchetet et coll. ayant montré des atteintes radiologiques plus sévères chez les patients PR ACPA+/anti-CarP+(SVF) de la cohorte nationale ESPOIR à un temps donné [3]. Notre objectif est de confirmer nos Résultats dans la cohorte ESPOIR et de déterminer la signification clinique des ACa-Fib IgG et IgA à différents temps, comparativement aux ACPA. Un objectif secondaire étant l’identification des épitopes immunodominants reconnus par les ACa-Fib et d’étudier leur réactivité vis-à-vis de ces épitopes, comparativement aux ACPA. Matériels et méthodes Nous avons mis au point des dosages sériques par ELISA pour la détection des ACa-Fib IgG et IgA chez les patients de la cohorte ESPOIR, et ainsi évalué leurs valeurs diagnostique et pronostique. Nous avons également réalisé un mapping épitopique sur la chaine α du fibrinogène carbamylé (15 peptides 27-Mer chevauchants sur 3 acides aminés) pour la reconnaissance des ACa-Fib IgG. Résultats 28 % des patients atteints de PR de la cohorte ESPOIR (n=575) sont positifs pour les ACa-Fib IgG et 8 % pour les ACa-Fib IgA. La prévalence des ACa-Fib IgG diminue après 2 ans (11 %) mais reste stable pour les ACa-Fib IgA (6 %). Parmi les patients ACPA-, 17 % d’entre eux sont ACa-Fib+ pour les IgG et 6 % pour les IgA, démontrant un intérêt diagnostique pour les ACa-Fib. Si la positivité ACa-Fib IgG ou IgA ne semble pas impacter l’activité de la PR, nous montrons que la double positivité ACPA/ACa-Fib IgG est associée à un pronostic structural significativement plus sévère à 2 ans comparativement aux ACPA seuls (p-value=0,0125). Nous montrons également la valeur pronostique des ACa-Fib IgG et IgA avec un pourcentage plus important de patients ayant une progression radiologique rapide lorsque ceux-ci possèdent une double positivité ACPA/ACa-Fib, comparativement aux ACPA seuls (42 % pour les patients ACPA+/ACa-Fib+ IgG contre 27 % pour les ACPA+ ; 50 % pour les ACPA+/ACa-Fib+ IgA contre 31 % pour les ACPA+). Enfin, nous avons mis en évidence, à l’aide de sérums de patients PR ACa-Fib+ IgG, 3 épitopes immunodominants sur la chaine α du fibrinogène carbamylé, avec une cross-réaction très faible, voire nulle, de la réponse anti-fibrinogène citrulliné. Ceci renforçant leur intérêt diagnostique dans la PR ACPA-. Conclusion Les ACa-Fib IgG et IgA possèdent un intérêt diagnostique chez les patients PR ACPA-, et également une valeur pronostique chez les patients ACPA+ constituant un facteur de gravité supplémentaire. De plus, la réponse auto-immune IgG contre le fibrinogène carbamylé reconnaît majoritairement des épitopes immunodominants sans cross-réaction avec la réponse citrullinée, renforçant leur intérêt diagnostique.
Background Inception cohorts can be defined as long term follow-up of patients suffering at entry visit from a recent onset disease. Despite their huge advantage to better approach the truth in terms of long term prognosis of the disease, they are usually facing 2 issues: a) the possibility of a change in the entry visit (initial) diagnosis b) the missing data due to patients lost of follow-up. Objectives To evaluate both the frequency and the baseline predisposing factors of a) a change in the initial diagnosis b) the risk of loss of follow-up in the DESIR axSpA cohort. Methods Study design:DESIR is an ongoing (10 year follow-up completed for all the patients) multicenter cohort of recent onset axSpA. Diagnosis: At entry visit and during the 10 year follow-up period, this diagnosis was based on the opinion of the treating rheumatologist with the requirement of a highly suspected diagnosis of axSpA at entry visit and after the first 2 years of follow-up the possibility to exclude the patients in case of a change in the initial axSpA diagnosis. Statistical analysis: Multiple imputation was used to address missing data and estimates the probabilities of a change in initial axSpA diagnosis for each patient lost of follow up. Predisposing factors of an unchanged initial axSpA diagnosis were