Neonatale Morbiditätsraten bei bilateraler Nierenagenesie werden immer noch bis 100% geschätzt – in erster Linie aufgrund respiratorischen Versagens infolge fehlender Lungenentwicklung bei Anhydramnie/schwerer Oligohydramnie. Bei pränataler Diagnose wird deshalb mit den Eltern zumeist eine „comfort-care-Versorgung“ des Neugeborenen, aber auch die Möglichkeit einer Abruptio besprochen. Wir präsentieren den Fall eines Neugeborenen, das trotz bestätigter beidseitiger Nierenagenesie eine fast normale Lungenfunktion entwickelte. Unseres Wissens nach ist dies erst der zweite in der Literatur bekannte Fall.
Background and aim The model of arterial-ventricular coupling (AVC) describes the interaction of the left ventricle (ELV) with the arterial system (EA) by the AVC-ratio (AVC = EA/ELV). Aim was to apply the model to preterm haemodynamics and to analyse time courses of AVC, EA and ELV in sick preterms with either pulmonary hypertension (PH-group) or haemodynamically significant patent ductus arteriosus (hPDA-group) and in stable preterms with uncomplicated postpartal course (control-group). Methods Study period was from 10/2009 to 12/2012. Patient recruitment criteria were as follows: anti-PH treatment due to (supra-) systemic pulmonary pressure on echocardiography (PH-group); presence of PDA with an enddiastolic maximal velocity in the left pulmonary artery (LPAdia) ≥ 0,2 m/s and negative history of PH (hPDA-group); neither anti-PH treatment nor catecholamines, PDA with an LPAdia < 0,2 m/s and a ratio of the left atrium/aorta < 1,4 (control-group). AVC was calculated from blood pressure and M-mode measurements. Selected time points were set from days 1–3, 4–7 and 8–30 respectively. Results Twentyone preterms were recruited to the PH-group, 19 to the hPDA-group and 63 to the control-group. AVC was lower in the PH- and hPDA-group than in the control-group (p = 0,05). ELV was higher in the PH-group (p = 0,007) and both EA and ELV were lower in the hPDA-group (EA: p = 0,0002; ELV: p = 0,02). Conclusion The AVC-ratio was lower in sick preterms. Higher ELV in PH results from interventricular interdependence with decreased LV-filling. Lower ELV and EA in PDA result from LV-volume-overload and systemic steal-effect. Applying the AVC-model may facilitate explaining preterm haemodynamics.
Introduction Acute renal failure is a severe complication in neonatal intensive care medicine. The only therapeutic option for renal replacement therapy in small infants or premature babies with acute renal failure is peritoneal dialysis. In this paper two case reports of sealing an untight peritoneal catheter with cyanoacrylate glue will be presented. Patient 1: Premature infant of 34 + 2 weeks gestational age. Dorsal cloaca malformation with megacystis and megaureter, peritoneal dialysis for 13 days. Patient 2: Term neonate of 40 + 4 weeks gestational age with meconium plug syndrome and volvulus, double ileostomy, necrotising enterocolitis and Morbus Hirschsprung. Alagille syndrome with renal malformation, peritoneal dialysis for 46 days. All catheters were extremely untight from the very beginning; effective dialysis was not possible. Sealing with fibrine glue was not efficient. Only sealing of the skin with cyanoacrylate glue allowed for effective peritoneal dialysis. Apart from gradually increasing irritation of the skin no serious side effects occurred. No dialysate entered the surrounding soft tissue although the subcutaneous part of the skin channel was always filled with dialysate. Conclusion It is possible to seal a catheter for peritoneal dialysis quickly and effectively with cyanoacrylate glue without serious complications. The existence of an ileostomy did not interfere with the dialysis.
