OBJECTIVES:The aim of this study was to investigate the characteristics of de novo malignancies arising in kidney transplant recipients followed in a tertiary hospital in Turkey and to examine the tumors in the head and neck region as a subgroup. MATERIALS AND METHODS:Data from kidney transplant recipients treated at our institution between January 2010 and July 2022 were retrospectively analyzed in this single-center study. Data regarding malignancies were noted according to the pathologists' reports. In situ malignancies and those arising after graft loss were not evaluated. RESULTS:The study population comprised 231 patients (165 men; 71.4%) with a median follow-up of 11 years (2853 patient-years). The recipients had a higher cancer risk than the general population (standardized incidence rate = 3.04; 95% CI, 1.82-4.26). Thirty de novo malignant tumors were detected in 24 patients (10.4%). The mean age at diagnosis of cancer was 54.88 ± 11.44 years. The median time from transplant to cancer diagnosis was 11.5 years (range, 7-18.8 y). Nonmelanoma skin cancers (56.7% of all tumors) were the most common malignancies. Twenty-two lesions (73.3%) that developed in 17 patients (7.4%) were localized to the head and neck region: 15 (68.2%) were cutaneous and 7 (31.8%) were noncutaneous. The median time from transplant to head and neck cancer diagnosis was 12 years (range, 7.5-17.5 y). Mortality rate was higher in cancer patients (10 [41.7%] vs 17 [8.2%]; P < 0.01). CONCLUSIONS:The incidence of de novo malignancy in kidney transplant recipients was relatively higher compared with previous data. Nonmelanoma skin cancers were the most common type. Three-quarters of all lesions were in the head and neck region, and two-thirds were of cutaneous origin.
Aim: Ependymomas are highly heterogeneous neuroepithelial tumors that occur in all age groups and in all compartments of the central nervous system. Because of the tumor heterogeneity, the current histopathological criteria for grading are insufficient. Therefore, auxiliary prognostic markers that are cost-effective and easy-to-use and may help predict clinical behavior are still needed. Neural Epidermal Growth Factor Like Like-2 (NELL2) and laminin alpha-2 (LAMA2) are two outstanding proteins with prognostic significance in ependymomas. In the present study, we aimed to investigate the correlation of NELL2 and LAMA2 expression with histopathological parameters in patients with ependymomas. Materials and Methods: A total of 64 ependymomas were retrospectively evaluated and representative paraffin blocks were selected. Demographic and histopathological characteristics of the patients were noted. Slides from selected blocks were immunohistochemically stained using NELL2, LAMA2 and Ki67 antibodies. The results were compared with other characteristics of the patients. Results: We found that the histological grades of the ependymomas correlated with age, tumor localization, histological subtype and number of mitosis. NELL2 staining was significantly higher in grade II ependymomas, while LAMA2 staining was significantly higher in grade III ependymomas. Moreover NELL2 and LAMA2 expression was associated only with the histological grade of the tumor. We found the Ki-67 proliferation index to be consistent with increasing histological grade. Conclusions: Our findings show that higher NELL2 expression than LAMA2 expression is associated with low histological grade and probably better prognosis. On the other hand LAMA2 expression is associated with high histological grade and probably poor prognosis.
