Atypical progeroid syndromes (APS) are premature aging syndromes caused by pathogenic LMNA missense variants, associated with unaltered expression levels of lamins A and C, without accumulation of wild-type or deleted prelamin A isoforms, as observed in Hutchinson-Gilford progeria syndrome (HGPS) or HGPS-like syndromes. A specific LMNA missense variant, (p.Thr528Met), was previously identified in a compound heterozygous state in patients affected by APS and severe familial partial lipodystrophy, whereas heterozygosity was recently identified in patients affected by Type 2 familial partial lipodystrophy. Here, we report four unrelated boys harboring homozygosity for the p.Thr528Met, variant who presented with strikingly homogeneous APS clinical features, including osteolysis of mandibles, distal clavicles and phalanges, congenital muscular dystrophy with elevated creatine kinase levels, and major skeletal deformities. Immunofluorescence analyses of patient-derived primary fibroblasts showed a high percentage of dysmorphic nuclei with nuclear blebs and typical honeycomb patterns devoid of lamin B1. Interestingly, in some protrusions emerin or LAP2α formed aberrant aggregates, suggesting pathophysiology-associated clues. These four cases further confirm that a specific LMNA variant can lead to the development of strikingly homogeneous clinical phenotypes, in these particular cases a premature aging phenotype with major musculoskeletal involvement linked to the homozygous p.Thr528Met variant.
IMPORTANCE Ataxia with oculomotor apraxia type 1 (AOA1) is an autosomal recessive cerebellar ataxia due to mutations in the aprataxin gene (APTX) that is characterized by early-onset cerebellar ataxia, oculomotor apraxia, axonal motor neuropathy, and eventual decrease of albumin serum levels. OBJECTIVES To improve the clinical, biomarker, and molecular delineation of AOA1 and provide genotype-phenotype correlations. DESIGN, SETTING, AND PARTICIPANTS This retrospective analysis included the clinical, biological (especially regarding biomarkers of the disease), electrophysiologic, imaging, and molecular data of all patients consecutively diagnosed with AOA1 in a single genetics laboratory from January 1, 2002, through December 31, 2014. Data were analyzed from January 1, 2015, through January 31, 2016. MAIN OUTCOMES AND MEASURES The clinical, biological, and molecular spectrum of AOA1 and genotype-phenotype correlations. RESULTS The diagnosis of AOA1 was confirmed in 80 patients (46 men [58%] and 34 women [42%]; mean [SD] age at onset, 7.7 [7.4] years) from 51 families, including 57 new (with 8 new mutations) and 23 previously described patients. Elevated levels of a-fetoprotein (AFP) were found in 33 patients (41%); hypoalbuminemia, in 50 (63%). Median AFP level was higher in patients with AOA1 (6.0 ng/mL; range, 1.1-17.0 ng/mL) than in patients without ataxia (3.4 ng/mL; range, 0.8-17.2 ng/mL; P < .01). Decreased albumin levels (rho = -0.532) and elevated AFP levels (rho = 0.637) were correlated with disease duration. The p. Trp279* mutation, initially reported as restricted to the Portuguese founder haplotype, was discovered in 53 patients with AOA1 (66%) with broad white racial origins. Oculomotor apraxia was found in 49 patients (61%); polyneuropathy, in 74 (93%); and cerebellar atrophy, in 78 (98%). Oculomotor apraxia correlated with the severity of ataxia and mutation type, being more frequent with deletion or truncating mutations (83%) than with presence of at least 1 missense variant (17%; P < .01). Mean (SD) age at onset was higher for patients with at least 1 missense mutation (17.7 [11.4] vs 5.2 [2.6] years; P < .001). CONCLUSIONS AND RELEVANCE The AFP level, slightly elevated in a substantial fraction of patients, may constitute a new biomarker for AOA1. Oculomotor apraxia may be an optional finding in AOA1 and correlates with more severe disease. The p. Trp279* mutation is the most frequent APTX mutation in the white population. APTX missense mutations may be associated with a milder phenotype.