Background: Intravenous sildenafil treatment has recently shown promising results and good tolerability in the treatment of refractory pulmonary hypertension (PH) in term and near-term neonates, while comparable data in preterm infants are still lacking. However, for critically ill preterm infants suffering from PH refractory to conventional treatment, sildenafil may represent a last treatment resort.Patients and methods : We reviewed the records of 6 critically ill extremely preterm infants who had suffered from PH refractory to conventional treatment and had obtained intravenous sildenafil after careful consideration as ultima ratio treatment.Aim: To describe the responses to sildenafil in terms of hemodynamic and respiratory changes during treatment and outcome.Results: 4/6 patients showed resolution of severe PH with full reversal of ductal shunt direction into pure left-to-right shunt within 82 +/- 35 h after sildenafil start. Remarkably, 2/6 patients developed pulmonary hemorrhage at a time point when significant improvement of PH had already taken place, both of them survived. Overall 4/6 patients died, two deaths were related to treatment-refractory PH.Conclusion: Intravenous sildenafil treatment seems effective in improving severe PH and hemodynamic instability in extremely preterm infants with refractory PH. Pulmonary hemorrhage may represent a distinct adverse effect of sildenafil treatment in these patients, presumably due to sudden reversal of ductal shunt. Accordingly, sildenafil should be restricted to most severe and refractory cases in this population.
Background and aim The need for ligation of a patent ductus arteriosus with a haemodynamically significant left to right shunt (hPDA) on echocardiography remains controversial. Aim was to determine echocardiographic and clinical differences of preterms with hPDA after ibuprofen therapy who underwent ligation and those who had no further intervention. Methods Echocardiographic and clinical parameters of preterms with hPDA (< 30 weeks of gestation) were retrieved for the “ligation group” before surgical ligation and for the “non-ligation group” after the last ibuprofen cycle. Recruitment criteria for hPDA were an enddiastolic maximal velocity of the left pulmonary artery (LPAdia) ≥ 0,2 m/s and/or a ratio of the left atrium/aorta (LA/Ao-ratio) ≥ 1,4. Preterms who died before ligation/ductal closure were excluded. Results In 53 of 461 preterms a hPDA was still present after ibuprofen. Thirty-nine preterms were included to the “ligation-group”, 14 to the “non-ligation group”. Significant differences were detected for diastolic and systolic blood pressure, LPAdia, LA/Ao-ratio, stroke volume, backward flow in the aorta abdominalis and the period until total enteral nutrition. Further differences were detected in airway support, hPDA flow patterns, gestational age, intraventricular haemorrhage and necrotising enterocolitis before and after ductal closure. Conclusions The hPDA of the “ligation-group” was haemodynamically more relevant and preterms were more morbid than in the “non-ligation group”. The observed differences reflect our policy of constraining the need for ligating a hPDA on echocardiography to selective ligation subject to both the severity of echocardiographic findings and the hPDA´s clinical impact.
Bronchopulmonary dysplasia (BPD) continues to represent a significant cause of morbidity among survivors of severe prematurity in the NICU. The increasing prevalence of BPD over the decades has been suggested to be related to the increased survival of extremely low birthweight infants.To evaluate differences in prevalence of BPD (BPD28d and BPD36wk) and as a function of survival rate in extremely low birth weight (ELBW) infants over time, and to explore its relationship with known associated risk factors.Survival rate and prevalence of oxygen-dependency =28 days (BPD28d) and oxygen-dependency =36 weeks postmenstrual age (BPD36wk) were evaluated in ELBW newborns (mean gestational age: 27.12.2 weeks; mean birth weight: 817142 g) consecutively admitted to the Brindisi NICU over the last 26 years. Two arbitrarily chosen time periods were compared: Period 1: July 1st, 1986 to June 30, 2002 vs. Period 2: July 1st, 2002 to December 31, 2012. Analyzed variables included gestational age, birth weight, intubation time, hours of O2 administration, NCPAP, and use of surfactant. Differences between the time periods were assessed by chi-square statistics, Fisher's tests or Mann–Whitney test, as appropriate. A two-tailed p value <0.05 was considered to indicate statistical significance.Survival rate of ELBW infants over the examined time periods dramatically improved from 42.3% to 72.6% (p < 0.0001), whereas changes in the prevalence of BPD28d and BPD36wk were not statistically significant (30.5% vs. 39.3%, p = 0.2137 and 5.5% vs. 13.1%, p = 0.1452, respectively). Likewise, BPD severity was not significantly different between the two time periods (p = 0.1635). Gestational age and birth weight of surviving neonates did not significantly change between the two time periods (p = 0.8050 and p = 0.6986, respectively), whereas significantly increased intubation time (median values: 144 hours vs. 33 hours, p <0.0001) and use of exogenous surfactant (89.3% vs. 48.6%, p < 0.0001) was evidenced for the second time period, as well as NCPAP (median values: 600 hours vs. 377 hours, p = 0.0005). A statistically non-significant trend for a prolonged O2 administration in period 2 (p = 0.0850) was also observed.Our findings indicate that a significantly increased survival is not necessarily associated with a significant difference in the prevalence of BPD among ELBW infants.