then evaluated using a multivariate logistic regression model on imputed data sets. A multivariate cox survival analysis exploring factors associated to the overtime risk of loss of follow-up was also performed. Results Of the 708 enrolled patients, 45 were excluded from the cohort because of a documented change in the initial axSpA diagnosis (mechanical low back pain n = 30, fibromyalgia n = 13, chronic inflammatory rheumatic disease (n = 1 and no information n = 1).The classification criteria were fulfilled by 16/45 (36%) versus 413/663 (63%) and 21/45 (47%) versus 522/663 (81%) patients with a documented change versus no change in their entry visit diagnosis according to the ASAS ax-SpA and the AMOR criteria respectively. During the 10 year follow-up period, 300 patients were lost of follow-up. Based on imputation, among these 300 patients, 19 patients were systematically suspected to have a change in their initial axSpA diagnosis in all imputations while 173 patients were never “suspected” for this change; 42 patients were considered as suspected for a change in their initial axSpA diagnosis in at least 70% of imputations. Predisposing factors of an unchanged initial axSpA diagnosis were (odds ratio [95% CI]): radiographic SIJ structural damage: 17.0 [4.1; 71.0]; past or present psoriasis: 5.3 [2.0; 14.3]; CRP ≥ 6 mg/l: 2.7 [1.3; 5.3]; good response to NSAID: 2.5 [1.5; 4.2]; HLA B27 positivity: 2.0 [1.3; 3.3]; anterior chest wall pain: 2.0 [1.2; 3.3] and female gender: 1.9 [1.2; 3.0]. Predisposing factors of the risk of loss of follow-up were: Age at back pain initiation< 45 years old: 1.8 [1.2; 2.9]; CRP < 6 mg/l: 1.5 [1.1; 1.9]; HLA B27 negativity: 1.4 [1.1; 1.8]; educational level < university: 1.4 [1.1; 1.8]; smoker: 1.3 [1.0; 1.6]. Conclusion These data suggest that a) a change in the entry visit diagnosis and the risk of follow-up have to be considered in inception cohorts b) statistical models including multiple imputations could facilitate the evaluation of long term prognosis of the disease. Introduction. - Prevalence of common low back pain is constantly increasing, especially among hospital staff. Numerous studies have highlighted the effectiveness of exercise programs in prevention and treatment of low back pain. The objective was to evaluate factors associated with six-month effectiveness of an exercise program in the prevention of common low back pain in hospital staff.Methods. - 67 hospital workers with or without low back pain were included in a single-center standardized program. They benefited from a supervised group educational intervention with learning of exercises, then performed at home for six months. The following items were evaluated: VAS pain at rest and during professional activity, occurrence of low back pain, sick-leave and work-accidents, FABQ, DALLAS, RMDQ, OMSPQ, Oswestry, BORG questionnaires, finger-to-foot distance, maximum core stability by planking time. The main endpoint was the improvement of VAS lumbar pain during professional activity at 6 months.Results. - At six months, 54.7% of the subjects showed an improvement of more than 50% in their VAS for low back pain during work activity. There was a significant reduction in the occurrence of low back pain, sick leave, degree of discomfort felt during work activity and the FABQ score (P < 0.001). Spinal flexibility and muscular endurance were significantly improved (P < 0.001). 43% of the subjects completed the program.Conclusion. - An exercise program combined with a therapeutic education session can improve low back pain experienced during work activity among hospital staff. Compliance could be improved by intermediate visits and ongoing supervision followed.
