Einleitung: Eine Streptozotozin-basierte Chemotherapie ist bei Patienten mit fortgeschrittenem neuroendokrinen Tumor des Pankreas trotz neu entwickelter zielgerichteter Therapien ein wichtiger Therapiebaustein. Bisher gibt es aber keine einheitlichen Empfehlungen zur Therapiedauer bei Patienten mit initialem Ansprechen auf die Therapie.
Hintergrund: Gastrin induziert durch Stimulation des Cholezystokinin2-Rezeptors (CCK2R) Histamin-Sekretion sowie Expression von Genen des Histamin-Metabolismus in gastralen neuroendokrinen ECL-Zellen. Darüber hinaus vermitteln Gastrin und der CCK2R Differenzierung und Proliferation von ECL-Zellen. Zu den extraintestinalen Effekten von Gastrin zählt die Freisetzung von Calcitonin aus neuroendokrinen C-Zell-Karzinomen, ein Effekt, der diagnostisch ausgenutzt wird (Pentagastrintest). Molekulares Korrelat des Pentagastrintests ist die Expression des CCK2R in diesen Tumoren. Wir konnten kürzlich zeigen, dass der CCK2R auch in benignen C-Zellen physiologisch exprimiert wird. Die Funktionen von Gastrin und dem CCK2R in C-Zellen sind unbekannt.
Hintergrund: Somatostatin-Analoga inhibieren Proliferation durch selektive Stimulation von Somatostatinrezeptoren (SSTR), von denen fünf Subtypen, SSTR1 bis SSTR5, bekannt sind. Die derzeit eingesetzten Somatostatin-Analoga weisen eine hohe Affinität für SSTR2, sowie mittelgradige Affinitäten für SSTR5 und SSTR3 auf. In den letzten Jahren wurden Somatostatin-Analoga zunehmend in der Therapie des fortgeschrittenen hepatozellulären Karzinoms (HCC) eingesetzt. Die Effektivität dieser Therapie ist angesichts uneinheitlicher Ergebnisse klinischer Studien umstritten. Wir konnten kürzlich mittels RT-PCR erstmals zeigen, dass HCCs differentielle Expressionsmuster für SSTR ausweisen. Ziel dieser Studie war die systematische Analyse der Inzidenz der einzelnen SSTR-Subtypen in HCCs.
H: Chronische Pankreatitis und Leberzirrhose sind Folgeerscheinungen vermehrten Alkoholkonsums. Für dessen Entstehen werden unterschiedliche Risikofaktoren suggeriert, da in der Literatur die Koinzidenz beider Erkrankungen mit 1% als gering beschrieben wird. Z: Ziel dieser Studie war die Ermittlung der Prävalenz des simultanen Erscheinens der chronischen Pankreatitis und Leberzirrhose in alkoholkranken Individuen. Methoden: Es handelt sich hierbei um eine post mortem Autopsiestudie an 601 alkoholkranken Individuen und einer Kontrollgruppe von 100 nicht Alkoholkranken. Die untersuchten Fälle wurden in folgende Kategorien eingeteilt: Pankreas: normales Pankreas, Pankreasfibrose, akute Pankreatitis, chronische Pankreatitis. Leber: normale Leber, Leberverfettung, Fettleber, Leberzirrhose. Ausschlusskriterien waren virale Hepatitiden, Adipositas permagna, Diabetes mellitus I. E: Von den 620 Fällen hatten 264 (42%) ein normales Pankreas, 242 (39%) hatten eine Pankreasfibrose, 29 (5, 8%) eine akute Pankreatitis, 85 (14%) eine chronische Pankreatitis, 50 (8%) hatten eine normale Leber, 157 (25%) eine Leberverfettung, 230 (37%) eine Fettleber und 183 (30%) eine Leberzirrhose. Keine Leber- oder Pankreasschädigung wurden in der Kontrollgruppe festgestellt (n=100). Von den 183 Individuen mit einer Leberzirrhose hatten 33 (18%) eine chronische Pankreatitis und 92 (51%) eine Pankreasfibrose. Von den 230 Individuen mit einer Fettleber hatten 37 (16%) eine chronische Pankreatitis und 97 (42%) eine Pankreasfibrose. 33 (39%) Individuen mit einer chronischen Pankreatitis hatten gleichzeitig auch eine Leberzirrhose und 37 (44%) eine Fettleber. Individuen mit einer Pankreafibrose hatten zu 38% auch eine Leberzirrhose und in 40% der Fälle eine Fettleber. Andere Risikofaktoren beider Erkrankungen wie Nikotinkonsum und BMI wurden ebenfalls erfasst. Nur 5% der 620 untersuchten Fälle waren Nichtraucher. 49% hatten einen BMI von 18,5–24,9, 22% von 25–29,9, 8% von 30–34,9 und 11% einen BMI unter 18,5. S: Entgegengesetzt der in der Literatur beschriebenen seltenen Koinzidenz von Pankreas- und Leberschädigung durch Alkoholkonsum im selben Individuum, zeigt die vorliegende Studie, dass beide Erkrankungen eng assoziiert sind. Auch die Vorläuferstadien beider Erkrankungen, die Pankreasfibrose und die Fettleber wurden häufig im selben Individuum gefunden. Diesen Daten suggerieren somit das Vorliegen gemeinsamer Risikofaktoren beider Erkrankungen.