Abstract Objectives: In recent years extracorporeal shock wave therapy has been reported to be useful in the treatment of a number of diseases including delayed bony union, calcifying tendinitis of the shoulder, tennis elbow, and plantar fascitis. In order to assess possible detrimental effects of this therapy on tendons and neighboring tissues a prospective study was carried out with rats.\nMethods: Thirty-two Wistar albino rats were equally divided into four groups. The first three groups were administered 1000 shock waves at 14 kV, 1500 at 14 kV, and 2000 at 18 kV, respectively. The fourth group included the controls. The effects of different shock waves at different power set-ups on rat achille tendons were investigated.\nResults: A marked histopathologic change was not observed with 1000 and 1500 shock waves at 14 kV. Mild disorganization in the leafs of collagen tissue and rare lymphocytes were detected with 2000 shock waves at 18 kV.\nConclusion: Observation of histopathologic changes with 2000 shock waves at 18 kV suggest that adverse tissue changes should be considered when applying high energy shock waves. Özet Amaç: Son yıllarda gündeme gelen ekstrakorporeal şok dalgası tedavisi, kaynama gecikmesi, omzun kalsifiye tendiniti, tenisçi dirseği ve plantar fasyitis tedavisinin de aralarında olduğu bir çok hastalıkta kullanılmaktadır. Bu tedavinin tendon ve komşu dokulara muhtemel zararlı etkilerini ortaya koymak amacıyla sıçanlarda deneysel bir araştırma gerçekleştirdik.\nÇalışma planı: Otuz iki adet Wistar albino cinsi sıçan sekizerlik dört gruba ayrıldı. İlk üç deney grubuna sırasıyla 14 kV’ lik 1000, 14 kV’lik 1500 ve 18 kV’lik 2000 şok dalgası uygulandı. Dördüncü grup kontrol grubunu oluşturdu. Bu enerji şiddeti ve şok dalgası uygulamasının sıçanların aşil tendonlarına etkisi araştırıldı.\nSonuçlar: 14 kV 1000 şok ve 14 kV 1500 şok gruplarında belirgin bir histopatolojik değişiklik görülmedi. 18 kV 2000 şok uygulanan grupta ise liflerde hafif derecede dizilim bozukluğu ve seyrek lenfositler görüldü.\nÇıkarımlar: Histopatolojik değişikliklerin 18 kV 2000 şok dalgasından itibaren gözlenmesi nedeniyle, yüksek enerjili şok dalgası uygulamalarında olası doku değişikliklerini göz önüne almak gerekir.
Background: Gastric cancer is the third most common cause of cancer-related death worldwide. Tumor budding is an easy to detect histopathological feature associated with a poor prognosis in patients with several types of cancer. The present study aimed to determine the relationship between tumor budding and clinicopathological parameters in gastric adenocarcinoma patients. Materials and Methods: This study retrospectively analyzed the H and E-stained slides of 146 patients that were diagnosed with gastric adenocarcinoma between 2013 and 2017. Tumor budding, large cell invasion, mitosis, fibrosis, and peritumoral lymphocytic response were recorded in all cases. The relationship between tumor budding and clinicopathological prognostic parameters was statistically analyzed. Results: Increased tumor budding density (>= 10 tumor buds) was observed in 62 (42.5%) of the patients. There was a significant relationship between increased tumor budding density and histological grade (P < 0.001), lymphovascular invasion (P = 0.016), perineural invasion (P < 0.001), lymph node involvement (P = 0.015), and tumor invasion depth (pT stage) (P < 0.001). There was also a significant relationship between a high fibrosis rate, and lymphovascular invasion (P < 0.001), lymph node involvement (P = 0.030), and pT stage (P = 0.002); however, there wasn't a significant association between prognostic parameters, and large cell invasion, the mitotic count, and peritumoral lymphocytic response. Conclusions: The present findings suggest that increased tumor budding density in gastric adenocarcinoma patients may be used to predict poor prognosis.
Aim:The identification of cellular pathways in colorectal tumor biology is essential for early diagnosis, treatment, and post-treatment follow-up.Osteopontin is an extracellular matrix protein that has regulatory physiological functions and roles in apoptosis, proliferation, adhesion, invasion, and tumor metastasis.In this study, we aimed to determine the prognostic and predictive value of osteopontin in colon adenocarcinoma.Our study investigated whether osteopontin expression had any prognostic or predictive use in colon adenocarcinoma and also if there were any differences between adenocarcinoma and adenoma.Methods: Fifty of these colonic specimens were adenocarcinoma, 16 were adenomatous polyps, and 10 were nontumoral colonic tissue that served as a control group.We used a two-tiered evaluation system that examined both the staining intensity and the percentage of staining.Results: The staining scores of tumors with vascular invasion were significantly higher than those of tumors without vascular invasion.In addition, the tumoral tissues' osteopontin staining scores were significantly higher than the score of polyps. Conclusion:If future studies support our results, we suggest that osteopontin may be an important biomarker for predicting or detecting vascular invasion in tumors and could be useful in tumor-adenomatous polyp differentiation.Therefore, osteopontin can provide helpful information in the diagnosis and prognosis of colon adenocarcinoma.