Le coût du traitement des principaux rhumatismes inflammatoires chroniques (RIC) a connu un bond en avant depuis l’avènement des biothérapies. L’apparition des premiers biosimilaires a cependant permis de réaliser d’importantes économies de santé. Toutefois, la perception parfois négative de ces traitements chez les patients et le praticien peut donner lieu à l’apparition d’un effet nocébo. L’objectif de ce travail a été de mesurer la prévalence de l’effet nocébo au décours du relais d’un princeps de l’adalimumab (ADA) ou de l’étanercept (ETA) vers un de leurs biosimilaires. Au total, 183 patients issus de 4 centres hospitaliers Normands (CHU de Rouen et de Caen, hôpitaux du Havre et d’Évreux) ont été inclus entre janvier 2018 et juillet 2022 dans cette étude rétrospective, dans le cadre de l’expérimentation sur les biosimilaires selon l’Article 51. Ils étaient atteints d’un RIC (PR ou SPA) en rémission, traités par biomédicament princeps de l’ADA ou de l’ETA et ayant bénéficié d’un switch vers un biosimilaire. La survenue d’un effet nocébo a été considérée chez les patients n’ayant pas maintenu le traitement à 12 mois et ayant présenté un effet secondaire subjectif. Les effets secondaires ayant motivé l’arrêt du biosimilaire ont été identifiés. Une analyse comparative des données quantitatives récoltées a été effectuée avant et après le switch. Enfin, une analyse univariée puis multivariée a été réalisée à la recherche de potentiels facteurs de risque associés à la survenue d’un effet nocébo. L’effet nocébo a été mesuré chez 13,1 % des patients. On notait par ailleurs que 9,2 % de l’effectif avait présenté un effet secondaire objectivé par le rhumatologue, et 1,6 % des effets secondaires mixtes. Ont été considérés comme effets secondaires subjectifs la sensation de recrudescence rhumatismale sans élément objectif retrouvé, ainsi qu’un certain nombre de plaintes. On comptait par ailleurs 8,2 % de patient ayant présenté une intolérance, 1,6 % un évènement infectieux, et 0,5 % une réaction allergique. Concernant l’évolution sous traitement, l’analyse comparative ne montrait pas de différence significative avant et après le switch pour les paramètres mesurés, en dehors de la durée du dérouillage matinal dans le groupe SPA (p = 0,01). L’analyse univariée puis multivariée n’a pas identifié de facteurs de risques associé à la survenue d’un effet nocébo. La survenue d’un effet nocébo au décours d’un switch vers un biosimilaire reste acceptable au vu des économies de santé effectuées, et s’avère moins fréquente lorsque la permutation est encadrée par le praticien, par opposition aux rares données de la littérature dont les résultats diffèrent lorsque la substitution est systématique comme dans certains pays (jusqu’à 33 %). La survenue d’un tel effet peut s’expliquer par une mauvaise perception du patient. L’éducation thérapeutique ainsi qu’une décision médicale partagée entre patient, rhumatologue mais aussi pharmacien semble indispensable chez une certaine population de malades qui reste toutefois à définir.
La prévalence des lombalgies communes est en constante augmentation, notamment au sein du personnel hospitalier. De nombreuses études ont mis en évidence l'efficacité des programmes d'exercices dans la prévention et le traitement des lombalgies. L'objectif était d'évaluer les facteurs associés à l'efficacité à six mois d'un programme d'exercices sur la prévention des lombalgies communes du personnel hospitalier. Soixante-sept travailleurs hospitaliers présentant ou non des lombalgies ont été inclus dans un programme standardisé monocentrique. Ils bénéficiaient d'une intervention éducative en groupe supervisée avec apprentissage des exercices, ensuite réalisés à domicile pendant six mois. Étaient évalués : EVA douleur au repos et à l'effort, survenue de lombalgie, d'arrêts et d'accidents de travail, questionnaires FABQ, DALLAS, RMDQ, OMSPQ, Oswestry, BORG, distance doigt-sol, temps maximal de gainage en planche. Le critère de jugement principal était l'amélioration de l'EVA douleur lors de l'activité professionnelle à 6 mois. À six mois, 54,7 % des sujets présentaient une amélioration de plus de 50 % de leur EVA douleur lombaire lors de l'activité professionnelle. Il existait une