Introduction Le syndrome myasthenique de Lambert Eaton non paraneoplasique (SMLE-NP) est une canalopathie auto-immune rare et de diagnostic difficile. Le SMLE est du a la presence d’auto anticorps diriges contre le versant presynaptique de la jonction neuromusculaire. Observation Nous rapportons 8 cas de SMLE-NP (2 hommes et 6 femmes). L’âge au debut varie entre 17 et 49 ans, la duree de la maladie varie entre 8 et 30 ans et le delai diagnostique varie entre 3 mois et 24 ans. Tous les patients presentent un deficit moteur predominant sur la ceinture pelvienne avec une hypo ou areflexie et une atteinte dysautonomique. Tous les patients ont le trace caracteristique du SMLE : diminution de l’amplitude du potentiel d’action au repos et un increment anormal de l’amplitude immediatement apres stimulations repetitives a haute frequence. Les anticorps anti-canaux calciques voltage-dependants (CCVD) doses chez 2 patientes sont augmentes. Trois patientes ont une autre pathologie auto-immune associee au SMLE. Le traitement a entraine une amelioration chez tous les patients. Une stabilisation sous traitement est observee 6 patients. Une remission sans sequelles depuis 6 ans est observee chez la plus jeune patiente. Discussion Le SMLE-NP peut se voir a tout âge avec une legere predominance feminine, alors que classiquement la forme paraneoplasique est plus frequente chez l’homme apres 50 ans. Le diagnostic est evoque devant un deficit moteur de la ceinture pelvienne, une areflexie et dysautonomie. Une potentialisation aux stimulations repetitives a haute frequence et la presence d’anticorps anti-CCVD confirment le diagnostic. Conclusion Le diagnostic du SMLE est souvent difficile et probablement sous diagnostique. Les formes non paraneoplasiques sont de meilleurs pronostics. De rares cas de remission ont ete rapportes.
Background Bilateral subthalamic nucleus deep brain stimulation (STN-DBS) of parkinson’s disease (PD) patients has demonstrated to improve motor performance and to reduce dopa-induced dyskinesia. An association between the occurrence of dyskinesias and LRRK2 (leucine-rich repeat kinase 2) G2019S gene mutations has recently been suggested. The aim of this study is to discover the impact of the G2019S mutation (with high incidence in the authors’ native Algeria) on the symptom response of PD in patients who underwent STN-DBS. Methods We carried out a comparative statistical study for the clinical evaluation and neuropsychological assessment of 27 Algerian PD STN-DBS patients, both G2019S mutation carriers (MC) and non-carriers (NC). A multiple correspondence analysis (MCA) was then conducted to compare the results with those from groups of individuals with similar modalities. Results The MCA revealed that MC and NC PD patients showed two different patterns of clinical evaluations. The group of idiopathic patients showed some differences compared to the clinical evaluations, depending on gender. No association was found between the G2019S mutation and the Mini Mental State Examination scores (MMSE), and MC patients appeared more susceptible to dyskinesia than NC patients. In NC patients, we found two cases with Parkin mutations who had a different “honeymoon” period and different initial symptoms. The results showed considerable improvement of motor unified parkinson’s disease rating scale III (UPDRS-III) in a situation of stimulation without medication in the MC patients with a percentage of improvement (51.1 %) over the required 30 % compared to the NC patients (25.5 %). The same result was observed for the Schwab and England’s activities of daily living scale (S and E scale), which thus demonstrated a greater effectiveness of DBS for MC patients than for NC patients. However, the Hoehn and Yahr scale (H and Y Scale) showed the same significance in a situation of stimulation for MC and NC patients. In this later group, the best scores of UPDRS-III were observed for patients with the Parkin mutation before they underwent surgery. Conclusions This study shows that surgical treatment probably has a more significant impact on LRRK2 G2019S MC than on idiopathic patients.