Continuous positive airway pressure (CPAP) systems for preterm infants work with conventional ventilators or use a jet ventilation system. It is assumed that the most important advantage of jet-CPAP systems is a lower expiratory resistance (R(E) ). We investigated the R(E) of seven different CPAP systems. We studied two primary-care CPAP systems, three jet-CPAP generators, and two conventional CPAP devices. All devices were adjusted at 6 mbar and connected with a test lung simulating a standardized expiration volume. Maximum pressure increase during expiration was measured and maximum R(E) was calculated. In primary-care CPAP devices, the maximum R(E) of the Benveniste valve was 9.7 mbar/L/s (SD 1.2) while that of the Neopuff was 102.8 mbar/L/s (SD 7.9) (P < 0.01). In jet-CPAP devices, the R(E) of the Infant Flow was 6.8 mbar/L/s (SD 1.7), the one of the Medijet REF 1000 was 43.5 mbar/L/s (SD 1.5), and that of the Medijet REF 1010 was 36.7 mbar/L/s (SD 0.3) (P < 0.01). In conventional CPAP systems, the R(E) of the Baby Flow was 29.7 mbar/L/s (SD 1.1) and that of the Bubble CPAP was 37.1 mbar/L/s (SD 4.3) (P < 0.01). All CPAP devices created an R(E). Jet-CPAP devices did not produce lower R(E) than conventional CPAP devices.
Aim of the study: To analyze arterial-ventricular coupling (AV-C) in a cohort of preterms before and after ductal closure. Patients and methods. Inclusion criteria: Gestational age < 33+0, normal intracardiac anatomy, no coarctation, minimum stay in the NICU from day 1 to day 3 of life in the period from 11/2009 - 11/2010. Further criteria were complete M-mode (left atrium, left ventricle) and Doppler recordings (left pulmonary artery, descending aorta and PDA), Patients were stratified into 4 subgroups according to GA. PDA flow patterns were classified according to Skinner (PDA type 1-6). As a measure for AV-C the ratio of effective arterial elastance to left-ventricular endsystolic elastance (EA /ELV ) was calculated based on M-mode derived volumes. Results: The study cohort comprised of 79 patients (subgroup 1 (GA23+0-24+6) n=17, subgroup 2 (GA 25+0-26+6) n=29, subgroup 3 (GA 27+0-28+6) n=21, subgroup 4 GA29+0 -32+6) n=12). PDA types 5 and 6 were found in 41%, 28%, 24% and 8% of patients in subgroups 1,2,3, and 4 respectively. 67/79 patients with a PDA type 3-6 received ibuprofen, in 12 of them ductal clipping was required. Calculation of EA / ELV before and after ductal closure revealed the following ratios: Subgroup 1: 0.28 vs. 0.27, subgroup 2: 0.31 vs.0.35, subgroup 3: 0.45 vs.0.39 and subgroup 4: 0.51 vs. 0.33. Conclusion: Our preliminary data indicate that the effect of ductal closure on either EA or ELV or both depends on gestational age.