diminution significative de la survenue de lombalgies, d'arrêts de travail, de la pénibilité ressentie lors de l'activité professionnelle, du score FABQ, ainsi qu'une amélioration de la souplesse rachidienne et de l'endurance musculaire (p < 0,001). Quarante-trois pour cent des sujets ont suivi le programme jusqu'au bout. Un programme d'exercices associé à une séance d'éducation thérapeutique permet d'améliorer la douleur lombaire ressentie lors de l'activité professionnelle chez le personnel hospitalier. Background Tofacitinib, an oral JAK inhibitor, is indicated for the treatment of adult patients with moderate to severe active rheumatoid arthritis (RA) for which we have no real-life effectiveness data in a French RA population. Objectives To describe tofacitinib effectiveness profile according to concomitant use of csDMARD in a French prospective observational study, DeFacTo. Methods DeFacTo, is an ongoing observational study with the primary objective of identifying predictive factors of Tofacitinib maintenance in real-life RA patients. The results described here are based on a descriptive interim analysis of its effectiveness at 18 months. Results Among 314 pts enrolled in the study, 301 (POP1) were included in this effectiveness analysis, including 274 (POP2) patients with follow-up ≥ 18 months on 03/15/2022 (date of analysis) with a median exposure duration of 538 [Q1; Q3: 381; 554] days. At baseline, among POP2: 168 (POP3) patients were treated with tofacitinib used concomitantly with at least one csDMARDs vs 106 (POP4) tofacitinib monotherapy. In POP3 vs POP4 respectively, 76.8% vs 84.9% were female, with a mean (± SD) age of 59.5 (10.8) vs 59.9 (12.1), median disease duration of 8.6 years [Q1;Q3: 3.2; 18.6] vs 11.1 [Q1;Q3: 5.3; 19.4)], 40.3% vs 52.4% patients had erosions, 80.5% vs 76.5% had FR+ and 77.7% vs 75.0% had ACPA+. Median CRP was 7.0 [Q1;Q3: 2.9; 18.0] vs 8.0 [Q1;Q3: 3.0; 18.1], and mean DAS28-4 CRP was 4.4 ±1.1 vs 4.7 ±1.0, in POP3 and POP4 respectively. Result showed a decrease in inflammatory markers, pain, and DAS28-CRP activity scores in both groups. At 18 months of follow-up, 24.4% and 24.5% of the pts achieved DAS28-CRP remission in POP 3 and POP 4 respectively (Table 1). Note that missing data and treatment discontinuation were considered as failures which resulted in an underestimation of LDA and remission at 18 months. Background Tofacitinib, an oral Janus Kinase inhibitor, is indicated for the treatment of adult patients with moderate to severe active rheumatoid arthritis (RA). Objectives To describe the safety profile of Tofacitinib in a French prospective observational study DeFacTo. Methods The safety profile of tofacitinib was assessed using interim data from a descriptive analysis of patients who took at least one dose of tofacitinib in the DeFacTo study. Results Among 314 patients enrolled in the study, 306 received tofacitinib and were included in the safety analysis, including 274 patients with a follow-up ≥ 18 months (POP2) on 03/15/22 date of analysis) with a median exposure duration of 538 [Q1; Q3: 381; 554] days. 113 patients (41.2%) were still on tofacitinib (39 discontinued, 122 missing data regarding prescription). At inclusion, 78.3% of the 306 pts were women with a mean (± standard deviation) age of 59.5 ± 11.5 years, median disease duration of 8.9 years [Q1; Q3: 4.1; 18.9]. There was a history of cardiovascular (CV) disease in 11.9% of cases (including 4.6% myocardial infarction (MI), 5.0% stroke/transient ischemic attack (TIA), 1.7% heart failure, and 1.7% peripheral arterial disease); 5.6% history of cancer, 16.8% history of infection, and 46.6% smoker/former smoker. Tofacitinib was prescribed in combination with a csDMARD in 61.1% of patients and corticosteroids in 56.2% of cases. The results showed that adverse events (AEs) were reported in 54.2% of the 306 patients, 14.4% were considered as serious. Infections were found in 22.2% of patients, no deaths were reported (Table 1). Conclusion These intermediate, descriptive results show a safety profile of tofacitinib in real-life RA similar to the one previously reported in clinical and observational studies. L’étude ORAL-SURVEILLANCE [1] a mis en avant un surrisque potentiel d’événements cardiovasculaires chez certains sujets traités pour une polyarthrite rhumatoïde et par tofacitinib comparativement aux biomédicaments anti-TNFα. Récemment toute la classe des inhibiteurs de Januskinases utilisés dans le traitement des maladies inflammatoires a fait l’objet d’une réévaluation globale de leur rapport bénéfices/risques. La Société française de rhumatologie a mis en place un groupe de travail qui en décembre 2022 a préconisé sur ce sujet 3 principes généraux et 11 recommandations. Les propositions de ce groupe de travail ont fait l’objet d’une analyse pragmatique. L’objectif principal étant de préciser leur application en pratique quotidienne. Il s’agit d’un travail de consensus d’experts ayant impliqué 8 rhumatologues universitaires et 1 cardiologue. Les modifications et/ou précisions apportées reposent sur une technique de consensus à l’unanimité et dans l’optique de préciser certains points concernant la prescription des JAKi avant et après les recommandations du PRAC. Quatre réunions présentielles ou distancielles se sont tenues entre le 17 juin 2022 et le 13 avril 2023. Sur les principes généraux, les experts reconnaissent ce rôle central du rhumatologue dans l’évaluation du risque cardiovasculaire. Ils proposent une check-list des principaux facteurs de risque d’accidents cardiovasculaires et conduisant à l’évaluation d’un niveau de risque notamment via l’application de l’équation score 2. Les experts s’interrogent quant à la pertinence actuelle du maintien du facteur multiplicatif de 1,5. (moindre sévérité actuelle de la maladie). Les experts proposent d’y associer l’implication du rhumatologue dans la vérification de la prise en charge même de ces principaux facteurs de risque (contrôle tensionnel, normalisation du LDL…). Les experts envisagent cette évaluation dès l’initiation de la 1re thérapie ciblée quelle qu’elle soit, puis une potentielle réévaluation annuelle. La recommandation sur le contrôle d’activité de la maladie pourrait préciser l’absence de synovite active et surtout la normalisation de la CRP. Une consultation de tabacologie devrait être proposée chez tout sujet fumeur. L’épargne cortisonique devrait conduire à envisager une possible modification de la stratégie de traitement de fond. En cas d’antécédents CV un suivi spécialisé s’impose. Conformément aux recommandations la présence d’antécédents de MACE/VTE et/ou de plusieurs facteurs de risque majeurs doit bien faire envisager une autre classe de thérapie ciblée. Pour la problématique des VTE il s’agit d’experts qui prennent en charge les maladies thromboemboliques. Des précisions complémentaires pourraient être apportées concernant les modalités de la thromboprophylaxie en présence de facteurs transitoires a fortiori permanents (déficit en protéine S…) et de prise conjointe des JAKi. Ce travail complémentaire devrait favoriser la diffusion et application de ces recommandations sur l’évaluation et la prévention des risques cardiovasculaires notamment à l’initiation d’un inhibiteur de Januskinases.
Les Spondyloarthrites (SpA) sont un groupe de rhumatismes inflammatoires aux formes cliniques variées. Leur sexe ratio, historiquement estimé à 1 femme pour 10 hommes, semble tendre vers l’équilibre. Toutefois, la spondylarthrite de la femme présente des spécificités, comme une moindre association au HLA-B27, des atteintes enthésitiques sévères plus fréquentes et une moins bonne réponse aux anti-TNFa. 18 à 28% de ces SpA surviennent dans la période du péri-partum. Cette période est marquée par un remodelage musculosquelettique via des hormones sexuelles ou non (comme la relaxine) et des métalloprotéases. L’implication de l’IL-17 dans l’accouchement est bien