Abstract Objectives: A rat model was designed to investigate the effect of low-energy extra-corporeal shock wave therapy (ESWT) (500 shock waves at 14 kV) on the healing potential of tendon injuries that may accompany fractures.\nMethods: The study included 48 rats for histopathologic (n=28) and biochemical (n=20) studies. The right Achilles tendons of all rats were cut and then sutured using the modified Kessler method. For histopathologic examination, ESWT was administered to the injured tendons under the guidance of fluoroscopy in 14 rats, while the remaining 14 rats remained as controls following sham operation. On the other hand, for biochemical studies 20 rats were divided into four equal groups where two groups received ESWT. On the postoperative third and ninth days, rats (one study with one control group, respectively) were sacrificed to measure the hydroxyproline levels.\nResults: Histopathologic examination in the second week showed highly cellular granulation tissue with more vascularization and massive inflammatory infiltration in ESWT-administered rats. There were many multinuclear giant cells. On the third week, the healing in the ESWT group was considerably better than that of the controls. In addition, the irregularity of the fibrils, intensity of lymphocyte and histiocyte infiltration were less and there was a reduction in the number of giant cells. Hydroxyproline levels were found to be high in the study groups.\nConclusion: Histopathological findings and increased hydroxyproline levels suggest that ESWT may facilitate tendon healing. Özet Amaç: Düşük enerjili ekstrakorporeal şok dalgası tedavisinin (ESWT) kırıklara eşlik edebilen tendon dokularındaki zedelenmelerin iyileşmesine etkileri sıçanlar üzerinde araştırıldı.\nÇalışma planı: Histopatolojik tetkik için 28 sıçan kullanıldı. Sıçanların sağ aşil tendonları kesilerek modifiye Kessler yöntemi ile sütüre edildi. Aşil tendonlarına skopi kontrolünde 14 kV gücünde 500 şok dalgası uygulandı. Cerrahi onarım yapılan ve ESWT uygulanmayan 14 sıçan kontrol grubuna alındı. Biyokimya çalışması için de 20 sıçan beşerli dört gruba ayrıldı. Sıçanların aşil tendonları kesilip sütüre edilerek birinci ve ikinci gruba 14 kV gücünde 500 şok dalgası uygulandı. Birinci grup üçüncü günde ikinci grup dokuzuncu günde öldürülerek hidroksiprolin düzeyleri ölçüldü. Üçüncü ve dördüncü grup kontrol grubunu oluşturdu.\nSonuçlar: Histolojik incelemede, ESWT uygulanan grupta ikinci haftada granülasyon dokusu belirgin olarak daha selüler, daha vaskülarize ve yoğun iltihabi infiltrasyon içermekteydi ve çok sayıda multinükleer dev hücre vardı. Üçüncü haftada ESWT grubunda iyileşmenin kontrol grubuna göre daha iyi ve düzenli olduğu, fibrillerde düzensizliğin, yoğun lenfosit ve histiosit infiltrasyonunun azaldığı ve çok daha az sayıda multinükleer dev hücre içerdiği görüldü. Deney grubunda hidroksiprolin düzeylerinin yükseldiği görüldü.\nÇıkarımlar: Deney grubundaki histopatolojik bulgular ve artmış hidroksiprolin düzeyleri ESWT uygulamasının tendon iyileşmesini hızlandırdığını gösterdi.