Les PIDC sont des neuropathies acquises, présumées d'origine dysimmunitaires et pouvant répondre favorablement à un traitement immunomodulateur. Analyser les données cliniques, électrophysiologiques et biologiques ainsi que la réponse au traitement et le pronostic. Nous avons essayé d'identifier des paramètres cliniques et évolutifs pouvant laisser prévoir une bonne réponse au traitement et un meilleur pronostic. Étude prospective sur 6,5 ans. Les 71 patients remplissent les critères EFNS/PNS. Le déficit moteur et le retentissement fonctionnel sont évalués par le MRC et l'ONLS. Nous avons évalué à 6 mois la réponse initiale au traitement et le pronostic à la fin de l'étude. Dans le but de rechercher des facteurs pronostiques, nous avons comparé les patients répondeurs et les non répondeurs et les patients bons répondeurs avec les répondeurs partiellement. Forme classique représente 41 %, les formes à début rapide 32 %, la forme à rechutes 30 %. La réponse initiale au traitement est bénéfique chez 68 %. Des facteurs prédisant une bonne réponse au traitement et un meilleur pronostic ont été identifiés : –précocité du traitement ; –formes à début rapide ou à rechutes ; –forme classique ; –présence d'une atteinte nerfs crâniens ou déficit moteur proximal ou score initial d'handicap sévère et l'absence d'atrophie. Quatre-vingt-quinze pour cent des cas initialement bons répondeurs sont asymptomatiques à la fin de l'étude. Le spectre clinique et évolutif des PIDC est très variable. L'efficacité initiale des traitements est observée dans 50 % à 90 % des cas. Certains facteurs pronostiques prédisant une réponse initiale favorable au traitement et un meilleur pronostic ont été mis en avant dans certaines études. Cette hétérogénéité clinique, évolutive, pronostique et de la réponse au traitement dans les PIDC est souvent rapportée. Dans certains cas, la précocité du traitement peut permettre un meilleur pronostic.
Autosomal recessive primary microcephaly is a neurodevelopmental disorder characterized by congenitally reduced head circumference by at least two standard deviations (SD) below the mean for age and gender. It is associated with nonprogressive mental retardation of variable degree, minimal neurological deficit with no evidence of architectural anomalies of the brain. So far, 12 genetic loci (MCPH1-12) and corresponding genes have been identified. Most of these encode centrosomal proteins. CASC5 is one the most recently unravelled genes responsible for MCPH with mutations reported in three consanguineous families of Moroccan origin, all of whom harboured the same CASC5 homozygous mutation (c.6125G>A; p.Met2041Ile). Here, we report the identification, by whole exome sequencing, of the same missense mutation in a consanguineous Algerian family. All patients exhibited a similar clinical phenotype, including congenital microcephaly with head circumferences ranging from -3 to -4 standard deviations (SD) after age 5 years, moderate to severe cognitive impairment, short stature (adult height -3 SD), dysmorphic features included a sloping forehead, thick eyebrows, synophris and a low columella. Severe vermis hypoplasia and a large cyst of the posterior fossa were observed in one patient. Close microsatellite markers showed identical alleles in the Algerian the previously and Moroccan patients. This study confirms the involvement of CASC5 in autosomal recessive microcephaly and supports the hypothesis of a founder effect of the c.6125G>A mutation. In addition, this report refines the phenotype of this newly recognized form of primary microcephaly.