Die Ergebnisse konservativer Therapien bei fortgeschrittenen Gastroenteropankreatischen Neuroendokrinen Tumoren (GEP-NET) sind enttäuschend und neue antitumorale Therapieansätze sind erforderlich. Aurorakinasen spielen eine wichtige Rolle bei der Kontrolle des Zellzyklus, und deren unkontrollierte Expression kann zu Aneuploidie und Tumorwachstum führen. Immunhistochemisch wurden 10 humane Karzinoide und zehn Insulinome untersucht; trotz guten Differenzierungsgrades zeigte sich eine hohe Expression von Aurorakinase A. Zunächst wurden die antiproliferativen Eigenschaften eines neuen kleinmolekularen Aurorakinase Inhibitors (AKI) in zwei humanen GEP-NET Zelllinien (BON-1 und QGP) untersucht. Beide Zelllinien exprimieren Aurorakinase A und B und weisen eine hohe Ser10-Phosphorylieriung von Histon H3 auf, einem Marker für Aurora B Aktivität. Ab einer Konzentration von 50nM führte die Behandlung der Zellen mit dem AKI zu einer nahezu vollständigen Hemmung der Tumorzellproliferation (Zellzählungen, 50nM vs. Kontrolle: p=0.0001, BON-1; p=0.0005, QGP; n=4 pro Zelllinie). Außerdem zeigte sich ab 50nM eine starke Hemmung der H3-Phosphorylierung. Die Behandlung der Zellen mit dem AKI führte zur Inhibierung des Zellzyklus mit Endoreduplikation und Polyploidie (DNA Gehalt >4N); höhere Konzentrationen des AKI führten zu einem vollständigen G2/M Phase Arrest. In einem Xenograft Modell wurden BON-1 und QGP Zellen in die Flanken immundefizienter NOD/SCID Mäuse injiziert.
Einleitung: Fibrose ist ein zentrales Merkmal chronischer Pankreatitis. Pancreatic stellate cells (PSC) erscheinen entscheidend für die Fibroseentwicklung. In PSC wird die Mitogen Activated Protein Kinase (MAPK) durch Wachstumfaktoren und unphysiologische Stimuli (Ethanol) aktiviert. In Azinuszellen aktiviert das GI-Hormon CCK, welches Apoptose und mitogene Signale vermittelt, focal adhesion- (FAK) und SRC-Kinasen. Durch CCK werden in Azinuszellen MAPK aktiviert. Bisher ist nur wenig über CCK bei der Aktivierung von PSC und dessen Signaltransduktion bekannt.
Hintergrund: Untersuchungen zu Skelettveränderungen an gastrektomierten Patienten deuten schon seit längerem auf eine Interaktion zwischen Gastrointestinalen Hormonen und Hormonen des Calcium- und Knochenstoffwechsels hin. Wir konnten kürzlich zeigen, dass Calcitonin-produzierende C-Zellen den Gastrinrezeptor (GR) exprimieren und dass Gastrin die Expression des knochenanabolen Hormons Calcitonin reguliert. Wir analysierten in dieser Arbeit die Bedeutung von Gastrin für C-Zellen, Calcitoninspiegel, sowie Parameter des Calcium- und Knochenstoffwechsels in GR-defizienten Mäusen.
Einleitung: Chromogranin A (CgA) ist ein etablierter Tumormarker für gastroenteropankreatische neuroendokrine Tumoren. Bei metastasierter Erkrankung kann eine Erhöhung von CgA-Serumspiegeln auf einen Tumorprogress hinweisen, aber Spiegeländerungen werden auch bei konstantem Tumorausmaß beobachtet. Dies könnte z.B. auf veränderte sekretorische Aktivität des Tumors zurückgeführt werden.

