Peptide nucleic acid fluorescence in situ hybridization (PNA FISH) is a molecular diagnostic tool for the rapid detection of pathogens directly from liquid media. The aim of this study was to prospectively evaluate PNA FISH assays in comparison with culture-based matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) identification, as a reference method, for both blood and cerebrospinal fluid (CSF) cultures, during a 1-year investigation. On the basis of the Gram stain microscopy results, four different PNA FISH commercially available assays were used ('Staphylococcus aureus/CNS', 'Enterococcus faecalis/OE', 'GNR Traffic Light' and 'Yeasts Traffic Light' PNA FISH assays, AdvanDx). The four PNA FISH assays were applied to 956 positive blood cultures (921 for bacteria and 35 for yeasts) and 11 CSF cultures. Among the 921 blood samples positive for bacteria, PNA FISH gave concordant results with MALDI-TOF MS in 908/921 (98.64%) samples, showing an agreement of 99.4% in the case of monomicrobial infections. As regards yeasts, the PNA FISH assay showed a 100% agreement with the result obtained by MALDI-TOF MS. When PNA FISH assays were tested on the 11 CSF cultures, the results agreed with the reference method in all cases (100%). PNA FISH assays provided species identification at least one work-day before the MALDI-TOF MS culture-based identification. PNA FISH assays showed an excellent efficacy in the prompt identification of main pathogens, yielding a significant reduction in reporting time and leading to more appropriate patient management and therapy in cases of sepsis and severe infections.
On December 2006, an outbreak of gastroenteritis occurred at a residential-care facility for the elderly in northern Italy. Thirty-five of 61 individuals interviewed (attack rate, 57.4%) fell ill. In 94.3% of cases, the onset of illness was within 48 h of a Christmas party at the facility. Norovirus (NoV) was detected by RT-PCR in 24 of 31 individuals examined, including three asymptomatic food-handlers, in whom there was evidence of long-lasting excretion of viral particles. The identification of a sequence referring to the ‘2006a GII.4 NoV variant’ in all examined strains supported the hypothesis of a common point source. This retrospective cohort study is the first report on an outbreak of NoV gastroenteritis in an Italian residential-care facility for the elderly.
Introduzione.La determinazione della carica virale di HIV-1 su sangue intero essiccato su cartoncino ("Dried Blood Spot"o DBS) presenta dei vantaggi di ordine pratico rispetto al plasma liquido (PL) ed è particolarmente indicata nei paesi in via di sviluppo.Lo scopo del nostro studio è stato quello di verificare l'utilizzo di DBS con il dosaggio Abbott RealTime HIV-1, un test che impiega un particolare formato di tecnologia real-time ("partially double stranded probe"), sviluppato appositamente per tollerare la diversità genetica di HIV-1.Metodi.Sono stati analizzati, per ciascuno dei 169 soggetti con infezione da HIV-1 inclusi nello studio, un campione di PL e uno di DBS.I campioni di PL sono stati testati con il dosaggio Abbott RealTime HIV-1 (protocollo con volume iniziale da 1.0 mL), utilizzando il sistema strumentale Abbott m2000, che prevede estrazione degli acidi nucleici, preparazione e dispensazione della mastermix in totale automazione (m2000sp), seguiti da amplificazione e rivelazione in real-time (m2000rt).I campioni di DBS (Whatman 903) sono stati conservati, prima del dosaggio, a temperatura ambiente per un tempo mediano di 26 giorni (range: 1-170).Per ogni paziente sono stati impiegati due spot da 50 microlitri, ottenuti con un punzonatore ed eluiti con tampone di lisi (kit di estrazione m2000sp).Il surnatante è stato poi analizzato con il sistema m2000sp/m2000rt (protocollo da 1,0 mL).Risultati.I campioni di DBS sono risultati positivi nel 90%, 94,7% e 100% dei campioni con viremia plasmatica compresa tra 2,3 e 3,0; 3,0 e 4,0; 4,0 e 6,1 log copie/mL, rispettivamente.L'analisi di regressione lineare ha mostrato un coefficiente di correlazione r di 0,871.La differenza (media±DS) tra DBS e PL è stata di 0.01±0.39log copie/mL.Il 78,52% (128/163) e il 99,4% (162/163) dei campioni hanno presentato differenza inferiore a 0,5 e 1,0 log, rispettivamente.La differenza DBS-PL non è correlata con il tempo di conservazione dei DBS (R 2 = 0,0008 e 0,0451 per i due gruppi di campioni provenienti dai due siti), a dimostrazione della stabilità dell'RNA di HIV-1 sui cartoncini Whatman 903.Conclusioni.I campioni di DBS possono essere usati come mezzo di raccolta e conservazione del sangue nel monitoraggio dell'infezione da HIV e della risposta alla terapia antiretrovirale.La totale automazione, la capacità di rilevare e quantificare correttamente tutti i sottotipi virali e le eccellenti prestazioni mostrate in questo studio sui DBS , rendono il dosaggio Abbott RealTime HIV adeguato per la routine nei paesi in via di sviluppo con una elevata prevalenza di infezione da HIV-1.

HBV genotypes were investigated in sera/plasma from 97 HBV positive subjects. Genotype D was revealed in 80.4% followed by E in 6.2%. Genotypes A, B, and C were also found, as well as for the first time a new combination of HBV D and G genotypes. In a cohort of subjects of this population, the relationship with lamivudine and/or famciclovir-resistant HBV mutants was also investigated. Among 12 untreated subjects, 25% carried HBV drug-resistant strains suggesting that drug-resistant variants naturally exist in untreated Italian HBV chronically infected subjects.