BACKGROUND:Autosomal recessive cerebellar ataxias (ARCA) are a complex group of neurodegenerative disorders with great genetic and phenotypic heterogeneity, over 30 genes/loci have been associated with more than 20 different clinical forms of ARCA. Genetic heterogeneity combined with highly variable clinical expression of the cerebellar symptoms and overlapping features complicate furthermore the etiological diagnosis of ARCA. The determination of the most frequent mutations and corresponding ataxias, as well as particular features specific to a population, are mandatory to facilitate and speed up the diagnosis process, especially when an appropriate treatment is available.METHODS:We explored 166 patients (115 families) refered to the neurology units of Algiers central hospitals (Algeria) with a cerebellar ataxia phenotype segregating as an autosomal recessive pattern of inheritance. Genomic DNA was extracted from peripheral blood samples and mutational screening was performed by PCR and direct sequencing or by targeted genomic capture and massive parallel sequencing of 57 genes associated with inherited cerebellar ataxia phenotypes.RESULTS:In this work we report the clinical and molecular results obtained on a large cohort of Algerian patients (110 patients/76 families) with genetically determined autosomal recessive ataxia, representing 9 different types of ARCA and 23 different mutations, including 6 novel ones. The five most common ARCA in this cohort were Friedreich ataxia, ataxia with isolated vitamin E deficiency, ataxia with oculomotor apraxia type 2, autosomal recessive spastic ataxia of Charlevoix-Saguenay and ataxia with oculomotor apraxia type 1.CONCLUSION:We report here a large cohort of patients with genetically determined autosomal recessive ataxia and the first study of the genetic context of ARCA in Algeria. This study showed that in Algerian patients, the two most common types of ataxia (Friedreich ataxia and ataxia with isolated vitamin E deficiency) coexist with forms that may be less common or underdiagnosed. To refine the genotype/phenotype correlation in rare and heteregeneous diseases as autosomal recessive ataxias, more extensive epidemiological investigations and reports are necessary as well as more accurate and detailed clinical characterizations. The use of standardized clinical and molecular protocols would thus enable a better knowledge of the different forms of ARCA.
Les paraplégies spastiques héréditaires représentent est un groupe d’affection neurodégénérative génétiquement hétérogène, caractérisées par une hyper-reflexie et une spasticité progressive des membres inférieurs. Il s’agit de 2 sœurs et un frère nés de parents apparentés âgés entre 11 et 20 ans sans antécédents pré- et post-nataux. La paraparésie spastique s’est installée vers l âge de 2 ans sauf pour la sœur aînée qui a présenté un retard à la marche. L’examen objectivé une microcéphalie entre moins 2–3 ds, une légère déficience intellectuelle et une petite taille chez les 2 sœurs. L’analyse cytochimique du LCR est normale. L’IRM et le scanner cérébral ne montrent ni calcifications ni anomalies de la myéline. Le séquençage de l’exome a permis d’identifier une mutation homozygote c.529G>A p.Ala177Thr du gène RNASEH2B chez les 3 patients. Le syndrome d’Aicardi-goutières est une affection du cerveau et de la peau à début précoce avec des calcifications cérébrales due à une mutation de six gènes dont RNASEH2B. La description récente d’une paraparésie spastique en rapport avec des mutations des gène ADAR1, IFIH1 et RNASEH2B a permis d’élargir le spectre phénotypique clinique du syndrome Aicardi-goutiéres. la paraparésie spastique héréditaire représente une forme clinique du syndrome Aicardi-goutiéres.
Les Ac anti-amphiphysines sont des Ac onco-neuronaux, mis en évidence dans les syndromes paranéoplasiques neurologiques. Leur présence doit faire rechercher le cancer du sein et le carcinome bronchique à petites cellules. Nous rapportons le cas d'une patiente âgée de 39 ans, qui présente depuis 9 mois des paresthésies à type de fourmillements, décharges éléctriques intenses et invalidantes, au niveau du membre supérieur gauche s'étant étendues au bout d'un mois au membre controlatéral et aux 2 membres inférieurs, 5 mois plus tard apparut une sensation de marcher sur du coton avec un déséquilibre à la marche. L'examen neurologique montrait un syndrome neurogène périphérique aux 4 membres (DN4 : 5/10) et un discret syndrome cérébélleux statique. L'EMG révéla une neuropathie périphérique sensitive axonale. Les Ac anti-amphiphysines étaient positifs(+++). L'ensemble des autres anticorps antineuronaux, les marqueurs tumoraux, le scanner thoracoabdominopelvien, la mammographie, l'échographie mammaire, la colorectoscopie et la fibroscopie digestive étaient normaux. Devant ce tableau de neuropathie sensitive douloureuse avec Ac anti-amphiphysines positifs, le diagnostic de neuropathie de Denny Brown a été retenu. Les cancers les plus fréquemment associés à ces Ac sont ceux du sein et du poumon, cependant dans la majorité des cas la tumeur n'est décelée que tardivement… Intérêt du dosage des Ac anti-neurones pour reconnaître l'origine paranéoplasique de l'affection neurologique et guider la recherche de la tumeur à un stade précoce où elle peut encore être traitée.