Hintergrund: Bereits im Mutterleib ist der Fetus einem kontinuierlichen Geräuschpegel von 40 bis 50 dB(A) ausgesetzt. Nach der Geburt steigt dieser Lärmpegel auf der neonatologischen Intensivstation sprunghaft auf bis zu 150 dB(A) an. Eine der Maschinen, die die höchste Lärmbelastung verursachen, ist der binasale cPAP Jet Ventilator. Fragestellung: In den letzten Jahren sind verschiedene neuere cPAP Generatoren auf den Markt gekommen. In der vorliegenden Studie wurde untersucht, ob diese neueren Modelle die Lärmbelastung Frühgeborener reduzieren können. Material und Methoden: Fünf verschiedene binasale cPAP Generatoren wurden im Hinblick auf den von ihnen erzeugten Lärmpegel untersucht. Drei dieser Modelle wurden auch in Kombination mit einem non-invasiven Beatmungsgerät für Frühgeborene (Infant Flow Advanced) untersucht. Der Lärmpegel in dB(A) wurde bei jedem Generator im Abstand von 2mm lateral vom Ende der nasalen Prongs gemessen.

Einleitung: Es konnte neben Atemzugvolumen und Beatmungsfrequenz auch die Dehnungsgeschwindigkeit der Lunge während der Einatmung als Ursache für den Beatmungsschaden identifiziert werden. Auf diesen Umstand wird jedoch wenig Rücksicht genommen. Die vorliegende Studie soll untersuchen, wie die Beatmung zur Minimierung der Dehnungsgeschwindigkeit eingestellt werden sollte. Methode: In die Studie wurden 12 konventionell beatmete Säuglinge aufgenommen. Bei diesen wurde die anhand internationaler Empfehlungen voreingestellte Beatmung so modifiziert, dass während der gesamten Inspirationszeit (TI) ein Flow in die Lunge herrschte. Hierfür wurde einzig der Beatmungsflow reduziert, alle anderen Einstellungen blieben unverändert. Die Reduktion des Flows wurde für fünf Minuten beibehalten. Ergebnisse: Die Kinder hatten zum Zeitpunkt der Messung ein Gewicht von 2065g (SD 1360). Sie wurden vor der Umstellung mit durchschnittlich 7,6l/min (SD 1,0) Flow beatmet. Der Druck erreichte sein Maximum (Pmax) nach 51% von TI (SD 7,1) und es herrschte während 60% von TI (SD 10,5) ein Flow in die Lunge. Um einen Flow während der gesamten TI zu erreichen konnte der Flow auf durchschnittlich 4,6l/min (SD 1,2) reduziert werden. Mit diesem Flow war Pmax nach durchschnittlich 85% von TI (SD 10,2) erreicht. Das Tidalvolumen (VT) war vor, während und nach der Umstellung unverändert. Schlussfolgerung: Bei der Beatmung nach internationalen Empfehlungen wird die Lunge innerhalb der halben TI auf ihr Maximum gedehnt und verbleibt den Rest von TI unter maximaler Dehnung. Das gesamte VT wird innerhalb von 60% von TI verabreicht. Für eine möglichst langsame Dehnung müsste Pmax erst nach 85% von TI erreicht werden.