décrite. Nous avons donc émis l’hypothèse que les SpA du péri-partum pourraient se manifester par des atteintes enthésitiques prédominantes et qu’elles seraient moins dépendantes du HLA-B27 et du TNFa. Nous avons réalisé une étude cas-témoin, rétrospective, observationnelle et monocentrique à partir de l’entrepôt de données de notre établissement. Les dates d’accouchement et de début des symptômes étaient disponibles pour 154 patientes atteintes de SpA. Les caractéristiques cliniques, biologiques et celles des imageries ont été comparées entre nos cas (SpA survenant dans le péri-partum) et nos témoins (SpA survenant à distance de l’accouchement). La maintenance thérapeutique des anti-TNFa a été évaluée dans les deux groupes à l’aide d’un modèle de survie multivarié, puis d’un modèle stratifié sur la présence d’un critère objectif d’inflammation. Nous avons identifié 58 cas et 96 témoins. Les SpA du péri-partum étaient proportionnellement moins associées au HLA-B27 (37,3 % contre 52,3 %, valeur de p : 0,040), présentaient plus de syndrome thoracique antérieur (74,1 % contre 54,7 %, valeur de p : 0,008) et de psoriasis (37,9 % contre 16,8 %, valeur de p : 0,006). Elles présentaient autant de syndrome inflammatoire biologique au moment du diagnostic, mais moins au moment de l’introduction de la première biothérapie (22,2 % contre 48,1 %, valeur de p : 0,003). On retrouvait également moins de sacro-iliite radiographique (4 % contre 18,6 %, valeur de p : 0,009) et magnétique (30,6 % contre 47,4 %, valeur de p : 0,033) chez les SpA du péri-partum. 100 % des patientes remplissaient les critères ESSG, mais seulement 38 % des SpA du péri-partum satisfaisaient les critères ASAS axiaux contre 71,6 % des témoins (valeur de p : 0,002). Le maintien thérapeutique des anti-TNFa dans le groupe péri-partum était de 45,6 % à 12 mois, contre 71,2 % chez nos témoins (Odd ratio : 2,56 [IC 95 % : 1,34–4,95]). L’analyse de survie prenant en considération la présence d’une sacro-iliite IRM et d’un syndrome inflammatoire biologique à l’introduction de la biothérapie a retrouvé un hazard ratio de 2,18 (IC 95 % : 1,47–3,22) comme risque d’arrêt de l’antiTNFa dans les deux premières années de traitement. L’IL-17 pourrait être un des facteurs centraux de l’entrée dans la maladie dans le péri-partum. Nous n’avons pas pu étudier l’efficacité des anti-IL17 dans notre cohorte, toutefois son potentiel thérapeutique incite à y consacrer un futur travail. Notre étude a mis en évidence un phénotype singulier aux SpA du péri-partum, ainsi qu’un maintien thérapeutique des anti-TNFa plus court suggérant une moins bonne réponse à ces traitements.
BackgroundTofacitinib, an oral Janus Kinase inhibitor, is indicated in the treatment of adult patients (pts) with active, moderate to severe rheumatoid arthritis (RA). At the present time we have no data concerning its efficacy in a French RA population in a real-life setting.ObjectivesDeFacTo, is an on-going observational study the principal goal of which is to identify factors predictive of Tofacitinib drug survival in RA patients in real life.MethodsThese are the results of interim descriptive analyses of effectiveness data after 1 year of follow up.ResultsOf the 313 pts enrolled in the study, 276 pts were included in the effectiveness analysis (POP1), 219 of whom were patients with a ≥1-year follow-up (POP2) including 122 patients who, as of 15 March 2021, were continuing to take Tofacitinib therapy. At inclusion, POP1 (n=276) was composed of 77.9% females of mean (± SD) age 59.7 ± 11.7 years, having a median disease duration of 9.1 years [Q1;Q3: 4.1; 19.2], TJC 7.5 ± 5.9 and SJC 5.3 ± 5.1; 115 patients presented with erosions, 206 had RF+ and 202 had ACPA+. The median CRP value was 7.4 [: 3.0; 20.0] mg/l, DAS28-4-CRP was 4.6 ±1.1, and the FACIT-Fatigue score was 28.3 ± 11.3. In all, 96.7% of the pts had received prior treatment with ≥1 csDMARDs, 65.2% with b/tsDMARDs (median = 2 [1; 4]). Tofacitinib was prescribed