Le syndrome de chevauchement est la forme la plus fréquente des myopathies inflammatoires idiopathiques, associant une myosite inflammatoire et une connectivite et ou des auto-anticorps spécifiques (Troyanov, 2005). Nous rapportons le cas clinique d'un patient âgé de 62 ans, qui a présenté brutalement une tête tombante associée un mois après à une diplégie faciale et un déficit moteur proximal aux 4 membres et des myalgies diffuses, ayant évolué rapidement vers l'aggravation avec survenue trois mois après des troubles de la déglutition et d'une détresse respiratoire aiguë. L'examen neurologique a montré une diplégie faciale, tête tombante, un syndrome myogène aux 4 membres. L'examen somatique a montré des lésions cutanées d'une sclérodermie. L'auscultation : des râles crépitant diffus. Biologiquement : un syndrome inflammatoire : Vs accélérée à 110 la 1re heure, à 115 la 2e heure, CRP positive 96 mg/L. FAN, AC anti-PM-Scl, Ac anti-RO-52 : positifs. Une atteinte myogène à l'EMG. La biopsie musculaire était en faveur d'une polymyosite. TDM thoracique montrait un aspect de fibrose pulmonaire. Devant ce tableau de myosite de chevauchement, le patient a été mis sous corticothérapie à forte dose entraînant une amélioration spectaculaire. L'association d'une myosite à une sclérodermie est la forme la plus fréquente des syndromes de chevauchement (40 %), souvent la connectivite est contemporaine à la myosite sur plusieurs années (70 % des cas). Or que chez notre patient, la tête tombante a précédé l'atteinte musculaire des membres et les troubles respiratoires, le tout a évolué rapidement vers l'aggravation au bout de 3 mois. Les Ac anti-PM-SCL sont rarement positifs 10 % (des cas). Savoir penser à ce syndrome qui malgré sa fréquence il est parfois de diagnostic difficile devant l'inauguration du tableau par des signes atypiques (tombante), une corticothérapie à temps peut éviter les complications.
L'insensibilité congénitale à la douleur est un syndrome clinique rare, elle se caractérise par une absence ou une diminution de la sensibilité douloureuse, des formes cliniques variées et hétérogènes sur le plan génétique. Nous rapportons une série de 10 cas d'insensibilité congénitale à la douleur (ICD) dont 3 familles et 3 cas sporadiques colligés dans notre service de neurologie entre 1994 et 2014. Les patients avaient un phénotype clinique compatible avec les différents types de neuropathies sensitives dysautonomiques et héréditaires décrites par Dyck et al. en 1993. Ils ont tous présenté une altération profonde de la sensibilité douloureuse associée ou non à un trouble des autres modalités sensitives, des manifestations cutanées (ulcérations, hyperkératose), arthropathies nerveuses et fractures, des signes dysautonomiques (fièvre, anhidrose), retard mental et hyperactivité. Chacun de nos patient a bénéficie d'un EMG qui a permis d'éliminer l'atteinte des fibres de gros diamètre, d'un examen ophtalmologique avec test de Schirmer, de tests neurophysiologiques. L'évolution a été marquée par la survenue de complications diverses (abcès dentaires indolores, arthropathies nerveuses récidivantes, ostéomyélites, fractures indolores, amputations de la langue, des phalanges, maux perforants plantaire, mais également par la difficulté diagnostique et de prise en charge). La moitie de nos patients ont été classés type IV selon la classification de Dyck et al. ont présenté des complications sévères ceci concorde avec les données de la littérature la présence d'une hyperactivité chez cinq patients associée parfois à un retard mental et des difficultés d'apprentissage, chez deux familles le diagnostique de canalopathie a été évoqué, le diagnostique est souvent tardif devant la survenue de fièvre récurrente inexpliquée. Le diagnostic d'ICD doit être évoqué devant une fièvre inexpliquée avec anhidrose chez le nourrissons de moins de 9 mois, des automutilations au delà de 9 mois, ce qui permet une meilleure prévention des complications.
