Use of polypharmacy in the treatment of diabetes is the norm; nonetheless, optimal control is often not achieved. Konjac-glucomannan-based fibre blend (KGB) and American ginseng (AG) have individually been shown to improve glycaemia and CVD risk factors in type 2 diabetes. The aim of this study was to determine whether co-administration of KGB and AG could improve diabetes control beyond conventional treatment.
PURPOSE:Studies on the effect of chronic interval training on appetite in the obese population are scarce. The aim of this study was to determine the effect of 12 wk of isocaloric programs of moderate-intensity continuous training (MICT), high-intensity interval training (HIIT), or short-duration HIIT on subjective feelings of appetite, appetite-related hormones, and reward value of food in sedentary obese individuals. METHODS:Forty-six sedentary obese individuals (30 women and 16 men), with a body mass index of 33.3 ± 2.9 kg·m and age of 34.4 ± 8.8 yr, were randomly assigned to one of the three training groups: MICT (n = 14), HIIT (n = 16), or short-duration HIIT (n = 16). Exercise was performed three times per week for 12 wk. Subjective feelings of appetite and plasma levels of acylated ghrelin, polypeptide YY3-36, and glucagon-like peptide 1 were measured before and after a standard breakfast (every 30 min up to 3 h), before and after the exercise intervention. Fat and sweet taste preferences and food reward were measured using the Leeds Food Preference Questionnaire. RESULTS:A significant increase in fasting and postprandial feelings of hunger was observed with the exercise intervention (P = 0.01 and P = 0.048, respectively), but no effect of group and no interaction. No significant effect of exercise intervention, group, or interaction was found on fasting or postprandial subjective feelings of fullness, desire to eat, and prospective food consumption or plasma concentration of acylated ghrelin, polypeptide YY3-36, and glucagon-like peptide 1. No changes in food preference or reward over time, differences between groups, or interactions were found. CONCLUSIONS:This study suggests that chronic HIIT has no independent effect on appetite or food reward when compared with an isocaloric program of MICT in obese individuals.
PURPOSE:The effect of acute exercise, and exercise intensity, on appetite control in obese individuals requires further study. The aim of this study was to compare the effects of acute isocaloric bouts (250 kcal) of high-intensity intermittent cycling (HIIC) and moderate-intensity continuous cycling (MICC) or short-duration HIIC (S-HIIC) (125 kcal) and a resting control condition on the appetite hormone responses, subjective feelings of appetite, energy intake (EI), and food reward in overweight/obese individuals. METHODS:This study is a randomized crossover study on 12 overweight/obese volunteers. Participants were assigned to the control, MICC, HIIC, and S-HIIC conditions, 1 wk apart, in a counterbalanced order. Exercise was performed 1 h after a standard breakfast. An ad libitum test lunch was served 3 h after breakfast. Fasting/postprandial plasma samples of insulin, acylated ghrelin, polypeptide YY3-36, and glucagon-like peptide 1 and subjective feelings of appetite were measured every 30 min for 3 h. Nutrient and taste preferences were measured at the beginning and end of each condition using the Leeds Food Preference Questionnaire. RESULTS:Insulin levels were significantly reduced, and glucagon-like peptide 1 levels significantly increased during all exercise bouts compared with those during rest. Acylated ghrelin plasma levels were lower in the MICC and HIIC, but not in S-HIIC, compared with those in control. There were no significant differences for polypeptide YY3-36 plasma levels, hunger or fullness ratings, EI, or food reward. CONCLUSIONS:Our findings suggest that, in overweight/obese individuals, isocaloric bouts of moderate- or high-intensity exercise lead to a similar appetite response. This strengthens previous findings in normal-weight individuals that acute exercise, even at high intensity, does not induce any known physiological adaptation that would lead to increased EI.