in combination with a csDMARD in 58.3% of cases. The baseline characteristics of the 122 patients still taking tofacitinib at 12 months were comparable to those of POP1. Results showed continued treatment with tofacitinib in 85.3% of pts with mean duration of treatment of 368 ± 197.1 days and reductions from baseline in the parameters of inflammatory markers, pain and the DAS28 activity scores; 20% of 122 pts were in DAS28-CRP remission (Table 1). Background The information related to the long term outcome of a disease is of huge interest for the clinicians in daily practice to facilitate the information of the patients recently diagnosed as axSpA. The data from inception cohorts are better approaching the truth of this long term prognosis in comparison to the conventional retrospective cohorts mainly conducted in patients with advanced and severe disease managed in tertiary referral centers. Objectives To evaluate the 10 year clinical outcome (and its predisposing factors) of patients suffering from recent onset axSpA in the DESIR cohort. Methods Study design: DESIR is an ongoing (10 year follow-up completed for all the patients) multicenter cohort of recent onset axSpA. Diagnosis: At entry visit and during the 10 year follow-up period, the diagnosis was based on the opinion of the treating physician with a requirement of a diagnosis of axSpA at entry and the possibility to exclude the patients after the first 2 years follow-up period in case of a change in this diagnosis. Management during the 10 year period: the investigators were in charge of the data collection required by the protocol but the management (treatment regimens) was only based on the decision of the treating rheumatologist. Statistical analysis: Data presented here are the ones issued a)from the analyses on the completers (observed data) and b) after multiple imputations considering the missing data due to the patients lost of follow-up (imputed data). Results Of the 708 enrolled patients, 45 were excluded from the cohort because of a change in the entry visit diagnosis, 3 patients died (suicide n = 1, colorectal cancer n = 1, sudden death n=1), 300 were lost of follow-up and 360 patients completed the 10 year period. A -Based on the analyses of the 10 year completers (n=360) No patient necessitated a spinal vertebrotomy, one single patient had a bilateral total hip replacement. A pension from the national health care system was provided to 16% patients because an invalidity related to the axSpA disease. A csDMARD (methotrexate and/or sulfasalazine) has been prescribed in 32% and a biotherapy in 55%. The prevalence of the main extra-musculoskeletal features increased from 18 to 30%, 10 to 18% and 5 to 10% from baseline to year 10 for psoriasis, acute anterior uveitis and inflammatory bowel disease respectively. The prevalence of the main comorbidities increased from 3 to 8%, 0 to 3%, 5 to 15%, 0 to 4%, 1 to 3% and 0 to 2%, from baseline to year 10 respectively for severe GI events, MACE, hypertension, diabetes, tuberculosis and other severe infections respectively. B- Based on the analyses of the 10 year completers (n=360) (observed data) and the 663 patients with unchanged initial axSpA diagnosis (imputed data) An acceptable status at year 10 was observed in 77%,70 [63; 77]%,;49% 43 [37; 49]%; 55% 48 [41; 56]% considering an acceptable PASS, BASDAI < 30, ASDAS < 2.1 for the observed (%) and imputed (% and [95%CI]) data respectively. The impact of the disease on the daily life of the patients was evaluated by different parameters: ASAS HI ≤ 5 (41 [35; 47]%, SF36 physical score (42 [40; 44]) and SF36 mental score (43 [40; 46]). The multivariate analysis of the baseline predisposing factors of an acceptable status at year 10 defined as an ASDAS score<2.1 picked-up the following variables: short (<1.5year)delay between the first symptoms and the baseline visit: OR= 1.46[0.93;2.29],socio-professional level (white collar): OR=1.87[1.20;2.90] and baseline