Abstract Objectives: We performed radiologic, histologic, and scintigraphic studies to evaluate the effect of extracorporeal shock-wave lithotripsy (ESWL) on the healing of experimental rat tibial fractures.\nMethods: Wistar albino rats aged 2.5 to 4 months (mean age 3 months) and with a mean weight of 200 g (range 180 to 220 g) were assigned to four- and six-week groups. A transverse fracture in the right tibial diaphyses was induced manually followed by intramedullary fixation with the use of a Kirschner wire. Each of the two (study and control) four-week and six-week groups consisted of 14 rats and five rats, respectively. Following fracture induction and fixation, four- and six-week study groups were administered ESWL at 200 shock waves (14 kV) to the fracture line on day 0, 5, 9, 14, and 19. The rats were radiologically examined in the forth and sixth weeks. The first two groups of rats were sacrificed at the end of four weeks for histologic examination. The remaining two groups underwent scintigraphic examination for osteoblastic activity at the end of six weeks and were then sacrificed for histologic examination.\nResults: Radiological scores were significantly higher in the study rats in the forth and sixth weeks (p=0.002 and p=0.011, respectively); similarly, significantly higher extent of bone healing was observed in the ESWT groups (p=0.027 and p=0.001, respectively). The 10-minute static scintigraphic counting resulted in higher values in the shock wave administered rats (p=0.039).\nConclusion: Radiologic, histologic, and scintigraphic results of our study showed that ESWT treatment increased the rate of clinical bone healing. Proliferating trichilemmal tumor (PTT) is a rare, mostly benign neoplasm which stems from the follicular outer root sheath epithelium. PTT occurs as a subcutaneous cystic nodule slowly enlarging to a larger noduler mass, and is usually localized on the scalp of elderly women. To the best of our knowledge, PTT localized on a psoriatic plaque has not been reported previously. Amaç: Bu çalışmanın amacı, yalın yönetim araçlarından olan 5S yöntemini bir kamu hastanesi patoloji laboratuvarında uygulamak, çalışma ortamındaki israf nedenlerini ortadan kaldırmak, çalışanlara daha düzenli ve verimli çalışabilecekleri bir iş alanı oluşturmaktır. Gereç ve Yöntemler: 5S yöntemi, çalışma alanlarında belirlenen israf nedenlerinin ortadan kaldırılması için kullanılan bir yalın yönetim tekniğidir. Bu çalışmada, 5S yönteminin uygulanması için literatür bilgilerine dayalı olarak belirlenen ve beş adımdan oluşan bir metodoloji geliştirilmiştir. Bu adımlar, uygulama ekibinin oluşturulması, 5S alanının belirlenmesi ve hazırlanması, amaç ve hedeflerin belirlenerek çalışanlara duyurulması, uygulama planının hazırlanması ve uygulamanın hayata geçirilmesi olarak belirlenmiştir. Bulgular: 5S uygulamasında sınıflandır ve sırala adımları ile laboratuvarda bulunan gereksiz malzemeler elimine edilmiş, mevcut durumda kullanılan malzemeler kullanım sıklığı dikkate alınarak yeniden konumlandırılmıştır. Bu sayede laboratuvarda malzeme, zaman ve hareket israflarının önüne geçilmiştir. Sil adımı ile daha temiz, düzenli, iş kazalarının ve hataların önceden engellenebileceği bir çalışma ortamı oluşturulmuştur. Standartlaştır adımında yöntemin düzenli kullanılması için '5S Kontrol Formu' oluşturulmuştur. Sürdür adımı için ise denetim ve eğitim programları planlanmıştır. Sonuç: 5S uygulaması sonucunda daha temiz, düzenli, iş kazalarının ve hataların önceden engellenebileceği bir çalışma ortamı oluşturulmuştur. Bu çalışma aynı zamanda patoloji laboratuarında yalın yönetim kültürünün oluşturulması için temel oluşturmaktadır. 5S uygulamasının sürdürülmesinin, laboratuvarda hata oranlarının azalmasını sağlayacağı ileri sürülmektedir.