La chorée de Sydenham, complication sévère mais curable du rhumatisme articulaire aigu, bien que rare de nos jours, sévit encore sporadiquement Elle atteint surtout les enfants, et ne doit pas être méconnue. Nous rapportons le cas d une fille de 12 ans, aux antécédents d angines à répétition non traitées, hospitalisée pour des mouvements anormaux involontaires anarchiques, touchant les quatre membres et la face associée à une régression intellectuelle, évoluant depuis un mois, dans un contexte de dyspnée à l effort. L IRM cérébrale était sans particularités. l’échographie cardiaque a objectivé des valvulopathies mitrale et aortique. Les bilans biologiques ont montré un syndrome inflammatoire évolutif avec des ASLO élevés. Le diagnostic de RAA compliqué d’une chorée de Sydenham et d’une cardite fut retenu et le traitement par halopéridol et corticoïdes a entraîné une amélioration spectaculaire de la symptomatologie autant sur le plan moteur que cognitif. La chorée de Sydenham, complication de l’infection par le streptocoque B hémolytique du groupe A, était il y a un siècle la cause la plus fréquente de chorée, actuellement beaucoup plus rare, elle serait en rapport avec un phénomène auto-immun de mimétisme moléculaire. Son pronostic n est pas toujours favorable et des rechutes sont possibles, d’où l’intérêt d’une prise en charge thérapeutique précoce et d’un suivi régulier des patients. La chorée de Sydenham se voit encore dans nos régions et seule une prophylaxie primaire par le traitement systématique des angines streptococciques permettra une réduction de son incidence et de ses conséquences.
Duchenne and Becker muscular dystrophies (DMD and BMD) represent the most frequent neuromuscular diseases in humans (1/3,500–6,000 live male births), characterized by an X-linked recessive pattern of inheritance and therefore affecting mainly male individuals. DMD and BMD are allelic disorders resulting from genetic defects, mostly intragenic deletions, in the dystrophin gene. Using multiplex polymerase chain reaction (PCR), we have analyzed 170 male patients from unrelated families originating from Algeria, showing that 68 % of them harbored deletion events affecting the known 5′ or 3′ hot spot regions. The distal portion was predominantly involved (85 %), whereas 37 distinctive patterns of deletion were identified in our panel. The extent of deletion varied from 1 to 32 exons, although the average number was about four exons. The lack of seven exons (45, 46, 47, 48, 50, 51 and 52), each alone or in combination, represented about 78 % of the alterations encountered, while exon 48 was most frequently involved (50 %). The effect of the deletions showed that the reading frame rule proved mostly true, correlating with the clinical diagnosis suggested. Moreover, the c.525delT mutation in the γ-sarcoglycan gene was present in non-deleted patients (7 %), suggesting that clinical features can still be misleading. Finally, multiplex PCR proved to be a simple, fast and low-cost approach for the molecular diagnosis of dystrophinopathies in Algeria, whereas our data could contribute to the creation of a national registry of DMD/BMD patients in our country, which would give them hope to an access to already available genotype-based therapies.
Tel2-interacting proteins 1 and 2 (TTI1 and TTI2) physically interact with telomere maintenance 2 (TEL2) to form a conserved trimeric complex called the Triple T complex. This complex is a master regulator of phosphoinositide-3-kinase-related protein kinase (PIKKs) abundance and DNA damage response signaling. Using a combination of autozygosity mapping and high-throughput sequencing in a large consanguineous multiplex family, we found that a missense c.1307T>A/p.I436N mutation in TTI2 causes a human autosomal recessive condition characterized by severe cognitive impairment, microcephaly, behavioral troubles, short stature, skeletal anomalies, and facial dysmorphic features. Immunoblotting experiment showed decreased amount of all Triple T complex components in the patient skin fibroblasts. Consistently, a drastically reduced steady-state level of all PIKKs tested was also observed in the patient cells. Combined with previous observations, these findings emphasises the role of the TTI2 gene in the etiology of intellectual disability and further support the role of PIKK signaling in brain development and functioning.