BASDAI score<40, OR=1.91[1.23;2.94] Conclusion: These data are suggesting a favorable 10 year outcome in terms of stringent measures such as the requirement to surgery contrasting with a relatively less favorable outcome with regard to patient reported outcomes. These data should improve and facilitate the information provided to the patients at the time of their diagnosis. Contexte : les cohortes d’inclusion ou de démarrage (inception cohort en anglais) peuvent être définies comme le suivi à long terme de patients souffrant d’une maladie récente au moment de la visite d’inclusion. Malgré leur énorme avantage pour mieux approcher la vérité en terme de pronostic à long terme de la maladie, elles sont généralement confrontées à deux problèmes : (a) la possibilité d’un changement dans le diagnostic initial (b) les données manquantes dues aux patients perdus de vue. Évaluer à la fois la fréquence et les facteurs prédisposant (a) d’un changement du diagnostic initial (b) du risque de perte de suivi dans la cohorte DESIR axSpA. Conception de l’étude : DESIR est une cohorte multicentrique en cours (10 ans de suivi pour tous les patients) d’axSpA d’apparition récente. Diagnostic : Lors de la visite d’entrée et au cours de la période de suivi de 10 ans, ce diagnostic a été basé sur l’opinion du rhumatologue traitant avec l’exigence d’un diagnostic fortement suspecté d’axSpA lors de la visite d’entrée et, après les 2 premières années de suivi, la possibilité d’exclure les patients en cas de changement de ce diagnostic initial. Analyse statistique : Imputation multiple pour traiter les données manquantes et estimer la probabilité d’un changement du diagnostic initial d’axSpA pour chaque patient perdu de vue. Modèle de régression logistique multivariée pour évaluer les facteurs prédisposant à un diagnostic initial inchangé d’axSpA. Enfin, analyse de survie cox multivariée pour explorer les facteurs associés au risque de perte de suivi dans la cohorte. Sur les 708 patients inclus, 45 ont été exclus de la cohorte au cours du suivi en raison d’un changement du diagnostic initial d’axSpA (lombalgie mécanique n = 30, fibromyalgie n = 13, rhumatisme inflammatoire chronique n = 1 et pas d’information n = 1). À la visite d’inclusion, les critères de classification (ASAS/AMOR) étaient satisfaits pour 36 %/47 % des 45 malades sortis de la cohorte pour un autre diagnostic versus 63 %/81 % des 663 patients sans changement de diagnostic initial. Au cours de la période de suivi de 10 ans, 300 patients ont été perdus de vue. Sur la base de l’imputation, parmi ces 300 patients, 19 patients ont été « suspectés » d’avoir un changement dans leur diagnostic initial d’axSpA dans toutes les imputations, tandis que 173 patients n’ont jamais été « suspectés » de ce changement ; 42 patients ont été considérés comme suspectés d’avoir un changement dans leur diagnostic initial d’axSpA dans au moins 70 % des imputations. Les facteurs prédisposant à un diagnostic initial inchangé d’axSpA au cours du suivi étaient (Odds ratio [IC 95 %]) : lésions structurelles radiographiques de l’articulation sacro-iliaque : 17,0 [4,1 ; 71,0] ; psoriasis passé ou présent : 5,3 [2,0 ; 14,3] ; CRP ≥ 6 mg/L : 2,7 [1,3 ; 5,3] ; bonne réponse aux AINS : 2,5 [1,5 ; 4,2] ; positivité HLA B27 : 2,0 [1,3 ; 3,3] ; douleur antérieure de la paroi thoracique : 2,0 [1,2 ; 3,3] et sexe féminin : 1,9 [1,2 ; 3,0]. Les facteurs prédisposant au risque de perte de suivi étaient les suivants : age au début du mal de dos < 45 ans : 1,8 [1,2 ; 2,9] ; CRP < 6 mg/L : 1,5 [1,1 ; 1,9] ; HLA B27 négativité : 1,4 [1,1 ; 1,8] ; niveau d’éducation < université : 1,4 [1,1 ; 1,8] ; fumeur : 1,3 [1,0 ; 1,6]. Ces données suggèrent que a) un changement de diagnostic de la visite d’inclusion et le risque de perte de suivi doivent être pris en compte dans les cohortes de démarrage b) des modèles statistiques incluant des imputations multiples facilitent l’évaluation du pronostic à long terme de la maladie.