Introduction: renal cell carcinoma and angiomyolipoma are well-known tumors of kidney. However, coexistence of these tumors in the same patient is a rare condition. synchronous occurrence of renal cell carcinoma and angiomyolipoma is more commonly seen in patients with tuberous sclerosis. case report: A 76-years-old female was admitted to urology clinic with a complaint of flank pain. she had pain and tenderness on her right flank on physical examination. radiological imaging revealed a solid mass in right kidney. she underwent right radical nephrectomy. conclusion: We report a case of clear cell renal cell carcinoma and angiomyolipoma occurring in the same kidney without stigmata of tuberous sclerosis and with the review of related literature.
Some of the non-parasitic cystic lesions of the liver are benign, small and asymptomatic lesions. They are often detected accidentally. In particular, the spread and increase of imaging methods led to an increase in coincidental findings and incidences of these lesions. Only 16% of cases are symptomatic. Symptoms are directly related to the size of the cyst or the pressure on the surrounding tissues. Abdominal pain and distension are the most common symptoms and occur in approximately 50% of cases. For asymptomatic cases, no treatment is required. In contrast, large, symptomatic and neoplastic cysts require treatment. In our study, we wanted to present two simple epithelial liver cyst cases that refer to our clinic with complications.
Introduction and Objective: In this study, a short message sending application was presented to inform physicians, clinical management officers and patients about pathology results in the determined time, patient satisfaction, service efficiency and speed increase, and time and labor loss losses. Material and Method: The study was conducted to evaluate the effectiveness of the Short Message System (SMS) application conducted in the Pathology Clinic of Diskapi Yildirim Beyazit Training and Research Hospital, between the dates 01.03.2017 - 31.10.2017. The study is a retrospective descriptive research and the data is analyzed in Microsoft Excel. If the first message is not reported within 24 hours from the report time-out, the first message is sent to the clinical supervisor of the second message, if the report has not been released within 24 hours, the message is sent to the assistant physician. SMS information and reminder messages are sent to patients in standard format at the time of pathology. Findings and Conclusion: As a result of the study, it was found that the outpatient report of the pathology results was 756 in March while it fell to 339 in October and the time out in the pathology results was 10.5% in 8 months.
INTRODUCTION: In this study, the effects of adrenaline bleeding control on myringosclerosis (MS) development after myringotomy in experimental model were investigated. METHODS: Sixteen Wistar-Albino healthy male rats were used in the experiment. The right and left ear of each rat were accepted as the control group and the ears were divided into 4 groups. Group 1: 8 rats, examined 48 hours after Miringotomy, with adrenaline applied to the left ear. Group 2: 8 rats, examined 120 hours after miringotomy, with adrenaline applied to the left ear. Group 3: 8 rats, examined 48 hours after miringotomy, with saline applied to the right ear. Group 4: 8 rats, examined 120 hours after miringotomy, with saline applied to the right ear.At the end of the experiment, ears of rats were examined otomicroscopically and histopathologically. RESULTS: There was no statistically significant difference between the groups in terms of vascular dilatation, congestion, inflammatory cell, leukocyte count, mast cell degranulation, connective tissue increase and sclerosis. DISCUSSION AND CONCLUSION: In this study, we have investigated the view that hemorrhage from the margins of myringotomy and the development of inflammatory cell migration to the myringotomy site and the development of inflammation may contribute to the development of myringosclerosis. It is important that the local administration of adrenaline, by virtue of its vasoconstrictor effect, may reduce the amount of bleeding and thereby prevent the development of myringosclerosis. However, there is a need for wider working groups to determine its statistical significance.