Einleitung: Die strukturell verwandten Kinasen FAK und PYK2 nehmen in einer Vielzahl von Signaltransduktionswegen sowohl in normalen, wie auch in malignen Geweben eine Schlüsselrolle ein. Trotz ihrer strukturellen Ähnlichkeit, spielen sie in Bezug auf wichtige zelluläre Funktionen, wie z.B. Apoptose, Zelladhäsion und Regulation des Zellzyklus unterschiedliche Rollen. Im Gegensatz zu PYK2 hat FAK zellproliferative und antiapoptotische Effekte. Das Pankreaskarzinom ist eines der häufigsten gastrointestinalen Tumoren und besitzt eine sehr schlechte Prognose. Es gibt zunehmende Evidenz, dass FAK in Pankreaskarzinomen eine wichtige Rolle spielt.
Hintergrund: Somatostatinanaloga können die Proliferation von Tumorzellen inhibieren durch die Induktion von Zellzyklusarrest und/oder Apoptose, in Abhängigkeit von den spezifischen Somatostatinrezeptor- (SSTR) Subtypen, die in den Tumorzellen exprimiert werden. In den letzten Jahren wurden Patienten mit hepatozellulären Karzinomen (HCC) zunehmend einer Therapie mit Somatostatinanaloga zugeführt. Studien zur Effektivität dieser Therapie zeigen widersprüchliche Ergebnisse. Ziel dieser Arbeit war es, wachstumsinhibierende Effekte des Somatostatinanalogons Octreotid und zugrundeliegende intrazelluläre Mechanismen in HCCs zu untersuchen.
BKG: The structurally related focal adhesion kinases FAK and PYK2 are involved in key signal transduction pathways in normal and malignant tissues. In spite of their structural homology, they have distinct roles in key cellular events such as cell adhesion, regulation of the cell cycle and apoptosis. In contrast to PYK2, FAK promotes cell-survival and proliferation. Hepatocellular carcinoma is one of the most common cancers. There is rising evidence for FAK to be implicated in hepatic malignancies. AIM: To study the expression of focal adhesion kinases, their activation by growth factors (GF) and their involvement in intracellular pathways. METH: HepG2-cells were cultivated in DMEM standard medium. FAK and PYK2 were detected by immunoprecipitation (IP), their activation assessed by western blotting with phosphotyrosine or phosphospecific antibodies. The involvement with other proteins was assessed by coimmunoprecipitation. RES: FAK, but not PYK2 was expressed in HepG2-cells as shown by WB. The stimulation with the GF IGF (1µM) and HGF (1µM) stimulated the tyrosine phosphorylation (TyrP) of FAK, as well as its autophosphorylation site pY397. Stimulation with HGF (1µM) and IGF (1µM) caused rapid tyrosine phosphorylation, reaching a maximum after 0,5 minutes followed by a rapid decrease. Both, HGF and IGF induced an association between FAK and the Src family kinase pp60Src. The addition of the specific Src-inhibitor PP2, but not of the inactive analogue PP3, inhibited FAK's tyrosine phosphorylation. CONCL: Similar to the expression in other malignant tissues and cell lines, we found FAK expression in HepG2 hepatoma cells. FAK gets activated upon growth factor stimulation as shown by its tyrosine phosphorylation and moreover of its autophosphorylation site. This TyrP is dependent on Src. The association with Src upon stimulation shows FAK's involvement in key signal transduction pathways. These data suggest that FAK mediates growth factor signaling in hepatoma cells, implying a central role in hepatic carcinogenesis for FAK.

It has been proposed that mutations that induce constitutive activity in G-protein-coupled receptors (GPCRs) concomitantly enhance the ability of partial agonists to trigger second-messenger signaling. Using the cholecystokinin type 2 receptor (CCK-2R) as a model system, we have explored whether this association applies to a diverse set of activating mutations. Consistent with established principles, constitutively active CCK-2Rs resulting from amino acid substitutions within the third intracellular loop each systematically increased partial agonist activities versus corresponding wild-type values. In contrast, activating mutations within transmembrane domain segments near the extracellular loops led to an increase in efficacy of only a subset of compounds but decreased or did not change the function of others. When transmembrane domain amino acid substitutions were introduced in combination with intracellular amplifying mutations, observed changes in ligand activity were defined by the product of two discernible factors 1) systematic amplification caused by an equilibrium shift from the inactive to the active receptor conformation and 2) ligand-specific alterations in signaling, which probably result from mutation-induced changes in the putative binding pocket. These findings illustrate functional heterogeneity among GPCR mutants with ligand-independent signaling. A subgroup of activating mutations facilitates receptor isomerization to the active state and in parallel perturbs ligand receptor interactions. These mutants do not adhere to the previously proposed "hallmark criteria" of constitutive activity.