La sclérose latérale amyotrophique est une affection dégénérative caractérisée par une dégénérescence du neurone moteur central et périphérique dans le territoire bulbaire et spinal. Le diagnostic en est souvent difficile en raison de l'hétérogénéité phénotypique et surtout de l'absence de marqueur paraclinique qui signerait l'affection. Dans la mesure où il est apparu indispensable d'établir des critères diagnostiques, trois conférences de consensus se sont tenues au cours des 20 dernières années. La première classification reposait sur l'existence de signes cliniques d'atteinte du NMc et du NMp au sein du système nerveux décomposé en quatre régions anatomiques. Compte tenu de la faible sensibilité de ces critères initiaux deux autres conférences ont eu lieu ensuite. La dernière qui s'est déroulée à Awaji en 2006 a insisté sur l'importance des données myographiques qui apportaient des informations semblables à la clinique pour dépister une atteinte du NMp ainsi que sur l'importance des fasciculations dans le diagnostic de SLA. Nous discuterons de l'intérêt et des limites de ces nouveaux critères dans le diagnostic de SLA.Amyotrophic lateral sclerosis is the most common motor neuron disorder in adults. Although the diagnosis appears obvious in theory, clinical practice shows the contrary as diagnosis is delayed in many patients; the average time between symptom onset and diagnosis can reach 12 months. The delay can be explained by the variability of the clinical presentation and by the absence of diagnostic markers. In order to standardize diagnosis for enrolment in clinical research, diagnostic criteria for ALS were created and revisited during the last 20 years. In 2006, the Awaji criteria for the diagnosis of ALS were proposed, adding two major points to the diagnostic criteria: electromyography is considered equivalent to clinical examination for the identification of LMN signs and fasciculation potentials resume their prominent place in the diagnosis. Comparisons of the accuracy of the revisited El Escorial and Awaji criteria support improved diagnostic sensitivity without any effect on specificity with the new classification. The only weakness of the new classification involves patients with UMN signs in one region and LMN in two regions; these patients were previously classified as laboratory-supported probable ALS and currently as possible ALS, a lower level of diagnostic certainty. In all other instances the accuracy appears to be improved by the Awaji criteria. Nevertheless, there is a body of evidence suggesting the need for a revision of these new criteria, giving more weight to clinical and complementary findings of UMN involvement. The need to diagnose and treat ALS quickly could be facilitated by the inclusion of complementary investigations that detect UMN signs.
In this work, using Sisko model, blood flow is simulated inside an artery which have cone shape of stenosis with different angles of φ = 0.25, φ = 0.5, φ = 0.75, φ = 1 and φ = 1.25 degree, respectively.In the first step, an artery radius of 0.002 m is fixed to study the effects of cone shape of arterial stenosis on the flow behavior. Then, stenosis angle of φ = 0.5 degree is fixed to study the effects of different Artery radii of 0.002 m, 0.0025 m, 0.003m, and 0.0035 m orderly on the flow behavior. For simulation the blood flow, Sisko model is used. Afterward, stenosis angle of φ = 0.5 degrees with a radius of 0.002m is fixed for investigating the influences of different behavior of blood fluid by manipulation of constant parameters of the Sisko model.It is reported that with increasing arterial stenosis angle, maximum blood flow velocity is sharply increased in central region of artery from 0.12 m/s to 0.16 m/s, 0.25 m/s, 0.36 m/s and 0.56 m/s in order of increasing stenosis angles from φ = 0.25 to φ = 0.5, φ = 0.75, φ = 1 and φ = 1.25 degree, respectively. Also, maximum shear stress of artery wall are as much as 64 Pa, 42 Pa, 24 Pa, 18 Pa and 16Pa respectively in order of stenosis angles of φ = 0.25, φ = 0.5, φ = 0.75, φ = 1 and φ = 1.25 degree. On the other side, the effect of increasing artery radius is against the influences of stenosis angle, and contradiction of these parameters is affected by the stress tension and viscosity of blood.Variations of blood behavior from non-Newtonian to Newtonian shows that shear stress in blood stream in the stenosis artery with non-Newtonian blood is higher than that of Newtonian blood due to differences in their viscous behaviors and reactions in exposure of stenosis and artery wall effects.