Background/Objective: Addition of viscous fiber to foods has been shown to significantly reduce postprandial glucose excursions. However, palatability issues and the variability in effectiveness due to different methods of administration in food limits it use. This study explores the effectiveness of a viscous fiber blend (VFB) in lowering postprandial glycemia using different methods of incorporation. Methods: Two acute, randomized, controlled studies were undertaken: Study 1: Twelve healthy individuals (mean ± SD, age: 36 ± 13 years, body mass index [BMI]: 27 ± 4 kg/m2) consumed 8 different breakfasts. All meals consisted of 50 g of available carbohydrate from white bread (WB) and 10 g margarine. Zero, 1, 2, or 4 g of the VFB was baked into WB or mixed with the margarine. Study 2: Thirteen healthy individuals (mean ± SD, age: 39 ± 17 years, BMI: 25 ± 5 kg/m2) consumed 6 test meals, consisting of 50 g of available carbohydrate from WB. Six capsules containing either cornstarch or VFB were taken at 4 different time points during the glucose tolerance test. After obtaining a fasting finger-prick blood sample, volunteers consumed the test meal over a 10-minute period. Additional blood samples were taken at 15, 30, 45, 60, 90, and 120 minutes from the start of the meal. For study 2, an additional fasting sample was obtained at −30 minutes. Results: Study 1: Irrespective of VFB dose, glucose levels were lower at 30 and 45 minutes when VFB was mixed into the margarine compared to the control (p < 0.05). Incremental areas under the curve were significantly lower compared to control when 4 g of VFB was mixed into the margarine. Study 2: There was no effect of the VFB on postprandial glucose levels when administered in capsules. Conclusion: Incorporation of VFB into margarine was more effective in lowering postprandial glycemia than when the VFB was baked into bread and no effect when given in capsules.
Prevention of weight gain in adults is a major public health target. Animal experiments have consistently demonstrated a relationship between fermentable carbohydrate intake, such as oligofructose, anorectic gut hormones, and appetite suppression and body weight control. This study was designed to determine the dose of oligofructose which would augment the release of anorectic gut hormones and reduce appetite consistently in non-obese humans. Twelve non-obese participants were recruited for a 5-week dose-escalation study. Following a 9-14-day run-in, participants increased their daily oligofructose intake every week from 15, 25, 35, 45, to 55 g daily. Subjective appetite and side effects were monitored daily. Three-day food diaries were completed every week. Appetite study sessions explored the acute effects of 0, 15, 35, and 55 g oligofructose on appetite-related hormones, glycaemia, subjective appetite, and energy intake. In the home environment, oligofructose suppressed hunger, but did not affect energy intake. Oligofructose dose-dependently increased peptide YY, decreased pancreatic polypeptide and tended to decrease ghrelin, but did not significantly affect appetite profile, energy intake, glucose, insulin, or glucagon-like peptide 1 concentrations during appetite study sessions. In conclusion, oligofructose supplementation at >= 35 g/day increased peptide YY and suppressed pancreatic polypeptide and hunger; however, energy intake did not change significantly. To determine the relative impact of three iso-caloric breakfast meals, of variable composition, on satiety, hunger and subsequent intake of energy.
While selenium (Se) is an essential micronutrient for humans, epidemiological studies have raised concern that supranutritional Se intake may increase the risk of developing Type 2 diabetes mellitus (T2DM). We aimed to determine the impact of Se at a dose and source frequently ingested by humans on markers of insulin sensitivity and signalling. Male pigs were fed either a Se-adequate (0.17 mg Se/kg) or a Se-supranutritional (0.50 mg Se/kg; high-Se) diet. After 16 weeks of intervention, fasting plasma insulin and cholesterol levels were non-significantly increased in the high-Se pigs, whereas fasting glucose concentrations did not differ between the two groups. In skeletal muscle of high-Se pigs, glutathione peroxidase activity was increased, gene expression of forkhead box O1 transcription factor and peroxisomal proliferator-activated receptor-γ coactivator 1α were increased and gene expression of the glycolytic enzyme pyruvate kinase was decreased. In visceral adipose tissue of high-Se pigs, mRNA levels of sterol regulatory element-binding transcription factor 1 were increased, and the phosphorylation of Akt, AMP-activated kinase and mitogen-activated protein kinases was affected. In conclusion, dietary Se oversupply may affect expression and activity of proteins involved in energy metabolism in major insulin target tissues, though this is probably not sufficient to induce diabetes.