Einleitung: Konventionelle CPAP Geräte alarmieren bei kleinen Frühgeborenen nicht immer adäquat bei PEEP-Verlust durch Verrutschen der Prongs. Die CPAP Geräte laufen mit herkömmlichen Beatmungsgeräten mit endständigem Expirationsventil, das große Leckageverluste kompensieren kann. Fragestellung: Welcher Zusammenhang besteht zwischen Alarmen bei PEEP- Verlust, Größe der Prongs und Beatmungsflow. Methode: Untersucht wurde der binasale CPAP BabyFlow in Kombination mit dem Babylog 8000plus im CPAP Modus. Um eine Regulation des PEEPS durch das Expirationsventil zu verhindern, wurde der PEEP auf 25 mbar gestellt. Beginnend mit 30l/min wurden alle verfügbaren Nasen-Prongs (XS, S, M, L, XL) und eine Nasenmaske nacheinander angeschlossen und der entstandene PEEP in einer Flow/Druck Graphik dokumentiert. Der Flow wurde schrittweise um je 2l/min reduziert bis bei allen Ansatzstücken ein PEEP von <2mbar erreicht war. Die Messungen wurden 3x wiederholt. Ergebnisse: Die Graphik ergibt, dass ein PEEP von z.B. 6mbar mit den XS-Prongs bei einem Flow von 9,5l/min erreicht wird, mit S-Prongs bei 14l/min, mit M-Pongs bei 16l/min, mit L-Prongs bei 19l/min, mit XL-Prongs bei 20,5l/min und mit der Maske bei 23l/min. Schlussfolgerung: Es zeigt sich, dass abhängig von der Größe der Prongs der gemessene PEEP im Schlauchsystem nur bis zu einem gewissen Maximalflow dem tatsächlichen PEEP im Kind entsprechen kann. Bei höherem Flow erzeugt der Strömungswiderstand der Prongs bereits den eingestellten Druck weshalb ein Abfallen des PEEP z.B. durch Verrutschen der Prongs keinen Alarm auslösen kann. Je kleiner die Prongs und je niedriger der gewünschte PEEP desto schneller ist dieser Maximalflow erreicht.
Einleitung und Zielsetzung: Die Studie soll zeigen, ob ROP-Screening Untersuchungen ohne Verwendung von Lidspreizern und ohne Intendieren des Bulbus weniger schmerzhaft für die Frühgeborenen sind, ohne dass dabei ROP-Erkrankungen übersehen werden. Patienten und Methoden: 38 Patienten, die ohne Verwendung von Lidspreizern und ohne Intendieren des Bulbus untersucht wurden, wurden mit 54 Patienten verglichen, die mit dieser Technik untersucht wurden. Schmerz und Stress der Patienten wurden an Hand des Herzfrequenzanstiegs sowie mittels des Neonatal Infant Pain Scores (NIPS) und eines subjektiven Scorings durch die behandelnde Schwester beurteilt. Resultate: Sowohl die Untersuchungsdauer als auch der NIPS und die subjektiv erhobenen Scores waren signifikant geringer bei den Patienten, die ohne Lidspreizer und Intendieren des Bulbus untersucht wurden (p=0,001). Auch die Zeitspanne bis zum Erreichen der Ausgangswerte vor der Untersuchung war signifikant kürzer (p=0,001). Trotzdem liegt die ROP-Inzidenz für diese Patientengruppe deutlich unter der ROP-Inzidenzrate des Vermont Oxford Neonatal Netzwerkes. Diskussion: Obwohl in den gültigen ophthalmologischen Richtlinien Lidspreizer und das Intendieren des Bulbus für ROP-Screeninguntersuchungen empfohlen werden, kann auch ohne diese Technik eine gründliche Beurteilung des Augenhintergrundes erfolgen. Die Untersuchung ist damit für die Frühgeborenen signifikant weniger schmerzhaft.