Les troubles mnésiques, plus particulièrement ceux de la mémoire épisodique verbale (MEV), constituent un motif fréquent de plainte cognitive chez les patients atteints de sclérose en plaques (SEP). Outre la gêne occasionnée dans la vie quotidienne, ces troubles peuvent avoir un impact sur les activités professionnelles. L’évaluation neuropsychologique des patients doit donc comporter des tests explorant la MEV, de façon à décrire le degré d’atteinte des différents processus cognitifs contribuant à la MEV et, le cas échéant, pouvoir proposer des stratégies rééducatives adaptées. L’objectif de cet article est de proposer une revue critique de la littérature portant sur les capacités de MEV dans la SEP. Cette revue permettra de présenter les références conceptuelles et les caractéristiques psychométriques des principaux tests de MEV appliqués dans cette pathologie (épreuves isolées ou intégrées à des batteries cognitives plus globales développées pour la SEP). Dans un second temps, nous proposons un inventaire, par processus cognitifs, des travaux réalisés dans cette affection. Cette approche permettra d’aborder les limites de chaque conception et les éventuelles ambiguïtés terminologiques. Les contributions de ces travaux aux connaissances sur les troubles mnésiques de la SEP seront précisées, ainsi que l’impact des caractéristiques de la maladie (formes de SEP, durée d’évolution, EDSS).Memory impairment, especially verbal episodic memory (VEM), represents a common ground for cognitive complaint in patients with multiple sclerosis (MS). Beyond the difficulty caused in daily life, these deficits may impact on occupational activities. Neuropsychological assessment of these patients has to include VEM tests, to describe the level of dysfunction of the different processes contributing to VEM and, if required, to guide adapted cognitive rehabilitation. The objective of the present paper is to propose a critique review of the literature on VEM abilities in MS. This review will present the conceptual references and the psychometric characteristics of the main VEM tests applied in MS (isolated tests or included within more general batteries developed specifically for MS). In a second phase, we propose an inventory of work on MS presented as a function of the cognitive processes involved. This approach provides an approach to the limitations of each conception and possible terminological ambiguities. Contributions to knowledge of MS memory impairments will be clarified, as well as the impact of the disease characteristics (MS forms, disease duration, EDSS).
The diagnosis of rare inherited diseases is becoming more and more complex as an increasing number of clinical conditions appear to be genetically heterogeneous. Multigenic inheritance also applies to the autosomal recessive progressive cerebellar ataxias (ARCAs), for which 14 genes have been identified and more are expected to be discovered. We used homozygosity mapping as a guide for identification of the defective locus in patients with ARCA born from consanguineous parents. Patients from 97 families were analyzed with GeneChip Mapping 10K or 50K SNP Affymetrix microarrays. We identified six families homozygous for regions containing the autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) gene, two families homozygous for the ataxia-telangiectasia gene (ATM), two families homozygous for the ataxia with oculomotor apraxia type 1 (AOA1) gene, and one family homozygous for the AOA type 2 (AOA2) gene. Upon direct gene testing, we were able to identify a disease-related mutation in all families but one of the two kindred homozygous at the ATM locus. Although linkage analyses pointed to a single locus on chromosome 11q22.1-q23.1 for this family, clinical features, normal levels of serum alpha-foetoprotein as well as absence of mutations in the ATM gene rather suggest the existence of an additional ARCA-related gene in that interval. While the use of homozygosity mapping was very effective at pointing to the correct gene, it also suggests that the majority of patients harbor mutations either in the genes of the rare forms of ARCA or in genes yet to be identified.