Hintergrund: Nahezu alle Patienten, bei denen sich der Verdacht auf ein Zollinger-Ellison-Syndrom (ZES) ergibt, erhalten eine Therapie mit Protonenpumpeninhibitoren (PPI). Dieses beeinträchtigt die Aussagekraft von Gastrin-Serumspiegelbestimmungen, da es unter Therapie mit Säuresekretionshemmern häufig zu einer reflektorischen Hypergastrinämie kommt. Gleichzeitig kann das Absetzen von PPI bei bestehendem ZES innerhalb kürzester Zeit zur Entwicklung von Ulzera und Komplikationen führen. Die für Gastrinome typische Stimulierbarkeit der Gastrinsekretion durch Sekretin ist die Grundlage des Sekretintests, der eine hohe Spezifität und Sensitivität in der Diagnostik des ZES besitzt. Unklar ist, ob auch für den Sekretintest das Absetzen einer säuresekretionshemmenden Therapie empfohlen werden muss. Wir führten daher Sekretintests unter PPI-Therapie bei Patienten mit gesichertem ZES und Patienten ohne ZES durch.
OBJECTIVE:The cholecystokinin(2)-receptor (CCK(2)R) promotes secretion and cell growth induced by its ligands cholecystokinin (CCK) and gastrin. The receptor has recently been shown to be expressed in human medullary thyroid carcinomas (MTCs). The objective of this study was to analyze CCK(2)R expression in MTC samples of different tumor stages as well as in non-malignant thyroid tissues.DESIGN AND METHODS:Using RT-PCR we investigated 19 MTC samples and TT-cells (a human MTC cell line), as well as samples of normal thyroid. In addition, we performed immunohistochemistry using calcitonin- and CCK(2)R-specific antibodies on MTCs and samples of C-cell hyperplasia.RESULTS:We demonstrate for the first time that CCK(2)R is expressed not only in MTCs but in all samples of normal thyroid tissue. Using immunohistochemistry the receptor could be localized on calcitonin-secreting C-cells. The highest incidence of CCK(2)R expression in MTCs was observed in early-tumor stages, whereas CCK(2)R could not be detected in advanced or metastasized tumors.CONCLUSIONS:The expression of CCK(2)R in C-cells suggests a physiological function for gastrin and/or CCK in the regulation of calcitonin release, presumably related to bone and calcium metabolism. Moreover, these ligands might act as growth factors in MTCs. Efforts in the development of CCK(2)R scintigraphy for the detection of MTC lesions might have to consider a lower incidence of the receptor in advanced tumor stages.
Recent efforts have focused on identifying small nonpeptide molecules that can mimic the activity of endogenous peptide hormones. Understanding the molecular basis of ligand-induced receptor activation by these divergent classes of ligands should expedite the process of drug development. Using the cholecystokinin-B/gastrin receptor (CCK-BR) as a model system, we have recently shown that both affinity and efficacy of nonpeptide ligands are markedly affected by amino acid alterations within a putative transmembrane domain (TMD) ligand pocket. In this report, we examine whether residues projecting into the TMD pocket determine the pharmacologic properties of structurally diverse CCK-BR ligands, including peptides and synthetic peptide-derived partial agonists (peptoids). Nineteen mutant human CCK-BRs, each including a single TMD amino acid substitution, were transiently expressed in COS-7 cells and characterized. Binding affinities as well as ligand-induced inositol phosphate production at the mutant CCK-BRs were assessed for peptides (CCK-8 and CCK-4) and for peptoids (PD-135,158 and PD-136, 450). Distinct as well as overlapping determinants of peptide and peptoid binding affinity were identified, supporting that both classes of ligands, at least in part, interact with the CCK-BR TMD ligand pocket. Eight point mutations resulted in marked increases or decreases in the functional activity of the synthetic peptoid ligands. In contrast, the functional activity of both peptides, CCK-8 and CCK-4, was not affected by any of the CCK-BR mutations. These findings suggest that the mechanisms underlying activation of G-protein-coupled receptors by endogenous peptide hormones versus synthetic ligands may markedly differ.