ABSTRACT We developed a nested reverse transcription-PCR (nRT-PCR) for the detection of noroviruses in stools, using random primers for RT, the JV12/JV13 primer pair in the first round of nPCR, and a set of nine inner primers for the second, comprising the reverse sequences of primers SR46, SR48, SR50, and SR52, and five novel oligonucleotide sequences (113-1, 113-2, 115-1, 115-2, and 115-3). The specificity of the nRT-PCR was confirmed by testing 61 stools containing enteric viruses other than noroviruses. In comparative assays on either stools or RNA dilutions from two genogroup I and three genogroup II (GII) norovirus-positive samples, nRT-PCR was always at least as sensitive as RT-PCR and Southern hybridization. With some of the samples tested, the increase in sensitivity was 10-fold or higher. For GII viruses, the detectable range of nRT-PCR was estimated to be 8.4 × 104 to 2 RNA viral particles. When used on 85 stools from pediatric patients with acute gastroenteritis negative for viruses by electron microscopy and cell culture, the nRT-PCR detected norovirus in 19 samples (22.3%), while it failed to detect one reference RT-PCR-positive sample containing a Desert Shield strain. Sixteen of the 19 nRT-PCR-positive samples gave concordant results with reference RT-PCR and Southern hybridization, and all with sequence analysis. Partial sequencing of the polymerase region revealed that from January to April 2000 all GII strains except two (Rotterdam- and Leeds-like viruses) formed a tight cluster related to Hawaii virus. The nRT-PCR described could prove suitable for large epidemiological studies and for specialized clinical laboratories performing routine molecular testing.

The cellular distribution of the human cytomegalovirus (HCMV)‐specific UL83 phosphoprotein (pp65) and UL123 immediate‐early protein (IEp72) in lytically infected human embryo fibroblasts was studied by means of indirect immunofluorescence and confocal microscopy. Both proteins were found to have a nuclear localization, but they were concentrated in different compartments within the nuclei. The pp65 was located predominantly in the nucleoli; this was already evident with the parental viral protein, which was targeted to the above nuclear compartment very soon after infection. The nucleolar localization of pp65 was also observed at later stages of the HCMV infectious cycle. After chromatin extraction (in the so‐called in situ nuclear matrices), a significant portion of the pp65 remained associated with nucleoli within the first hour after infection, then gradually redistributed in a perinucleolar area, as well as throughout the nucleus, with a granular pattern. A quite different distribution was observed for IEp72 at very early stages after infection of human embryo fibroblasts with HCMV; indeed, this viral protein was found in bright foci, clearly observable in both non‐extracted nuclei and in nuclear matrices. At later stages of infection, IEp72 became almost homogeneously distributed within the whole nucleus, while the foci increased in size and were more evenly spread; in several infected cells some of them lay within nucleoli. This peculiar nuclear distribution of IEp72 was preserved in nuclear matrices as well. The entire set of data is discussed in terms of the necessity of integration for HCMV‐specific products into the pre‐existing nuclear architecture, with the possibility of subsequent adaptation of nuclear compartments to fit the needs of the HCMV replicative cycle. © 2003 Wiley‐Liss, Inc.
I metodi molecolari per la rivelazione di enterovirus (EV) in campioni clinici offrono il vantaggio rispetto al metodo colturale convenzionale di rivelare EV difficilmente o affatto coltivabili e di fornire i risultati nel giro di 24 ore; essi pertanto possono essere utili per il trattamento tempestivo del paziente. In questo studio è stato valutato il saggio Enterovirus NucliSens EasyQ Real-Time NASBA basato sull’impiego della tecnologia di amplificazione NASBA in associazione alla rivelazione mediante “molecular beacons” in tempo reale. Al saggio di specificità tutti i campioni clinici (3 feci e 2 secrezioni respiratorie) contenenti virus a RNA diversi da EV, hanno dato esito negativo mediante Real-Time NASBA. Il saggio di sensibilità, condotto mediante Real-Time NASBA in triplicato ed in tre giorni diversi su diluizioni seriali di RNA estratto da una sospensione fecale negativa per EV addizionata di una sospensione di virus ECHO 6 (concentrazione finale 105 TCID50/ml), ha dato costantemente esito positivo fino alla diluizione contenente 1 TCID50/ml di EV e meno costantemente rispetto all’nRT-PCR in presenza di 0,1 TCID50/ml. Impiegato su 45 campioni clinici (32 feci, 6 liquor, 7 secrezioni respiratorie), Real-Time NASBA ha dato esito concordante con l’nRT-PCR in 43 campioni (36 positivi e 7 negativi) e discordante in 2 campioni che sono risultati Real-Time NASBA negativi e nRT-PCR e/o esame colturale convenzionale positivi, rivelando una sensibilità del 94,7% e una specificità del 100%. Impiegato su 7 campioni del “QCMD EV Proficiency Programme 2002” contenenti EV a diverse concentrazioni, RealTime NASBA ha concordato con l’esito positivo in 6 casi; al contrario ha dato esito negativo su un campione contenente 3,6 x 10-1 TCID50/ml. Grazie alla rapidità e semplicità di esecuzione (3 ore per il completamento della singola fase della RealTime NASBA rispetto a 6 ore per le tre fasi dell’nRTPCR) oltre a buona specificità e sufficiente sensibilità, Enterovirus NucliSens EasyQ Real-Time NASBA si propone come una conveniente alternativa all’nRTPCR per la rivelazione di RNA di EV in tipi diversi di campioni clinici. volume 18, numero 2, 2003 COMUNICAZIONI ORALI