Objective: Acute lung injury is a complication after hepatic ischemia/reperfusion (I/R) and might be responsible for high mortality rate. The aim of this study was to demonstrate that adrenomedulline administration may prevent lung injury after hepatic I / R by downregulation of proinflammatory cytokines. Materials and Methods: This study was performed by using 54 male Wistar rats. The rats were randomly allocated into 3 groups and in groups were randomly allocated into 1st, 2nd, and 4th hour subgroups. After I/R, AM (12μg/kg) was infused for 30 minute via portal vein. Blood and tissue samples were collected 1, 2 and 4 hour after reperfusion. Hepatic I/R induced lung injury, as characterized by lung edema, histopathologic changes and proinflammatory cytokines including tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) levels were evaluated. Results: The TNF-α enzyme activity in the first hour was found to be significantly higher in the I/R group than in the Sham and AM group (p=0.027 and p=0.041, respectively). The levels of TNF-α in the sham-operated group were similar in the AM group (p=0.310). Significant differences were found only in the second hour IL-6 measurements (p=0.038). Similar differences in caspase-9 enzyme activity in the sham-operated and AM group (p=0.291). Treatment with AM decreased lung injury after hepatic I/R as demonstrated by reduction in histologic damage. Conclusions: Acute lung injury was decreased by AM treatment after hepatic I/R injury. the I/R in of time AM starts proinflammatory cytokine downregulation. Lung injury can be further aggravated by increased apoptosis of epithelial cells. Apoptosis is induced through two pathways. The intrinsic pathway is mitochondria-dependent and is activated by reactive oxygen species or cytokines. The extrinsic pathway is dependent on inflammatory molecules, such as TNF-α or Fas-ligand. Activating production of reactive oxygen species oxidase, TNF-α contributes to the intrinsic pathway, as well. Mycosis fungoides (MF) is the most common type of primary cutaneous T-cell lymphoma. Hypopigmented MF is a rare atypical clinical variant and is mostly seen in dark-skinned individuals of Africa, India, Latin America and Asian origin, especially in children and young adults. As well as it is seen very rare in Caucasians, patients usually consult with asymptomatic, irregular, non-squamous hypopigmented patches which distributed in the trunk and proximal extremities. A 25-year-old female patient clinically and histopathologically hypopigmented MF who presented with diffuse white stains on the trunk and upper extremities was presented because of its rarity.
PURPOSE: Adiponectin is a protein stemming from adipose tissue and having strong anti-inflammatory properties. We aimed to assess the damage diminishing effects of recombinant adiponectin (rAD) through NF-kB in the experimental acute pancreatitis (AP) model. MATERIALS AND METHODS: Acute pancreatitis was created by applying 50 mu g/kg dose of intraperitoneal cerulean. The rats were randomised and divided into 3 groups as control, AP and rAD groups. Moreover, the rats in each group were divided into two sub-groups as 24th and 48th hour subgroups. rAD was injected in the study group intraperitoneally. Tissue and blood samples were taken after 24 and 48 hours. Histopathological assessment and NF-kB activity were investigated in pancreatic tissue. RESULTS: Serum TNF-a, IL-1b and IL-6 levels were found to be statistically significant in the AP group compared to the rAD group in the 24th and 48th hour (p < 0.05). Similarly, NF-kB activity was also found to be significant in the AP group both in the 24th and 48th hour (p < 0.05). There were significant differences in the AP and the rAD groups histopathologically in terms of edema, inflammation, vacuolisation and necrosis (p < 0.001). CONCLUSION: rAD has significantly reduced NF-kB activity, cytokine levels and tissue damage.

Poliklinikte sabah ilk hasta muayene kabulu gecikmelerinde kademeli olarak 10’ar dakika araliklar ile uzman doktora, klinik idari sorumlu doktoruna, polikliniklerden Sorumlu Bashekim Yardimcisina SMS hatirlatma mesaji gonderilmektedir. Calisma sonucunda, az maliyetli, basit guncel teknoloji (cep telefonu - SMS) kullanilarak 08:30 olarak bildirilen poliklinik muayene baslama saatlerinin saat 08:55’den 08:26’a dustugu gorulmustur. Hasta memnuniyet oraninda %3,5 luk bir artisin oldugu tespit edilmistir.