Einleitung: Obwohl die Vorteile einer Ernährung mit Muttermilch gegenüber Formula eindeutig belegt sind, ist die Stillrate bei Müttern von Frühgeborenen relativ nieder. Ziel der vorliegenden Studie war es, Parameter zu identifizieren, die das Stillverhalten von Müttern Frühgeborener beeinflussen. Patienten und Methoden: Retrospektiv wurden alle Patienten mit einem Geburtsgewicht<1500g, die zwischen 01.01.2000 und 31.12.2005 an der neonatologischen Abteilung der Universitätsklinik für Kinder- und Jugendheilkunde behandelt und nicht vorzeitig transferiert wurden, entsprechend ihrer Ernährung zum Zeitpunkt der Entlassung in zwei Gruppen eingeteilt. Folgende Parameter wurden untersucht: Parität, Einlings- vs. Mehrlingsschwangerschaft, Gestationsalter, Geburtsgewicht, Geschlecht, schwere Komplikationen und Aufenthaltsdauer. Ergebnis: Von den 239 in die Studie eingeschlossenen Patienten wurden zum Zeitpunkt der Entlassung 142(60%) zumindest teilweise mit Muttermilch ernährt, 97(40%) erhielten ausschließlich Formulanahrung. Von allen untersuchten Parametern korrelierte einzig die Aufenthaltsdauer signifikant mit der Ernährung zum Zeitpunkt der Entlassung (p=0,006). Je länger die Kinder im Krankenhaus waren, desto eher waren sie bei Entlassung mit Formula ernährt. In der Untergruppe der Kinder mit schweren Komplikationen ergab sich eine signifikante Korrelation mit den Parametern: Parität (p=0,002), Einlings- vs. Mehrlingsschwangerschaft (p=0,009), Geburtsgewicht (p=0,03) und Aufenthaltsdauer (p=0,007). Diskussion: Die vorliegende Studie zeigt eindeutig, dass die Muttermilchernährung von Frühgeborenen mit der Dauer des Krankenhausaufenthaltes signifikant abnimmt. Eine intensivere Stillberatung während des gesamten stationären Aufenthaltes wäre daher dringend notwendig.
Einleitung: Für peripher verlegte zentrale Katheter (=Schwemmkatheter) werden erst ab einem Durchmesser von 4-Fr Einführhilfen angeboten, die mittels modifizierter Seldinger Technik verlegt werden können. Dünnere Katheter müssen bisher mittels Einführhilfen, die wie peripher venöse Zugänge verlegt werden eingeschwemmt werden. Wir möchten hier eine modifizierte Seldinger Technik für 2- und 3-Fr Schwemmkatheter sowie erste klinische Erfahrungen mit dieser neuen Methode vorstellen. Methode: Hierfür wurde ein mittels Seldinger Technik verlegbarer Arterienkatheter mit einer Einführkanüle für Schwemmkatheter kombiniert. Zuerst wird die Stahlnadel aus der Einführkanüle entfernt und an deren Stelle der Arterienkatheter eingeführt (Abb. A). Danach wird ein peripherer Venenzugang in eine passende Vene verlegt (24-G für 2-F Ketheter, 22-G für 3-F Katheter). Über diese wird die Seldingerspirale des Arterienkatheters eingeführt (Abb. B). Nach Entfernung des periphervenösen Zuganges und Vergrößerung der Hauteintrittsstelle mittels einer minimalen Stichinzision werden über die Spirale Arterienkanüle und Einführkanüle in die Vene vorgeschoben (Abb. C). Spirale und Arterienkatheter können dann entfernt und der Katheter eingeschwemmt werden (Abb. D).
Einleitung: Chronische Lungenschäden, mitverursacht durch maschinelle Beatmung, sind ein wesentliches Morbiditätsrisiko bei extrem kleinen Frühgeborenen. Ursache dafür ist die Überdehnung der Lungenarchitektur durch die oft notwendigen hohen Tidalvolumina. Eine Möglichkeit zur Reduktion des Tidalvolumens ist die Minimierung des Technischen Totraumes. Wir haben ein neues Beatmungssystem entwickelt, wobei 10% des Beatmungsflows über eine Bypass (Splitflow) Leitung zum Tubusansatz geleitet werden und von dort retrograd den Totraum auswaschen. In zwei Pilotstudien konnte durch dieses Auswaschen des Totraumes der Beatmungsaufwand um bis zu 40% reduziert werden. Herkömmliche Flowsensoren sind für diese neue Form der totraumreduzierten Beatmung allerdings nicht geeignet, da sie den retrograden Splitflow als zusätzlichen exspiratorischen Flow detektieren, wodurch eine genaue Messung des Tidalvolumens unmöglich wird. Wir haben deshalb einen Prototyp eines Flowsensors entwickelt, der auch bei Splitflowbeatmung eine genaue Volumsmessung und damit eine synchronisierte Beatmung ermöglicht.