Many factors thought to influence the control of food intake have been investigated independently, but the relative importance of each of these factors remains unknown. This study investigated the concurrent role of 21 factors in accurate compensation for energy consumed 60 min previously. Energy compensation was assessed by measuring the difference in energy intake at an ad libitum test meal following two preloads of differing energy content, in 105 participants. Using regression, energy compensation was associated only with age (B = -2.39, beta = -0.345, p < 0.01), and accuracy of energy compensation was associated only with age (B = 1.81, beta = 0.376, p < 0.01) and order of preload presentation (B = -21.80, beta = -0.233, p = 0.01). These findings suggest that our ability to detect and/or adjust for energy intake deteriorates with age, and that individuals adjust more easily for missing, as opposed to additional, energy. Notably however, only these predictors were associated with energy compensation and they account for only 11-18% of total variance. (C) 2011 Elsevier Ltd. All rights reserved.
Subjects working a 12 h offshore night shift for 2 weeks normally adapt to the night shift and are out of synchrony when they return home to day life, with consequent problems of poor sleep. The aim of this study was to investigate the effectiveness of timed light treatment to hasten circadian adaptation and improve sleep after the night shift. Ten male shift workers worked 19.00–07.00 h (n = 4) or 18.00–06.00 h (n = 6) offshore shift schedules. They were assessed for the last 7 days of a 14 or 21 day offshore night shift and for the following 14 days at home. Either timed light treatment/sunglasses or no light treatment/no sunglasses were scheduled in a crossover design during days 1–5 after the nightshift, theoretically timed to advance the circadian system. Subjects completed the Horne Östberg questionnaire. They wore an Actiwatch-L throughout the study to monitor light/activity and completed daily sleep diaries. Actigraphic sleep efficiency after the light/sunglasses treatment was significantly improved (days 1–5), that is, 86.7 ± 5.8% (mean ± SD; light treatment) compared to 79.4 ± 10.3% (no light treatment), P< 0.05. Objective sleep duration (days 6–14) was significantly improved in the light treatment leg; actigraphic sleep duration was longer after light treatment (6.75 ± 0.50 h) compared to 5.76 ± 0.73 h, P< 0.05. If appropriately timed, light and darkness has beneficial effects on sleep efficiency and sleep duration following a night shift.
Methods: The D-FINES study (Vitamin D, Food Intake, Nutrition and Exposure to Sunlight in Southern England) is currently investigating the interaction between diet and sunlight exposure on vitamin D status in Caucasian and Asian women aged 19–70 years living in Southern England. Fasted blood samples were collected from a total of 279 Caucasian and 94 Asian women in each season. Bone quality was assessed using broadband ultrasound attenuation BUA during Autumn and Spring seasons of 2007. Aim: The aim of the present subsidiary study was to examine: 1) the association between serum lipids and bone quality in Autumn and Spring 2007; 2) the association between vitamin D status and BUA according to ethnicity groups. Results: As is shown in the Table below, there was a significant negative correlation between lipid profiles and BUA in Caucasian women but not Asian women, with effect greatest in the Spring. These finding remained significant for cholesterol, TAG and LDL after adjustment for BMI and vitamin D status.. Table. The impact of eating behaviours on circulating levels of appetite-regulating hormones remains largely unknown. The aims of this study were to assess the role of restraint and disinhibition on fasting/postprandial peptide YY (PYY) plasma levels and subjective feelings of appetite in normal-weight individuals and to determine whether the effect was energy load dependent. 33 participants (12 men) were classified as restrained/unrestrained and low/high in disinhibition based on Three Factor Eating Questionnaire-18R and Dutch Eating Behaviour Questionnaire. The impact of restraint/disinhibition on PYY plasma levels and feelings of appetite was measured, after a 500kcal and 1000kcal breakfast, using a randomised crossover design. Restraint did not impact on either fasting or postprandial PYY plasma levels, but participants with high disinhibition had a tendency towards a blunted postprandial PYY response. Moreover, restrained eaters reported lower ratings of prospective food consumption postprandially, and a tendency towards higher fullness/lower hunger. In conclusion, circulating PYY is unaffected by restrained eating behaviour, despite being associated with increased fullness and reduced hunger in the fed state. High levels of disinhibition tend to be associated with a blunted PYY response and this may contribute towards the susceptibility to overconsumption and increased risk of weight gain characteristic of this trait.
